While pharmaceutical industry involvement in producing, interpreting, and regulating medical knowledge and practice is widely accepted and believed to promote medical innovation, industry-favouring biases may result in prioritizing corporate profit above public health. Using diabetes as our example, we review successive changes over forty years in screening, diagnosis, and treatment guidelines for type 2 diabetes and prediabetes, which have dramatically expanded the population prescribed diabetes drugs, generating a billion-dollar market. We argue that these guideline recommendations have emerged under pervasive industry (...) influence and persisted, despite weak evidence for their health benefits and indications of serious adverse effects associated with many of the drugs they recommend. We consider pharmaceutical industry conflicts of interest in some of the research and publications supporting these revisions, and in related standard-setting committees and oversight panels. We raise concern over the long-term impact of these multifaceted involvements. Rather than accept industry conflicts of interest as normal, needing only to be monitored and managed, we suggest challenging that normalcy, and ask: what are the real costs of tolerating such industry participation? We urge the development of a broader focus to fully understand and curtail the systemic nature of industry’s influence over medical knowledge and practice. (shrink)

The 1964 theorem of John Bell shows that no model that reproduces the predictions of quantum mechanics can simultaneously satisfy the assumptions of locality and determinism. On the other hand, the assumptions of signal locality plus predictability are also sufficient to derive Bell inequalities. This simple theorem, previously noted but published only relatively recently by Masanes, Acin and Gisin, has fundamental implications not entirely appreciated. Firstly, nothing can be concluded about the ontological assumptions of locality or determinism independently (...) of each other—it is possible to reproduce quantum mechanics with deterministic models that violate locality as well as indeterministic models that satisfy locality. On the other hand, the operational assumption of signal locality is an empirically testable (and well-tested) consequence of relativity. Thus Bell inequality violations imply that we can trust that some events are fundamentally unpredictable, even if we cannot trust that they are indeterministic. This result grounds the quantum-mechanical prohibition of arbitrarily accurate predictions on the assumption of no superluminal signalling, regardless of any postulates of quantum mechanics. It also sheds a new light on an early stage of the historical debate between Einstein and Bohr. (shrink)

As the technician inserted the rather ominous cotton-tipped probe into my nostril, she told me that it was going to feel as if she were tickling my brain. Indeed… This experience, shared by many during the past three years, and likely multiple times, prompted me to think about my nose. Not since cocaine reentered American mainstream culture (...) in the 1970s and '80s—and found its way into countless movies that reflect the trend—has the nose been so central to a certain state of public life. It's difficult to find a film dealing with American culture from that period and beyond, from Scarface to The Wolf of Wall Street, in which there is not a scene depicting characters snorting the obligatory line or spoon of cocaine. Yes, snorting, sniffing, sneezing, ingesting the line through the nose and finishing leftover dust by collecting it with a moist finger, rubbing it lovingly against the front gum of the mouth. Freud's "Über Coca" looms ominously in the background, the work of a phase in Freud's professional career complicated by his obscure relationships with Parke-Davis (now a subsidiary of Pfizer) and Merck, pharmaceutical companies that supplied him with early samples to tickle his nose (Freud; Merkel). The Sacklers of the moment. In our day, Pfizer wants to transform our noses into defenses against the virus rather than conduits toward mind-altering dreams. In the wake of the Freud and William Halsted cocaine addictions chronicled by Howard Merkel, the irony of the contemporary link between Pfizer and the nose is patent.The nose: that orifice through which the virus creeps, the first stage in breathing, the first filter for air-borne pollutants. Recently, important immunological analyses of COVID have focused on the question of viral load, how much virus is necessary to overcome the nose's filtering function and reach the deeper airways leading to the lungs. The processes triggered by any of the major vaccines prepare antibody blueprints and actual antibodies that can be dispatched to the linings of the nose to counter the virus as soon as the body detects its presence. The Delta, Omicron, and subsequent variants remind us that the virus has mechanisms to challenge this response, namely, increased viral load that intensifies transmissibility and the potential to overwhelm antibodies. It seems, however, that [End Page 231] so-called breakthrough infections are still well controlled by the body's vaccinated immune system. Hysteria