Objectives To characterize the prevalence and content of pediatric triage policies.Methods We surveyed and solicited policies from U.S. hospitals with pediatric intensive care units. Policies were analyzed using qualitative methods and coded by 2 investigators.Results Thirty-four of 120 institutions (28%) responded. Twenty-five (74%) were freestanding children’s hospitals and 9 (26%) were hospitals within a hospital. Nine (26%) had approved policies, 9 (26%) had draft policies, 5 (14%) were developing policies, and 7 (20%) did not have policies. Nineteen (68%) institutions shared (...) their approved or draft policy. Eight (42%) of those policies included neonates. The polices identified 0 to 5 (median 2) factors to prioritize patients. The most common factors were short- (17, 90%) and long- (14, 74%) term predicted mortality. Pediatric scoring systems included Pediatric Logistic Organ Dysfunction-2 (12, 63%) and Score for Neonatal Acute Physiology and Perinatal Extensions-II (4, 21%). Thirteen (68%) policies described a formal algorithm. The most common tiebreakers were random/lottery (10, 71%) and life cycles (9, 64%). The majority (15, 79%) of policies specified the roles of triage team members and 13 (68%) precluded those participating in patient care from making triage decisions.Conclusions While many institutions still do not have pediatric triage policies, there appears to be a trend among those with policies to utilize a formal algorithm that focuses on short- and long-term predicted mortality and that incorporates age-appropriate scoring systems. Additional work is needed to expand access to pediatric-specific policies, to validate scoring systems, and to address health disparities. (shrink)

Aesthetics is the subject matter concerning, as a paradigm, fine art, but also the special, art-like status sometimes given to applied arts like architecture or industrial design or to objects in nature. It is hard to say precisely what is shared among this motley crew of objects (often referred to as aesthetic objects), but the aesthetic attitude is supposed to go some way toward solving this problem. It is, at the very least, the special point of view we take toward (...) an object that results in our having an aesthetic experience (an experience of, for example, beauty, sublimity, or even ugliness). Many aesthetic theories, however, have taken it to play a central role in defining the boundary between aesthetic and non-aesthetic objects. These theories, usually called aesthetic attitude theories, argue that when we take the aesthetic attitude toward an object, we thereby make it an aesthetic object. (shrink)

This article establishes a dialogue between Virginie Despentes’s 2006 memoir-cum-manifesto, King Kong théorie and Jack Halberstam’s theorization of ‘shadow feminism’. For Halberstam, ‘not succeeding at womanhood can offer unexpected pleasures (…) Shadow feminisms take the form not of becoming, being, and doing but of shady, murky modes of undoing, un-becoming, and violating’. In King Kong théorie, I argue, Despentes embraces her failure to ‘become woman’, and her accounts of rape and rape fantasy present a refusal of mastery wherein (...) the subject might unravel, come undone. Through her use of the King Kong metaphor, Despentes connects ‘unbecoming woman’ to an unravelling of the human subject; King Kong figures in the text as a composite, cyborgian creature, with whom Despentes herself identifies. In King Kong théorie, then, Despentes adopts a shadowy, hybrid positionality, forging a textual space for all creatures who fail to be or become woman. (shrink)

