Slowing the aging process would be one of the most dramatic and momentous ways of enhancing human beings. It is also one that mainstream science is on the brink of pursuing. The state of the science, together with its possible impact, make it an important example for how to think about research into all enhancement technologies.

Unter dem Begriff ›Anti-Aging‹ wird seit etwa drei Jahrzehnten eine wachsende Anzahl medizinischer wie außermedizinischer Präparate bzw. Verfahren konzipiert, entwickelt und eingesetzt, die allgemein der Bekämpfung des Alterns dienen sollen. Die so genannte Anti-Aging-Medizin will in den Alterungsprozess des menschlichen Körpers eingreifen, um ihn zu verlangsamen, aufzuhalten oder rückgängig zu machen. Dabei variieren die konkreten Angriffspunkte, Vorgehensweisen und Zielsetzungen der einzelnen Ansätze durchaus beträchtlich.

Durch Fortschritte in der modernen Reproduktionsmedizin ist es Frauen heute möglich, auch nach der Menopause mit eigenen Eizellen schwanger zu werden. Damit wird die Fortpflanzung im homologen System auch im Alter zu einer realistischen Option. Gegen derartige späte Schwangerschaften gibt es vielfältige Argumente, die vor allem auf mögliche Schädigungen aufgrund des hohen Alters der Mutter verweisen. Maßnahmen der Anti-Aging-Medizin zum Erhalt bzw. zur Verbesserung der kognitiven und physischen Leistungsfähigkeit im fortgeschrittenen Alter könnten diese Einwände gegen den Einsatz von (...) Reproduktionstechniken nach der Menopause entkräften. Der Beitrag geht der Frage nach, wie erstrebenswert postmenopausale Schwangerschaften unter diesen Voraussetzungen eigentlich sind. Es wird die These vertreten, dass eine Schadensargumentation für eine ethische Bewertung allein nicht ausreichend ist, sondern auch Überlegungen zu Status und Authentizität des Kinderwunsches, zum Selbstverständnis der Medizin, zur Autonomie der Betroffenen sowie zur Natürlichkeit der Lebensführung berücksichtigt werden müssen. Darüber hinaus zeigt sich, wie Entwicklungen in zwei spezifischen Anwendungsbereichen (Anti-Aging- und Reproduktionsmedizin) in einer gemeinsamen Betrachtung zusammengeführt und mit Gewinn für allgemeinere medizinethische Überlegungen bewertet werden können. Dieses Prozedere führt jedoch zu keinem brauchbaren Ergebnis. Unter Berufung auf den Krankheitsbegriff (...) allein ist der Umfang solidarischer Gesundheitsversorgung nicht zu bestimmen. Anti-aging medicine has emerged in the past two decades as both a medical practice and scientific objective largely aimed at intervening into the process of aging itself rather than its “associated” diseases. This has provoked a both excitement and concern in bioethical deliberations on the meaning and potential impact of an effective intervention. In this article, I examine the different ways in which bioethicists, other social scientists, and anti-aging proponents frame anti-aging goals, in (...) particular, the construction of immortality as its implicit and explicit aim. This research is based upon over 9 years of anthropological, ethnographic interview- and observation-based research in the field and draws substantially from the US President’s Council on Bioethics’ deliberations on this topic as well as from interview data and other publications/discussion on anti-aging medicine. I argue that while the framework of life and death provide the primary structure for many bioethical and social science critiques of anti-aging medicine, many if not most anti-aging practitioners, researchers, and advocates employ the alternative structure of health and pain to orient their work. These divergent orientations of life/death and health/pain beget competing conversations around anti-aging medicine; including voices from the front lines of anti-aging practice complicate bioethical critiques and ultimately beg different questions. Positioning the painful, physiological decline of aging as the ultimate adversary rather than death challenges traditional models of biomedical intervention based on “nature” and “disease” constructions. This paper explores how the exercise of the ethics of ‘responsibility’ for health care advanced through ‘healthy ageing’ and ‘successful ageing’ narratives in Western countries animates an array of ‘authorities’, including the ‘anti-ageing medicine’ movement as a strategy to address the anxieties of growing old in Western societies and as a tool to exercise the ethos of ‘responsibility’. The choice of this type of authority as a source of guidance for self-constitution and the exercise of the ‘responsible self’, this (...) paper will argue, enables the enactment of a type of late modernity notion of citizenship for ageing individuals based on principles of agelessness, health, independence and consumption power. Through interviews with anti-ageing consumers, however, it is also possible to argue the existence of tensions and contradictions that such a rigid model of self-constitution in later life produces, and the potential forms of resistance and contestations that may emerge as a result. In this way the current ‘war on anti-ageing medicine’ (Vincent 2003) becomes also symptomatic of bigger ‘wars’ taking place not only between institutions competing for control over knowledge and management of ageing, but between those in