In this review article we describe the current scope, methods, and contents of medical ethics education in medical schools in Western English speaking countries (mainly the United Kingdom, the United States, and Australia). We assess the strengths and weaknesses of current medical ethics curricula, and students’ levels of satisfaction with different teaching approaches and their reported difficulties in learning medical ethics concepts and applying them in clinical practice. We identify three main challenges for medical ethics education: counteracting the bad effects (...) of the “hidden curriculum,” teaching students how to apply ethical knowledge and critical thinking to real cases in clinical practice, and shaping future doctors’ right character through ethics education. We suggest ways in which these challenges could be addressed. On the basis of this analysis, we propose practical guidelines for designing, implementing, teaching, and assessing a medical ethics program within a four-year medical course. (shrink)

In making assertions one takes on commitments to the consistency of what one asserts and to the logical consequences of what one asserts. Although there is no quick link between belief and assertion, the dialectical requirements on assertion feed back into normative constraints on those beliefs that constitute one's evidence. But if we are not certain of many of our beliefs and that uncertainty is modelled in terms of probabilities, then there is at least prima facie incoherence between the normative (...) constraints on belief and the probability-like structure of degrees of belief. I suggest that the norm-governed practice relating to degrees of belief is the evaluation of betting odds. (shrink)

In this work, Alan Milne builds on the argument of his earlier book Ethical Frontiers of the State that limits on governmental action are to be understood in terms of humanistic social ethics. Here Milne considers the role of the market in politics, and in particular the relation of the market to the obligations of government to advance human wellbeing. Issues covered include contingency in politics, the command economy, capitalism, the welfare state, inequality, and representative democracy.

Desert theories of distributive justice have been attacked on the grounds that they attempt to found large inequalities on morally arbitrary features of individuals: desert is usually classified as a meritocratic principle in contrast to the egalitarian principle that goods should be distributed according to need. I argue that there is an egalitarian version of desert theory, which focuses on effort rather than success, and which aims at equal levels of well‐being; I call it a ‘well‐being desert’ theory. It is (...) argued that this egalitarian conception of desert is preferable to a meritocratic conception, and that its adoption would encourage greater clarity in arguments over wage differentials and in debates about criteria for job and educational competitions. “Readiness cohorts” are an innovation in clinical trial design to tackle the scarcity of time and people in drug studies. This has emerged in response to the challenges of recruiting the “right” research participants at the “right time” in the context of precision medicine. In this paper, we consider how the achievement of “readiness” aligns temporalities, biologies, and market processes of pharmaceutical innovation: how the promise of “willing bodies” in research emerges in relation to intertwined economic and biological time imperatives. Drawing on long-term engagement with the field of Alzheimer’s disease prevention and interviews with researchers from academia and the pharmaceutical industry, we describe the discursive construction and practical arrangement of readiness. This paper contributes to understandings of temporal specificity, or “timing,” within prevention research and casts critical light on the way this specificity—the threshold for “trial readiness”—relates to an opaque and highly speculative drug development pipeline. Extending the study of biomedical potential, as that which holds promise but may not yet exist, we consider how absences operate in adaptive trials. By highlighting these absences, we outline an opportunity for socio-ethical research to intervene in the speculative gaps of drug development. (shrink)

