Background Parental refusal of routine childhood vaccination remains an ethically contested area. This systematic review sought to explore and characterise the normative arguments made about parental refusal of routine vaccination, with the aim of providing researchers, practitioners, and policymakers with a synthesis of current normative literature. Methods Nine databases covering health and ethics research were searched, and 121 publications identified for the period Jan 1998 to Mar 2022. For articles, source journals were categorised according to Australian Standard Field of Research (...) codes, and normative content was analysed using a framework analytical approach. Results Most of the articles were published in biomedical journals (34%), bioethics journals (21%), and journals that carry both classifications (20%). Two central questions dominated the literature: (1) Whether vaccine refusal is justifiable (which we labelled ‘refusal arguments’); and (2) Whether strategies for dealing with those who reject vaccines are justifiable (‘response arguments’). Refusal arguments relied on principlism, religious frameworks, the rights and obligations of parents, the rights of children, the medico-legal best interests of the child standard, and the potential to cause harm to others. Response arguments were broadly divided into arguments about policy, arguments about how individual physicians should practice regarding vaccine rejectors, and both legal precedents and ethical arguments for vaccinating children against a parent’s will. Policy arguments considered the normative significance of coercion, non-medical or conscientious objections, and possible reciprocal social efforts to offset vaccine refusal. Individual physician practice arguments covered nudging and coercive practices, patient dismissal, and the ethical and professional obligations of physicians. Most of the legal precedents discussed were from the American setting, with some from the United Kingdom. Conclusions This review provides a comprehensive picture of the scope and substance of normative arguments about vaccine refusal and responses to vaccine refusal. It can serve as a platform for future research to extend the current normative literature, better understand the role of cultural context in normative judgements about vaccination, and more comprehensively translate the nuance of ethical arguments into practice and policy. The idea of a role in nursing that includes expertise in ethics has been around for more than 30 years. Whether or not one subscribes to the idea that nursing ethics is separate and distinct from bioethics, nursing practice has much to contribute to the ethical practice of healthcare, and with the strong grounding in ethics and aspiration for social justice considerations in nursing, there is no wonder that the specific role of the nurse ethicist has emerged. Nurse ethicists, expert (...) in nursing practice and the application of ethical theories and concepts, are well positioned to guide nurses through complex ethical challenges. However, there is limited discussion within the field regarding the specific job responsibilities that the nurse ethicist ought to have. The recent appearance of job postings with the title “nurse ethicist” suggest that some healthcare institutions have identified the value of a nurse in the practice of ethics and are actively recruiting. Discomfort about the possibility of others defining the role of the nurse ethicist inspired this paper (and special issue). If the nurse ethicist is to be seen as an integral part of addressing ethical dilemmas and ethical conflicts that arise in healthcare, then nurse ethicists ought to be at the forefront of defining this role. In this paper, we draw upon our own experiences as nurse ethicists in large academic healthcare systems to describe the essential elements that ought to be addressed in a job description for a nurse ethicist practicing in a clinical setting linked to academic programs. Drawing upon our experience and the literature, we describe how we perceive the nurse ethicist adds value to healthcare organizations and teams of professional ethicists. (shrink)

Las nuevas prácticas políticas que se insertan en el campo de la participación política son ahora mucho más directas con respecto a los sectores marginados, y la forma en la que éstos se han hecho presentes a través de las protestas y cualquier tipo de desobediencia civil en el espacio público, logrando enfrentar al Estado con los reclamos de quienes siendo la mayoría no han dejado de sufrir la explotación y alineación económica y política. This article defends epiphenomenalism against criticisms raised in Dwayne Moore’s “On Robinson’s Response to the Self-Stultifying Objection”.

Forest tenure reforms are occurring in many developing countries around the world. These reforms typically include devolution of forest lands to local people and communities, which has attracted a great deal of attention and interest. While the nature and level of devolution vary by country, all have potentially important implications for resource allocation, local ecosystem services, livelihoods and climate change. This book helps students, researchers and professionals to understand the importance and implications of these reforms for local environmental quality, climate (...) change, and the livelihoods of villagers, who are often poor. It is shown that local forest management can often be more successful than top-down management of common pool forest resources. The relationship of local forest tenure reform to the important climate change initiative REDD+ is also considered. The work includes a number of generic chapters and also detailed case studies from China, Ethiopia, Kenya, Nepal, Tanzania and Uganda. Using specific examples and a wide variety of disciplinary perspectives, including quantitative and qualitative analytical methods, the book provides an authoritative and critical picture of local forest reforms in light of the key challenges humanity faces today. BackgroundSuicide research aims to contribute to a better understanding of suicidal behaviour and its prevention. However, there are many ethical challenges in this research field, for example, regarding consent and potential risks to participants. While studies to-date have focused on the perspective of the researchers, this study aimed to investigate the views and experiences of members of Human Research Ethics Committees (HRECs) in dealing with suicide-related study applications.MethodsThis qualitative study entailed a thematic analysis using an inductive approach. We conducted semi-structured (...) interviews with a purposive sample (N = 15) of HREC Chairs or their delegates from Australian research-intensive universities. The interview guide included questions regarding the ethical concerns and challenges in suicide-related research raised by HREC members, how they dealt with those challenges and what advice they could give to researchers.ResultsThe analysis identified four main themes: (1) HREC members’ experiences of reviewing suicide-related study applications, (2) HREC members’ perceptions of suicide, suicide research, and study participants, (3) Complexity in HREC members’ decision-making processes, and (4) HREC members’ relationships with researchers.ConclusionsReliance on ethical guidelines and dialogue with researchers are crucial in the assessment of suicide-related study applications. Both researchers and HREC members may benefit from guidance and resources on how to conduct ethically sound suicide-related studies. Developing working relationships will be likely to help HRECs to facilitate high quality, ethical suicide-related research and researchers to conduct such research. (shrink)

