Surgical expertise does not have a clear definition and is often culturally associated with power, authority, prestige, and case number rather than more objective proxies of excellence. Multiple models of expertise progression have been proposed including the Dreyfus model, however, they all currently require subjective evaluation of skill. Recently, efforts have been made to improve the ways in which surgical excellence is measured and expertise is defined using artificial intelligence, video recordings, and accelerometers. However, these aforementioned methods of assessment are (...) still subjective or indirect proxies of expertise, thus uncovering the neural mechanisms that differentiate expert surgeons from trainees may enhance the objectivity of surgical expertise validation. In fact, some researchers have already suggested that their neural imaging-based expertise classification methods outperform currently used methods of surgical skill certification such as the Fundamentals of Laparoscopic Surgery scores. Such imaging biomarkers would not only help better identify the highest performing surgeons, but could also improve residency programs by providing more objective, evidence-based feedback and developmental milestones for those in training and perhaps act as a marker of surgical potential in medical students. Despite the potential advantages of using neural imaging in the assessment of surgical expertise, this field of research remains in its infancy. This systematic review identifies studies that have applied neuromonitoring in assessing surgical skill across levels of expertise. The goals of this review are to identify the strongest neural indicators of surgical expertise, the limitations of the current literature on this subject, the most sensible future directions for further study. We found substantial evidence that surgical expertise can be delineated by differential activation and connectivity in the prefrontal cortex across multiple task and neuroimaging modalities. Specifically, novices tend to have greater PFC activation than experts under standard conditions in bimanual and decision-making tasks. However, under high temporal demand tasks, experts had increased PFC activation whereas novices had decreased PFC activation. Common limitations uncovered in this review were that task difficulty was often insufficient to delineate between residents and attending. Moreover, attending level involvement was also low in multiple studies which may also have contributed to this issue. Most studies did not analyze the ability of their neuromonitoring findings to accurately classify subjects by level of expertise. Finally, the predominance of fNIRS as the neuromonitoring modality limits our ability to uncover the neural correlates of surgical expertise in non-cortical brain regions. Future studies should first strive to address these limitations. In the longer term, longitudinal within-subjects design over the course of a residency or even a career will also advance the field. Although logistically arduous, such studies would likely be most beneficial in demonstrating effects of increasing surgical expertise on regional brain activation and inter-region connectivity. Abstract:In 1965 the “beautification” movement, spearheaded by “Lady Bird” Johnson, ushered in a new phase for American utility companies, under increasing pressure from environmentalists, regulatory bodies and the public, who protested against the continued expansion of energy facilities. In response, utilities such as Southern California Edison, incorporated aesthetics into their corporate strategies to manage an increasingly strained relationship with their consumer base. This ranged from painting infrastructure to launching new design models for transmission lines and converting overhead lines underground. Far (...) from universally welcomed, Southern California Edison's beautification programmes were often met with resistance in communities, due to economic costs being passed on to the consumer. This led to an extensive debate about who benefited from the aesthetics of the energy landscape: was it the utility company, the developer, the homeowner, or the communities in which they were set? Considering three distinct groups with an interest in the aesthetics of the energy landscape, Southern California Edison, federal and state regulatory bodies, and homeowner groups, this article examines how the battle over the visibility of energy infrastructure was set within the social and urban politics of Los Angeles in the late 1960s. By throwing into relief this set of competing interests, it explores how the aesthetics of the energy landscape was not unitary, but fractured along conflicting political, social, and class-based lines. It argues that energy aesthetics was less the experience of the eye alone than a reflection of broader powers struggles within community, state and nation. (shrink)

