BackgroundAccurate reporting on sex and gender in health research is integral to ensuring that health interventions are safe and effective. In Canada and internationally, governments, research organizations, journal editors, and health agencies have called for more inclusive research, provision of sex-disaggregated data, and the integration of sex and gender analysis throughout the research process. Sex and gender analysis is generally defined as an approach for considering how and why different subpopulations (e.g., of diverse genders, ages, and social locations) may experience (...) health conditions and interventions in different or similar ways.The objective of this study was to assess the extent and nature of reporting about sex and/or gender, including whether sex and gender analysis (SGA) was carried out in a sample of Canadian randomized controlled trials (RCTs) with human participants.MethodsWe searched MEDLINE from 01 January 2013 to 23 July 2014 using a validated filter for identification of RCTs, combined with terms related to Canada. Two reviewers screened the search results to identify the first 100 RCTs that were either identified in the trial publication as funded by a Canadian organization or which had a first or last author based in Canada. Data were independently extracted by two people from 10% of the RCTs during an initial training period; once agreement was reached on this sample, the remainder of the data extraction was completed by one person and verified by a second.ResultsThe search yielded 1433 records. We screened 256 records to identify 100 RCTs which met our eligibility criteria. The median sample size of the RCTs was 107 participants (range 12–6085). While 98% of studies described the demographic composition of their participants by sex, only 6% conducted a subgroup analysis across sex and 4% reported sex-disaggregated data. No article defined “sex” and/or “gender.” No publication carried out a comprehensive sex and gender analysis.ConclusionsFindings highlight poor uptake of sex and gender considerations in the Canadian RCT context and underscore the need for better articulated guidance on sex and gender analysis to improve reporting of evidence, inform policy development, and guide future research. What I would add, and what Reichert seems unable to see, is that the facts of the text do not identify themselves. He faults Roskill for failing to see that coherence is not a function of the text but of "principles we bring to the text"; yet he himself does not see that the text, insofar as one can point to it, is produced by those same principles. Indeed, Reichert is continually doing the very thing for which he criticizes Roskill, (...) attributing to the text qualities and features that are the product of interpretive strategies. Thus, for example, he cites the instance of "the interpreter . . . noticing something in the text that makes his former reading seem implausible" as evidence that the text is at some level independent of interpretation; but noticeability is a function of what it is possible to notice given a particular set of assumptions: a reader innocent of the principles of typology would be incapable of "noticing" a typological pattern, whereas for a reader like Madsen, the pattern will seem to announce itself; and a reader who "notices" something he didn't "notice" before is a reader who is proceeding within a different set of interpretive assumptions. That which is noticeable, in short, can never be the means of confirming or constraining interpretations because it is always a product of one. The same argument dissolves the distinction, invoked by Reichert, between extratextual and textual evidence; it is not that such a distinction is never in force but that what counts as internal and external evidence will vary according to the interpretive principles one espouses. Just what is and what is not extratextual is a matter of continual debate, and when the debate has been concluded, it is not because the matter has been settled by the facts but because one set of interpretive principles has won the right to say what the facts are. There is no agreed minimum standard with regard to what is considered safe, competent nursing care. Limited resources and organizational constraints make it challenging to develop a minimum standard. As part of their everyday practice, nurses have to ration nursing care and prioritize what care to postpone, leave out, and/or omit. In developed countries where public healthcare is tax-funded, a minimum level of healthcare is a patient right; however, what this entails in a given patient’s actual situation is unclear. Thus, (...) both patients and nurses would benefit from the development of a minimum standard of nursing care. Clarity on this matter is also of ethical and legal concern. In this article, we explore the case for developing a minimum standard to ensure safe and competent nursing care services. Any such standard must encompass knowledge of basic principles of clinical nursing and preservation of moral values, as well as managerial issues, such as manpower planning, skill-mix, and time to care. In order for such standards to aid in providing safe and competent nursing care, they should be in compliance with accepted evidence-based nursing knowledge, based on patients’ needs and legal rights to healthcare and on nurses’ codes of ethics. That is, a minimum standard must uphold a satisfactory level of quality in terms of both professionalism and ethics. Rather than being fixed, the minimum standard should be adjusted according to patients’ needs in different settings and may thus be different in different contexts and countries. (shrink)

The authors of the preceding articles1 raise legitimate questions about patient and staff rights and the unintended consequences of allowing ABC News to film inside teaching hospitals. We explain why we regard their fears as baseless and not supported by what we heard from individuals portrayed in the filming, our decade-long experience making medical documentaries, and the full un-aired context of the scenes shown in the broadcast. The authors don’t and can’t know what conversations we had, what documents we reviewed, (...) and what protections we put in place in each televised scene. Finally, we hope to correct several misleading examples cited by the authors as well as their offhand mischaracterization of our program as a “reality” show. (shrink)

We describe a broad class of problems, called “uncompromising problems,” which are characterized by the requirement that solutions must perform optimally on each of many test cases. Many of the problems that have long motivated genetic programming research, including the automation of many traditional programming tasks, are uncompromising. We describe and analyze the recently proposed “lexicase” parent selection algorithm and show that it can facilitate the solution of uncompromising problems by genetic programming. Unlike most traditional parent selection techniques, lexicase selection (...) does not base selection on a fitness value that is aggregated over all test cases; rather, it considers test cases one at a time in random order. We present results comparing lexicase selection to more traditional parent selection methods, including standard tournament selection and implicit fitness sharing, on four uncompromising problems: 1) finding terms in finite algebras; 2) designing digital multipliers; 3) counting words in files; and 4) performing symbolic regression of the factorial function. We provide evidence that lexicase selection maintains higher levels of population diversity than other selection methods, which may partially explain its utility as a parent selection algorithm in the context of uncompromising problems. The author continues the preceding exchange of views on the nature of metaphor with the American philosopher Nelson Goodman that has been published in 'Poetics Today'. He offers his own theory of metaphor, which assumes a middle ground between Goodman's semantic theory and Davidson's pragmatic conception. The article also presents another argument in support of the thesis that there are only true metaphors, and that the logical space assumed by Goodman for the metaphorical falsehood is already reserved for the category (...) of statements that are nonsensical. This “Legal Briefing” column covers recent legal developments involving patient decision aids. This topic has been the subject of recent articles in JCE. It is included in the 2010 Patient Protection and Affordable Care Act. And it has received significant attention in the biomedical literature, including a new book, a thematic issue of Health Affairs, and a recent article in the New England Journal of Medicine. Moreover, physicians and health systems across the United States are increasingly integrating decision aids into (...) their clinical practice. Both federal and state laws play a significant role in promoting this expanded use. On the other hand, concerns about liability could stymie development and implementation. We categorize legal developments concerning patient decision aids into the following five sections:1. Development of decision aids2. Effectiveness of decision aids3. As ethical attitudinists say, ethical statements cannot be strictly true or false, since they express wishes or attitudes, not beliefs. However, the wishes expressed by basic moral judgments about human rights are such that it is a necessary truth that those who know what human beings are have them, and those who do not acknowledge these rights show their lack of a living sense of human reality. The same goes for basic judgments about the rights of animals, and it is (...) blindness about the ontology of animal reality which lies behind the cruelty to animals inherent in vivisection and factory farming. One current source of this blindness may be the physicalist metaphysics which is typical of our day, and which should be sharply distinguished from metaphysical naturalism. (shrink)
