There is a global shortage of organs for transplantation and despite many governments making significant changes to their organ donation systems, there are not enough kidneys available to meet the demand. This has led scientists and clinicians to explore alternative means of meeting this organ shortfall. One of the alternatives to human organ transplantation is xenotransplantation, which is the transplantation of organs, tissues, or cells between different species. The resurgence of interest in xenotransplantation and recent scientific breakthroughs suggest that genetically-engineered (...) pigs may soon present a realistic alternative as sources of kidneys for clinical transplantation. It is therefore important for nurses and allied health professionals to understand what is involved in xenotransplantation and its future implications for their clinical practices. First, we explore the insufficiency of different organ donation systems to meet the kidney shortage. Second, we provide a background and a summary of the progress made so far in xenotransplantation research. Third, we discuss some of the scientific, technological, ethical, and economic issues associated with xenotransplantation. Finally, we summarise the literature on the attitudes of healthcare professionals toward xenotransplantation. (shrink)

Formal clinical trials of pig-to-human organ transplant—known as xenotransplantation—may begin this decade, with the first trials likely to consist of either adult renal transplants or pediatric cardiac transplant patients. Xenotransplantation as a systematic scientific study only reaches back to the latter half of the 20th century, with episodic xenotransplantation events occurring prior to that. As the science of xenotransplantation has progressed in the 20th and 21st centuries, the public's knowledge of the potential therapy has also increased. With this, there have (...) been shifting ethical stances toward xenotransplantation in key areas, such as religious and public viewpoints towards xenotransplantation, animal rights, and public health concerns. Florence Ashley has argued that requiring patients with gender dysphoria to undergo an assessment and referral from a mental health professional before undergoing hormone replacement therapy (HRT) is unethical and may represent an unconscious hostility towards transgender people. We respond, first, by showing that Ashley has conflated the self-reporting of symptoms with self-diagnosis, and that this is not consistent with the standard model of informed consent to medical treatment. Second, we note that the model of informed consent involved in cosmetic (...) surgery resembles the model Ashley defends, and that psychological assessment and referral is recognised as an important aspect of such a model. Third, we suggest that the increased prevalence of psychiatric morbidity in the transgender population arguably supports the requirement of assessment and referral from a mental health professional prior to undergoing HRT. This paper applies models of the onset of adolescent sexual intercourse using national data from Denmark and the USA. The model gave excellent fits to data on Danish Whites and a good fit to American Whites, but the model-fits for American Blacks and Hispanics were not as good. The weakness of the latter model fits may reflect either real processes that the model does not capture or problems in the reliability of adolescent sexuality data.

Florence Ashley has responded to our response to ‘Gatekeeping hormone replacement therapy for transgender patients is dehumanising.’ Ashley criticises some of our objections to their view that patients seeking hormone replacement therapy (HRT) for gender dysphoria should not have to undergo a prior psychological assessment. Here we clarify our objections, most importantly that concerning the parity between cosmetic surgery and the sort of intervention Ashley has in mind. Firstly, we show Ashley’s criticism of our comparison is insubstantial. We then examine (...) their comments regarding the analogy between abortion and HRT, and the language of informed consent. We also scrutinize the data upon which Ashley relies to defend the claim that informed consent without assessment always yields positive outcomes, finding this conclusion to be overstated. Finally, even if Ashley’s arguments were vindicated, the claim that the gatekeeping of HRT is motivated by hostility towards transgender people remains unsupported. (shrink)