Then she logically progresses to more radical ideas such as parenting in prison and the total abolition of prisons. The stories in this book are accompanied by warm, colorful illustrations by Julia Hunt. These drawings help the reader visualize the person behind the story and the very real life that was affected by reproductive law and/or social norms. Paynter holds a PhD in nursing, and it is from this background that she writes, seeking to educate nurses and other healthcare professionals alongside a broader readership garnered through her clear, approachable writing style.Chapter 1 introduces the reader to many of the book's key themes by considering the right and need to have bodily autonomy, and the multiple ways this manifests itself in ordinary (and less-ordinary) life. Paynter considers the disparate stories of five women, all fighting in some way for bodily autonomy. The stories cover events of gender-based gun violence, sexual assault, transgender rights in prison, the legalization of sex work, police violence, and institutional racism. Each story recounted by Paynter catalogs tragedies and injustices with blunt and brutal honesty—perhaps to an unnecessary extent. Every horrific fact is laid bare for the readers as they uncover the intricacies of gender-based, imperialist, and racist violence. Moreover, Paynter suggests that access is also bounded by financial constraints, finite public funding, and the limited availability of inclusive clinics that prioritize healthcare for LGBTQ2S+ people.Chapter 3 considers the topic of having children; in particular, the right to reproductive technologies and freedom from forced sterilization. Paynter quickly makes clear that while many white feminists might see having and not having children as diametrically opposed, they are two equally essential parts of reproductive justice. Paynter draws attention to the Sexual Sterilization Act of 1928 that was only overturned in 1972. She describes it as part of the "genocidal colonial project" (84), a collection of laws and policies aimed at fulfilling eugenicist, neoimperial ideals of increasing production of the white "race" and promoting the genocide of Indigenous peoples. Furthermore, Paynter argues that evidence of forced sterilization of Indigenous peoples, and other physical violence and neglect, persists to the present day and must be stopped and reparations made for previous harms. The Infectious Diseases Act entered into force officially on 14 November 2018 in Bangladesh. The Act is designed to raise awareness of, prevent, control, and eradicate infectious or communicable diseases to address public health emergencies and reduce health risks. A novel coronavirus disease was first identified in Bangladesh on 8 March 2020, and the Ministry of Health and Family Welfare issued a gazette on 23 March, listing COVID-19 as an infectious disease and addressing COVID-19 as a public health emergency. The (...) gazette empowers the government to monitor the spread of infection. Despite there being an infrastructure of research ethics committees in almost all hospitals in Bangladesh, a lack of such committees in the clinical setting often forces healthcare professionals to allocate scarce healthcare resources to the task. These personnel are often either influenced by materialistic matters or guided by the emergency policies, without reaching a consensus on how to allocate scarce resources in times of need, especially in the time of the COVID-19 pandemic. Ethical dilemmas often arise when a number of patients with COVID-19, especially in poor and middle-class areas, are denied care while elites are prioritized to receive such scarce resources. Resource allocation in healthcare during the COVID-19 pandemic in Bangladesh appears to be unethical and in direct conflict with the biomedical principles of non-maleficence and procedural justice. The findings of this study suggest that the Act needs substantive changes in the stipulation of policy directing hospitals in the provision of resource allocation framework. Furthermore, parliament should produce guidance outlining how to successfully implement the law with the aim of protecting public health in times of emergency, especially the COVID-19 pandemic. Philosophers have often posited a foundational calling voice, such that hearing its call constitutes subjects as responsive and responsible negotiators of normative claims. I give the name ldquo;transcendental conscience to that which speaks in this founding, constitutive voice. The role of transcendental conscience is not – or not merely – to normatively bind the subject, but to constitute the possibility of the subject's being bound by any particular, contentful normative claims in the first place. I explore the ontological and temporal (...) status of transcendental conscience, using Heidegger's account of conscience in Being and Time as my textual touchstone. I ask what performative structure the call of conscience might have that would enable it to constitute normative responsiveness, and I raise some temporal conundrums surrounding this structure. I argue that it is incoherent to attempt to give a literal, chronological account of the origin of normative grip and response. I suggest that we can best understand the founding calls of conscience, not as literal events occurring in regular time, but as events that can only show up retrospectively, as occurring in an ever-receding, unlocalizable past, and that these calls can only be figured mythically and metaphorically. Appropriating a Derridean term, I claim that the voice of transcendental conscience must be that of a lsquo;ghost, whose call binds us by haunting us – a haunting that is no less transcendentally necessary for its inability to be translated into a literal historical event. When Sandra Harding called for an epistemology of science whose systematic attention to the gendered Status of epistemic agents renders it ‘less partial and distorted’ than ‘traditional’ epistemologies, some commentators recoiled in horror. Propelled by ‘a mad form of the genetic fallacy’ they said, she descends ‘the slide to an arational account of science.’ On a less melodramatic reading, feminist epistemologies such as Harding's advocate not irrationalism, but senses of rationality more expanded than those which they associate with ‘traditional’ epistemology.