The late antique and early medieval Mediterranean was characterized by wide-ranging cultural and linguistic diversity. Yet, under the influence of Christianity, communities in the Mediterranean world were bound together by common concepts of good rulership, which were also shaped by Greco-Roman, Persian, Caucasian, and other traditions. This collection of essays examines ideas of good Christian rulership and the debates surrounding them in diverse cultures and linguistic communities. It grants special attention to communities on the periphery, such as the Caucasus and (...) Nubia, and some essays examine non-Christian concepts of good rulership to offer a comparative perspective. As a whole, the studies in this volume reveal not only the entanglement and affinity of communities around the Mediterranean but also areas of conflict among Christians and between Christians and other cultural traditions. By gathering various specialized studies on the overarching question of good rulership, this volume highlights the possibilities of placing research on classical antiquity and early medieval Europe into conversation with the study of eastern Christianity. The debate about how neurotechnologies impact authenticity has focused on two inter-related dimensions: self-discovery and self-creation. In this paper, we develop a broader framework that includes the experimental and relational dimensions of authenticity, both understood as decisive for shaping one’s narrative identity. In our view, neurointerventions that alter someone’s personality traits will also impact her very own self-understanding across time. We argue that experimental authenticity only needs a minimum conception of narrative coherence of the self and that reversibility should remain (...) a key feature in the ethical assessment of neurotechnologies. The relational dimension of authenticity derives from the fact that a process of self-constitution is never a solitary business. The significant others act not just as subsequent judges of one’s life decisions, but as real co-authors of one’s self. We discuss relevant cases in which others are actively contributing to the meaning-making process, as to the direction one’s life is taking. We also claim that the experimental and the relational dimensions of authenticity should aim to correct each other. The relational dimension introduces socially approved standards as well as limitations to the array of available possibilities of self-fashioning or human enhancement. The experimental side enables us to avoid confusing one’s personal identity with a social and narrative construct that only bears the mark of social pressure and conformism. (shrink)

Those who want to deny the ‘ought implies can’ principle often turn to weakened views to explain ‘ought implies can’ phenomena. The two most common versions of such views are that ‘ought’ presupposes ‘can’, and that ‘ought’ conversationally implicates ‘can’. This paper will reject both views, and in doing so, present a case against any pragmatic view of ‘ought implies can’. Unlike much of the literature, I won't rely on counterexamples, but instead will argue that each of these views fails (...) on its own terms. ‘Ought’ and ‘can’ do not obey the negation test for presupposition, and they do not obey the calculability or the cancelability tests for conversational implicature. I diagnose these failures as partly a result of the importance of the contrapositive of ‘ought implies can’. I end with a final argument emphasizing the role the principle plays in moral thinking, and the fact that no pragmatic account can do it justice. (shrink)

This article examines the processes of investigation and gathering evidence about victims of the September 11 attacks to better understand the inability of state and nonstate institutions to effectively deal with the invisibility of undocumented migrants in terms of providing assistance and recognition at a moment of tragedy. The failure to make the invisible visible or to address the very question of visibility publicly is explained by three major reasons: 1) A general fear of coming forward on the part of (...) undocumented migrants or their families, partly as a result of their legal status and their lack of trust in government agencies, which was compounded by ineffective communication about available relief services; 2) different procedural requirements and logics of evidence used by government and nongovernmental relief agencies, which, in some cases, made it impossible for undocumented migrants or their families to provide proof of their presence at the site or employment in the businesses affected; 3) the context of 9/11 as a disruptive event that influenced the overall climate in which issues of victimhood and immigration status could be addressed. When a term is used in science, we tend to integrate its origins, functions, and history to see if the term is a scientific one or comes from other fields. The term «empathy» is an example to such a case. This article challenges the widespread view that empathy is the capability of a person to understand emotions and thoughts of others. We will deconstruct the concept of empathy as an academic one by focusing on its limits. We will discuss the (...) possibility of empathy for humans in critically reconstructing the meaning of the word and its meanings from its ancient origins to contemporary research. Our question is centered on cognitive actions that enable empathy as an emotional way of understanding others. Based on our findings, we argue that the concept of empathy is a recently established academic term. As a concept, it matches with some alternative concepts such as apathy and sympathy. In rhetoric, it is the pathos of the speech that affects the speaker by emotions since no specific receptive ability of the speaker is needed. Similarly, when we use the term empathy, we should use it as a category of the sender, not that of the receiver. This concept of logical empathy opposes to the classical view of empathy studies emphasizing that empathy is a receptive category of the receiver. So, empathy is nothing else but the ability to send emotional or intellectual content suitable to a capable receiver. Also, the comparative study in the concept of empathy in Buddhism gives us evidence that empathy is a spiritual or mental concept whose applications are in the area of pre-linguistic states and esoteric contexts. In rhetorical terms we could define it as a persuasive mood of pathos-driven speech. (shrink)

