Cancer rates in men and women of reproductive age have continued to increase in recent years; however, therapy has dramatically decreased the mortality rates. Since 1990, the prevalence of cancer survivors in young adults increased from 1 in 1,000 to 1 in 250 patients due to more aggressive therapies. Current therapies may have profound toxic effects on gamete function with infertility as an expected consequence of cancer therapy. Depending on the site and stage of cancer, age of the patient, and (...) the type of treatment, approximately 90% of men and women diagnosed with cancer may be at risk of permanent infertility.Fertility preservation has emerged as a discipline dedicated to improving the future reproductive potential of cancer survivors. Significant progress in the advancement of fertility preservation therapies and a heightened awareness of the availability of therapies has occurred in the past 10 years. The American Society of Clinical Oncology and the American Society of Reproductive Medicine have advanced these efforts by formally recognizing the importance of fertility awareness. (shrink)

Despite extinction-based processes demonstrating efficacy in the animal extinction and human anxiety literatures, extinction for substance use disorders has shown poor efficacy. Reasons for this lack of success include common threats to extinction, such as renewal and reinstatement. In recent decades, research on mindfulness for SUD has flourished, and a key aspect of these mindfulness-based interventions includes teaching individuals to stay present with whatever experience they have, even if unpleasant, without trying to change/escape/avoid it. Similarly, CET teaches individuals to not (...) escape/avoid conditioned responses by engaging in drug use behavior. This paper discusses how mindfulness-based research and practices could positively influence CET through future research, with the long-term goal of improving SUD treatment. (shrink)

The Waltham Chronicle is an interesting example of a twelfth-century historia fundacionis. Written by one of the secular canons of Waltham just after the refoundation of the house as an Augustinian priory in 1177, it records the legends of the original foundation and miracle stories, together with historical information about the pre-Conquest benefactors and the internal organization of the community. Its value is much more than that of a local history, because of its connection with the literary romances of Harold (...) Godwineson, the religious context of the theology of the Cross, and the general movement to replace sector colleges by houses of regular Benedictine monks or Augustinian canons. This is the first published translation, printed with the original text editied from the two manuscripts containing the Chronicle, with full introduction, historical notes and apparatus criticus, by Leslie Watkiss and Marjorie Chibnall. ABSTRACT The nature of two influential theories on the moral status of abortion logically commits them to welcoming the advent of ectogenesis as a solution to the abortion conflict. However, qualitative research into women's response to ectogenesis reveals that both women in favour and women opposed to abortion rights reject the technology on surprisingly similar grounds. The abortion framework which led women to reject ectogenesis as an ethical resolution to unwanted pregnancy is contrasted with the moral framework which shapes formal (...) ethical discussions of abortion. Quantum mechanics and probability theory share one peculiarity. Both have well established mathematical formalisms, yet both are subject to controversy about the meaning and interpretation of their basic concepts. Since probability plays a fundamental role in QM, the conceptual problems of one theory can affect the other. We first classify the interpretations of probability into three major classes: inferential probability, ensemble probability, and propensity. Class is the basis of inductive logic; deals with the frequencies of events in repeatable experiments; describes (...) a form of causality that is weaker than determinism. An important, but neglected, paper by P. Humphreys demonstrated that propensity must differ mathematically, as well as conceptually, from probability, but he did not develop a theory of propensity. Such a theory is developed in this paper. Propensity theory shares many, but not all, of the axioms of probability theory. As a consequence, propensity supports the Law of Large Numbers from probability theory, but does not support Bayes theorem. Although there are particular problems within QM to which any of the classes of probability may be applied, it is argued that the intrinsic quantum probabilities are most naturally interpreted as quantum propensities. This does not alter the familiar statistical interpretation of QM. But the interpretation of quantum states as representing knowledge is untenable. The broad use of social networking and user-generated content has increased the online footprint of many individuals. A generation of healthcare professionals have grown up with online search activities as part of their everyday lives. Sites like Facebook, Twitter, and Instagram have given the public new ways to share intimate details about their public and private lives and the lives of their friends and families. As a result, careproviders have the ability to find out more about their patients with just (...) the tap of a key or the click of a mouse. This type of online searching for patient information is known as patient-targeted googling or PTG.This article provides an overview of the emergence of PTG, identifies the potential benefits and possible pitfalls of engaging in PTG, and explores current ethical frameworks that guide decisions about PTG. The article describes the development of a critical thinking tool developed by the Behavioral Health Ethics Committee at CHI Health, that can serve as a best-practice model for other hospitals and health systems. Called TTaPP (Together Take a Pause and Ponder), this tool is designed to help healthcare professionals across settings practice collaborative critical thinking skills as they consider the ethical questions of whether or not to engage in PTG. Finally, this article suggests areas for further study, including ways to prompt collaboration and appropriate documentation by maximizing electronic medical records systems, exploring the effectiveness of the TTaPP tool as a way to promote a culture of collaborative critical thinking practices, and the attitudes of patients and the public regarding PTG. The historical-grammatical approach to exegesis, while noteworthy for uncovering historical realities, can often lead to a dry, lifeless reading of Scripture. More subjective models seek the text's contemporary relevance, but run the risk of missing the historical truth communicated by the Spirit-inspired author. This paper explores whether these two horizons can work together, and then advance toward a transformative hermeneutic. Such an interpretive method includes an openness to the Spirit's leading and guidance, a robust historical investigation, and a contextualization of (...) the message into similar contemporary situations and contexts. This paper considers Pindar's diverse appropriations of elements of the sacred topography of Aegina for different purposes in epinikia composed for Aeginetan victors. It focuses on poems likely performed in the vicinity of the Aiakeion for their different mobilizations of a monument that we know from Pausanias stood beside the Aiakeion—the tomb of Phokos, an earth mound topped with the “rough stone” that killed him. The more speculative final part of the paper suggests that it may also be possible to (...) track a coherent ideology attached to the island's sacred topography across several Aeginetan odes, thereby detecting a broader structural unity that accompanies and frames the different individual appropriations of different poems. This part starts from Pausanias’ mythic narrative of the exemplary justice of Aiakos banishing his own son Telamon as the aetiology for a distinctive Aeginetan justice system inscribed in a whole set of man-made monuments that ring the island with concentric circles of rough stones. From the very first scene of The Good Place, monumental duplicity is at work. Everything is contrived and everyone is lying, both to themselves and to one another. The Good Place raises the issue of how to determine whether a person is ethical or not. Both Eleanor and Tahani struggle with significant feelings of inadequacy, compelling them to commit ethical infractions to land them in “The Good Place”. Although Tahani and Eleanor come from divergent social stations, they share a common (...) legacy of familial abuse that deeply affected their sense of self‐worth, causing them to relentlessly exploit and abuse others in their attempts to gain a positive self‐image. The Good Place reminds that in our lives we are constantly confronted by ethical dilemmas that challenge us to make choices, and all we have for guidance are flawed, often conflicting ideals that we pick up along the way. Intellectually, the Nineteenth Century was marked by a struggle for a world vision — a struggle which shows itself in the minds and works of the British poets who often sought to reconcile science and religion. Blake feared that sophisticated concepts were destroying our contact with reality. Coleridge drew on Plato, Kant and Hegel and wrestled with the materialism of Hartley. Shelley used ideas from Plato and Berkeley. Wordsworth sought a visio if nature in which people could be comfortable. Hopkins (...) sought the idea of individuality in Duns Scotus.