During the 1980s, Jean Baudrillard has been promoted in certain circles as the new McLuhan, as the most advanced theorist of the media and society in the so-called postmodern era.[1] His theory of a new, postmodern society rests on a key assumption that the media, simulations, and what he calls "cyberblitz" constitute a new realm of experience and a new stage of history and type of society. To a large extent, Baudrillard's work consists in rethinking radical social theory and politics (...) in the light of developments of the consumer, media, information, and technological society. Baudrillard's earlier works focus on the construction of the consumer society and how it provides a new world of values, meaning, and activity, and thus inhabit the terrain of Marxism and political economy. From the mid-1970s on, however, reflections on political economy and the consumer society disappear almost completely from his texts, and henceforth simulations and simulacra, media and information, science and new technologies, and implosion and hyperreality become the constituents of a new postmodern world which -- in his theorizing -- obliterate all the boundaries, categories, and values of the previous forms of industrial society while establishing new forms of social organization, thought, and experience. In this article, I argue that concern with the public sphere and the necessary conditions for a genuine democracy can be seen as a central theme of Jurgen Habermas's work that deserves respect and critical scrutiny in the contemporary moment, when throughout the world liberal democracies are in crisis. My study intends to point to the continuing importance of Habermas' problematic of the public sphere and its relevance for debates over democratic politics and social and cultural life in the present (...) age, in an era in which the Occupy Movements, Black Lives Matter, #MeToo, the Trump Resistance Movement, and Ukraine and Palestine Solidarity groups use social media to struggle against multiple forms of oppression. At stake is delineating a concept of the public sphere which facilitates maximum public participation and debate over the key issues of the contemporary era and which consequently promotes the cause of radical democracy and social transformation. Ethical sensitivity is a precursor to mora1 judgment in that a person must recognize the existence of an ethical problem before such a problem can be resolved. It is an important concept, yet it has received little attention from ethics scholars. This preliminary and exploratory study indicates that ethical sensitivity can be identified in viewers' reactions to and evaluations of ethically controversial television news stories, that diferent levels of ethical sensitivity are evident in discussions of television news stories, and that (...) there are at least 3 dimensions by which ethnic sensitivity may be evaluated: time, breadth, and depth. Truth-telling is a project that is both gripping and problematic for Rousseau, as he is both captured by an ideal of telling as complete, undistorted discernment, documentation and communication, and also haunted by the fear that telling can never be this innocent. For Rousseau, as for Kant, telling does not leave the told untouched; rather, telling gives us a type of contact with objects that is marked and mediated by the process of telling itself, and hence the possibility of immediately (...) grasping objects through telling is forever lost to us. The drive to capture things in themselves, which originates, according to Kant, in a formal principle of reason, shows up in Rousseau's writings as a nostalgia that governs and animates inquiry. I will read Rousseau, and the traumas of truth-telling he articulates, as important pretexts for Kant's critical epistemology. Rousseau discloses tensions that infect his truth-telling practices. Kant seeks to neutralize these tensions, not by dissolving them, but rather by translating them into the terms of transcendental philosophy and thus showing how they can be defused and rendered harmless in their empirical form, so as to secure the possibility of proper truth-telling. A novel Protocol Ethics Tool Kit (‘Ethics Tool Kit’) has been developed by a multi-stakeholder group of the Multi-Regional Clinical Trials Center of Brigham and Women9s Hospital and Harvard. The purpose of the Ethics Tool Kit is to facilitate effective recognition, consideration and deliberation of critical ethical issues in clinical trial protocols. The Ethics Tool Kit may be used by investigators and sponsors to develop a dedicated Ethics Section within a protocol to improve the consistency and transparency between clinical trial (...) protocols and research ethics committee reviews. It may also streamline ethics review and may facilitate and expedite the review process by anticipating the concerns of ethics committee reviewers. Specific attention was given to issues arising in multinational settings. With the use of this Tool Kit, researchers have the opportunity to address critical research ethics issues proactively, potentially