In natural deduction classical logic is commonly formulated by adding a rule such as Double Negation Elimination (DNE) or Classical Reductio ad Absurdum (CRA) to a set of introduction and elimination rules sufficient for intuitionist first-order logic with conjunction, disjunction, implication, negation and the universal and existential quantifiers all taken as primitive. The natural deduction formulation of intuitionist logic, coming from Gentzen, has nice properties:— (i) the separation property: an intuitionistically valid inference is derivable using only the introduction and elimination (...) rules governing the connectives and/or quantifiers that occur in the premises (if any) and conclusion; (ii) the (strict) subformula property: more narrowly, there is a derivation of any intuitionistically valid inference that employs only subformulas of the formulas occurring in premises (if any) and conclusion. Wikipedia is a goldmine of information; not just for its many readers, but also for the growing community of researchers who recognize it as a resource of exceptional scale and utility. It represents a vast investment of manual effort and judgment: a huge, constantly evolving tapestry of concepts and relations that is being applied to a host of tasks. This article provides a comprehensive description of this work. It focuses on research that extracts and makes use of the concepts, relations, (...) facts and descriptions found in Wikipedia, and organizes the work into four broad categories: applying Wikipedia to natural language processing; using it to facilitate information retrieval and information extraction; and as a resource for ontology building. The article addresses how Wikipedia is being used as is, how it is being improved and adapted, and how it is being combined with other structures to create entirely new resources. We identify the research groups and individuals involved, and how their work has developed in the last few years. We provide a comprehensive list of the open-source software they have produced. This dissertation is concerned with theories of mental content, and in particular with the relationships between a theory of content and truth and rationality. My strategy is to examine the metaphysics of various approaches to content and ask certain questions. What is it for a belief to be true on a particular theory? What is it for a thought process to be rational? Are truth and rationality useful explanatory properties on each theory? Are they useful normative or action-guiding properties? Does (...) a theory allow truth and rationality to be valuable? ;I argue that there is a useful categorization of theories of mental content in terms of the kinds of connection they make between content and truth and rationality. Some theories, most notably Donald Davidson's and other theories inspired by his work, make it a constitutive truth, and thus necessary, that any entity that has intentional states must believe mostly truths and be largely rational. This holistic theories can be contrasted with other atomistic approaches, including those championed by Jerry Fodor and others, that deny such necessary connections, and leave the question of just how much truth and falsehood can be believed, and how rational or irrational a believer can be, to be settled by empirical investigation. ;The various chapters of this dissertation examine these different types of theories, and also the teleological theory of Ruth Millikan. I argue that atomistic theories of content have certain undesirable consequences: truth and rationality will no longer be explanatorily or normatively useful properties. On the other hand, holistic theories of content that make truth and rationality necessary features of systems with intentional states face different problems, because the norms they claim are constitutive of content are unlikely to be true of evolved creatures. Teleological theories, which presume a weak, historical connection between content and truth and rationality, fare best against these tests--they make truth and rationality important parts of explanations, and provide some motivation for pursuing them. George schlesinger has characterized justified belief probabilistically. I question the propriety of this characterization and demonstrate that with respect to it certain principles of epistemic logic that he considers plausible are unsound.

The significance of big data for driving health research and improvements in patient care is well recognised. Along with these potential benefits, however, come significant challenges, including those concerning the sharing and linkage of health and social care records. Recently, there has been a shift in attention towards a paradigm of data sharing centred on the ‘trusted research environment’ (TRE). TREs are being widely adopted by the UK’s health data initiatives including Health Data Research UK (HDR UK),1 Our Future Health2 (...) and Genomics England.3 A recent review commissioned by the UK’s Department of Health and Social Care (hereafter referred to as the ‘Goldacre Review’) places TREs at the heart of its recommendations around the future of National Health Service (NHS) health data sharing for research, describing them as the ‘clear path forward’ to a health data system in which trust is ‘earned’ through ‘provable, credible steps to protect patient privacy, and by being transparent with everyone about everything that is done with their deepest medical secrets’.4 We argue that rather than building public trust, the TRE model actually reduces the need for trust in the use and sharing of patient health data. This is because trust is importantly connected to vulnerability and uncertainty; an essential part of trusting someone is accepting that one’s trust could be disappointed or betrayed. In attempting to provide assurances or guarantees of data privacy and security, TREs strive to remove this vulnerability, and so remove the need for trust. We do not see this as a problem and are broadly supportive of this kind of data sharing model because of the increased security and oversight it provides. Recent results in kinematics, obtained by myself and those working with me, have convinced me that the philosophical status of physics, as it has come down to us from Renaissance days, requires reconsideration. The reason can be stated in a couple of sentences: it has been found possible to establish certain laws of physics—laws of motion, the law of gravitation, the laws known under the name of the Lorentz transformation, and some others—purely deductively, without specific assumptions, and without empirical appeals (...) save only to the empirical awareness of something we call “the passage of time.” Yet physicists have claimed ad nauseam that the ultimate sanction for “laws of nature” is observation and experiment, that this is what distinguishes physics from other branches of exact inquiry. The contrast in these two conceptions of natural law needs no emphasis. It is the purpose of this article to attempt to clarify this confused situation. (shrink)