about the dangers of such incursions is partially a product of biased and often incorrect or incomplete media coverage of the COVID vaccine, which has terrified the vaccinated and fueled the skepticism of the unvaccinated.The virus tries to overpower and confuse the filtering nose in another way, more immediately apparent to our conscious selves. As many as eighty-five percent of COVID infections result in the loss of smell, generally recovered in about two to three weeks in sickened people who overcome the infection. About five percent of COVID victims, however, have not recovered their sense of smell six months or more after they have shed the virus.1 What might it mean to lose one's sense of smell? In our vision-oriented culture, both sound (the ear) and smell (the nose) have been less recognized as primary defense mechanisms than in other cultures. When one hears a sound, the first reaction is to prepare for a threat, before the sonic source is seen or understood. Orientation in the world, knowing from which direction the sound emanates, is key to preparing the body for an encounter with an unknown phenomenon: "To sound (an alarm) means to warn. Isn't any emerging sound understood as a warning? Any unexpected sonic occurrence is seen as an indication of something that is to come and that must be clarified rapidly" (Deshays 34). By the same token, to sample surroundings by sniffing what is in the air signals whether one should inhale or somehow try to hold one's breath in search of cleaner, more palatable air. We first examine Howard's analysis of the Bell factorizability condition in terms of 'separability' and 'locality' and then consider his claims that the violations of Bell's inequality by the statistical predictions of quantum mechanics should be interpreted in terms of 'nonseparability' rather than 'nonlocality' and that 'nonseparability' implies the failure of spacetime as a principle of individuation for quantum-mechanical systems. We will argue that his argument for the first claim is less than compelling and that any argument (...) for the second claim will be interpretation-dependent and, hence, not generally valid. In the context of stochastic hidden variable theories, Howard has argued that the role of separability—spatially separated systems possess distinct real states—has been underestimated. Howard claims that separability is equivalent to Jarrett‘s completeness: this equivalence should imply that the Bell theorem forces us to give up either separability or locality. Howard's claim, however, is shown to be ill founded since it is based on an implausible assumption. The necessity of sharply distinguishing separability and locality is emphasized: (...) a quantitative formulation of separability, due to D'Espagnat, is reviewed and found unsatisfactory, in that it basically conflates separability and locality in a single notion. This paper addresses arguments that “separability” is an assumption of Bell’s theorem, and that abandoning this assumption in our interpretation of quantum mechanics (a position sometimes referred to as “holism”) will allow us to restore a satisfying locality principle. Separability here means that all events associated to the union of some set of disjoint regions are combinations of events associated to each region taken separately.In this article, it is shown that: (a) localised events can be consistently defined without implying (...) separability; (b) the definition of Bell’s locality condition does not rely on separability in any way; (c) the proof of Bell’s theorem does not use separability as an assumption. If, inspired by considerations of non-separability, the assumptions of Bell’s theorem are weakened, what remains no longer embodies the locality principle. Teller’s argument for “relational holism” and Howard’s arguments concerning separability are criticised in the light of these results. Howard’s claim that Einstein grounded his arguments on the incompleteness of QM with a separability assumption is also challenged. Instead, Einstein is better interpreted as referring merely to the existence of localised events. Finally, it is argued that Bell rejected the idea that separability is an assumption of his theorem. No word is more central to the contemporary political imagination and action than ‘resistance'. In its various manifestations - from the armed guerrilla to Gandhian mass pacifist protest, from Wikileaks and the Arab Spring to the global eruption and violent repression of the Occupy movement - concepts of resistance are becoming ubiquitous and urgent. THE CLOSE CONNECTION BETWEEN mathematics and philosophy has long been recognized by practitioners of both disciplines. The apparent timelessness of mathematical truth, the exactness and objective nature of its concepts, its applicability to the phenomena of the empirical world— explicating such facts presents philosophy with some of its subtlest problems. Let me begin by reminding you of some celebrated past attempts made by philosophers and mathematicians to explicate the nature of mathematics.

In this volume, thirteen distinguished contributors from the United States, Canada, Britain, and Germany cast light on important aspects of Kant's liberal thinking.

