AimTo examine the relative cost-effectiveness of predictive genetic tests for familial breast and ovarian cancer provided by Genetic Services of Western Australia.MethodsThe relative cost-effectiveness was assessed using a decision analytic model.ResultsThe cost and outcomes of genetic testing was compared in first-degree relatives of known BRCA1/2 mutation-carriers who have a 50% risk of carrying the mutated gene (intervention group) to individuals with the same a priori risk but who do not undergo a genetic test (control subjects).Since genetic testing enables the restriction (...) of intensive surveillance to individuals with an identified BRCA1/2 gene mutation, net savings in the period observed (age 25-70) were $980-$1008 per woman in the ovarian intervention group and $1681-$1795 per woman in the breast intervention group, and delayed the onset of breast cancer (6mths BRCA1, 3mths BRCA2).Compared to control subjects undergoing population surveillance, it was estimated the onset of breast cancer could be delayed at a total net cost of $3055 (5.1yrs) to $3389 (3.2yrs) for women in the breast intervention group with BRCA1/2 mutations. Since population surveillance is not currently recommended for ovarian cancer, control subjects undergoing no surveillance were compared with the intervention group. The onset of ovarian cancer was delayed at a net cost of $1630 (3.5yrs) to $2509 (1.2years) for women with BRCA1/2 mutations.ConclusionsTesting allows targeted high-level surveillance for gene mutation carriers, which ensures the cost-effective use of resources and reduces cancer-related morbidity if clinical recommendations for intervention are adopted. Relational Quantum Mechanics is an interpretation of quantum mechanics proposed by Carlo Rovelli. Rovelli argues that, in the same spirit as Einstein’s theory of relativity, physical quantities can only have definite values relative to an observer. Relational Quantum Mechanics is hereby able to offer a principled explanation of the problem of nested measurement, also known as Wigner’s friend. Since quantum states are taken to be relative states that depend on both the system and the observer, there is no inconsistency in (...) the descriptions of the observers. Federico Laudisa has recently argued, however, that Rovelli’s description of Wigner’s friend is ambiguous, because it does not take into account the correlation between the observer and the quantum system. He argues that if this correlation is taken into account, the problem with Wigner’s friend disappears and, therefore, a relativization of quantum states is not necessary. I will show that Laudisa’s criticism is not justified. To the extent that the correlation can be accurately reflected, the problem of Wigner’s friend remains. An interpretation of quantum mechanics that provides a solution to it, like Relational Quantum Mechanics, is therefore a welcome one. (shrink)

Prominent accounts of language use (those of Grice, Lewis, Stalnaker, Sperber and Wilson among others) have viewed basic communicative acts as essentially involving the attitudes of the participating agents. Developmental data poses a dilemma for these accounts, since it suggests children below age four are competent communicators but would lack the ability to conceptualise communication if philosophers and linguists are right about what communication is. This paper argues that this dilemma is quite serious and that these prominent accounts would be (...) undermined if an adequate more minimal alternative were available. Just such a minimalist account of communication is offered, drawing on ideas from relevance theory and situation theory. (shrink)

Philosophy of language in the 21st century has cultivated a concern for the hateful, the coercive, and the lethal. Amidst this shift of attention toward politically significant speech, ‘non-ideal’ philosophers of language investigate whether common conceptual toolkits from ‘mainstream’ philosophy of language manage to make contact with our non-ideal world in the first place. Drawing on a tradition of Wittgensteinian critical social thought, I contend that philosophers of language risk their (ideology‐)critical bite when they isolate our words from the activities (...) into which they are woven. That is, when philosophers fail to register our ordinary investment in the words we put on display—the interests, stakes, and concerns in light of which we voice them—we make their political import unavailable to philosophy of language. But the non-ideal philosopher’s concept of idealization is itself a normatively non-neutral tool; and, as such, it exemplifies the sort of theoretical resource that may be mobilized with an investment in shaping not only our epistemic resources but also our senses of what matters. The structural approach to alternatives :669–690, 2007; Fox and Katzir in Nat Lang Semant 19:87–107, 2011; Katzir in Semantics, pragmatics and the case of scalar implicatures, Palgrave Macmillan, London, pp 40–71, 2014) is the most developed attempt in the literature at solving the symmetry problem of scalar implicatures. Problematic data with indirect and particularised scalar implicatures have however been raised :249–270, 2015). To address these problems, Trinh and Haida proposed to augment the theory with the Atomicity Constraint. Here we show (...) that this constraint falls short of explaining minimal variants of the original problems, and moreover that it runs into trouble with the inferences of sentences involving gradable adjectives like full and empty. We furthermore discuss how the structural approach suffers at times from the problem of ‘too many lexical alternatives’ pointed out by Swanson :31–36, 2010), and