In this article we provide a case history of the development of a communicative system in songbirds. In particular, we explore how brown-headed cowbirds, male and female, cooperate in the development and use of species-typical song. The goal is to show how social interactions between and within sexes create a platform for the production and perception of song. We consider six perspectives. First, we discuss the nature of the acoustic signal. Second, we look at the process of song learning. Third, (...) we describe a specific song mechanism, social shaping. Fourth, we look at the more general developmental process of neophenogenesis. Fifth, we consider the developmental ecology for social learning. Finally, we describe how social networks measures can be used to capture the nature of social interactions as the engines of song learning. Taken as a whole, we argue that culturally transmitted behaviors structure social interactions that predict the acquisition of species' typical behaviors necessary for successful reproduction. Para darle respuesta, en el inicio del artículo se trabajará una breve observación respecto al ansia de pureza cristalina, que, de algún modo, fue el plan que marcaba la pauta en la filosofía del Tractatus logico-philosophicus. A continuación, desde la discutida noción del significado como uso, (...) se abordarán los principales elementos que constituyen la nueva concepción del lenguaje del segundo Wittgenstein, a saber: el criterio definidor, los juegos de lenguaje, las formas de vida y los parecidos de familia. Zoos are typically public attractions that do not explicitly, or through a more implicit culture, expect quietness from their guests. This paper will explore whether quietness is something we should aim for when we are visiting zoos. Primarily through analogy with other public spaces which share some of the key characteristics of zoos (libraries and schools, cinemas, theatres and galleries, war memorials, and hospitals and gardens), we suggest that quiet is indeed appropriate in zoos (more appropriate than being noisy). A (...) major component of this argument will be the exploration of what is meant by quiet (and noise), and outlining a concept of quietness based on an idea of attention. The central premise here, drawing upon theories of attention and love, is that noise involves a certain kind of outward expression, which leaves less room for the appreciation of, and attention to, the animals and information that the zoo provides. The article will reflect on how a call for quiet may create scope for enhancing the educational possibilities of zoos, and how zoos might profit by taking the acoustic dimension of visitor behaviour into account when considering their values. W. E. Hocking has written recently that Whitehead's descriptive generalization of concrete fact, namely, his actual occasion, is “… not a term of description in the direct sense. It is an hypothesis. It cannot be kept in place by pointing to its presence as a datum: it can only hold its own if it proves to be a valuable conceptual tool.” I further advance the thesis that all generality is hypothetical, and holds it own only if it proves to be (...) conceptually useful. This is in accord with Whitehead's statement, the full implications of which have not been taken seriously enough, that metaphysical categories are not dogmatic statements of the obvious; they are tentative formulations of the ultimate generalities. But when these tentative formulations prove conceptually useful or are conceptually employed, they lose their “hypothetical air,” and often pass into what Whitehead calls a false state of “sober obviousness:”“An old established metaphysical system gains a false air of adequate precision from the fact that its words and phrases have passed into current literature. Thus propositions expressed in its language are more easily correlated to our flitting intuitions into metaphysical truth. When we trust these verbal statements and argue as though they adequately analysed meaning, we are led into difficulties which take the shape of negations of what in practice is presupposed. But when they are proposed as first principles they assume an unmerited air of sober obviousness.”. (shrink)

This chapter argues that the philosophical problem Boethius addresses in his Consolation of Philosophy is the Problem of Desert, why people do not get what they deserve. He rejects the proposal that any undeserved fortunes in this life are compensated for in the Afterlife, arguing instead that, contrary to appearances, people do get exactly what they deserve. Later medieval thinkers recognize that this is Boethius’s solution, but several disagree with its adequacy.