It is commonly claimed that Kant's critical philosophy aims to limit reason's speculative use and its metaphysical pretensions. This paper argues that such claims should be amended in light of a technical distinction between negative limits and positive boundaries that Kant held throughout his career. Kant's only extended discussion of this distinction appears in §§57–60 of the Prolegomena, a division entitled “On pure reason's boundary‐determination”. I examine these sections in detail in order to elucidate the account of the limits and (...) boundaries of the activity of different cognitive faculties in the Critique of Pure Reason. I argue that once we understand a key ambition of the Critique to be the positive determination of the boundary between objects of experience and things in themselves, we gain insight into the essential contribution that Kant's theory of the positive utility of ideas of reason makes to the critical philosophy. Abstract:Vulnerability is an important criterion to assess the ethical justification of the inclusion of participants in research trials. Currently, vulnerability is often understood as an attribute inherent to a participant by nature of a diagnosed condition. Accordingly, a common ethical concern relates to the participant’s decisionmaking capacity and ability to provide free and informed consent. We propose an expanded view of vulnerability that moves beyond a focus on consent and the intrinsic attributes of participants. Despite establishing the gendered construction of infertility, most research on the subject has not examined how individuals with such reproductive difficulty negotiate their own sense of gender. I explore this gap through 58 interviews with women who are medically infertile and involuntarily childless. In studying how women achieve their gender, I reveal the importance of the body to such construction. For the participants, there is not just a motherhood mandate in the United States, but a fertility mandate—women are not just (...) supposed to mother, they are supposed to procreate. Given this understanding, participants maintain their gender by denying their infertile status. They do so through reliance on essentialist notions, using their bodies as a means of constructing a gendered sense of self. Using the tenets of transgender theory, this study not only informs our understanding of infertility, but also our broader understanding of the relationship between gender, identity, and the body, exposing how individuals negotiate their gender through physical as well as institutional and social constraints. The problem of the ‘Unity of the Proposition’ is the problem of explaining the difference between a content-expressing declarative sentence and a ‘mere list’ of referents. The prevailing view is that such a problem is to be solved metaphysically, either by reducing our ontology to exclude propositions or universals, or by explaining how it is possible for a certain kind of complex entity – the ‘proposition’– to ‘unify’ its constituents. I argue that these metaphysical approaches cannot succeed; instead the only (...) viable approach is linguistic, identifying features of the (type–) sentence itself that enable it to express a content. Thus the problem of the ‘Unity of the Proposition’ (distinguishing sentences from lists) is distinct from the problem of ‘propositional unity’ (explaining how the constituents of propositions form a unified content). I suggest that, while the latter problem is not pressing, the former does not permit of an answer which applies in generality in all languages; we can only fully explain the Unity of the Proposition for single languages or groups of similar languages. This is a collection of articles which represents current research on the metaphysics of causation and laws of nature, mostly by authors working in or active in the Australasian region. The book provides an overview of current work on the theory of causation, including counterfactual, singularist, nomological and causal process approaches. It also covers work on the nature of laws of nature, with special emphasis on the scientific essentialist theory that laws of nature are, at base, the fundamental dispositions or (...) capacities of natural kinds of things. Because the book represents a good cross-section of authors currently working on these themes in the Australasian region, it conveys something of the interest and excitement of an active philosophical debate between advocates of several different research programmes in the area. In the article “Nudging and Informed Consent”, Cohen argues that the use of “nudging” by physicians in the clinical encounter may be ethically warranted because it results in an informed consent where obligations for beneficence and respect for autonomy are both met. However, the author's overenthusiastic support for nudging and his quick dismissal of shared decision-making leads him to assume that “soft” manipulation is un-problematic and that “wisdom” on the side of medical professionals will suffice to guard against abuse. Opposing (...) this view, we propose that: 1) patient preferences and values should not be boiled down to rational or irrational decisions based on cognitive reasoning alone, 2) potential threats to the physician-patient must be mitigated and could be minimized by employing alternative strategies that acknowledge the impact of cognitive heuristics in shaping decision-making but maintain transparency and trust, and 3) patient preferences and values can be legitimately influenced only under specific circumstances. The latter include: (1) exhaustion of other means; (2) evidence that nudging is effective in a specific context; (3) evidence of clear and active public misinformation (e.g., industry promoting smoking); (4) explicit pubic dialogue between various stakeholders including medical professional societies about nudging; (5) upholding shared decision-making as a preferred approach; and (6) a rich understanding of clinical reality beyond the simple dichotomy of beneficence-autonomy. This book is the product of a multi-year dialogue between leading human rights theorists and high-level representatives of international human rights NGOs. It is divided into three parts that reflect the major ethical challenges discussed at the workshops: the ethical challenges associated with interaction between relatively rich and powerful northern-based human rights INGOs and recipients of their aid in the South; whether and how to collaborate with governments that place severe restrictions on the activities of human rights INGOs; and the (...) tension between expanding the organization's mandate to address more fundamental social and economic problems and restricting it for the sake of focusing on more immediate and clearly identifiable violations of civil and political rights. A review of the philosophical debate on theoretical models for the physician-patient relationship over the past fifteen years may point to some of the more productive questions for future research. Contractual models have been criticized for promoting a legalistic and minimalistic image of the relationship, such that another form of model (such as convenant) is required. Shifting from a contractual to a contractarian model (in keeping with Rawls' notion of an original position) provides an adequate response to many criticisms of (...) this type. A deeper criticism, however, is one that advocates a shift to a virtue-based approach. A creative amalgam between a contractarian model and elements of the virtue-based approach, combined with appropriate empirical investigation, may yield richer models in the future. (shrink)