at other times from the opposite problem of ‘too few lexical alternatives’. These three problems epitomise the challenge of constructing just enough alternatives under the structural approach to solve the symmetry problem in full generality. Finally, we also sketch another recent attempt at solving the symmetry problem, Bergen et al., 2016), which is based on relative informativity and complexity. We argue that Bergen et al. do not provide a general solution to the symmetry problem either, by pointing to some of the open problematic cases that remain for this approach as well. We conclude that while important progress has been made in the theory of alternatives for scalar implicatures in the last few years, a full solution to the symmetry problem has not yet been attained. Brain-computer interfaces are communication bridges between a human brain and external world, enabling humans to interact with their environment without muscle intervention. Their functionality, therefore, depends on both the BCI system and the cognitive capacities of the user. Motor-imagery BCIs rely on the users’ mental imagination of body movements. However, not all users have the ability to sufficiently modulate their brain activity for control of a MI-BCI; a problem known as BCI illiteracy or inefficiency. The underlying mechanism of this phenomenon (...) and the cause of such difference among users is yet not fully understood. In this study, we investigated the impact of several cognitive and psychological measures on MI-BCI performance. Fifty-five novice BCI-users participated in a left- versus right-hand motor imagery task. In addition to their BCI classification error rate and demographics, psychological measures including personality factors, affinity for technology, and motivation during the experiment, as well as cognitive measures including visuospatial memory and spatial ability and Vividness of Visual Imagery were collected. Factors that were found to have a significant impact on MI-BCI performance were Vividness of Visual Imagery, and the personality factors of orderliness and autonomy. These findings shed light on individual traits that lead to difficulty in BCI operation and hence can help with early prediction of inefficiency among users to optimize training for them. (shrink)

BackgroundDespite its ubiquity, it is often not clear what organizations and services mean by well-being. Visual impairment has been associated with poorer well-being and well-being has become a key outcome for support and services for adults living with VI. A shared understanding of what well-being means is therefore essential to enable assessment of well-being and cross-service provision of well-being support.ObjectivesTo provide an overview of the ways in which well-being has been conceptualized in research relating to adults living with VI.Eligibility criteriaArticles (...) were included in the review if the article discussed well-being in the context of adults living with VI, was available in English and as a full text.Data sourcesA systematic search using search terms relating to VI and well-being was conducted in EBSCOHost and Ovid.ChartingA team of three reviewers screened titles, abstracts and full-texts articles and extracted data. Ambiguous articles were referred to the research group and discussed.ResultsOf 10,662 articles identified in the search, 249 were included in the review. These referred to 38 types of well-being. The most common types were general well-being emotional well-being and psychological well-being. Most articles referred to one type only, with a maximum of 9 listed in one article. A large number of articles did not clearly define well-being. A wide range of indicators of well-being related to the domains of hedonia, mood, positive and negative affect, quality of life, mental health, eudaimonia, self/identity, health, psychological reactions to disability and health problems, functioning, social functioning and environment, were extracted, many of which were used just once.ConclusionsThere remains a lack of consensus on how well-being is conceptualized and assessed in the context of adult VI. A standardized multi-domain approach derived with input from adults with VI and practitioners working with them is required to enable comparison of findings and cross-organizational provision of support. Most consumers are aware that climate change is a growing problem and admit that action is needed. However, research shows that consumers’ behavior often does not conform to their value and orientations. This value-behavior gap is due to contextual factors such as price, product design, and social norms as well as individual factors such as personal and hedonic values, environmental beliefs, and the workload capacity an individual can handle. Because of this conflict of interest, consumers have a hard time identifying (...) the true drivers of their behavior, as they are either unaware of or unwilling to acknowledge the processes at play. Therefore, consumer neuroscience methods might provide a valuable tool to uncover the implicit measurements of pro-environmental behavior (PEB). Several studies have already defined neurophysiological differences between green and non-green individuals; however, a behavior change intervention must be developed to motivate PEB among consumers. Motivating behavior with reward or punishment will most likely get users engaged in climate change action via brain structures related to the reward system, such as the amygdala, nucleus accumbens, and (pre)frontal cortex, where the reward information and subsequent affective responses are encoded. The intensity of the reward experience can be increased when the consumer is consciously