Quality health care relies upon communication in a patient's preferred language. Language access in health care occurs when individuals are: Welcomed by providers regardless of language ability; and Offered quality language services as part of their care. Federal law generally requires access to health care and quality language services for deaf and Limited English Proficient patients in health care settings, but these patients still find it hard to access health care and quality language services.Meanwhile, several states are implementing Medicaid Accountable (...) Care Organization initiatives to reduce health care costs and improve health care quality. Alternative payment methods used in these initiatives can give Accountable Care Organizations more flexibility to design linguistically accessible care, but they can also put ACOs at increased financial risk for the cost of care. If these new payment methods do not account for differences in patient language needs, ACO initiatives could have the unintended consequence of rewarding ACOs who do not reach out to deaf and LEP communities or offer quality language services.We reviewed public documents related to Medicaid ACO initiatives in six states. Some of these documents address language access. More could be done, however, to pay for language access efforts. This article describes Medicaid ACO initiatives and explores how different payment tools could be leveraged to reward ACOs for increased access to care and quality language services. We find that a combination of payment tools might be helpful to encourage both access and quality. (shrink)

The break necessary to establish a rigorous science of cultural works is something more and something else than a simple methodological reversal.1 It implies a true conversion of the ordinary way of thinking and living the intellectual enterprise. It is a matter of breaking the narcissistic relationship inscribed in the representation of intellectual work as a “creation” and which excludes as the expression par excellence of “reductionist sociology” the effort to subject the artist and the work of art to a (...) way of thinking that is doubly objectionable since it is both genetic and generic.It would be easy to show what the most different kinds of analysis of the work of art owe to the norms that require treating works in and for themselves, with no reference to the social conditions of their production. Thus in the now-classic Theory of Literature, René Wellek and Austin Warren seem to advocate “an explanation in terms of the personality and the life of the writer.” In fact, because they accept the ideology of the “man of genius” they are committed, in their own terms, to “one of the oldest and best-established methods of literary study”—which seeks the explanatory principle of a work in the author taken in isolation .2 In fact, this explanatory principle resides in the relationship between the “space” of works in which each particular work is taken and the “space” of authors in which each cultural enterprise is constituted. Similarly, when Sartre takes on the project of specifying the meditations through which society determined Flaubert, the individual, he attributes to those factors that can be perceived from that point of view—that is, to social class as refracted through a family structure—what are instead the effects of generic factors influencing every writer in an artistic field that is itself in a subordinate position in the field of power and also the effects specific to all writers who occupy the same position as Flaubert within the artistic field. 1. The concept of exploitation brings many of our ordinary moral intuitions into conflict. Exploitation—or to use the commonly accepted ordinary language definition, taking unfair advantage—is often thought to be morally impermissible. In order to be permissible, transactions must not be unfair. The claim that engaging in mutually beneficial transactions is morally better than not transacting is also quite compelling. However, when combined with the claim that morally permissible transactions are better than impermissible transactions, these three imply the counterintuitive claim that (...) it is obligatory to engage in mutually beneficial transactions. In this paper I outline the conditions that comprise this ‘paradox of exploitation’ along with a solution that involves replacing one of the original conditions with a condition I call Weak Non-worseness. The solution captures the priority of our concerns about exploitation by making a concern for the fairness of a transaction subsidiary to a concern for the welfare of the would-be exploited. Globalization generates new structures of human interdependence and vulnerability while also posing challenges for models of democracy rooted in territorially bounded states. The diverse phenomena of globalization have stimulated two relatively new branches of political theory: theoretical accounts of the possibilities of democracy beyond the state; and comparative political theory, which aims at bringing non-Western political thought into conversation with the Western traditions that remain dominant in the political theory academy. This article links these two theoretical responses to globalization by (...) showing how comparative political theory can contribute to the emergence of new global “publics” around the common fates that globalization forges across borders. Building on the pragmatist foundations of deliberative democratic theory, it makes a democratic case for comparative political theory as an architecture of translation that helps deliberative publics grow across boundaries of culture. In _Ontological Terror_ Calvin L. Warren intervenes in Afro-pessimism, Heideggerian metaphysics, and black humanist philosophy by positing that the "Negro question" is intimately imbricated with questions of Being. Warren uses the figure of the antebellum free black as a philosophical paradigm for thinking through the tensions between blackness and Being. He illustrates how blacks embody a metaphysical nothing. This nothingness serves as a destabilizing presence and force as well as that which whiteness defines itself against. Thus, the function (...) of blackness as giving form to nothing presents a terrifying problem for whites: they need blacks to affirm their existence, even as they despise the nothingness they represent. By pointing out how all humanism is based on investing blackness with non-Being—a logic which reproduces antiblack violence and precludes any realization of equality, justice, and recognition for blacks—Warren urges the removal of the human from its metaphysical pedestal and the exploration of ways of existing that are not predicated on a grounding in Being. In a review of a book by the British idealist, A. E. Taylor, some years ago, C. D. Broad commented: “What of the nightmarish appearance, stupid perseveration and meaningless fecundity in organic nature? If the teleologist would consider the ways of the locust and the lemming, he would be a sadder and perhaps a wiser man.” Of course, others besides idealists are teleologists, but in the idealist tradition since Plato, the question of overall teleology has been a fundamental one. It (...) may be that idealists have a peculiar problem in trying to reconcile apparent dysteleological facts with their overall belief in purpose. The question is a basic one. It may also be regarded as a watershed dividing Eastern idealists, especially Indian, from their Western counterparts. (shrink)

