This paper aims to critique the phenomenon of advanced patient autonomy and choice in healthcare within the specific context of self-testing devices. A growing number of self-testing medical devices are currently available for home use. The premise underpinning many of these devices is that they assist individuals to be more autonomous in the assessment and management of their health. Increased patient autonomy is assumed to be a good thing. We take issue with this assumption and argue that self-testing provides a (...) specific example how increased patient autonomy and choice within healthcare might not best serve the patient population. We propose that current interpretations of autonomy in healthcare are based on negative accounts of liberty to the detriment of a more relational understanding. We also propose that Kantian philosophy is often applied to the healthcare arena in an inappropriate manner. We draw on the philosophical literature and examples from the self-testing process to support these claims. We conclude by offering an alternative account of autonomy based on the interrelated concepts of relationality, care and responsibility. (shrink)

U.S. researchers and scholars often point to two legal factors as significant obstacles to the inclusion of pregnant women in clinical research: the Department of Health and Human Services’ regulatory limitations specific to pregnant women's research participation and the fear of liability for potential harm to children born following a pregnant woman's research participation. This article offers a more nuanced view of the potential legal complexities that can impede research with pregnant women than has previously been reflected in the literature. (...) It reveals new insights into the role of legal professionals throughout the research pathway, from product conception to market, and it highlights a variety of legal factors influencing decision-making that may slow or halt research involving pregnant women. Our conclusion is that closing the evidence gap created by the underrepresentation and exclusion of pregnant women in research will require targeted attention to the role of legal professionals and the legal factors that influence their decisions. This book presents a survey of approaches to dealing with 'rival histories' in the classroom, arguing that approaching this problem requires great sensitivity to differing national, educational and narrative contexts. Contested narratives and disputed histories have long been an important issue in history-teaching all over the world, and have even been described as the 'history' or 'culture' wars. In this book, authors from across the globe ponder the question "what can teachers do (and what are they doing) to address conflicting (...) narratives of the same past?", and puts an epistemological issue at the heart of the discussion: what does it mean for the epistemology of history, if it is possible to teach more than one narrative? Divided into three sections that deal with historical cultures, multicultural societies and multiperspectivity, the chapters of the book showcase that dealing with rival histories is very much dependent on context, and that diverse teaching traditions and societal debates mean that teachers' abilities in engaging with the teaching of rival narratives are very different. Adults who stutter (AWS) have demonstrated atypical coordination of motor and sensory regions during speech production. Yet little is known of the speech-motor network in AWS in the brief time window preceding audible speech onset. The purpose of the current study was to characterize neural oscillations in the speech-motor network during preparation for and execution of overt speech production in AWS using magnetoencephalography (MEG). Twelve AWS and twelve age-matched controls were presented with 220 words, each word embedded in a carrier (...) phrase. Controls were presented with the same word list as their matched AWS participant. Neural oscillatory activity was localized using minimum-variance beamforming during two time periods of interest: speech preparation (prior to speech onset) and speech execution (following speech onset). Compared to controls, AWS showed stronger beta (15-25Hz) suppression in the speech preparation stage, followed by stronger beta synchronization in the bilateral mouth motor cortex. AWS also recruited the right mouth motor cortex significantly earlier in the speech preparation stage compared to controls. Exaggerated motor preparation is discussed in the context of reduced coordination in the speech-motor network of AWS. It is further proposed that exaggerated beta synchronization may reflect a more strongly inhibited motor system that requires a stronger beta suppression to disengage prior to speech initiation. These novel findings highlight critical differences in the speech-motor network of AWS that occur prior to speech onset and emphasize the need to investigate further the speech-motor assembly in the stuttering population. BackgroundThis paper represents a portion of the findings from one of the first research studies eliciting the lived experience of adults with an early diagnosis of Phenylketonuria living in Ireland. Ireland has one of the highest prevalence rates of PKU in Europe, however, little is known about the experience of Irish adults with PKU. Furthermore, Ireland is one of the first countries in the world to introduce neonatal screening followed by the introduction of long-term dietary therapy over 50 years ago. (...) This study presents the first comprehensive assessment of the lived experience of Irish adults with PKU on long term dietary therapy.MethodsNarrative data was collected from eleven self-selected participants, using semi-structured interviews. The interviews were divided into five sections focused on eliciting a holistic understanding of the lived experience of adults with PKU living in Ireland. Thematic analysis was guided by Colaizzi's Framework in conjunction with NVivo qualitative data analysis software.FindingsFindings from the original research encompassed a broad understanding of the lived experience of adults with PKU living in Ireland, including factors influencing dietary therapy and managing PHE blood levels. The themes being discussed within this article are those which appear to be least represented within current literature: living with PKU, including reproductive health, the importance of self-management and establishing routine, support networks in adulthood and concerns regarding aging with PKU.ConclusionIt was evident from the findings that a diagnosis of PKU can influence how adults with PKU may experience aging and their own mortality. These findings offer new insight into the vulnerability attached to the experience of aging with PKU and may be beneficial to advocacy groups and for future development of policy and practice. Men often make riskier decisions than women across a wide range of real-life behaviors. Whether this sex difference is accentuated, diminished, or stable under stressful conditions is, however, contested in the scientific literature. A critical blind spot lies amid this contestation: Most studies use standardized, laboratory-based, cognitive measures of decision making rather than complex real-life social simulation tasks to assess risk-related behavior. To address this blind spot, we investigated the effects of acute psychosocial stress on risk decision making in men (...) and women using a standardized cognitive measure and a novel task that simulated a real-life social situation. Participants were exposed to either an acute psychosocial stressor or an equivalent control condition. Stressor-exposed participants were further characterized as high- or low-cortisol responders. Results confirmed that the experimental manipulation was effective. On the IGT, participants characterized as low-cortisol responders made significantly riskier decisions than those characterized as high-cortisol responders. Similarly, in the online chatroom, participants characterized as low-cortisol responders were, relative to those in the Non-Stress group, significantly more likely to make risky decisions. Together, these results suggest that at lower levels of cortisol both men and women tend to make riskier decisions in both economic and social spheres. (shrink)