considering the action to achieve it. This makes goal-directed behavior the potential aim of behavior change interventions. This article provides an extensive review of the neuroscientific evidence for consumer attitude, behavior, and decision-making processes in the light of sustainability incentives for behavior change interventions. Based on this review, we aim to unite the current theories and provide future research directions to exploit the power of affective conditioning and neuroscience methods for promoting PEB engagement. Cross-cultural comparisons are more difficult in intellectual than in economic or social history both because patterns of belief vary even more than patterns of society and because there is no valid way to prove the relative importance of different ideas. In Asia, perhaps even more than elsewhere, the borders between intellectual history and political expediency are also often cloudy, so that it may be necessary to deal on the same terms with new ideas and with political propaganda which may not (...) even be firmly believed by its author. The particularly close ties between intellectual, political, and social issues in modern Asia, however, mean that analytical studies of intellectual history can shed light on the whole transformation of Asian society. The very difficulty of finding valid comparisons should encourage attempts to do so in order to help explain the bewildering series of intellectual changes which have occurred in each Asian culture. A scholarly comparative study of intellectual history would require extraordinary linguistic ability and patience. The growing number of works on various aspects of modern Asian intellectual history and of translations of sources, however, enables scholars to attempt generalizations regarding the basis of material available in Western languages. Such generalizations can illuminate specific problems of intellectual history and can indicate basic similarities and differences in the Western impact on different Asian countries. (shrink)

In 1977 John Money published the first modern case histories of what he called ‘apotemnophilia’, literally meaning ‘amputation love’ [Money et al., The Journal of Sex Research, 13(2):115–12523, 1977], thus from its inception as a clinically authorized phenomenon, the desire for the amputation of a healthy limb or limbs was constituted as a sexual perversion conceptually related to other so-called paraphilias. This paper engages with sex-based accounts of amputation-related desires and practices, not in order to substantiate the paraphilic model, but (...) rather, because the conception of these (no doubt) heterogeneous desires and practices as symptoms of a paraphilic condition (or conditions) highlights some interesting cultural assumptions about ‘disability’ and ‘normalcy’, their seemingly inherent (un)desirability, and their relation to sexuality. In critically interrogating the socio-political conditions that structure particular desires and practices such that they are lived as improper, distressing and/or disabling, the paper constitutes an exercise in what Margrit Shildrick [Beyond the body of bioethics: Challenging the conventions. This paper examines the ways in which two methods of horse training generally considered divergent approach the concepts of partnership and conflict in human-horse relations. It focuses on finding similarities between the methods, both of which, it is argued, demonstrate the significance of communication in improving human-horse relations. Using interview material, the paper analyzes the practices and beliefs of individuals involved in natural horsemanship. In doing so the paper shows that communication between human and horse works to promote relations between (...) them that are free of conflict. This analysis is offered as a potential “solution” to welfare problems that exist both within competitive dressage practice and within individual human-horse relations. The paper also examines how the fundamental similarities between dressage and natural horsemanship could point to universalized theories of horse training focused on improving human-horse relations. Advances in genomic medicine have lead to debate about the potential inclusion of genetic tests for susceptibility to common complex disorders in newborn screening programmes. Empirical evidence concerning psychosocial reactions to genetic testing is a crucial component of both ethical debate and policy development, but while there has been much speculation concerning the possible psychosocial impact of screening newborns for genetic susceptibilities, there remains a paucity of data. The aim of the study reported here is to provide some of this (...) missing empirical evidence, using type 1 diabetes as an example of a common disorder with multiple significant genetic contributors to its aetiology. Semi-structured interviews were conducted with 11 parents of babies who had received increased risk results in a study that involved newborn screening for genetic susceptibility to type 1 diabetes. Interpretative phenomenological analysis was used to evaluate the data. The interview data suggest that the probabilistic nature of results of genetic susceptibility tests impacts upon all aspects of parents' psychosocial reactions, resulting in a complex and dynamic process quite different to that described in relation to current newborn screening programmes. While parents generally reported fairly minor levels of concern in response to news of their child's increased genetic risk, these worries frequently recurred, and perception of risk also varied and fluctuated over time. Both individual and contextual factors appeared to interact with the inherent uncertainty of the test result to contribute to the dynamic nature of parental reactions, and their behavioural responses. The implications of these findings for future research and for the debate concerning potential expansion of newborn screening are discussed. (shrink)