There is a fundamental divergence of opinion between the EU and the US over how food products derived from genetically modified organisms should be labeled. This has less to do with safety, as moves towards the international harmonization of safety standards continue apace, and rather more to do with the consumers' right to know about the origins of the food they are consuming. Cet article s'attache à montrer le processus de création d'un espace public de proximité. À travers une expérience , nous présentons les grandes phases constitutives de ce processus. En effet, la création d'un espace public de proximité suppose une première phase de concertation à travers la mise en présence des différents acteurs et de différentes logiques d'action. Cet espace de concertation doit ensuite se transformer en un espace d'intermédiation. En passant d'un espace public de concertation à un espace public d'intermédiation, (...) on passerait d'une phase de confrontation d'idées à une phase d'articulation de ces idées. L'espace public de proximité correspondrait donc à cet espace d'intermédiation dont le fonctionnement serait basé sur les principes de démocratie et de participation.This article attempts to show the process of creation of a public sphere of proximity. Through an experiment , we present the main phases that made up this process. Indeed, the creation of a community-based development programme requires the first phase of dialogue between those involved concerning the possible spheres of action to be adopted. This dialogue phase must then be transformed into a programme of intermediation which implies moving from discussion of opposing ideas to the articulation of those ideas. The community-based development programme would thus correspond to this mediation process which would be based on the principles of democracy and participation. (shrink)

Le fragment 129 d'Empédocle fait état du savoir prodigieux et du pouvoir des prapides d'un Super-Sage du passé en qui les sources citatrices et les interprètes modernes reconnaissent trop facilement Pythagore de Samos. Le but de la présente étude est de reprendre à nouveaux frais l'examen de ces six vers afin d'ouvrir le débat autour de la sagesse et des pouvoirs attribués à la figure anonyme du Super-Sage. Interprétant « Empédocle à partir d'Empédocle », mais aussi à l'aide des références (...) culturelles majeures de son temps, on essaie d'examiner quel type de sagesse était attribué à cette figure par le poète philosophe, de quelles conditions, physiologiques ou autres, relève l'acquisition de cette dernière et en quoi consiste son caractère exceptionnel. Empedocle's fragment 129 praises the exceptional knowledge and the power of the prapides of a Super-Sage of the past. In this sage both ancient testimonies and modern interpreters all too easily recognize Pythagoras of Samos. The aim of the present paper is to re-examine these six verses in order to open up the discussion on the wisdom and powers attributed to this unnamed figure. Interpreting « Empedocles by Empedocles », and also taking into account the major cultural references of his time, we try to define what kind of wisdom was attributed to this distinguished sage, under which conditions, physiological or otherwise, he was able to acquire it, and what constitutes the exceptional character of his knowledge. This article describes a project which involved an attempt to integrate an expert system with a hypertext database of primary and secondary text materials. Our chosen legal domain was that of the Convention on Jurisdiction and the Enforcement of Judgments in Civil and Commercial Matters (The Brussels Convention 1968). In this article, we address three dimensions of system design. With regard to the legal dimension, we consider the choice of domain and the representation of both knowledge and data in the (...) system. On the technological dimension, we discuss the selection of software development tools and problems associated with keeping knowledge bases and databases up-to-date. Finally, we pay particular attention to the Cinderella dimension of legal expert system development — the user interface. (shrink)

In recent years innovative approaches have emerged across the United States and Canada to improve access to healthful foods. Mobile markets—traveling food retailers that specifically target food deserts—are one such strategy. Given the recent emergence of mobile markets, and their positioning as a solution to disparities in food access, research is needed to understand potentials and limitations of the model. In this article, we report on findings from a survey of mobile market operators in the United States and Canada. Results (...) identify tensions between the intended goals of mobile markets and constraints of the model itself. Further study, including applied research, is needed to better understand potential opportunities to strengthen mobile markets as an intervention strategy. (shrink)
