In this paper today, I would like to offer a new analysis of causation and of causal claims. It is an unorthodox one, as you will see, but I suspect that in the not too distant future it will be seen as intuitively, perhaps even trivially, true. I hardly need defend the urgency of my project. Ever since Hume, philosophers have wondered whether there are causes. This is a desperate situation. With no causes, it's hard to see how brushing my (...) teeth is likely to prevent tooth decay. Indeed, it would not be unreasonable to read Hume as an advocate of rotten teeth, which might explain the sad state that many British mouths find themselves in today. The attentive listener will have noted that I said Hume's advocacy of rotten teeth might explain the abysmal state of British oral hygiene. Of course, if Hume is right about causation then nothing explains anything, and that explains why I have been tentative in my claim. The account I would like to propose is this. The claim ‘x causes y’ is to be understood in the following way: ‘x makes y happen’. That is, to say that x is the cause of y is just to say that x makes y happen. Or, to put it more succinctly, if x is the cause of y, then x makes y happen. This is no doubt a startling claim, and one in need of further clarification and defense. To begin, I should like to contrast my analysis with another that might, on its surface, appear similar. Suppose one were to claim that 'x is the cause of y' means that x brings y about. But ‘bringing about’ is hardly an informative verbal clause, and does little ampliative work. This way of putting it lacks the opaque transparency that we’ve come to expect of philosophical analyses of causation. Now this new account is not necessarily inconsistent with other, more traditional analyses, such as Lewis and Hausman's analyses of causation in terms of counterfactuals or Eells' probabilistic theory of causation. Cette contribution apporte des éclaircissements sur le rôle du médecin et psychanalyste Heinrich Meng dans les réseaux de l’Institut de recherche sociale, en particulier sur sa perspective hygiéniste en santé mentale. Directeur adjoint de l’Institut de psychanalyse de Francfort au côté de Karl Landauer, Meng fut le porteur d’un programme de réformes sociales et de prévention appelé Psychohygiene, une conception de l’hygiène mentale adossée à la psychanalyse. Ce cadre de référence explique pourquoi et comment Meng a surtout été force de (...) propositions en prophylaxie plutôt qu’en théorie, à l’unisson avec les pédagogues et les associations engagées dans la prévention en santé mentale. Même si Meng fut l’éditeur de nombreux ouvrages de psychanalyse appliquée aux institutions sociales, il est resté en marge de la Théorie critique, à la fois à l’écart des sciences sociales et en dehors des premiers cercles de l’institut en exil. Nineteenth century hygiene might be a confusing concept. On the one hand, the concept of hygiene was gradually becoming an important concept that was focused on cleanliness and used interchangeably with sanitation. On the other hand, the classical notions of hygiene rooted in the Hippocratic teachings remained influential. This study is about two attempts to newly theorise such a confusing concept of hygiene in the second half of the century by Edward. W. Lane and Thomas R. (...) Allinson. Their works, standing on the borders of self-help medical advice and theoretical treatises on medical philosophies, were not exactly scholarly ones, but their medical thoughts - conceptualised as _hygienic medicine_ - show a characteristically holistic medical view of hygiene, a nineteenth-century version of the reinterpretation of the nature cure philosophy and vitalism. However, the aim of this study is to properly locate their conceptualisations of hygienic medicine within the historical context of the second half of the nineteenth century rather than to simply introduce the medical ideas in their books. Their views of hygiene were distinguished not only from the contemporary sanitary approach but also from similar attempts by contemporary orthodox and unorthodox medical doctors. Through a chronological analysis of changes in the concept of hygiene and a comparative analysis of these two authors’ and other medical professionals’ views of hygiene, this paper aims to help understand the complicated picture of nineteenth-century hygiene, particularly during the second half of the century, from the perspective of medical holism and reductionism. Academic physiology, as it was taught by John Hughes Bennett during the 1870s, involved an understanding of the functions of the human body and the physical laws which governed those functions. This knowledge was perceived to be directly relevant and applicable to clinical practice in terms of maintaining bodily hygiene and human health. The first generation of medical women received their physiological education at Edinburgh University under Bennett, who emphasised the importance of physiology for women due to its relevance (...) for the hygienic needs of the family and of society. With the development of laboratory-based science as a distinct aspect of medical education during the later nineteenth century, however, so the direct application of physiology to clinical practice diminished. The understanding of physiology as hygiene was marginalised by the new orthodoxy of scientific medicine. This shift in the physiological paradigm enabled medical women to stake out a specific field of interest within medicine which was omitted from the new definition of physiology as pure medical science: hygiene and preventive medicine. Women physicians were able to take advantage of the shift towards science as the basis of medical theory and practice to define their own specific role within the profession. (shrink)

