Dual antiviral therapy with pegylated interferon-2a and ribavirin only reaches sustained virological response rates of ~30% after liver transplantation. With the approval of viral NS3/4A (...) protease inhibitors telaprevir, boceprevir, and simeprevir and the NS5B polymerase inhibitor sofosbuvir, combination therapy offers new therapeutic options for HCV-infected patients, resulting in considerably higher sustained virological response rates in the nontransplant setting. Case presentation: We report three cases of TVR-based triple antiviral therapy in HCV genotype 1 reinfected patients after liver transplantation, of whom a 57-year-old Caucasian female and a 43-year-old Caucasian male were therapy naïve, and a 49-year-old Caucasian male patient was pretreated ineffectively. After 4 weeks of therapy, viral load decreased one to three log10 and became negative in weeks 6 to 8 in the therapy naïve patients. The pretreated patient showed a negative viral load in week 4. TVR was administered over 12 weeks, 750 mg thrice daily. Doses of immunosuppression with cyclosporine were reduced four to six fold. Initial peg-IFN and RBV doses ranged from 135–180 µg/week and 800–1,200 mg/day, according to the patient's body weight. Doses of peg-IFN and RBV were adapted to 90–135 µg/week and 400–800 mg/day after 2 to 12 weeks of protease inhibitor therapy. Dual therapy was continued for 36 weeks with total treatment duration of 48 weeks in the therapy naïve patients leading to a sustained virological response 12 weeks after the end of therapy. In the pretreated patient a breakthrough was detected in week 24 and therapy was discontinued. Overall, antiviral therapy was well tolerated. Side effects included dysgeusia and anemia leading to erythropoietin application and blood transfusions. Conclusion: This case series emphasizes that triple therapy with TVR is an efficient treatment for therapy naïve HCV genotype 1 reinfected patients after liver transplantation. Foucault argues that in modern Western societies, the political community and, more specifically, politics itself, has been the space of a war waged by other means. The dilemma of whether and how to disclose a diagnosis of cancer or of any other terminal illness continues to be a subject of worldwide interest. We present the case of a 62-year-old Japanese woman afflicted with advanced gall bladder cancer who had previously expressed a preference not to be told a diagnosis of cancer. The treating physician revealed the diagnosis to the family first, and then told the patient: "You don't have any cancer yet, but if we don't (...) treat you, it will progress to a cancer". In our analysis, we examine the role of family consent, communication patterns (including ambiguous disclosure), and advance directives for cancer disclosure in Japan. Finally, we explore the implications for Edmund Pellegrino's proposal of "something close to autonomy" as a universal good. (shrink)

The bilingual Philoxenus Glossary drew some of its materials from Festus de Signif. Verb. and occasionally mentions his name. Its Festus glosses have been collected in a Jena dissertation by Dammann. The Abolita Glossary seems to have begun with Festus excerpts. Before we can glean from these two glossaries every available scrap of evidence about Festus, we must try to complete and correct them. For of the Philoxenus Glossary we have practically only one MS., and that of the ninth century. (...) It must have omitted many items and perverted the words of many others. The Abolita Glossary was associated with another, the Abstrusa Glossary , in Spain, and this composite collection passed into Italy. The MSS. are, we may say, only two, and of each a certain number of leaves are missing. The older and better MS. , an uncial codex of the eighth century, was written apparently in Central Italy; the other was written two centuries later at Monte Cassino. They show us Abol. not in its original form, but rearranged in a not too strict alphabetical order . The St. Gall Glossary was compiled from Philox., the composite Abstr.-Abol., and supplies some of those Festus glosses of Philox. and Abol. which are omitted in our MSS. It seems to have been a Bobbio compilation . Our best MS. was written in rude uncials at St. Gall in the eighth century. (shrink)