Previous research has shown that women are disproportionately negatively affected by a variety of socio-economic hardships, many of which COVID-19 is making worse. In particular, because of gender roles, and because women’s jobs tend to be given lower priority than men’s (since they are more likely to be part-time, lower-income, and less secure), women assume the obligations of increased caregiving needs at a much higher rate. This unfairly renders women especially susceptible to short- and long-term economic insecurity and decreases in (...) wellbeing. Single-parent households, the majority of which are headed by single mothers, face even greater risks. These vulnerabilities are further compounded along the dimensions of race, ethnicity, class, and geography. Drawing upon the philosophical literature on political responsibility and structural injustice (specifically, the work of Iris Marion Young), I argue that while the state may not have had either foresight into, or control over, the disproportionate effect the pandemic would have on women, it can nonetheless be held responsible for mitigating these effects. In order to do so, it must first recognize the ways in which women have been affected by the outbreak. Specifically, policies must take into account the unpaid labor of care that falls on women. Moreover, given that this labor is particularly vital during a global health pandemic, the state ought to immediately prioritize the value of this work by providing financial stimuli directly to families, requiring employers to provide both sick leave and parental leave for at least as long as schools and daycares are inoperational, and providing subsidized emergency childcare. This paper considers some issues for traditional and radical views of the semantic content of utterances. It suggests that, as the radical view denies that linguistic meaning solely determines explicit content, it is required to come up with an alternative account of content. We focus on cognitively oriented radical theories and argue that none of the current alternatives for delimiting content is adequate. An alternative radical account of content is sketched. We also consider Stanley's (2000) binding argument in support of (...) the traditional view and suggest that it fails as it is based on an overly strong conception of binding in natural language. Abstractabstract:In research, in policy, and in the media, there is a clear focus on alleviating child poverty. Child poverty is cast as an urgent societal problem, in part reflecting recognition of the impact of early life circumstances on health across the life course. However, focusing on child poverty can have unintended consequences. First, calls to alleviate child poverty position children as a worthy investment in future population health, while adult poverty is represented as a misallocation of scarce resources. Second, children (...) are positioned as blameless in talk about child poverty, whereas adults are viewed as potentially blameworthy for living in poverty. This ignores the ways early poverty shapes opportunities throughout the life course. These accounts are inconsistent: if a childhood of poverty shapes long-term outcomes, then poverty in adulthood cannot be an individual failing. Building arguments for poverty alleviation on social investment and personal responsibility ultimately supports rather than undermines the social structural arrangements that entrench inequalities. Recognizing the interconnectedness of families and communities and the destructiveness of poverty at every point in the life course has the potential to improve health in a way that can never be achieved by focusing solely on child poverty. On January 29, 2021, a police officer with the Rochester, New York, Police Department pepper-sprayed a 9-year old Black girl who had been handcuffed and forced into the back of a police car. In the struggle that proceeded this moment, an officer yelled at the girl with obvious frustration, “You’re acting like a child!” In this essay, I consider how the girl’s quick retort —“I AM a child!”—interjected a truth into the struggle that had been all but ignored by the (...) armed adults on the scene. I consider how the truth embedded in this girl’s call exposes the lies of law enforcement and, in doing so, lay the seeds of abolitionist imaginings—a call for a system, a world, that would treat a Black girl as if she were a child. Abstract:The “Ashley treatment” has provoked much debate and remains ethically controversial. Given that more children are being referred for such treatment, there remains a need to provide advice to clinicians and ethics committees regarding how to respond to such requests. This article contends that there is one particularly important gap in the existing literature about growth attenuation therapy (GAT) (one aspect of the Ashley treatment): the views of parents of children with profound cognitive impairment (PCI) remain significantly underrepresented. The article (...) attempts to redress this balance by analyzing published accounts both from parents of children who have received GAT and from parents who oppose treatment. Using these accounts, important points are illuminated regarding how parents characterize benefits and harms, and their responsibilities as surrogate decisionmakers. This analysis could contribute to decisionmaking about future requests for GAT and might also have wider relevance to healthcare decisionmaking for children with PCI. (shrink)