ObjectivePatient involvement is used to describe the inclusion of patients as active participants in healthcare decision-making and research. This study aimed to investigate incoming year 1 medical (MBChB) students’ attitudes and opinions regarding patient involvement in this context.MethodsWe established a staff–student partnership to formulate the design of an online research survey, which included Likert scale questions and three short vignette scenarios designed to probe student attitudes towards patient involvement linked to existing legal precedent. Incoming year 1 medical students (n=333) were (...) invited to participate in the survey before formal teaching commenced.ResultsSurvey data (49 participants) indicate that students were broadly familiar with, and supportive of, patient involvement in medical treatment. There was least support for patient involvement in conducting (23.9%), contributing to (37.0%) or communicating research (32.6%), whereas there was unanimous support for patients choosing treatment from a selection of options (100%).ConclusionIncoming members of the medical profession demonstrate awareness of the need to actively involve patients in healthcare decision-making but are unfamiliar with the utility and value of such involvement in research. Further empirical studies are required to examine attitudes to patient involvement in healthcare. (shrink)

Background Research shows that undocumented migrants have difficulties in accessing healthcare. Act 2013:407 came into force in 2013 and entitled undocumented migrants to healthcare that cannot be deferred. To date, studies about undocumented migrants’ access to care in Sweden and the impact of Act 2013:407 are sparse. Hence, the aim of this study was to describe professionals’ experiences of access to healthcare for undocumented migrants in Sweden and the impact of Act 2013:407. Methods A qualitative design with semi-structured interviews was (...) employed. Nine interviews were carried out in 2015 with nurses at two NGO healthcare centres for undocumented migrants – and an additional seven interviews in 2022 with staff at an NGO healthcare centre for undocumented migrants and personnel at a regional health and medical care administration. Interpretive description was used for the analyses. Ethical considerations Permission to carry out the study was obtained from managers at the participating NGOs and the regional health and medical care administration. Participants received verbal and written information about the study, and informed consent was obtained from all participants. Findings Six categories emerged from the analysis: Changes since the Act was introduced, General problems with healthcare access, Care for undocumented migrants – politics and social economy, Lack of knowledge, ‘Healthcare that cannot be deferred’ and Being an undocumented migrant. Conclusion Undocumented migrants’ social needs are as great as their needs for healthcare. Healthcare staff are burdened with healthcare cost considerations which affect their judgement of care provision and prioritization. Healthcare staff attitudes towards undocumented migrants affect their access to healthcare. Undocumented migrants in need of healthcare are especially vulnerable due to their legal status, being ill and the fear of being reported and deported. To assure undocumented migrants’ access to healthcare and maintain healthcare ethics, the only possible solution is to provide healthcare based on needs. (shrink)