In a recent article in this journal, Abram Brummett argues that new and future assisted reproductive technologies will provide challenging ethical questions relating to lesbian, gay, bisexual and transgender (LGBT) persons. Brummett notes that it is likely that some clinicians may wish to conscientiously object to offering assisted reproductive technologies to LGBT couples on moral or religious grounds, and argues that such appeals to conscience should be constrained. We argue that Brummett's case is unsuccessful because he: does not adequately interact (...) with his opponents’ views; equivocates on the meaning of ‘natural’; fails to show that the practice he opposes is eugenic in any non‐trivial sense; and fails to justify and explicate the relevance of the naturalism he proposes. We do not argue that conscience protections should exist for those objecting to providing LGBT people with artificial reproductive technologies, but only show that Brummett's arguments are insufficient to prove that they should not. BackgroundIn the Canadian Alliance for Healthy Hearts and Minds (CAHHM) cohort, participants underwent magnetic resonance imaging (MRI) of the brain, heart, and abdomen, that generated incidental findings (IFs). The approach to managing these unexpected results remain a complex issue. Our objectives were to describe the CAHHM policy for the management of IFs, to understand the impact of disclosing IFs to healthy research participants, and to reflect on the ethical obligations of researchers in future MRI studies.MethodsBetween 2013 and 2019, 8252 participants (...) (mean age 58 ± 9 years, 54% women) were recruited with a follow-up questionnaire administered to 909 participants (40% response rate) at 1-year. The CAHHM policy followed a restricted approach, whereby routine feedback on IFs was not provided. Only IFs of severe structural abnormalities were reported.ResultsSevere structural abnormalities occurred in 8.3% (95% confidence interval 7.7–8.9%) of participants, with the highest proportions found in the brain (4.2%) and abdomen (3.1%). The majority of participants (97%) informed of an IF reported no change in quality of life, with 3% of participants reporting that the knowledge of an IF negatively impacted their quality of life. Furthermore, 50% reported increased stress in learning about an IF, and in 95%, the discovery of an IF did not adversely impact his/her life insurance policy. Most participants (90%) would enrol in the study again and perceived the MRI scan to be beneficial, regardless of whether they were informed of IFs. While the implications of a restricted approach to IF management was perceived to be mostly positive, a degree of diagnostic misconception was present amongst participants, indicating the importance of a more thorough consent process to support participant autonomy.ConclusionThe management of IFs from research MRI scans remain a challenging issue, as participants may experience stress and a reduced quality of life when IFs are disclosed. The restricted approach to IF management in CAHHM demonstrated a fair fulfillment of the overarching ethical principles of respect for autonomy, concern for wellbeing, and justice. Background: Moral distress has been described as a major problem for the nursing profession, and in recent years, a considerable amount of research has been undertaken to examine its causes and effects. However, few research projects have been performed that examined the moral distress of an entire nation’s nurses, as this particular study does. Aim/objective: The purpose of this study was to determine the frequency and intensity of moral distress experienced by registered nurses in New Zealand. Research design: The research (...) involved the use of a mainly quantitative approach supported by a slightly modified version of a survey based on the Moral Distress Scale–Revised. Participants and research context: In total, 1500 questionnaires were sent out at random to nurses working in general areas around New Zealand and 412 were returned, giving an adequate response rate of 27%. Ethical considerations: The project was evaluated and judged to be low risk and recorded as such on 22 February 2011 via the auspices of the Massey University Human Ethics Committee. Findings: Results indicate that the most frequent situations to cause nursing distress were (a) having to provide less than optimal care due to management decisions, (b) seeing patient care suffer due to lack of provider continuity and (c) working with others who are less than competent. The most distressing experiences resulted from (a) working with others who are unsafe or incompetent, (b) witnessing diminished care due to poor communication and (c) watching patients suffer due to a lack of provider continuity. Of the respondents, 48% reported having considered leaving their position due to the moral distress. Conclusion: The results imply that moral distress in nursing remains a highly significant and pertinent issue that requires