For women’s studies professors, it’s been heartening to find the world outside our classrooms taking up conversations about sex and power that we’ve been having for decades. In this piece, I will reflect on three questions: What (...) is going on? Why is it happening now? And what forms of feminism have been overlooked in the coverage of the #MeToo movement? I spend the longest time on the third question, because I’m concerned about how #MeToo has advanced a version of public feminism that is, in some ways, out of step with currents in academic feminism. What’s going on? Although feminists have long championed public speak-outs for survivors of sexual violence—whether in Take Back the Night open mics since the 1980s or the workshops also called “MeToo” that Tarana Burke started in Alabama in 2007—the viral force of the hashtag #MeToo in mid-October 2017 took most people by surprise. Within the first twenty-four hours, it had been retweeted half a million times. According to Facebook, nearly 50 percent of US users are friends with someone who posted a message about experiences of assault or harassment. #MeToo was by no means just a US phenomenon: Facebook and Twitter feeds in various parts of the world, notably Sweden, India, and Japan, were rocked for days by this hashtag. Then came the slew of powerful cis-men, largely in the US 198 News and Views media and entertainment industries, who were forced to swiftly resign after allegations of sexual misconduct. This toppling continues and has expanded beyond the media to other industries where reputations matter: politics, music, architecture, and, somewhat belatedly, higher education. In an important way, the ground beneath us has shifted. #MeToo has tilted public sympathy in favor of survivors by changing the default response to belief, rather than suspicion; the hashtag has revealed how widespread sexual coercion is. Why Now? We need to theorize, on a cultural scale, why this movement against sexual harassment and violence in the United States has happened now rather than, say, three years ago, when Bill Cosby was accused by multiple women, or after Roger Ailes, CEO of Fox News, was deposed. #MeToo’s impact may seem sudden, but it is a part of a groundswell in women’s activism since the November 2016 elections. The Women’s March was the largest globally coordinated public gathering in history. The 3-million strong Facebook group Pantsuit Nation saw hundreds of thousands of posts about experiences of misogyny. Unprecedented numbers of women are running for US political office this year. The signature affective note running through this political moment is a fierce rage about the election of Donald Trump. Trump’s impunity, I suggest, serves as a trigger provoking the fury at the heart of #MeToo. There are many reasons to find fault with Trump, but it is distinctly galling that he faced no consequences after acknowledging being a sexual predator. For victims of sexual trauma, it is already painful to watch perpetrators roam free because of how high the burdens of proof are in legal cases. When a person such as Trump is grandly affirmed by an election, it retraumatizes victims. Right after the election, therapists and counseling centers were flooded with patients seeking help with processing past events. The ballast provided by women’s feverish organizing and the instant power of social media has facilitated a collective emboldening. Trump has made the comeuppance of all powerful men feel more urgent. But from the inception of #MeToo, I have also watched its racial and class politics with some wariness: whose pain was being centered, I wondered? A colleague recently asked aloud: is #MeToo a white women’s movement? Another wondered, is this a moral panic? Causal Modeling Semantics (CMS, e.g., Galles and Pearl 1998; Pearl 2000; Halpern 2000) is a powerful framework for evaluating counterfactuals whose antecedent is a conjunction of atomic formulas. We extend CMS to an evaluation of the probability of counterfactuals with disjunctive antecedents, and more generally, to counterfactuals whose antecedent is an arbitrary Boolean combination of atomic formulas. Our main idea is to assign a probability to a counterfactual (A ∨ B) € C at a causal model M as a weighted (...) average of the probability of C in those submodels that truthmake A ∨ B (Briggs 2012; Fine 2016, 2017). The weights of the submodels are given by the inverse distance to the original model M, based on a distance metric proposed by Eva, Stern, and Hartmann (2019). Apart from solving a major problem in the epistemology of counterfactuals, our paper shows how work in semantics, causal inference and formal epistemology can be fruitfully combined. (shrink)

Reuchlin 1977), reflete uma adesão — raramente discutida — a uma concepção internalista. Segundo essa concepção, a psicologia seria animada por uma dinâmica própria, um processo evolutivo totalmente endógeno, e seria independente de fatores externos tais como os domínios religiosos, sociopolíticos e econômicos. Além do mais, os partidários dessa história aceitam (...) ver a psicologia influenciada por disciplinas situadas em suas fronteiras, como a biologia, a fisiologia e, em menor medida, a física. Esses domínios fronteiriços geralmente dizem respeito a objetos psicológicos habitualmente qualificados de inferiores, como reflexos, sensações e percepções, em oposição à linguagem e ao pensamento, qualificados como processos superiores. Esses mesmos domínios fronteiriços engendraram por sua vez subdomínios