How can law embrace -- in other words --the 'posthuman' condition -- a condition in which non-human forces such as climate change and Covid-19 signal the impossibility of clinging to the existing imaginaries of Western legal systems and international law? This article illustrates how a collaborative research process can successfully engage an underserved minority community to address health disparities. Pacific Islanders, including the Marshallese, are one of the fastest growing US populations. They face significant health disparities, including extremely high rates of type 2 diabetes. This article describes the engagement process of designing patient‐centered outcomes research with Marshallese stakeholders, highlighting the specific influences of their input on a randomized control trial to address diabetes. Over 18 months, an interdisciplinary research team (...) used community‐based participatory principles to conduct patient‐engaged outcomes research that involved 31 stakeholders in all aspects of research design, from defining the research question to making decisions about budgets and staffing. This required academic researcher flexibility, but yielded a design linking scientific methodology with community wisdom. (shrink)

Previous research indicates that individuals with depressive disorders have aberrant resting state functional connectivity and may experience memory dysfunction. While resting state functional connectivity may be affected by experiences preceding the resting state scan, little is known about this relationship in individuals with DD. Our study examined this question in the context of object memory. 52 individuals with DD and 45 healthy controls completed clinical interviews, and a memory encoding task followed by a forced-choice recognition test. A 5-min resting state (...) fMRI scan was administered immediately after the forced-choice task. Resting state networks were identified using group Independent Component Analysis across all participants. A network modeling analysis conducted on 22 networks using FSLNets examined the interaction effect of diagnostic status and memory accuracy on the between-network connectivity. We found that this interaction significantly affected the relationship between the network comprised of the medial prefrontal cortex, posterior cingulate cortex, and hippocampal formation and the network comprised of the inferior temporal, parietal, and prefrontal cortices. A stronger positive correlation between these two networks was observed in individuals with DD who showed higher memory accuracy, while a stronger negative correlation was observed in individuals with DD who showed lower memory accuracy prior to resting state. No such effect was observed for HC. The former network cross-correlated with the default mode network, and the latter cross-correlated with the dorsal attention network. Considering that the DMN and DAN typically anticorrelate, we hypothesize that our findings indicate aberrant reactivation and consolidation processes that occur after the task is completed. Such aberrant processes may lead to continuous “replay” of previously learned, but currently irrelevant, information and underlie rumination in depression. (shrink)

Background Genome scientists and Ethical, Legal, and Social Implications of genetics (ELSI) scholars commonly inhabit distinct research cultures – utilizing different research methods, asking different research questions, and valuing different types of knowledge. Collaborations between these two communities are frequently called for to enhance the ethical conduct of genomics research. Yet, little has been done to qualitatively compare genome scientists’ and ELSI scholars’ perspectives on collaborations with each other and the factors that may affect these collaborations.Methods 20 semi-structured interviews with (...) US-based genome scientists and ELSI scholars were conducted between June–September 2021. Interviews were analyzed using inductive thematic analysis.Results Genome scientists and ELSI scholars provided different understandings of the value and goals of their collaborations with each other. Genome scientists largely perceived ELSI expertise to be relevant for human subjects research; they described ELSI scholars as communicators who help the public and/or study participants better understand genomics research. In comparison, ELSI scholars viewed themselves as developing and implementing policies; they expressed frustration at how scientists can misunderstand their research methods or negatively perceive them. A combination of factors – both structural (e.g., criteria for promotion) and cultural (e.g., perceptions of what colleagues value and respect) – seemed to shape these diverging perspectives.Conclusion Academic institutions, funders, and researchers commonly call for collaborations between genome scientists and ELSI scholars, but under-consider how their different conceptual frameworks, research methods, goals, norms, and values, conjoin to affect such partnerships. Acknowledging, exploring, and addressing the complex interplay between these factors could help to more effectively facilitate collaborations between genome scientists and ELSI scholars. (shrink)