greater consideration by health service managers, policymakers and nurse educators. The Roman Senate in 144 B.C. instructed the urban praetor, Q. Marcius Rex, to repair the conduits of Rome's two existing aqueducts, the Appia and the Anio , and to put an end to illegal use of their water by private citizens. Urban growth now demanded a more copious water supply, and so the Senate further I instructed Marcius to secure additional water for the city. Money was appropriated for this work, and Marcius' praetorship was prorogued for 143. At this (...) point the decemviri objected to a plan for bringing water to the Capitol. The issue was debated in the Senate in 143, and again in 140; but on both occasions Marcius' gratia prevailed, and water reached the Capitol in a new aqueduct which Marcius himself had built. A statue of Marcius was erected in later times on the Capitol, behind the temple of Jupiter, to commemorate this grand achievement.1 An everlasting glory was in fact to be his in the name of the aqueduct whose waters earned poets' praises. Henry Charlton Bastian's support for spontaneous generation is shown to have developed from his commitment to the new evolutionary science of Darwin, Spencer, Huxley and Tyndall. Tracing Bastian's early career development shows that he was one of the most talented rising young stars among the Darwinians in the 1860s. His argument for a logically necessary link between evolution and spontaneous generation was widely believed among those sympathetic to Darwin's ideas. Spontaneous generation implied materialism to many, however, and it had (...) associations in Britain with radical politics and amateur science. Huxley and the X Club were trying to create a public posture of Darwinism that kept it at arm's length from those negative associations. Thus, the conflict that developed when Huxley and the X Club opposed Bastian was at least as much about factional in-fighting among the Darwinians as it was about the experiments under dispute. Huxley's strategy to defeat Bastian and define his position as "non-Darwinian" contributed significantly to the shaping of Huxley's famous address "Biogenesis and Abiogenesis." Rhetorically separating Darwinism from Bastian was thus responsible for Huxley's first clear public statement that a naturalistic origin of life was compatible with Darwin's ideas, but only in the earth's distant past. The final separation of the discourse on the meaning of Brownian movement and "active molecules" from any possible link with spontaneous generation also grew out of Huxley's strategy to defeat Bastian. The Paradox of Prime Matter DANIEL W. GRAHAM TRADITIONAL INTERPRETATIONS OF Aristotle hold that he posited the existence of prime matter–a purely indeterminate substratum underlying all material composition and providing the ultimate potentiality for all material existence. A number of revisionary interpretations have appeared in the last thirty years which deny that Aristotle had a concept of prime matter, provoking an even larger number of vigorous defenses claiming that he did have the concept? The traditionalists are clearly in the majority, (...) but some obstacles stand in the way of a general acceptance of prime matter as an Aristotelian concept. In a recent contribution to the debate, William Charlton, an opponent of prime matter, has pointed out that the opposing parties have reached a stalemate in large measure because most of the relevant texts are ambiguous; consequently, "the question whether or not Aristotle believed in prime matter really comes down to the question how far, if at all, it is demanded by his philosophy as a whole. Given Aristotle’s assumptions and commitments, the doctrine of prime matter is not only dialectically inevitable but also systematically incoherent. In this paper I shall not defend the existence of a doctrine of prime matter in Aristotle’s philosophy, although my argument will provide an incidental justification for it by exhibiting its function within Aristotle’s system. My aim here is to explain what is wrong with the doctrine of prime matter. (1) I shall examine a problem concerning prime matter–a problem of which Aristotle was aware and which he thought he had solved. (2) I shall argue that he did not solve the problem, for the doctrine of prime matter entails a paradox for his system. (3) I shall reply to some objections, and (4) I shall offer a tentative diagnosis of how Aristotle could have come to embrace a paradoxical position. 