Despite recent increases in research on emotions and regulation in collaborative learning, measuring both constructs remains challenging and often lacks structure. Researchers need a systematic method to measure both the formation of emotions and subsequent regulation in collaborative learning environments. Drawing from the Formation and Regulation of Emotions in Collaborative Learning model, I introduce a new observational coding procedure that provides comprehensive guidelines for coding these phenomena. The FRECL coding procedure has been implemented successfully in other studies and is described (...) here in detail. Specifically, I detail the ideal situations for using the procedure, discuss background information and present a codebook and empirical examples for each stage of the FRECL model, and provide additional considerations that allow researchers flexibility based on their own experiences and preferences. This procedure extends past research by providing an accessible observational protocol that is both systematic and comprehensive. The FRECL coding procedure can benefit future research by providing more organized consistency to the measurement of collaborative emotions and regulation. (shrink)

By the time I graduated high school I’d hit 250 lbs.Even (...) today, after losing a considerable amount of weight from having Roux–en–Y gastric bypass surgery, I am still considered overweight according to the body mass index.All this is to say that the way I think about my health, my body and my life is very much from the perspective of someone who has never experienced the so–called condition of being “normal.”Enough is EnoughI’ve been asked many times how I decided to have bariatric surgery. The answer isn’t simple. Because I’d been overweight all my life, the notion of being anything but overweight seemed a bit absurd to me. Not absurd as in, “I would not be happy if I were smaller” but more like, “I don’t know if this could ever happen for me and, if it did, who would I be?”Truth be told, I wasn’t entirely unhappy as an overweight woman. By the account of my primary care physician I also was not necessarily unhealthy for an overweight person. I didn’t have high blood pressure or diabetes (I wasn’t even pre–diabetic). My heart seemed to work fine. I was just very, very large.However, I’d been large (or larger than others) my entire life. In many ways my very identity was inextractable from that fact. While I can’t speak for my entire culture, my experience as an African–American woman is one where women of size were not reviled, but celebrated. Being too thin was always presented to me as a negative cultural value, I believe because it was associated with drug use.That means that growing up I didn’t have some of the experiences other obese people had. There were a few people who teased me but they were the exception, not the norm. I don’t recall being lonely because of my size. I dated avidly throughout my adolescence. I was asked to, and went to, both my proms.I proceeded into young adulthood with a few bumps along the way. I suffered a bout of depression in my sophomore year of college and just after my 21st birthday I found the first signs of what would be massive hair loss from alopecia areata (although did not know that’s what it was at the [End Page 107] time). Otherwise, I was fairly healthy and no more or less happy than any other 21 year–old I knew.At age 22, I had my first child, a daughter. There has been considerable debate on how to teach ethics within the marketing curriculum to accommodate the AACSB requirements requiring emphasis on ethical issues within the business curricula. Since introducing a separate course on marketing ethics has limited reach, we propose incorporating the ethical dimension through guest speeches and reflective learning in a mandatory Marketing course for all business majors. Through phenomenographic analysis of 121 student reflections, we report evidence supporting the effectiveness of this approach in significantly raising awareness of (...) ethics in the marketing domain and in initiating the nurturing of ethical reasoning abilities in undergraduate business students early in their college careers. Students absorbed substantial amounts of knowledge at different levels and demonstrated significant cognitive processing within the revised Bloom’s taxonomy. Using this method to teach marketing ethics promotes awareness and application of ethical reasoning without compromising disciplinespecific instruction. (shrink)

By comparing two time periods, the early and late 20th century, this article examines the ambiguities and ambivalences in the state promotion of women in the nation-building projects of Mexico. I argue that in both cases, the state was keen to promote itself as modern and progressive and used women's status in society to these ends. Despite the explicit focus on women, there were many ambiguities and ambivalences resulting from the competing state projects in the political, socio-economic and cultural arenas (...) offering women both privileged spaces and constraints in the development of gendered citizenship. The contradictions arise from simultaneously promoting women's rights, extolling traditional gender roles and fearing women's political activism – both conservative and more radical. Although these ambivalences and ambiguities remain a constant feature, there is a key difference in the two time periods: in one the regime is inward looking, economically protectionist and corporatist, while in the other a new vision of Mexico has attempted to dismantle the corporatist structures and state development project with