Grandparental presence is known to correlate with the number of grandchildren born, and this effect may vary according to grandparental sex and lineage. However, existing studies of grandparental effects on fertility mostly concern traditional subsistence societies, while evidence from contemporary developed societies is both scarce and mixed. Here, we explore how grandparents affect the transition to second and subsequent children in the contemporary United Kingdom. The longitudinal Millennium Cohort Study (n = 10,295 families) was used to study the association between (...) grandparental investment and parents’ probability of having a new child within 4.5 years. Results show that contact with paternal grandparents is associated with higher probability of parents having a second child. In contrast, contact with maternal grandparents is associated with lower probability of having a third or subsequent child. Kin may have opposite effects on fertility even in contemporary societies, which may explain the lack of consistent effects of grandparental investment on fertility in previous studies. (shrink)

This paper attempts to provide a rough conceptual map of the terrain of public health ethics. We begin by briefly defining public health and identifying general features of the field that are particularly relevant for a discussion of public health ethics.Public health is primarily concerned with (...) the health of the entire population, rather than the health of individuals. We propose that those skills could be based on simpler detection systems humans could share with other animal tool users. More specifically, we discuss the impact of object affordances on the understanding and the social learning of tool use.

The article presents four erotic poems by Franciszek Dionizy Kniaźnin: Spocznienie, Z Anakreonta, Ognie młodości and Pasterka, together with an editorial compilation. All poems come from the first book of Erotics published in 1779 and the goal underlying their selection was to present the variety of motifs intertwining in this anthology. The book has its direct addressee, namely the mythological Roman goddess of love, and a considerable part of Kniaznin’s erotics are dedicated to this emotion in which Cupid, Venus and (...) Nymphs get people entangled. The first book also includes poems translated from Anacreon, Horace, Claudian, Durini or Sappho. These are paraphrases whose prevailing theme is Bacchic. Several poems reflect on the irreversibly passing human existence that cannot be prevented or stopped, or the ideal realm of shepherds governed by the natural impulse of the heart. The poems discussed herein show richness of forms and motifs introduced on the pages of Erotics by Franciszek Dionizy Kniaźnin, scantily discussed before. (shrink)

What effect does witnessing other students cheat have on one's own cheating behavior? What roles do moral attitudes and neutralizing attitudes (justifications for behavior) play when deciding to cheat? The present research proposes a model of academic dishonesty which takes into account each of these variables. Findings from experimental (vignette) and survey methods determined that seeing others cheat increases cheating behavior by causing students to judge the behavior less morally reprehensible, not by making rationalization easier. Witnessing cheating also has unique (...) effects, controlling for other variables. (shrink)

The paper is aiming to present and analyse the notions used by Adam Smith: ‘instinct’ and ‘ appetites’. They appear in two of the Scottish philosopher’s works: in ‘The Theory of Moral Sentiments’ and in an essay ‘ Of the External Senses’. Smith noticed that there are certain inborn mechanisms that suggest the existence of crucial needs and means leading to satisfy those needs. That concerns, mostly, hunger and thirst, but also sex drive. He regarded the need of self-preservation and (...) of caring for one’s own well-being as crucial for a man’s behaviour. The essay ‘ Of the External Senses’ mentions instinctive correlation of the objects of sight with the objects of touch as crucial for the human survival, basing on George Berkeley’s theory of vision. Smith’s remarks on appetites give a new perspective on how the mechanisms given us by Nature influence moral and social individuals, at the same time completing the philosopher’s theory. The need to care for self ’s prosperity and survival can be regarded not only as an instinct, but also as actions leading to realisation of the virtue of prudence. Moreover, we learn how to fulfil our instinctive needs and appetites within the society, searching for acceptable means. In the paper Smith’s view of instincts and appetites is also being briefly confronted with David Hume’s and Thomas Hobbes’ remarks on the subject. (shrink)