The hygiene hypothesis offers an explanation for the correlation, well-established in the industrialized nations of North and West, between increased hygiene and sanitation, and increased rates of asthma and allergies. Recent studies have added to the scope of the hypothesis, showing a link between decreased exposure to certain bacteria and parasitic worms, and increased rates of depression and intestinal auto- immune disorders, respectively. What remains less often discussed in the research on these links is that women have higher (...) rates than men of asthma and allergies, as well as many auto-immune disorders, and also depression. The current paper introduces a feminist understanding of gender socialization to the epidemiological and immunological picture. That standards of cleanliness are generally higher for girls than boys, especially under the age of five when children are more likely to be under close adult supervision, is a robust phenomenon in industrialized nations, and some research points to a cross- cultural pattern. I conclude that, insofar as the hygiene hypothesis successfully identifies standards of hygiene and sanitation as mediators of immune health, then attention to the relevant patterns of gender socialization is important. The review also makes clear that adding a feminist analysis of gender socialization to the hygiene hypothesis helps explain variation in morbidity rates not addressed by other sources and responds to a number of outstanding puzzles in current research. Alternative explanations for the sex differences in the relevant morbidity rates are also discussed (e.g., the effects of estrogens). Finally, new sources of evidence for the hygiene hypothesis are suggested in the form of cross-cultural and other natural experiments. (shrink)

The Covid-19 pandemic accelerated the global trend towards spending increasing amounts of time online. I explore some of the potential negative consequences of lockdown-induced increases in time spent online, and I argue that the stressful context of the pandemic and lockdowns is exacerbated by being online beyond that which is required for essential purposes. Time spent online may increase stress levels by perpetuating the sympathetic nervous system's fight-or-flight response, draining a person’s energy and diminishing one’s ability to deal with illness. I frame the situation as one in which the pandemic context, combined with a mandatory need to be online more, forces many people into what Daniel Kahneman calls “System 1 thinking”, or “fast thinking”. I argue that digital hygiene requires the suspension of System 1 thinking, and that “philosophical perception” resonates with potential remedies in this regard. (shrink)

The article explores the inner logic and defining features of Daoist freedom. It argues that Daoist freedom can be meaningfully understood as psychological hygiene, and it suggests that Daoist xuan-jie (懸解) can be rendered possible only if one can rid oneself of intensional suffering—an idea ultimately inspired by Friedrich Nietzsche. This comparative approach enables the article to contribute to the received way of understanding Daoist freedom by stressing its dialectics: by being at ease with one’s social and political environment, (...) Daoist freedom demands social criticism, calling for room for people to be able to act spontaneously so that all can nurture their distinctive natures and have a full and free exercise of their natural abilities. The article aims to achieve two objectives at once: it aims to argue for the understanding of Daoist freedom as psychological hygiene, and it defends this reading by bringing two dimensions of Daoist freedom into a consistent whole. In a constantly changing and increasingly pluralistic world, psychological hygiene and social criticism are both necessary for leading a robust and healthy human life. (shrink)

The twentieth century must be seen as the era of the political efficiency of biology, that is to say, of politics as bio-cybernetics. This provides the context in which a definition of social hygiene makes sense as bio-politics or as the practice of biologism through the power of politics. But the point is that social hygiene provided legitimacy to the Holocaust, insofar as it constituted an ethical framework that made sense, in some way, to a significant part of (...) the ruling elites of Germany. The Holocaust as a sanitary operation represented the ultimate consequence of social hygiene. In other words, there would have been no Holocaust without the theory and practice of social hygiene as a commonly accepted social and political phenomenon, and its “barbarity” had little to do with that of foreign cultures or ancient times. The Holocaust happened as the consequence of the unchecked development of social hygiene as political common sense in twentieth-century European culture. (shrink)