speeding the time and easing the process to final protocol approval. As we move into a new millennium fraught with terror and danger, a global postmodern cosmopolis is unfolding in the midst of rapid evolutionary and social changes co-constructed by science, technology, and the restructuring of global capital. We are quickly morphing into a new biological and social existence that is ever-more mediated and shaped by computers, mass media, and biotechnology, all driven by the logic of capital and a powerful emergent technoscience. In this global context, science is no longer merely (...) an interpretation of the natural and social worlds, rather it has become an active force in changing them and the very nature of life. In an era where life can be created and redesigned in a petri dish, and genetic codes can be edited like a digital text, the distinction between “natural” and “artificial” has become greatly complexified. The new techniques of manipulation call into question existing definitions of life and death, demand a rethinking of fundamental notions of ethics and moral value, and pose unique challenges for democracy. Our lead article in this issue of the Kennedy Institute of Ethics Journal, Jonathan Kaplan’s “Self-Care as Self-Blame Redux: Stress as Personal and Political,” opens up an entirely new and clearly important topic for bioethicists: the concept and role of ‘self-care.’ Advice for ‘self-care’ is everywhere, and often this advice takes the form of a kind of moral imperative: we owe ourselves self-care and have a responsibility to care for ourselves. Meanwhile, typical suggested self-care practices focus on individual behaviors and (...) purchases, rather than social interventions. Since the 9/11 terrorist attacks and subsequent Terror War, Jean Baudrillard has written a series of reflections on the contemporary moment that have evoked the excitement and controversy of his earlier work. For many years, Baudrillard had complained that the contemporary era has been one of “weak events,” that the energies of history seemed to be depleted, and that politics has become increasingly banal and boring. He claimed in an essay "Anorexic Ruins," published in 1989, that the Berlin wall was (...) a sign of a frozen history, of an anorexic history, in which nothing more can happen, marked by a "lack of events" and the end of history, taking the Berlin wall as a sign of a stasis between communism and capitalism. Likewise, at one time, Baudrillard read the New York Twin Towers of the World Trade Center as symbols of the stasis of global capitalism and a frozen history in which the two superpowers develop a system of binary regulation. Paul Lazarsfeld, one of the originators of modern communications studies, distinguished between what he called a "administrative research," that deployed empirical research for the goals of corporate and state institutions, and “critical research,” that he associated with the Frankfurt School. Critical research situates the media within the broader context of social life and interrogates its structure, goals, values, messages, and effects. It develops critical perspectives by which media are evaluated and appraised. Since the 1940s, an impressive variety of critical approaches (...) to the media and television have developed. In this study, I will first present the Frankfurt School as an inaugurator of critical approaches to television studies and will then discuss how a wide range of theorists addressed what later became known as the politics of representation in critical television studies, engaging problematics of class, gender, race, sexuality, and other central components of media representation and social life. Then, I discuss how a postmodern turn in cultural studies contested earlier critical models and provided alternative approaches to television studies. I conclude with some comments that argue for a critical approach to television and media culture and in this text sketch out a comprehensive critical model that embraces production and political economy of television; textual analysis; and investigation of the effects and uses of television by audiences. As this study will indicate, such a multidimensional approach to critical media and television studies is found initially in the Frankfurt School and was developed by many other television theorists in diverse locations and from often conflicting perspectives, ranging from British cultural studies to critical feminism. The developing countries are currently undergoing a perhaps unprecedented technological revolution that has given new credence and life to the concept of alienation after a period of relative decline in which M arxian, existentialist, and other modern discourses were replaced with postmodern perspectives skeptical or critical of the concept of alienation. In this paper, I want to suggest that emergent information and communication technologies and the restructuring of global capitalism require us to rethink the problematics of technology and alienation. If (...) it is true that we are undergoing a Great Transformation, one of