We argue the need for academics to resist and challenge the hegemonic discourse of sustainable development within the corporate context. Laclau and Mouffe’s discourse theory provides a useful framework for recognizing the complex nature of sustainable development and a way of conceptualizing counter-hegemonies. Published empirical research that analyzes sustainable development discourse within corporate reports is examined to consider how the hegemonic discourse is constructed. Embedded assumptions within the hegemonic construction are identified including sustainable development as primarily about economic development, progress, (...) growth, profitability, and “responsibly” managed levels of resource depletion. We call for multiple voices in the discursive field to debate and to resist closure and highlight the possibilities for academic researchers to actively resist the hegemonic construction. Specifically, we advocate vigilance and awareness, critical and reflective analyses, challenge and resistance based on other frames of reference, and strategies for communicating both within and outside the academy. I think that to Lord Kelvin is attributed the saying that the scientific attitude to a thing, if you can't do anything else with it, is to measure it. This is the attitude I propose to adopt towards Time . The situation is to some extent analogous to the situation with regard to electricity . Science is unable to say what electricity is, and so it almost denies the word any entrance into a treatise on the subject. It replaces it (...) by the word charge , which is something that can be measured. It is important to avoid hypostatizing time into a thing-in-itself. Instead of trying to say what time is we try to find a way of measuring it. (shrink)

According to the axiologist the value concepts are basic and the deontic concepts are derivative. This paper addresses two fundamental problems that arise for the axiologist. Firstly, what ought the axiologist o understand by the value of an act? Second, what are the prospects in principle for an axiological representation of moral theories. Can the deontic concepts of any coherent moral theory be represented by an agent-netural axiology: (1) whatever structure those concepts have and (2) whatever the causal structure of (...) the world happens to be. We show that the answer is "almost always". The only substantive constraint is that autonomous moral agents cannot have the power to simultaneously block the options open to other autonomous moral agents. But this seems to be part and parcel of the notion of an autonomous moral agent. (shrink)

In this piece we explore the COVID pandemic as an opportunity for the articulation and realization of digital health futures. Our discussion draws on an engagement with emergent discourse around COVID-19 and ongoing work on imaginaries of future care associated with digital tools for the detection of cognitive decline and the risk of dementia. We describe how the post-COVID futures of digital health are narrated in terms of the timing and speed with which they are being brought into being, as (...) market actors attempt to establish the scale and durability of the COVID transformation. However, we also point to the particularly spatial changes to medical practice they envisage. In a time of distancing and isolation, the ability to operate effectively at a distance has become integral to the future of medical assessment, diagnosis and care. However, spatialized promises of digital health and the ability to act remotely are unevenly spread – some organizations and entities inevitably have greater reach. (shrink)

This paper offers a critique of sustainability reporting and, in particular, a critique of the modern disconnect between the practice of sustainability reporting and what we consider to be the urgent issue of our era: sustaining the life-supporting ecological systems on which humanity and other species depend. Tracing the history of such reporting developments, we identify and isolate the concept of the ‘triple bottom line’ (TBL) as a core and dominant idea that continues to pervade business reporting, and business engagement (...) with sustainability. Incorporating an entity’s economic, environmental and social performance indicators into its management and reporting processes, we argue, has become synonymous with corporate sustainability; in the process, concern for ecology has become sidelined. Moreover, this process has become reinforced and institutionalised through SustainAbility’s biennial benchmarking reports, KPMG’s triennial surveys of practice, initiatives by the accountancy profession and, particularly, the Global Reporting Initiative (GRI)’s sustainability reporting guidelines. We argue that the TBL and the GRI are insufficient conditions for organizations contributing to the sustaining of the Earth’s ecology. Paradoxically, they may reinforce business-as-usual and greater levels of un-sustainability. (shrink)

We study companies that do not produce a sustainability report in contexts where institutionalisation is assumed. Based on a careful analysis of interaction patterns between non-reporting companies, sustainability interest groups, and peer organisations, we find patterns of discursive and material isomorphism that suggest sustainability reporting is confined to an issues-based field, rather than spreading as an institutionalised practice across the business community. We argue that the issues-based field exerts only weak pressure for sustainability reporting, and that encouraging more firms to (...) report rests on understanding what influences companies to interact more widely to become part of this field. This article begins by outlining some of the history—beginning with brief remarks of Quine's—of work on conditional assertions and conditional events. The upshot of the historical narrative is that diverse works from various starting points have circled around a nexus of ideas without convincingly tying them together. Section 3 shows how ideas contained in a neglected article of de Finetti's lead to a unified treatment of the topics based on the identification of conditional events as the objects of conditional bets.
