Background: The ethics and value bases in healthcare are widely acknowledged. There is a need to improve and raise awareness of ethics in complex systems and in line with competing needs, different stakeholders and patients’ rights. Evidence-based strategies and interventions for the development of procedures and practice have been used to improve care and services. However, it is not known whether and to what extent ethics can be developed using interventions. Objectives: To examine ethics interventions conducted on healthcare professionals and (...) healthcare students to achieve ethics-related outcomes. Research design: A systematic review. Methods: Five electronic databases were searched: CINAHL, the Cochrane Library, Philosopher’s Index, PubMed and PsycINFO. We searched for published articles written in English without a time limit using the keywords: ethic* OR moral* AND intervention OR program OR pre-post OR quasi-experimental OR rct OR experimental AND nurse OR nursing OR health care. In the four-phased retrieval process, 23 full texts out of 4675 citations were included in the review. Data were analysed using conventional content analysis. Ethical consideration: This systematic review was conducted following good scientific practice in every phase. Findings: It is possible to affect the ethics of healthcare practices through professionals and students. All the interventions were educational in type. Many of the interventions were related to the ethical or moral sensitivity of the professionals, such as moral courage and empowerment. A few of the interventions focused on identifying ethical problems or research ethics. Conclusion: Patient-related outcomes followed by organisational outcomes can be improved by ethics interventions targeting professionals. Such outcomes are promising in developing ethical safety for healthcare patients and professionals. (shrink)

Klein, Renate The practice of surrogacy in Australia has been controversial since its beginning in the late 1980s. In 1988, the famous 'Kirkman case' in the state of Victoria put surrogacy on the national map. This was a two-sisters surrogacy - Linda and Maggie Kirkman and the resulting baby Alice - in which power differences between the two women were extraordinarily stark: Maggie was the glamorous and well spoken woman of the world; Linda who carried the baby, was the (...) demure school teacher in child-like frocks and pig tails. Their IVF doctor applauded altruistic surrogacy. He called it 'gestational surrogacy' and proclaimed that if the so-called surrogate mother didn't use her own eggs, thus wasn't the baby's 'genetic' mother, no attachment would ensue! This statement is haunting us to this day. It is patently absurd: as a baby grows in a woman's body over the nine months of the pregnancy, it is hard to see why the 24/7 presence of the baby inside her body, its growth, its interaction with her would be any different whether s/he has the mother's genes! The efforts of monitoring and forecasting of disastrous events can illustrate benefits and threats of technicization and science. In ecophilosophy the forecasting of hazards is today extremely needed. It is not about creating theoretical unified structures or practical return to holistic harmony of a primordial man with nature. It is about, as Félix Guattari once held it, the complexity of the relationship between humans and their natural environment. Patients' wishes regarding health care and dying must be taken into consideration by their physicians. Competent patients need to record directives about their care in advance of a crisis situation. The primary care physician, seeing the patient at the time of a routine office visit, is in a favorable position to explore and record attitudes. A patient's value system should be part of a medical history before hospital admission. Details in a Value History Questionnaire facilitate guiding an incompetent patient through (...) a terminal illness in accordance with wishes previously expressed.An instrument in the form of a questionnaire was designed to record the attitudes of 200 patients regarding health care and dying. Respondents ranged in age from 17 to 84 years, and all were members of one family practice. They reacted positively to the opportunity to record their values, opinions, and wishes about their health care and process of dying. The article begins by addressing the main conceptual issues. Use of this term is the hallmark of an approach that is critical of the explanatory deficits of correlational analysis and of the covering-law model, advocating instead the causal reconstruction of the processes that account for given macro-phenomena. The term "social mechanisms" should be used to refer to recurrent processes generating (...) a specific kind of outcome. Explanation of social macro-phenomena by mechanisms typically involves causal regression to lower-level elements, as stipulated by methodological individualism. While there exist a good many mechanism models to explain emergent effects of collective behavior, we lack a similarly systematic treatment of generative mechanisms in which institutions and specific kinds of structural configurations play the decisive role. As COVID-19 continues to spread, a variety of COVID-19 tracking apps (CTAs) have been introduced to help contain the pandemic. Deployment of this technology poses serious challenges of effectiveness, technological problems and risks to privacy and equity. The ethical use of CTAs depends heavily on the protection of voluntariness. Voluntary use of CTAs implies not only the absence of a legal obligation to employ the app but also the absence of more subtle forms of coercion such as enforced exclusion from (...) certain social and work activities. The protection of individual rights to voluntary use can be enhanced through an ethics by design approach in the development of CTAs that treat the introduction of CTAs for what it is: a complete novelty that is being tested for the first time in democracies. This paper offers a contribution to ecophilosophy from the perspective of the scientific research of the environment. The problem considered in the paper deals with a specific issue of environmental risk, namely, the problem of radon ionizing radiation and the highest permissible security norms of it. This problem, now rarely discussed in ecological communities, is one of more important for humankind’s health and safe existence. The awareness of harmful and beneficial biological effects of various environmental factors is a basic step (...) towards ensuring the security of public health. The admissible norms of radon radiation are different in different countries. The article suggests possible causes of these differences and puts forward the thesis that today’s science alone is not a sufficient ground of resolving ecological problems. (shrink)