Belief in propositions has had a long and distinguished history in analytic philosophy. Three of the founding fathers of analytic philosophy, Gottlob Frege, Bertrand Russell, and G. E. Moore, believed in propositions. Many philosophers since then have shared this belief; and the belief is widely, though certainly not universally, accepted among philosophers today. Among contemporary philosophers who believe in propositions, many, and perhaps even most, take them to be structured entities with individuals, properties, and relations as constituents. For example, the (...) proposition that Glenn loves Tracy has Glenn, the loving relation, and Tracy as constituents. What is it, then, that binds these constituents together and imposes structure on them? And if the proposition that Glenn loves Tracy is distinct from the proposition that Tracy loves Glenn yet both have the same constituents, what is about the way these constituents are structured or bound together that makes them two different propositions? In The Nature and Structure of Content, Jeffrey C. King formulates a detailed account of the metaphysical nature of propositions, and provides fresh answers to the above questions. In addition to explaining what it is that binds together the constituents of structured propositions and imposes structure on them, King deals with some of the standard objections to accounts of propositions: he shows that there is no mystery about what propositions are; that given certain minimal assumptions, it follows that they exist; and that on his approach, we can see how and why propositions manage to have truth conditions and represent the world as being a certain way. The Nature and Structure of Content also contains a detailed account of the nature of tense and modality, and provides a solution to the paradox of analysis. Scholars and students working in the philosophy of mind and language will find this book rewarding reading. (shrink)

Empowerment, an already central concept in public health, has gained additional relevance through the expansion of mobile health (mHealth). Especially direct-to-consumer self-testing app companies mobilise the term to advertise their products, which allow users to self-test for various medical conditions independent of healthcare professionals. This article first demonstrates the absence of empowerment conceptualisations in the context of self-testing apps by engaging with empowerment literature. It then contrasts the service these apps provide with two widely cited empowerment definitions by the WHO, (...) which describe the term as a process that, broadly, leads to knowledge and control of health decisions. We conclude that self-testing apps can only partly empower their users, as they, we argue, do not provide the type of knowledge and control the WHO definitions describe. More importantly, we observe that this shortcoming stems from the fact that in the literature on mHealth and in self-testing marketing, empowerment is understood as a goal rather than a process. This characterises a shift in the meaning of empowerment in the context of self-testing and mHealth, one that reveals a lack of awareness for relational and contextual factors that contribute to empowerment. We argue that returning to a process-understanding of empowerment helps to identify these apps’ deficits, and we conclude the article by briefly suggesting several strategies to increase self-testing apps’ empowerment function. In this paper, we put forward an interdisciplinary framework describing different levels of self-representations, namely non-conceptual, conceptual and propositional self-representations. We argue that these different levels of self-representation are differently affected by cultural upbringing: while propositional self-representations rely on “theoretical” concepts and are thus strongly influenced by cultural upbringing, non-conceptual self-representations are uniform across cultures and thus universal. This differentiation offers a theoretical specification of the distinction between an independent and interdependent self-construal put forward in cross-cultural psychology. Hence, this does (...) not only allow for a deeper understanding of different self-conceptions, but also for a formulation of new hypotheses regarding the cultural influence on self-representations. As one example, we will highlight the role of the proposed levels of self-representation for emotional experience and formulate some major implications of our interdisciplinary framework for future empirical research. One striking observation in the history of rational choice models is that those models have not only been used in economics but spread widely across the social and behavioral sciences. How do such model transfers proceed? By closely studying the early efforts to transfer such models by William Riker – a major protagonist in pushing the adoption of game theoretic models in political science – this article examines the transfer process as one of ‘translation’ by which abstract and mathematical rational (...) choice models were constructed and modified such that they applied to a specific target system in a new domain. In this paper, we argue that to overcome a set of challenges that hampered the straightforward transfer of game theoretic models into political science, Riker complemented theoretical and conceptual modifications of von Neumann and Morgenstern’s game schemes with the use of narratives to turn them into applicable and testable models. As such, those narratives played a crucial role in enabling their transfer and ultimately facilitated the applicability of game theoretic models in political science. (shrink)