The maternal deprivation hypothesis was arguably the most discussed debate in midcentury psychiatry. Combined with the gender ideology prevalent in America and Britain, it solidified the idea that the mother-child relationship had formative influence on personality development. This essay explores the formation of this hypothesis by situating its knowledge claims against an institutional innovation set to prevent juvenile delinquency and promote mental hygiene, the establishment of child guidance clinics on both sides of the Atlantic in the late 1920s. It (...) then tracks the development of investigative agendas by child guidance practitioners, examining the practices they adopted to construct psychological knowledge claims in conformity with preventive objectives and positivist standards. Shifting the historiographical focus toward the clinical scenes in which psychological assertions were made, the essay examines how investigators sought to determine the “pathogenic” influence of early parental environment by way of estimating its emotional qualities. It argues that it was this positivist-determinist effort to achieve preventive goals that foregrounded the role of the mother in the etiology of personality disturbance and marked the knowledge field that came to be called the maternal deprivation debate. Background: Nosocomial infections represent a serious challenge for healthcare systems worldwide. Adherence to hand hygiene plays a major role in infection prevention and control. These adherence rates can be improved through behaviour tracking innovations. This requires the systems to be widely implemented and accepted. Therefore, both a systematic analysis of the normative issues related and the evaluation of technology acceptance are equally important. Objectives: To explore and describe relevant aspects regarding the acceptance of technology and ethical implications using a (...) tracking device to measure and improve adherence to hand hygiene. Research design: A quantitative study with a descriptive design was performed. Participants and research context: A total of 75 questionnaires were collected in three hospitals in Germany. Acceptance of technology was measured with n = 60 participants (n = 50 nurses; n = 9 physicians; n = 1 not disclosed) and ethical assessment with n = 15 participants (nurses only). Ethical considerations: Ethical approval for this study was obtained from the institutional review board. Findings: The acceptance of technologies improving hygiene in general was good (median = 80.5, interquartile range = 28, range: 0–100). The experience with technologies in general (median = 48.5, interquartile range = 57, range: 0–100) and the acceptance of the specific technology (mean = 82.23, standard deviation = 15.16 (range: 23–138)) was moderate. There was a significant positive correlation between the acceptance and experience of technology in general ( r = 0.217, p =.025). Ethical concerns played a minor role. The need for practical support was a key topic. Discussion: Study participants accepted technologies improving hygiene; however, the specific device implemented was only moderately accepted. This creates specific opportunities in the implementation process for higher later acceptance. More practical support and an increase in experience may create opportunities for device implementation with high acceptance and low ethical concerns. Conclusion: Study results indicate a vast potential for improving the implementation process of hand hygiene technologies. Ethical concerns in this study did not seem to be a relevant barrier for successful implementation of hand hygiene technologies. (shrink)

This cross sectional study was carried out among the workers of British American Tobacco Company, Dhaka with a view to explore their nutritional status, personal hygiene and health seeking behavior as because they are working on a tobacco processing company. The sample size was 179 which were selected purposively. The study showed that out of 179 respondents 89 (49.7%) were in the age groups of 30-39 years and the mean age of the respondents were 31.99 ± 6.01 years. A (...) large number of respondents (55.9%) had monthly family income of Taka 10001-20000 and the mean family income was Taka 12776.54 ± 5230.13. Maximum respondents (73.7%) were Muslim, more than half (54.2%) were shift in charge, 39.1% of the respondents consisted of 4 family members, 43.6% respondents were accustomed to other type of eating habit and 38.5% respondents knew that malnutrition was the effect of lack of proper nutrition, 59.8% of the respondents knew that night blindness was the disease due to malnutrition, most of the respondents (91.6%) performed duties to maintain health, majority (62.0%) respondents used to do nothing to maintain healthcare for their children and 35.9% visited doctor’s single time in a month, 40.2% of the respondents told regular tooth brushing as type of healthy habits. Majority (64.8%) respondents used to brush twice a day, majority (50.8%) respondents used to wash hand after toileting, majority (62.08%) respondents used to bath daily, 43.0% and 31.8% of the respondents told that dysentery and diarrhea was due to eating without proper hand washing respectively. Majority (53.6%) respondents informed that they learned about personal hygiene from television, 45.8% respondents understood that use of safe water in every work as sanitation. Majority (50.84%) came from nuclear family; most (84.92%) had exercise habit and 40.22% had education level of class VIII. Most (75.42%) of the respondents had semi pucca houses and majority (69.83%) of the respondents used only water as materials for hand washing. This study provided some important information which might help the concerned authority to take appropriate measures to improve the health status of the workers. (shrink)