No doubt taking his clue from a book published by Friedrich Paulsen under the title Philosophia Militans, Albert C. Knudson placed a chapter in his memorable history of personalistic idealism called “Militant Personalism”. And he raised by that very title, as Paulsen had earlier, the question of the actual forcefulness of philosophical ideas on history and society. Another book, issued three years after Knudson’s, was called Behaviorism: A Battle Line. This volume of collected essays, edited by W. P. King, made (...) the point that behaviorism as a philosophy of mind had such serious negative implications for human beings and the social order that it must be vigorously opposed and literally fought against. Idealistically oriented thinkers have persistently fought against any tendencies on the part of diverse philosophies to interpret or explain the fact of self-experience in terms of something less than the self knows itself to be. But this insistence on the centrality of the knowing subject carries with it the obligation to explain not only what that knowing subject is but why it is central and why one must in some way begin with it in his philosophical explorations. The need for (...) such an explanation is laid upon the idealist by those who insist that all explanations, all philosophizing, must begin, rather, with the observable objective facts of the natural sciences and the “external order.” But those who dote so much on alleged “objective” facts then have to try to squeeze the claims of the self-conscious subject into some explanatory formula which accounts for those data causally and views the self as some kind of a product of something more basic ontologically. Is a thinker unalterably “subjectivistic” if he insists on the epistemic centrality of the knower? Is that merely another form of “foundationalism”? Of course, there are other philosophical alternatives, but can idealistic methodology be characterized wholly by the title of Quentin Lauer’s book on Husserl, The Triumph of Subjectivity? (shrink)

The present essay describes how one small, rural, Canadian town relied on its community sprit to support locals affected by the current pandemic. Such spirit might increasingly attract people, now working from home in large cities, who seek a sense of community beyond what work offers. The attraction could bring new life to small towns.

Rankings of countries by perceived corruption have emerged over the past decade as leading indicators of governance and development. Designed to highlight countries that are known to be corrupt, their objective is to encourage transparency and good governance. High rankings on corruption, it is argued, will serve as a strong incentive for reform. The practice of ranking and labeling countries "corrupt," however, may have a perverse effect. Consistent with Social Labeling Theory, we argue that perceptual indices can encourage the loss (...) of needed investment and, thus, contribute to higher rates of corruption within unfavorably ranked countries. In effect, corruption indices may inhibit foreign direct investment, the effect of which is to encourage the status quo in terms of corruption ranking. Using an experimental study design, we test the effects of country corruption rankings on the assessment of country investment desirability and find ranking exposure causes shifts in country investment desirability for 10 of 12 countries studied. These findings suggest that corruption rankings, which are based on perceptions of corruption, may cause country isolation and a reduction in legitimate means of investment. (shrink)