Students of the relations between Greeks and Persians in classical antiquity usually depend entirely on Greek authors, as there are no extensive narrative texts among the cuneiform inscriptions of the Achaemenid kings, our extant Oriental sources for the history of the Persian empire. Hence modern scholars have raised the question of the reliability of the Greek sources and emphasized the need to reveal the ideology and presuppositions of the Greek writers. For, if language embodies social reality, the assimilation of information (...) is conditioned by the character of the mind, individual or collective, which comprehends the data within its own terms of reference, fits them into its own set of concepts and records them in that form. Thus, it has been argued that ‘the impression we get of the Persians in the Greek authors is in some ways a deceptive one. Too much emphasis is laid on what is pejorative’, that the labelling of Persia ‘as an Oriental Monarchy: a state and society ruled less by rational actions than by the writing and caprices of its king and court… is to be traced back directly to the Greek sources on Persian history’; and that the ‘discourse of barbarism’ which projected upon the Persians ‘the opposite of qualities admired’ in the Athenian society, ‘is ultimately to be referred to the ideology binding together democratic Athens and her empire’. The mathematical Platonism is the most popular standpoint amongst mathematicians. The Platonism better than other conceptions explains the fact, that mathematicians discover some properties of the mathematical objects, which are independent of them and exceed the boundary of our intuition and experience. On the other hand Platonism generates some difficulties. In spite of this the Platonism is the idea worth the defence. In the paper I modify the traditional formulation of Platonism, which avoids some of the difficulties.

Predictiveness (or testability) can be ascribed to empirical hypotheses in different degrees. Seven reasons why predictiveness is gradable are outlined. They are reflected in seven ways in which hypotheses can be compared according to the degree of their predictiveness. Hence additional specifications are required to accept predictiveness as a criterion of whether given hypotheses are scientific. Generally speaking a new hypothesis has little, if any, predictive power. Its predictive power will increase as science progresses. The paper is devoted to the history of considerations on the denotation of propositions, or on that which is denoted, or described by propositions. n the first part of the paper, aside to the views of ancient authors (Plato, Aristotle, Stoics) concerning this issue, the debate about the existence and nature of the denotation of propositions has been presented. It was alive especially in the later scholastic philosophy (two objectivistic standpoints – among others – Gregory of Rimini, and one subjectivistic). (...) The second part of the paper discusses the position of modern and contemporary authors, i.e. two ways with which the question of the denotation of propositions was reconstructed in the 19th century, the so-called Brentano’s schools (Twardowski, Meinong), Frege’s thought and his followers. The texts of many authors have been analysed in order to present (and compare) all the more important views in the question of the denotation of propositions, starting from antiquity until the contemporary times. OF SENSES AND PERCEPTION—WHAT SMITH TELLS US ABOUT BERKELEY AND VICE VERSA Adam Smith’s epistemology, described primarily in the essay Of the External Senses, was strongly inspired by George Berkeley’s thought expressed in his New Theory of Vision (1709). Both philosophers distinguished between the Objects of Sight and the Objects of Touch and analyzed the perception of distance between objects and size of objects (Berkeley’s thorough analysis was limited to the sense of sight, whereas Smith described also the features of (...) other senses.). They also noticed the observer’s role in the process. Both philosophers made a parallel between their epistemology and moral/ social theories. However, their conclusions turned out to be quite different, despite some similarities in their views on the problem of external senses. Smith focused on the issue of sympathy and the Impartial Spectator, and underlined the fact that our moral judgements are strongly altered by our relation to those who are judged and that we learn how to give opinions on the basis of how others judge our behaviour. Buscar o significado da “racionalidade” pressupõe perscrutá-la “em ação” – não há como “sair fora” do racional para encontrá-la. O valor de face que apresenta é o lado “direito” de sua tessitura. Mas a condição (auto)reguladora da racionalidade demanda que se lhe busque também o “avesso”, lado nem sempre visível das costuras que dão a feitura do “direito”. Em (...) face da contextualidade da racionalidade “em ação”, cabe examinar essa distinção através de dois “casos exemplares”: a visão aristotélica de racionalidade e a argumentação de Charles Darwin na Origem das espécies. De um lado, encontramos uma outra vertente da Retórica aristotélica, mostrando a interpenetração de demonstrar e persuadir no “avesso” da racionalidade; de outro, encontramos no “avesso” da argumentação científica darwiniana a essencialidade de procedimentos ditos “retóricos”. In a flexible way, the religious services of the UCKG are always in line with the demands of certain social groups, more precisely to those who are more sensitive to the use of the popular imagination outline as a complementary substance in the plausibility of everyday life. This article is a polemic with the article by Marian Przełęcki, O pewnych filozoficznych konsekwencjach semantycznej definicji prawdy published by “Studia Filozoficzne” 6, 1973. Przełęcki claimed that certain evaluative propositions have a logical value by virtue of containing evaluative predicates which are commonly understood. The functioning of these predicates in natural languages makes it possible to identify their denotation in these languages, and thus it is possible to assign logical values to the propositions that predicate about different elements of the (...) world their values in all cases when these elements do not fall into the range of vagueness of the predicates used. The authors of the polemic points out that the propositions analysed by Przełęcki are in fact descriptive and not normative because their logical value is determined by a common understanding of the evaluative predicates which they contain. These propositions do not say what is morally good, but, what is commonly believed to be morally good. Consequently, the problem of logical value of evaluative propositions has not been settled in Przełęcki’s article, while the ascription of logical values to propositions belonging to descriptive ethics, which is the real subject matter of the article, hardly poses any questions. The authors of this polemic contend that it is not possible to settle the problem of logical value of evaluative propositions without an analysis of the meaning of evaluative predicates. (shrink)