This paper describes an exploratory comparative study of knowledge workers and their challenges in high tech global project teams. More specifically we focus on the tension between perceived collocation and actual geographical distributed project work as a function of: (1) the demand to distribute and shift attention in multi-teaming, (2) virtuality i.e. number of virtual teams participants engage in, (3) the continuous adjustment and re-adjustment to new places they perform their activity, and (4) the collaboration technologies they use. We present (...) the methodology for data collection that included semi-structured interviews, surveys, and on site shadowing of the project participants, and discuss the findings from the data analysis. The study is based on the bricks-bits-interaction framework. It is at the intersection of the design of physical spaces, i.e., bricks; rich digital information and collaboration technology (ICT) content, mobile devices and network infrastructures, i.e., bits, and emergent work practices, process, and new ways people behave in communicative events using the affordances of ICT augmented physical, virtual spaces and digital content, i.e., interaction. "This book addresses recent developments in the study of tense from a cross-paradigm and cross-linguistic point of view. Leading international scholars explore challenging ideas about tense at the interfaces between semantics and syntax as well as syntax and morphology. The book is divided into three main subsections: 1) Tense in tenseless languages; 2) Tense, mood, and modality, and 3) Descriptive approaches to some tense phenonema. Although time is a universal dimension of the human experience, some languages encode reference to time (...) without any grammatical tense morphology of the verb. Some of these exceptional "tenseless" languages are investigated in this volume: Kalaallisut, Paraguayan Guaraní and Movima. Modal verbs are polyfunctional in the sense that they express both tense and modality. In this volume, an untypical modal is analyzed, a modal analysis of imperatives is argued for, and sentential mood, which is closely related to modality, is analyzed. It is always interesting to look at the expression of tense in understudied languages, which is done here for Scottish Gaelic, Austronesian Rukai and German dialects. The epistemological problems of unification of two distinct theories are discussed. An approach related to the work of Soviet authors (Stepin, Podgoretzky and Smorodinsky) is used and developed. The notion of ‘crossbred objects’—theoretical objects with contradictory properties which are part of the domain of application of two independent theories—is introduced which helps to describe the dynamics of revolutionary theory change. The occurrence of the cross-contradiction of two theories is reconstructed and the reductionistic and the synthetic means of its elimination are (...) proposed. Moral injury emerged in the healthcare discussion quite recently because of the difficulties and challenges healthcare workers and healthcare systems face in the context of the COVID-19 pandemic. Moral injury involves a deep emotional wound and is unique to those who bear witness to intense human suffering and cruelty. This article aims to synthesise the very limited evidence from empirical studies on moral injury and to discuss a better understanding of the concept of moral injury, its importance in the healthcare (...) context and its relation to the well-known concept of moral distress. A scoping literature review design was used to support the discussion. Systematic literature searches conducted in April 2020 in two electronic databases, PubMed/Medline and PsychInfo, produced 2044 hits but only a handful of empirical papers, from which seven well-focused articles were identified. The concept of moral injury was considered under other concepts as well such as stress of conscience, regrets for ethical situation, moral distress and ethical suffering, guilt without fault, and existential suffering with inflicting pain. Nurses had witnessed these difficult ethical situations when faced with unnecessary patient suffering and a feeling of not doing enough. Some cases of moral distress may turn into moral residue and end in moral injury with time, and in certain circumstances and contexts. The association between these concepts needs further investigation and confirmation through empirical studies; in particular, where to draw the line as to when moral distress turns into moral injury, leading to severe consequences. Given the very limited research on moral injury, discussion of moral injury in the context of the duty to care, for example, in this pandemic settings and similar situations warrants some consideration. Due to the potential ethical and psychological implications of screening, and especially inregard of screening on children without available and acceptable therapeutic measures, there is a common view that such procedures are not advisable. As part of an independent research- and bioethical case study, our aim was therefore to explore and describe bioethical issues among a representative sample of participant families (n = 17,055 children) in the ABIS (All Babies In South-east Sweden) research screening for Type 1 diabetes (IDDM).The primary (...) aim is the identification of risk factors important for the development of diabetes and other multifactorial immune-mediated diseases. Four hundred, randomly chosen, participant mothers were asked to complete a questionnaire exploring issues of information, informed consent, bio-material, confidentiality and autonomy, and of prevention/intervention. 293 completed the questionnaire, resulting in a response rate of 73.3%. The majority of questions had the form of 6-point Likert-type response scales (1–6).We found that the majority of respondents felt calm in regard of samples and written material, and also concerning the possibility of their child in the future being identified as having high risk of developing Type 1 diabetes. An important finding concerning access and control of mainly biological data was indicated, with the respondents expressing concern for potential future use. We believe our findings indicate that this kind of empirical studies can substantially contribute to our understanding of bioethical issues of medical research involving genetics. Issues, such as safeguards ensuring theethical criteria of autonomy and respect, were emphasised by our respondents. We believe theissues brought up may promote further discussion, and do suggest issues for consideration by, among others, researchers, bioethicists and Institutional Review Boards. (shrink)

