In recent decades, debate on the quality and safety of healthcare has been dominated by a measure and manage administrative rationality. More recently, this rationality has been overlaid by ideas from human factors, ergonomics and systems engineering. Little critical attention has been given in the nursing literature to how risk of harm is understood and actioned, or how patients can be subjectified and marginalised through these discourses. The problem of assuring safety for particular patient groups, and the dominance of technical (...) forms of rationality, has seen the word ‘unavoidable’ used in connection with intractable forms of patient harm. Employing pressure injury policy as an exemplar, and critically reviewing notions of risk and unavoidable harm, we problematise the concept of unavoidable patient harm, highlighting how this dominant safety rationality risks perverse and taken‐for‐granted assumptions about patients, care processes and the nature of risk and harm. In this orthodoxy, those who specify or measure risk are positioned as having more insight into the nature of risk, compared to those who simply experience risk. Driven almost exclusively as a technical and administrative pursuit, the patient safety agenda risks decentring the focus from patients and patient care. (shrink)

In a crisis, managers are confronted with a dilemma between their ethical responsibility to respond to victims and their fiduciary responsibility to protect shareholder value. In this study, we use a unique and comprehensive dataset of 223 non-financial crises between 1983 and 2013 to investigate how corporate apologies affect stock prices. Our empirical evidence shows that the stock price response from apologizing depends on the firm’s level of responsibility for the crisis. We find that to protect shareholder value, management needs (...) to match its formal response strategy with the degree of its responsibility for the crisis. Further, failing to match the proper response is harmful to shareholders: Both apologizing when the firm is not directly responsible for the crisis and failing to apologize when the firm is directly responsible reduce shareholder wealth. However, apologizing for a crisis when the firm is directly responsible mitigates losses to shareholder value that arise because of the crisis, as does refraining from apologizing when the firm is not directly responsible. ABSTRACT:This article outlines the policy context for teachers’ learning and continuing professional development in Scotland and considers this in relation to the perspectives of key informants gained through interview. The analysis draws on a triple-lens conceptual framework and points to some interesting contradictions between the policy text and the expressed aspirations of the interviewees. Current policy and the associated structural arrangements are viewed as broadly positive, but interviewees express concerns that an unintended emphasis on contractual arrangements might inhibit the more (...) transformative elements of professional learning. BackgroundThe COVID-19 pandemic has had a significant impact on the health system worldwide. The organ and tissue donation and transplantation (OTDT) system is no exception and has had to face ethical challenges related to the pandemic, such as risks of infection and resource allocation. In this setting, many Canadian transplant programs halted their activities during the first wave of the pandemic.MethodTo inform future ethical guidelines related to the COVID-19 pandemic or other public health emergencies of international concern, we conducted a (...) literature review to summarize the ethical issues.ResultsThis literature review identified three categories of ethical challenges. The first one describes the general ethical issues and challenges reported by OTDT organizations and transplantation programs, such as risks of COVID-19 transmission and infection to transplant recipients and healthcare professionals during the transplant process, risk of patient waitlist mortality or further resource strain where transplant procedures have been delayed or halted, and resource allocation. The second category describes ethical challenges related to informed consent in the context of uncertainty and virtual consent. Finally, the third category describes ethical issues related to organ allocation, such as social considerations in selecting transplant candidates.ConclusionThis literature review highlights the salient ethical issues related to OTDT during the current COVID-19 pandemic. As medical and scientific knowledge about COVID-19 increases, the uncertainties related to this disease will decrease and the associated ethical issues will continue to evolve. (shrink)

Indeed, if we grasp Virginia Held’s care ethics, we acknowledge that all humans are interdependent and that the vulnerable among us deserve particularly conscientious consideration—some level of care. Accordingly, I agree with Rosemarie Tong when she proposes that those who dodge caring roles marginalize themselves from society. This marginalization can occur if I squirm out of attending to my ailing family members’ needs, or if I avoid (employment or (...) volunteer) opportunities to care for those other than my family. As the term regenerative agriculture caught fire in public discourse around 2019, it was promptly labelled a buzzword. While the buzzword accusation tends to be regarded as negative, these widely used terms also reflect an important area of growing public interest. Exploring a buzzword can thus help us understand our current moment and offer insights to paths forward. In this study, we explored how and why different individuals and groups adopt certain key terms or buzzwords, in this case the term (...) “regenerative agriculture”. We used an interpretivist approach to understand how “regenerative agriculture” is being constructed, interpreted, understood, and employed, drawing from 19 semi-structured interviews conducted with farmers, researchers, private companies, and NGO/nonprofits. Several interviewees felt that regenerative agriculture is making an important societal shift in thinking towards addressing major issues like climate change and parity in our food and agricultural systems. However, farmers in particular felt that the term is being greenwashed, coopting the work they do, and even diluting the meaning. We also found that regenerative agriculture is being advanced as mobilizing “win-wins”—for farmers, for consumers, for society—but that this discourse may be veiling the political and economic agendas of the big companies using the term. Our findings further illustrated the debates over standardizing the term regenerative agriculture, with some contending that there should be room for “continuous improvement” but others felt it is meaningless without a definition. A number of recent studies have examined the effects of phonological variation on the perception of speech. These studies show that both the lexical representations of words and the mechanisms of lexical access are organized so that natural, systematic variation is tolerated by the perceptual system, while a general intolerance of random deviation is maintained. Lexical abstraction distinguishes between phonetic features that form the invariant core of a word and those that are susceptible to variation. Phonological inference relies on the (...) context of surface changes to retrieve the underlying phonological form. In this article we present a model of these processes in speech perception, based on connectionist learning techniques. A simple recurrent network was trained on the mapping from the variant surface form of speech to the underlying form. Once trained, the network exhibited features of both abstraction and inference in its processing of normal speech, and predicted that similar behavior will be found in the perception of nonsense words. This prediction was confirmed in subsequent research (Gaskell & Marslen-Wilson, 1994). (shrink)

The promotion of the self‐managing capacities of people has become a marker of contemporary mental health practice, yet self‐management remains a largely uncontested construct in mental health settings. This discourse analysis based upon the work of Foucault investigates self‐management practices for bipolar disorder and their action upon how a person with bipolar disorder comes to think of who they are and how they should live. Using Foucault's framework for exploring the ethical self and transcripts of interviews with people living with (...) bipolar disorder, this discourse analysis finds that the discursive practices of self‐management for bipolar disorder are prescribing a restricted regime for living based upon the quintessential neo‐liberal subject using practices that are focused upon managing an unreliable and problematic self. The article concludes with the proposition that the problem of bipolar disorder (for which self‐management is an answer) is less about the object itself and more about how we construct the nature of the self. A Māori indigenous ontology is used here as an example of an alternative discursive resource through which people make sense of the self and which allows ways of thinking about self‐management that engage with ambivalence and ambiguity rather than restriction and regulation. Mary Shepherd (1777–1847) was a fierce and brilliant critic of Berkeley and Hume, who moreover offered strikingly original positive views about the nature of reality and our access to it which deserve much more attention (and credit, since she anticipates many prominent views) than they have received thus far. By way of illustration, I focus on Shepherd's 1824 Essay Upon the Relation of Cause and Effect, Controverting the Doctrine of Mr. Hume, Concerning the Nature of that Relation (ERCE). After (...) a brief setup, I canvas certain of her trenchant objections to Hume’s argumentation; I then present the positive core of her response to Hume, which consists in providing novel accounts of how reason alone or reason coupled with experience can justify, first, that every effect must have a cause, and second, that it is necessary that like causes produce like effects. Among other contributions here, Shepherd provides a distinctively metaphysical argument for the claim that nothing can begin to exist 'of itself' (going beyond an appeal to the Principle of Sufficient Reason, in particular), and leverages difference-making considerations to make the case that a single 'experimentum crucis' can justify causal belief (anticipating Mill's 'method of difference'). I close by highlighting salient features of Shepherd's metaphysics of causation, whereby causation is singularist and local (anticipating Ducasse and Anscombe) and involves synchronic interactions (anticipating Mill's and certain contemporary accounts), and according to which objects are essentially characterized by their causes and effects (anticipating contemporary causal or dispositional essentialist positions). This paper reports on the experiences of creative practice graduate researchers and academic staff as they seek to comply with the requirements of the Australian National Statement on the Ethical Conduct of Research Involving Humans. The research was conducted over a two-year period (2015 to 2017) as part of a wider project ‘iDARE – Developing New Approaches to Ethics and Research Integrity Training through Challenges Presented by Creative Practice Research’. The research identified the appreciation of ethics that the participants acquired (...) through their experience of institutional research ethics procedures at their university. It also revealed a disjunction between the concepts of ethics acquired through meeting institutional research ethics requirements, the notion of ethics that many researchers adopt in their own professional creative practice and the contents of professional codes of conduct. A key finding of the research was that to prepare creative practice graduates for ethical decision-making in their professional lives, research ethics training in universities should be broadened to encompass a variety of contexts and enable researchers to develop skills in ethical know-how. : Catholic teaching has no moral difficulties with research on stem cells derived from adult stem cells or fetal cord blood. The ethical problem comes with embryonic stem cells since their genesis involves the destruction of a human embryo. However, there seems to be significant promise of health benefits from such research. Although Catholic teaching does not permit any destruction of human embryos, the question remains whether researchers in a Catholic institution, or any researchers opposed to destruction of human embryos, (...) could participate in research on cultured embryonic stem cells, or whether a Catholic institution could use any therapy that ultimately results from such research. This