1. According to Aristotle’s theory of change, there is a substratum which underlies every change (Ph. 1.7.19oa33ff). When a thing changes its features, we call that accidental change and identify the substratum as substance. When a thing comes into being or ceases to be, we call that substantial change3 and identify the substratum as matter.4 The most simple bodies of the Aristotelian cosmos are the four traditional "elements": earth, air, fire, and water.5The elements are characterized by the contrary powers hot, cold, wet, and dry. This doesn't mean they acted suddenly, or impulsively, only that they believe they could not have done otherwise. Trying to make sense of this near universal answer to the question "Why did you do it?" the essay draws on narrative theory. Narrative theory distinguishes between actant and sender—that is, between actor and his or her values. I was moved to make it the subject of this evening's lecture by the fact that I have lately been reading the book in which the most important of the late Professor Prichard's scattered writings on Sense-perception have been collected by Sir W. D. Ross. Like everything that Prichard wrote, these essays are extremely acute, transparently honest, and admirably thorough. ISBN: 978-0-8132-3445-8.Nature as Guide is an intriguing reevaluation of the philosophical legacy of Ludwig Wittgenstein in the light of Thomistic moral theology after the Second (...) Vatican Council. Goodill helpfully lays out his fundamental argument in the book’s introduction: Wittgenstein can help the philosopher to reconcile the “modern dichotomy between nature and reason,” and the theologian to reevaluate “the associated oppositions typical of modern thought, such as that between moral agency and natural agency or between grace and nature” (2). The author is overt in his appreciation for Fergus Kerr’s 1997 monograph Theology after Wittgenstein. Nonetheless, Nature as Guide is unique insofar as it focuses on specifically moral theological issues in a moment of theological “renewal”—one in which we can recognize “the restoration of metaphysics to the center of theological debate” (12).The first four chapters of Nature as Guide consider Wittgenstein in relation to themes associated with classical philosophy. Goodill’s “objective in the first part of the book is to construct a dialogue between Wittgenstein and the metaphysical tradition which illuminates both conversation partners” (38). This initial part of the book culminates in an account of human nature that serves as the foundation for the moral theological inquiry that follows in the second part (the fifth through the eighth chapters).In the first chapter (“What Lies Hidden: Wittgenstein, Metaphysics, and Moral Theology”), Goodill introduces Wittgenstein’s “philosophical practice”—specifically his “insistence on grounding philosophical enquiry in everyday thinking” that effectively “connects him to the [metaphysical] tradition that begins with Plato and Aristotle” (16). Goodill is perceptive and nuanced in his analysis of Wittgenstein’s texts. He recognizes that “it would be wrong to treat [Wittgenstein] as an unproblematic inheritor of this tradition” (37). Nonetheless, he wants his readers to appreciate the careful balance between theory and praxis maintained in Wittgenstein’s thought vis-à-vis the priorities of classical philosophy. To this end, he examines G. E. M. Anscombe’s account of how Wittgenstein helped her to overcome the “trap” of phenomenalism.In the second chapter (“Nature, Logic, and Grammar”), Goodill attends to the progress of key tenets in Wittgenstein’s philosophy. He identifies as “the guiding theme” of this narrative “the development of Wittgenstein’s understanding of the relationship between the meaning and the application of linguistic expressions” (40–41). Specifically, he suggests that Wittgenstein’s “mistake” in the Tractatus (51) can be reduced a fundamental point: the erroneous presupposition that there is “one general notion of clarity which is required in all contexts. It was precisely this requirement which Wittgenstein came later to reject in the Investigations” (52). Indeed, between the Tractatus [End Page 703] and the Investigations Wittgenstein abandoned his efforts to establish a “forced unity” of language through “the general form of the proposition”—turning, instead, to “our actual linguistic practices” (85). The whole of human life, thus, “provides the unity of our grammar” (ibid.). Nonetheless, Goodill also points out that Wittgenstein does not reject “the central insight” of the Tractatus, namely, “that meaning is internal to our forms of representation” (59). This immanence remains throughout the Tractatus and the Investigations.The third chapter (“Grammar, Teleology, and Understanding”), summarizes objections to Goodill’s proposal that Wittgenstein can be read in continuity with the metaphysical tradition. William Charlton “argues that in the absence of a teleological conception of nature Wittgenstein’s