Cleaning the floor, stripping the bed, arranging a bouquet of flowers—such tasks are essential to keeping a hospital room clean and creating a pleasant atmosphere. They usually fall under the purview of female* nurses, cleaning staff and housekeepers. In everyday hospital life, the demands for hygienic cleanliness commingle with the imperatives of economization, marketing logic, and attention to the affective and emotional needs of the actors in these rooms. Although the standards of clinical hygiene are based on medical knowledge, (...) the division of labor and the demands for cleanliness at various hierarchical levels also reveal gendered and partly racialized ideas that point beyond the clinical context. This blending of imperatives in the hospital environment invites deeper consideration of the history of bacteriology: The logic and language of defense against infection in science and everyday life is also interwoven with social markers of difference.Drawing on the findings of an ethnography on cleanliness and cleaning work in hospitals, as well as a history of knowledge approach, the article links the question of (feminized) care for the environment with the question of the atmosphere of clinical rooms. In what ways, and to what effect, does scientific knowledge about medical hygiene also carry with it cultural and aesthetic perceptions of beautiful and pleasant cleanliness that reveal feminine connotations rooted in the nineteenth century? Die historisch-ethnologische Abteilung der 1911 in Dresden veranstalteten Internationalen Hygiene Ausstellung war ein großer Publikumserfolg. Organisiert von Karl Sudhoff, dem einflussreichsten Medizinhistoriker der Zeit, versuchte diese Abteilung, die Geschichte der Hygiene zu ‘verkaufen’. Auf einer Ausstellungsfläche von mehr als 2.400 m2 wurden über 20.000 Objekte in mehr als siebzig Räumen, Hallen und Gallerien präsentiert. Die Dresdener Hygiene-Ausstellung war zwar nicht die erste ihrer Art, aber die erste, die eine derartig große Ausstellungsfläche für die Inszenierung einer Geschichte der (...) Hygiene reservierte. Der Beitrag, der auf einer Auswertung der Korrespondenz Sudhoffs mit seinen Mitarbeitern beruht, zeigt, wie diese überaus erfolgreiche historisch-ethnologische Ausstellung möglich wurde. Das Archivmaterial, das einen Blick hinter die Kulissen erlaubt, belegt wie das Aufeinanderprallen unterschiedlicher Charaktere, verschiedener intellektueller Ziele und Organisationskulturen die spezifische Präsentation dieser Abteilung prägten. Resumo: O objetivo deste texto é refletir sobre a produção de anormalidades. de um ponto de vista histórico-hermenêutico. Partindo do marco teórico proposto por Michel Foucault, será possível ver como o binômio normalidade-anormalidade, seja de modo disciplinar, seja biopolítico, serve à produção de indivíduos mediante relações de saber-poder. Nesses termos, observa-se como se dá a produção de anormalidades na obra de Foucault. Na segunda seção, verifica-se como a discussão sobre o biopoder pode ser utilizada no intuito de problematizar situações e (...) casos ocorridos neste continente. Trata-se de relacionar a produção de anormalidades com o desenvolvimento do Higienismo na América Latina, dando a perceber algumas estratégias adotadas na normalização e produção de corpos higiênicos. Hospital acquired infections (HAIs) are a major threat to patient safety. This paper addresses the following question: given what is known about the causes of and possible interventions on HAIs, to whom or what should the moral responsibility for preventing these infections be attributed? First, we show how generating robust evidence on the effectiveness of preventive hygiene measures is a complex endeavour and review the existing evidence on the causes of HAIs. Second, we demonstrate that the existing literature on (...) the ethical aspects of infection control has focused on responsibility at the individual-level. Thirdly, we argue that these accounts do not accommodate systemic factors relevant for HAI prevention. We show that the notion of collective responsibility is useful for making understandable how systemic factors, such as employment conditions in hospitals, are both causally and ethically relevant in infection control. The Hygiene Hypothesis has been recognized as an important cornerstone to explain the sudden increase in the prevalence of asthma and allergic diseases in modernized culture. The recent epidemic of allergic diseases is in contrast with the gradual implementation of Homo sapiens sapiens to the present-day forms of civilization. This civilization forms a gradual process with cumulative effects on the human immune system, which co-developed with parasitic and commensal Helminths. The clinical manifestation of this epidemic, however, became only visible (...) in the second half of the twentieth century. In order to explain these clinical effects in terms of the underlying IgE-mediated reactions to innocuous environmental antigens, the low biodiversity of antigens in the domestic environment plays a pivotal role. The skewing of antigen exposure as a cumulative effect of reducing biodiversity in the immediate human environment as well as in changing food habits, provides a sufficient and parsimonious explanation for the rise in allergic diseases in a highly developed and helminth-free modernized culture. Socio-economic tendencies that incline towards a further reduction of environmental biodiversity may provide serious concern for future health. This article explains that the “Hygiene Hypothesis”, the “Old Friends Hypothesis”, and the “Skewed Antigen Exposure Hypothesis” are required to more fully explain the rise of allergy in modern societies.