private economic initiatives and political individualism. In both periods, women gained important rights but romanticized imagery of the self-sacrificing mother was mobilized to underpin change: women were expected both to change and remain the same. Nikki Lane Bringing Flesh to Theory: Ethnography, Black Queer Theory, and Studying Black Sexualities As Dorothy Hodgson tells us, the most common features of an ethnographic project involve “talking to, participating with, and observing the people who produce... texts, exploring the contexts of their ideas and actions, and often studying how their situations, ideas, and actions (...) change over time.”1 Practically, this often involves spending months, and more often years, in a particular field site, where one develops relationships with members of the group, community, or institution being studied. Ethnographers are positioned in a place to observe, but also place their bodies on the line—participating, when possible, in the quotidian practices of the group. This observation and participation is captured in the form of “field notes” that may relay in the form of “thick description,” what the ethnographer sees as she observes and participates in various cultural and social practices.2 As such, ethnography requires reflexivity because the ethnographer must constantly consider how her body is affecting and is effected by the communities and institutions in which she is embedded. Nikki Lane 633 communities, social movements, and institutions, which frequently run along class, race, and generational lines, and that might easily be missed by more deductive and quantitative methodologies.”3 MarlonM.Bailey’s ButchQueensUpinPumps, Jafari S. Allen’s ¡Venceremos?, Mignon Moore’s Invisible Families, and Mireille Miller-Young’s A Taste for Brown Sugar represent an exciting trend within an interdisciplinary body of research that I am referring to as Black sexuality studies. What links these projects is their use of ethnographic methodologies to understand how Blackness informs racialized gender and sexuality in the everyday experiences of their interlocutors. In relying on ethnography, to varying degrees, they are in conversation with and expand upon methodological trends within Black feminist studies and Black queer studies. Further, the Black sexuality studies projects reviewed here question the (hetero)normative bent within the field of African American studies, the normatively white subject position that exists within queer theory, and the lack of attention to issues of sexual pleasure within Black feminist theory. Patrick Johnson: “What is the utility of queer [or feminist] theory on the front lines, in the trenches, on the street, or anyplace where the racialized and sexualized body is beaten, starved, fired, cursed—indeed, where the body is the site of trauma?”4 In my discussion of these texts, I will focus on how each utilizes ethnographic methodologies in distinct ways. I will argue that regardless of how they employ ethnography’s methods, they make two very important contributions to fields of Black feminist theory and Black queer theory. First, they add information about the lived experiences of Black sexual subjectivity to the ethnographic record. Reproduction and immunity are energy intensive, intimately linked processes in most organisms. In women, pregnancy is associated with widespread immunological adaptations that alter immunity to many diseases, whereas, immune dysfunction has emerged as a major cause for infertility in both men and women. Deciphering the molecular bases of this dynamic association is inherently challenging in mammals. This relationship has been traditionally studied in fast‐living, invertebrate species, often in the context of resource allocation between life history traits. More recently, these studies (...) have advanced our understanding of the mechanistic underpinnings of the immunity‐fertility dialogue. Here, we review the molecular connections between reproduction and immunity from the perspective of human pregnancy to mechanistic discoveries in laboratory organisms. We focus particularly on recent invertebrate studies identifying conserved signaling pathways and transcription factors that regulate resource allocation and shape the balance between reproductive status and immune health. Treating physicians have key roles to play in expanded access to investigational drugs, by identifying investigational treatment options, assessing the balance of risks and potential benefits, informing their patients, and applying to the regulatory authorities. This study is the first to explore physicians’ experiences and moral views, with the aim of understanding the conditions under which doctors decide to pursue expanded access for their patients and the obstacles and facilitators they encounter in the Netherlands. In this mixed-methods study, semi-structured interviews (...) and a questionnaire were conducted with medical specialists across the country and analysed thematically. Typically, our respondents pursue expanded access in “back against the wall” situations and broadly support its classic requirements. They indicate practical hurdles related to reimbursement, the amount of time and effort required for the application, and unfamiliarity with the regulatory process. Some physicians are morally opposed to expanded access, with an appeal to safety risks, lack of evidence, and “false hope.” Some of these moral concerns and practical obstacles may be essential targets for change, if expanded access to unapproved drugs is to become available for wider groups of patients for whom standard treatment options are not—or no longer—available, on a more consistent and equal basis.