This article explores the concept of blame in organizations. Existing work suggests that ‘no-blame’ approaches may be conducive to organizational learning and may foster innovation. However, both the apparently strong public appetite for blaming, and research into no-blame approaches, suggest that wider application of ‘no-blame’ in organizations may not be straightforward. The article explores the contribution of the rich philosophical literature on blame to this debate, and considers the implications of philosophical ideas for the no-blame idea. In doing so, it (...) identifies conceptual and practical issues, sheds light on why the benefits of ‘no-blame’ may be difficult to realize, and offers the basis for an alternative approach. The article also contributes by providing foundations for future research, and identifies some fruitful lines of enquiry. (shrink)

There has developed since around the end of the Second World War, a body of writing that has become known as the Philosophy of Technology. This writing now includes work by those who can be identified as "seminal thinkers," as well as developments of their original ideas, and fresh work and commentary on specific issues, to produce what is now a quite wide ranging body of literature. This chapter is primarily a descriptive one that summarizes the general field with an (...) eye to its significance for teaching and teachers, highlighting some perennial issues in educational thought and practice that the literature of Philosophy of Technology invites us to revisit. We are, however, also invited to reconsider a perennial debate in education, one that originates in the dialogues between Socrates and the Sophists of Classical Greece and which can be understood in terms of many of the themes of Philosophy of Technology, and of this, volume. Moreover, it suggests why that debate must be re-engaged in societies governed by advanced technology, and how the activity of teaching and the role of the teacher as a "reflective practitioner" are critical factors in that re-engagement. Thus, description provides the background for both analytic and critical dimensions of the Chapter. (shrink)

In this paper we examine the weaknesses of development strategies which have been applied in Puerto Rico. The process of industrialization by invitation, referred to as Operation Bootstrap, was instituted by the United States of America by the end of the 1940s. This involved tax incentives and subsidies for companies and was dependent on industrial peace and low wages in labor-intensive, low-wage industries, especially those of textile and clothing. Naturally, women's labor was encouraged as a result of the lower cost, (...) as well as assumed dexterity, of the female in such areas. While these new activity areas for women also allowed other benefits in the form of legislation and increased social services, the inherent problems of rapid, labor-intensive industrialization also led to displacement and increased underemployment and impoverization of female headed families from the 1960s onwards. The paper explores some of the changes in gender relations which resulted from these policies and looks at the challenges which the feminist movement in Puerto Rico has made, particularly with regard to state processes to bring about beneficial changes in the economic, legal, political and social status of women in Puerto Rico. At a time when there is general speculation about civilization, or civilizations, as well as on what the relationships are between Western and other civilizations, it is logical to try to define precisely what the men of ancient Greece thought about the question, since Western civilization owes so much to them.It may be that they did not think about it at all, or at least they thought nothing that could be expressed in modern terms. The fact is that ancient Greece (...) did not have a word for civilization: the French and English words come from Latin, not from Greek, while the first examples given in French and English dictionaries are the privileged example of Greece, “the civilization of Greece.” Did the Greeks use a paraphrase? Or a different word? If we consult a English-Greek dictionary we are surprised to see that the word given for “civilized” is hèmeros) that is, “docile,” “tamed.”. (shrink)

Genetic analysis of Drosophila has shown that a morphogenetic gradient of the Transforming Growth Factor‐β family member dpp patterns the embryonic dorsalventral axis. Molecular and embryological evidence from Xenopus has strongly suggested a similar role for Bmp‐4, the dpp homolog, in patterning the dorsalventral axis of chordates. A recent report has now identified mutations in two genes, dino and swirl, that disrupt dorsal‐ventral patterning in the zebrafish Danio rerio(1). Characterization of these mutations parallels findings from Drosophila, thus establishing a genetic (...) framework for the analysis of dorsalventral patterning in a vertebrate. (shrink)

Government, media, and medical accounts of Emma Goldman converged to create her public presence in the U.S. as a "dangerous individual." The prevailing discourses constituted Goldman as violent, utilizing her alleged menace to distract attention from far more egregious violence against labor by state and corporate forces. Goldman responded by denying, confronting, and redirecting the alarmed gaze toward greater risks left underarticulated in hegemonic accounts. Goldman's bold confrontations with authorities constituted a kind of anarchist parrhesia, fearless speech, a relentless truth-telling (...) practice that risked her own security in pursuit of her "beautiful ideal." The labor of remembering America's history of class violence hones our attention to the complex discursive processes by which some historical facts come to count in prevailing narratives, while others fade into obscurity.