account of language collapses into pragmatism” (88-89). Rush Rhee “accuses Wittgenstein of being misled by his analogy between language and games to neglect the basis of our linguistic practices in the human search for universal understanding” (89). What ecological advantages do animals gain by being able to detect, extract and exploit wavelength information? What are the advantages of representing that information as hue qualities? The benefits of adding chromatic to achromatic vision, marginal in object detection, become apparent in object recognition and receiving biological signals. It is argued that this improved performance is a direct consequence of the fact that many animals' visual systems reduce wavelength information to combinations of four basic hues. This engenders a simple categorical (...) scheme that permits a rich amount of sensory information to be rapidly and efficiently employed by cognitive machinery of limited capacity. (shrink)

This paper builds on some important recent work by Amie Thomasson, wherein she argues that recent disputes about the existence of ordinary objects have arisen due to eliminiativist metaphysicians’ misunderstandings. Some, she argues, are mistaken about how the language of quantification works, while others neglect the existence and significance of certain analytic entailments. Thomasson claims that once these misunderstandings are cleared away, it is trivially easy to answer existence questions about ordinary objects using everyday empirical methods of investigation. She reveals (...) how two conflicting metaontologies can lead to different positions in the first-order debate. In this paper, I bring a third metaontological perspective to the table: one that enables us to maintain that ontological disputes about ordinary objects are not trivially easy to settle, even if we agree with Thomasson that they are merely verbal. The Holocaust marks a decisive moment in modern suffering in which it becomes almost impossible to find meaning or redemption in the experience. In this study, C. Fred Alford offers a new and thoughtful examination of the experience of suffering. Moving from the Book of Job, an account of meaningful suffering in a God-drenched world, to the work of Primo Levi, who attempted to find meaning in the Holocaust through absolute clarity of insight, he concludes that neither strategy works well (...) in today's world. More effective are the day-to-day coping practices of some survivors. Drawing on testimonies of survivors from the Fortunoff Video Archives, Alford also applies the work of Julia Kristeva and the psychoanalyst Donald Winnicot to his examination of a topic that has been and continues to be central to human experience. Анотація. У статті проаналізовані досягнення Соціального Вчительства Католицької Церкви, репрезентовані працями Лева ХІІІ, Пія ХІ, Пія ХІІ, Івана Павла ІІ, що розкривають змістовні характеристики поняття «принцип субсидіарності», його роль і значення в системі християнських цінностей. Принцип субсидіарності робить можливими такі взаємовідносини в соціальному житті, коли спільнота вищого порядку не втручається у внутрішнє життя спільноти нижчого порядку, перебираючи на себе належні тій функції; заради спільного добра, спільного блага вона надає їй у разі потреби підтримку й допомогу, узгоджуючи у такий спосіб її (...) взаємодію з іншими соціальними структурами. Принцип субсидіарності скеровує соціальну практику на утвердження спільного блага в людському співтоваристві. Поширення і застосування принципу субсидіарності протистоїть небезпеці «одержавлення» суспільства та найзагрозливіших проявів колективізму, обмежує абсолютизацію влади, бюрократизацію державних і соціокультурних структур, стає одним із гарантів дотримання прав і свобод громадян своєї країни. The development of gene-editing technologies, such as the clustered regularly interspaced short palindromic repeats and associated Cas9 endonuclease system, coincides with a rapidly expanding knowledge of the role of genes in the human ageing process. This raises the prospect that, in addition to the treatment of genetic diseases and disorders, it may become possible to use gene-editing technologies to alter the ageing process and significantly extend the maximum human lifespan. Germline editing poses distinctive problems due to its implications for individual (...) members of future, unborn generations. In this essay, I wish to home in, narrowly, on a single ethical objection to extending the lifespan of future generations by editing the human germline. The objection suggests that to extend lifespans is to unethically inflict the harm of loneliness on future people. I claim that the argument rests on assumptions that ought to be rejected. (shrink)