O manuscrito clandestino Jordanus Brunus redivivus ou Traité des erreurs populaires foi publicado em 1771, não trazendo indicação alguma de sua autoria. Entretanto, a despeito do anonimato da obra, ela tornou-se um dos textos mais famosos dentre a filosofia clandestina do século XVIII, aludindo a um filósofo que foi uma das maiores vítimas da superstição e do fanatismo de sua época: Giordano Bruno. O título do escrito é uma espécie de homenagem ao pensador italiano, mas não há no decorrer da (...) obra uma menção sequer que comprove que o autor anônimo tenha lido uma linha de suas obras, mesmo que seja considerado o elo filosófico de união entre o brunismo da primeira metade do século XVII e o da segunda metade do século XVIII. É em um encadeamento de ideias progressivo no Jordanus Brunus que nos permite encontrar o fio condutor das reflexões do autor: a sua formulação do ateísmo caracteriza-se por sua abordagem não dogmática, dissimulada, preparando o solo para o racionalismo ateu que estaria por vir. O presente artigo limitar-se-á à terceira e quarta partes do manuscrito, nas quais o autor anônimo vai tratar diretamente da questão da existência de deus e fazer diversas objeções às provas de sua existência. This article is an investigation of neurodivergent reading practices. It is a collectively written paper where the focus is as much on an autoethnographic exploration of our autistic readings of autism/autistic fiction as it is on the read texts themselves. The reading experiences described come primarily from Yoon Ha Lee’s _Dragon Pearl_ (2019) and Dahlia Donovan’s _The Grasmere Cottage Mystery_ (2018), which we experience as opposite each other in how they depict their neurodivergent characters and speak to us as autistic (...) readers. Through the article, we describe a formation of neurodivergent (critical) collective readings of autism/autistic fiction. The article contributes to an academic and activistic discourse around neurodivergent reader responses and power relations between neurodivergent and neurotypical readers and authors. (shrink)

The aim of three studies was to examine the differences between business majors and non-business majors, in their level of implicit and explicit power motivation.It was predicted that there are no differences between these two groups in the general level of power motivation, but that differences exist in the way it is explicitly expressed: through desire for leadership and prominence vs. desire for helping. Results of Study 1 indicated that business majors declared a higher leadership motive and a lower helping (...) motive than non-business majors.Study 2 addressed question whether the above differences in power motivation stem from socialization at the university level or from pre-selection. The relationship between high school students’ academic major preferences and their power motivation was tested. It was found that the more they were business-oriented, the higher their scores were on leadership, and lower on helping scales. In Study 3, business majors and non-business majors completed the same questionnaire as participants in previous studies, as well as performed the IAT. Non-business majors declared stronger explicit helping motive, while business majors expressed stronger prominence and leadership motives. Furthermore, for non-business majors, IAT results could be predicted by their helping score. Implications and possible limitations of the presented results are discussed. (shrink)