position paper examines how such research could be conducted legitimately in a Catholic institution by using an ethical analysis involving a narrative context, the nature of the moral act, and the principle of material cooperation, along with references to significant ethical assessments. Hirschi and Gottfredson (1983) claim that the relationship between age and crime is similar in all social and cultural conditions and that no current sociological or criminological theory can account for this similarity. We introduce the new field of evolutionary psychology and extend Daly and Wilson's (1988) work on homicide to construct a general theory of male criminality, which explains why men commit violent and property crimes. The theory can also explain the age-crime curve. Halford, Wilson & Phillips argue that cognitive development is driven by increases in processing capacity. We suggest that changes in relational knowledge are as important or more so. We present evidence that 3-year-olds' analogical abilities are sharply improved by teaching them relational labels; over a 30-minute experimental session children gained approximately 2 years in effective performance. These results mandate caution interpreting age-related change as indicating maturational change, and call for a deeper consideration of the role of epistemological change in (...) cognitive development. In discussing letter-writing, Whitman made his own views clear. Simplicity and naturalness were his guidelines. ”I like my letters to be personal—very personal—and then stop.“ The six volumes in The Correspondence comprise nearly 3,000 letters written over a (...) half century, revealing Whitman the person as no other documents can. This volume, together with Volume V, covers the last seven years of Whitman’s life, giving an almost day-by-day account of his long struggle with various ailments, his stoical acceptance of constant pain, but also his continuing energy. This period saw his supervision and publication of two complete editions of Leaves of Grass, as well as November Boughs and Good-bye My Fancy. Although Whitman himself admitted that many of his later poems were “pot boilers,” designed primarily to make money, his recognition and popularity continued to grow as his health declined. Mary Midgley's Science and Poetry tackles so many topics of importance that one wants it to be very good. Yet Midgley, a moral philosopher, makes one idea the measure of all things, so that the book is just good enough. Her topic is not really “science and poetry” but the failure of neurobiological reductionism to understand the human mind. That poets understand the mind better than scientists is the subtext of this collection of essays, but the poetic theories Midgley (...) quotes are too general to make much headway against science's prejudices about the arts' naive handling of the serious matters of consciousness, the operation of the brain, and connections between mind and body. Midgley needs to dig deeper into poetry and to provide better analyses of works like Wordsworth's “Tintern Abbey” and Shelley's A Defence of Poetry. She also needs to update her poetic references to include contemporaries such as A. R. Ammons, Patti Ann Rodgers, and Alison Deming, who make the science‐based topics that preoccupy Midgley the overt focus of their work. The valuable first section of the book presents capsule histories of science and its march toward dualism, from the Greeks to the Enlightenment, interspersed with poets' contrary views. Midgley succinctly traces the evolution of the theories of the mind/body split that such a campaign produced. She drops the “science and poetry” focus in the second section for her major concern—showing that contemporary scientists themselves, in pursuing scientific understanding, employ choice, intention, and cognition, the very factors denied by strict scientific materialist explanations of how the mind works. This is the neatest feature of the book—its witty turning of scientific discourse about the purely mechanistic operations of the mind back on the scientists themselves, who violate their own theories in stating them. Her favorite scientists to tweak in this way are the sociobiologist E. O. Wilson and the geneticist Richard Dawkins. When she needs to, Midgley lets a philosopher of language like John Searle handle others, such as Daniel Dennett and his claim that cultural ideas are simply genetically produced “memes.” Midgley's summary, however, does not do justice to the complexity of the debate about the nature of language as a window to the operation of the mind. Midgley's strength is in testing fundamental theoretical assumptions by applying common‐sense reasoning to them. For example, Descartes's mind/body dualism fails on the face of it because he uses socially constructed language, a product of mind, as he asserts the complete independence and autonomy of individual minds. Midgley's deft use of simple but revealing thought‐problems also helps make her case, and she is particularly good at alerting us to ordinary words that need clarification, such as “cause.” She's like a poet when she sensitizes us to the power of simple words in shaping scientific discourse. In the miscellaneous last section of the book Midgley provides examples of the contemporary dangers of the reductivist way of thinking, but the first two parts could have used livelier, more pointed dramatizations of the consequences of splitting the world into subjects and objects. Universal human rights and the right of all creatures to live in cooperation are what Midgley believes science ultimately sacrifices in its vision of competitive, reductive materialism. Scientists and social scientists such as Freud and Marx who use that “vision” have led us down disastrous paths. That science should learn to see the world as a cooperating, interacting, and living whole is her wish—and the subject of many of her other books. Science and Poetry would be more effective if condensed into a single, focused essay that concentrated on convincing Midgley's presumed audience—scientists who may have found themselves at the dead end of materialism as their own disciplines discover their limits and branch out into territory more familiar to poets. The book should interest students of literature who want an introduction to the many topics it treats, but they will find little to convince them that poets do indeed have important ideas about the human mind.