relativamente autônomos, tais como a psicofísica ou a psicofisiologia. Correlativamente a essa concepção internalista, o desenvolvimento científico é apresentado como um caminho ao estado de psicologia positiva, tal como A. Comte a definiu em 1837 na 45a lição do Curso de Filosofia Positiva. A caminhada até a positividade foi indicada pelo próprio A. Comte: estudo da anátomo-fisiologia do sistema nervoso (a frenologia de Gall lhe parece a esse respeito uma contribuição decisiva), estudos comparados, análise de casos patológicos, estudo dos comportamentos animais e do desenvolvimento individual. Considerando a evolução da psicologia nos últimos cem anos, seríamos tentados a sustentar que ela realizou o projeto positivista. Psicologia diferencial, psicopatologia, etologia animal e psicologia da criança são vários domínios que concorrem para essa realização. Entretanto, esses domínios estão longe de parecerem homogêneos quanto aos métodos empregados e os modelos epistemológicos aos quais se referem. The development of executive functions is recognizably correlated to culture, contextual and social factors. However, studies considering all the basic EF are still scarce in Brazil, most notably in the Northeast region, which is known for its social inequality and economic gap. This study aimed to analyze the developmental trajectories and structure of four EF, namely inhibition, flexibility, working memory and planning. In addition, the potential effects of socioeconomic status and gender were examined. The sample included 230 Brazilian children between (...) 7-12 years old, homogeneously distributed by age, gender and type of school. The EF were assessed through the Brazilian version of the Child Executive Functions Battery. A global effect of age was found for most of the EF measures evaluated. Gender effect was mostly non-significant, except for 4 of the 12 tasks. There was a significant SES effect on 8 tasks, all in favor of private school children. Exploratory factorial and correlation analysis showed a 4-factor EF structure, corroborating the theoretical distribution considered in the CEF-B. A developmental progression is evident in the results for all of the EF measures evaluated. While gender had little influence on EF, SES seems to significantly impact the development of EF. As normative data are still lacking in Northeast Brazil, this study may help to understand EF development trajectories and provide tools for neuropsychological evaluation. (shrink)

This article, which is an adaptation of a lecture delivered at Villanova University in the Fall of 2015, proposes a reading of Augustine’s Confessions with the assistance of the notions of absorption and theatricality. The very use of those notions is meant to counterbalance the readings generated by our overfamiliarity with Augustinian interiority. By replacing interiority with a concept that, heretofore, is alien to the Augustinian vocabulary, it becomes possible to block facile access to mystical interpretations of conf. on the (...) one hand, and to embark upon the task of reassessing the nature of “confessing” on the other. This new reading demonstrates the difficulties involved in approaching the confessor fully involved in his act of sustained confessing. A comparison is also made with the notion of absorption in the visual arts. Just as spectatordom becomes problematic vis-à-vis a painting whose personae look inward rather than outward, so too the position of the reader vis-à-vis a text whose confessing creator uninterruptedly addresses his Confessee demands a redefinition of the reader’s role and place in the process. The medieval Islamic philosophers held a certain conception of the divine unity that assumes the necessary existent to be both one and simple. The oneness of the necessary existent meant that it is the only necessary existent and its simplicity meant that it admits no composition whatsoever – it is pure essence and its essence is necessary existence. In The Incoherence of the Philosophers al-Ġazālī presents, with elaboration, an exposition of the philosophers' conception of the divine unity, several arguments for (...) its two components, and his critique of these arguments. In this paper I focus on six of the arguments attributed to the philosophers. Following the textual evidence, I reconstruct these arguments and offer two possible interpretations of them. The first interpretation, which I call the many-argument interpretation, sees one of the arguments as employing the simplicity of the necessary existent to establish its oneness and the other five arguments as invoking oneness to establish simplicity. The second interpretation, which I call the one-argument interpretation, doesn't offer a new reading for the first argument but sees the other five arguments as defending the simplicity of the necessary existent based on its basic concept. I argue for the superiority of the