the epochal shifts within the history of capitalism, that the new technologies are taking us into a novel field of cultural experience and that the very nature of human identity and social relations are changing, then obviously we need to develop fresh theories to analyze these changes and politics to respond to them.2 For many, the changes underway on a global scale are as thorough-going and dramatic as the shift from the stage of market and competitive and laissez-faire capitalism theorized by M arx to the stage of state monopoly capitalism critically analyzed by the Frankfurt School in the 1930s.3 Theorizing this ongoing and epic transformation requires critical social theory to engage anew the relations between the economy, state, culture industry, science and technology, social institutions and everyday life as radically as the Frankfurt School revised classical M arxism in the 1930s. In this context, talking about technology and alienation is not just an academic affair, the latest twist in the discourse of alienation or of technology, but rather concerns the fate of the human being in the contemporary world and thus requires serious reflection and discussion whether the changes in society, culture, and human existence are or are not beneficial, and what we can do to promote a positive outcome and prevent a harmful one.. Fredric Jameson is generally considered to be one of the foremost contemporary Englishlanguage Marxist literary and cultural critics. Over the past three decades, he has published a wide range of works analyzing literary and cultural texts, while developing his own neo-Marxist theoretical perspectives. In addition, Jameson produced many important critiques of opposing theoretical schools and positions. A common challenge arises from the difficulty of assessing trust levels in ecologically invalid environments: we present in this paper two independent laboratory studies, totalling 160 participants, where we investigate the impact of different types of errors on resulting trust, using both behavioural and subjective measures of trust. While we found a general effect of errors on reported and observed level of trust, no (...) significant differences between the type of errors were found in either of our studies. We discuss this negative result in light of our experimental protocols, and argue for the community to move towards alternative methodologies to assess trust. (shrink)

In The Virtual, Rob Shields puts virtuality in with the key categories of contemporary social theory such as subjectivity, agency, structure, and the spaces and temporalities between the modern and the postmodern. Shields has rescued the term and the idea of the virtual from utopian futurists like Howard Rheingold and Nicholas Negroponte who use it to hype emergent technologies and forms of culture as the magical vehicles and entry points to new worlds and identities. The works of these digerati, ideologues (...) for multimedia technology and culture, now appear ideological, outdated, and no more than huckstering of the new when confronted with the current state of affairs in the technoculture and its attendant war- and terrorism-torn world. In the current ongoing Terror War, both George W. Bush and Osama bin Laden deploy certain similar figures of speech, fusing their metaphysical and political discourses while reserving the demonology. In his speech to Congress on September 20, 2001 declaring his war against terrorism, Bush described the conflict as a war between freedom and fear. The coming Terror War was, he explained, a conflict between “those governed by fear” who “want to destroy our wealth and freedoms,” and those on the (...) side of freedom. Bush insisted that “you’re either with us, or you’re with the terrorists,” and laid down a series of non-negotiable demands to the Taliban while Congress wildly applauded. Bush’s popularity soared with a country craving blood-revenge and the head of Osama bin Laden. Moreover, proclaiming what his administration and commentators would describe as “the Bush doctrine,” Bush also asserted that his administration held accountable those nations who supported terrorism –- a position that could nurture and legitimate military interventions for years to come. Both New French Theory and Critical Theory explode the boundaries established in the division of labor which separates our academic disciplines into such things as economics, political science, philosophy, sociology, etc. Both claim that there are epistemological and metaphysical problems with abstracting from the interconnectedness of phenomena in the world, or from our experience of it. On this view, philosophy, for example, that abstracts from sociology and economics, or political science that excludes, say, economics or culture from its conceptual boundaries, (...) is by nature one-sided, limited, and flawed. Both Critical Theory and New French Theory therefore transgress established disciplinary boundaries and create new disciplines, theories, and discourses that avoid the deficiencies of the traditional academic division of labor.