This paper discusses ethics in the context of Aboriginal Studies. Taking the example of a late-nineteenth century missionary work, a collection of out-of-print Mi’kmaq stories, it examines the ethical implications of the potential re-publication of such a text. It is argued that the Baptist missionary Silas T. Rand, who translated and transcribed the narratives, did his work from a Eurocentric perspective. The biases of a colonial ideology built into his translations/interpretations which are often quoted as authoritative would be further perpetuated (...) if his work is republished without critical commentary. As Aboriginal oral traditions generally form the basis of Aboriginal cultures and contemporary Aboriginal literatures but have been demeaned for centuries thereby further colonizing the peoples, an edition of Rand’s work informed by a postcolonial ethics is crucial. The paper therefore concludes with the suggestion that the stories collected by the missionary should be repatriated in the respective Mi’kmaq communities in a way that the people can decide how to go about their publication (if they want them published at all). A non-Aboriginal scholar may facilitate the process but should follow the directions of the communities. (shrink)

We explore in this paper the relation between activities, communication channels and media, and common ground building in global teams. We define re-representation as a sequence of representations of the same concept using different communication channels and media. We identified the re - representation technique to build common ground that is used by team members during multimodal and multimedia communicative events in cross-disciplinary, geographically distributed settings. Our hypotheses are as follows: (1) Significant sources of information behind decisions and request for (...) actions are embedded within the fabric of communicative events in which participants use both informal and formal media to express their ideas. Capturing these information sources can facilitate common ground building and accelerate the execution of action requests. (2) Re-representations of concepts, i.e., sequences of representations using diverse media and communication channels, mediate and accelerate common ground building. (3) The use of intra- or interdisciplinary re-representations correlates with high team performance, i.e., effective team process and high product quality. We used AEC Global Teamwork course offered in 2008–2009 as the testbed for our study to validate our hypothesis. (shrink)

Being a member of both local and global teams requires constant distribution and re-distribution of attention, engagement, and intensive communication over synchronous and asynchronous channels with remote and local partners. We explore in this paper the increasing number of social worlds such participants distribute their attention to, how this affects their level of engagement and attention, and how the workspace, collaboration technologies, and interaction modes afford and constrain the communicative events. The use of information and collaboration technologies (ICT) shapes and (...) reshapes work spaces, processes, and social interactions among team members, and team members reshape ICT and the way it is used. We use as a testbed the AEC Global Teamwork course established at Stanford in 1993 in collaboration with universities worldwide and provide examples from two sites—Stanford PBL Lab, California USA and Chalmers University, Goteborg Sweden. We used temporal analysis and qualitative methods of inquiry in order to study participants’ attention, how they used the site and the means, i.e., their social and material resources, to accomplish their interaction needs, and how they engaged throughout project reviews. Gegenstand dieser politikwissenschaftlichen Arbeit ist weder die >Textur des Gewissens von Politikern, noch das sozialöffentliche Gewissen, also die Untersuchung von Wohlfahrts-sytemen. Es geht vielmehr um Konzeptionen des moralisch-individuellen Gewissens und der darin konturierten Beziehung zum politischen System. Zentrales Thema ist mithin die Frage nach dem Verhältnis von individueller Freiheit und politischer Ordnung. Dabei wird das semantische Feld im Kontext der Termini Gewissen, Moral und Ethik neu formuliert. Mittels einer Analyse der Gewissensdiskurse in der Frühmoderne sowie in der Spätmoderne läßt sich (...) die These entfalten, daß die Erkenntnis der neuartigen normativen Prob-lemstrukturen im technischen Zeitalter eine politologische Aufklärung erfordern, welche die zu-nehmend erkennbar werdenden Grenzen des Wissens in die Theoriebildung einbezieht. Die Poli-tik- und Sozialwissenschaften befinden sich gegenwärtig in einer Schwellenphase, in der die herkömmlichen normativen Erklärungsmuster, die in der Tradition des ethischen Humanismus verwurzelt sind, zu erodieren beginnen - doch werden die weitreichenden Konsequenzen des gegenwärtig sich vollziehenden sozio-technischen Umbruchs für die politische Ethik allenfalls halbherzig gezogen. Arabidopsis thaliana (Thale cress, Arabidopsis) is an ideal model organism for the molecular genetic analysis of many plant processes. The availability of a complete physical map would greatly facilitate the gene cloning steps in these studies. The small genome size of Arabidopsis makes the construction of such a map a feasible goal. One of the approaches to construct an overlapping library of the Arabidopsis genome takes advantage of the many mapped markers and the availability of Arabidopsis yeast artificial chromosome (YAC) (...) libraries. Mapped molecular markers are used to identify corresponding YAC clones and thereby place them on the genetic map. Subsequently, these YAC clones provide the framework for directed walking experiments aimed at closing the gaps between the YAC contigs. Adopting this strategy, YAC clones comprising about 10% of the genome have been assigned to the top halves of Arabidopsis chromosomes 4 and 5. Extensive walking experiments in a 10 cM interval of chromosome 4 have resulted in two contiguous regions in the megabase size range. (shrink)