one-argument interpretation. Résumé Les philosophes de l'Islam classique ont une doctrine de l'unité divine selon laquelle l'existant nécessaire est à la fois unique et simple. Son unicité signifie qu'il est le seul existant nécessaire, sa simplicité qu'il n'admet aucune sorte de composition; il est pure essence et son essence est existence nécessaire. Dans la Destruction des Philosophes, al-Ġazālī présente avec force détails un exposé de la doctrine de l'unité divine des philosophes, plusieurs arguments en faveur de ses deux composantes, ainsi que sa critique de ces arguments. Je me concentre ici sur six des arguments qu'il attribue aux philosophes. En suivant les données textuelles, je reconstruis ces arguments et en propose deux interprétations possibles. La première, que j'appelle l'interprétation “à arguments multiples”, identifie l'un des arguments comme s'appuyant sur la simplicité de l'existant nécessaire pour établir son unicité, et les cinq autres comme s'appuyant sur son unicité pour établir sa simplicité. Dillon LOVE IN WOMEN IN LOVE: A PHENOMENOLOGICAL ANALYSIS Despite his sexism, his turgid prose, and his antiquated social conscience, Lawrence is on every bookshelf. This is not merely because of the vicarious erotic entertainment to be found in the saga of John Thomas and Lady Jane, but because Lawrence remains a major guru of romance. We take him seriously, look to him for guidance, measure ourselves (...) against Ursula and Birkin. If he is on our shelves—and his thoughts not far from our pillows—then his ideas starid in need of scrutiny. Women in Love is as much a treatise as a novel: we witness more talking about than making of love. The agon of ideas engulfs the agon of flesh. Hence, the book suits the present purpose. It cannot be taken as Lawrence's definitive statement on love—later works, notably Lady Chatterly's Lover, move in different directions—but it is his most philosophical treatment and, I think, most clearly expresses his conceptual framework, the parameters which define and limit his thinking. What he says about love in Women in Love may therefore illumine his depictions of the behavior of lovers in other works. Lawrence is a romantic. In the popular sense of the word he is one who makes of romance or erotic adventure a dominant life value. Lawrence is also a romantic in another sense insofar as he takes the transcendence or unattainability of the other as essential to the project of loving. These two senses are conceptually compatible but existentially in conflict. That is, if one's major undertaking in life is to seek erotic satisfaction, and if one desires only so long as the object is beyond reach, then one has established the groundwork for continual frustration. This, I will try to show, is what Lawrence has done. I Many writers have tried to bring the ideas implicitly present in and constitutive of romantic love to the level of explicit reflective awareness. 190 M.C.Dillon191 The most successful treatment I know is given by a writer who did not know—or at least did not say—that he was writing about romantic love. Sartre purports to reveal the inner structures of love itself—the unqualified, unrestricted essence of love. As I have tried to show elsewhere,1 he fails at this. But he succeeds admirably in unveiling the inner contradictions of romantic love. Our project here is to draw upon Sartre's insights: first, to develop a conception of romantic love, and then to discern the model of Eros which informs Women in Love. The key idea in romantic love is transcendence. Something is transcendent when it is beyond me, when I cannot adequately prehend it conceptually or master it with my will. The role of transcendence in romantic love is demonstrated clearly in Denis de Rougemont's book on Love in the Western World.2 There he shows that Tristan's passion for Iseult is directly proportional to her distance from him: he is increasingly attracted as she becomes further removed. On the other hand, when through various deeds of gallantry and subterfuge, he succeeds in winning her to his side, he loses interest and must create circumstances which put her again beyond his reach. The culmination of romantic love, according to de Rougemont, is death—it sets the other absolutely beyond reach. It also arrests and preserves at their apices the youth and beauty of the lovers and the intensity of the erotic relationship. It is, I believe, this complex of ideas that led Kierkegaard to this cynical reflection on the nature of romantic love: "When two beings fall in love with one another and begin to suspect that they were made for each other, it is time to have the courage to break it off; for by going on they have everything to lose and nothing to gain."3 The same idea, qualified by increased optimism, underlies Stendhal's notion of crystallization in his book On Love.* Love is predicated on the attribution of perfection to the beloved.