favour and against the homogenisation of life under the language of universal science, reason and market rationality. This article investigates the extent to which the emerging trend of do-it-yourself anti-ageing skin-whitening products represents a re-articulation of Western colonial concerns with environmental pollution and racial degeneracy into concern with gendered vulnerability. This emerging market is a multibillion dollar industry anchored in the USA, but expanding globally. Do-it-yourself anti-ageing skin-whitening products purport to address the needs of those looking to fight the visible signs of ageing, often promising to remove hyper-pigmented age spots from women’s skin, and replace (...) it with ageless skin, free from pigmentation. In order to contextualize the investigation of do-it-yourself anti-ageing skin-whitening practice and discourse, this article draws from the literature in colonial commodity culture, colonial tropical medicine, the contemporary anti-ageing discourse, and advertisements for anti-ageing skin-whitening products. First, it argues that the framing of the biomedicalization of ageing as a pigmentation problem caused by deteriorating environmental conditions and unhealthy lifestyle draws tacitly from European colonial concerns with the European body’s susceptibility to tropical diseases, pigmentation disorders, and racial degeneration. Second, the article argues that the rise of do-it-yourself anti-ageing skin-whitening commodities that promise to whiten, brighten, and purify the ageing skin of women and frames the visible signs of ageing in terms of pigmentation pathology. This paper provides both an overview of and a personal perspective on the field of ‘anti-ageing’. In the late 20th century, progress in the science of ageing re-invigorated activity designed to avoid biological ageing. For some the objective was to abolish the need to die of old age. This anti-ageing movement includes a diverse range of people: hard scientists working in well-funded and established university laboratories, slick corporate-marketing executives and new-age entrepreneurs selling herbal elixirs. The movement has attracted (...) anti-anti-ageing critical comment from three sources: moral critique located in philosophy and religion, scepticism from established scientists in the field of biology and gerontology, and challenge from critical social gerontology. Anti-ageing activists have labelled their critics as ‘conservative’ for opposing the inevitable progress of science. The paper argues that the alternative perspective, which sees the anti-ageing movement as a route by which a biologisation of old age inhibits people from achieving a culturally valued final part to life, is the progressive one. Since the late 1990s, a constellation of professional associations, journals and health promotion materials has emerged that has constructed the ‘aging male’ as a medical problem. Central to this construction has been a revival of a hormonal model of the male body in which anti-aging is linked to the restoration of masculinity. In this paper I revisit the association of aging and demasculinization that animated the rejuvenation movement of the early 20th century, and contrast this with (...) the initial mainstream medical interest in testosterone therapy in the mid-20th century. Then I will demonstrate how the association between anti-aging and re-masculinization has been given new life in the remedicalized ‘andropause’, and as a contemporary focus on maintaining life-long virility has emerged as an important indicator of ‘health aging’. In this paper, which brings together aging research and media research, we will contribute to the mapping of the complicated cartography of anti-aging by analyzing the press coverage of anti-aging medicine. The mass media decisively shape societal impacts of the expert scientific discourse on anti-aging. While sensitivity towards the heterogeneity of the field of anti-aging is increasing to some degree in the social-gerontological discussion, the role of the media in transmitting the (...) various anti-aging messages to the general public has so far not been systematically scrutinized. Current opinions on the press coverage of anti-aging medicine range from proponents’ complaints of a media witch-hunt against them to opponents’ reproaches about uncritical reporting of this complex topic. This paper discusses whether the media act accordingly to the ideal of the fourth estate, controlling the increased power of science in the 20th century. Our areas of investigation are two Western countries: the USA and Germany. Both countries have experienced an apparent increase in the average age of their populations, which has led to increasingly vigorous public debate since the late 20th century. The subjects of our study are three organizations that represent different approaches to anti-aging medicine: the American Academy of Anti-Aging Medicine, the German Society of Anti-Aging Medicine, and the Methuselah Foundation. The article discusses the programs, goals, and media strategies of these organizations and compares them with the anti-aging messages that actually make their way to the reader in the media “interdiscourse.” The article asks whether transatlantic learning processes within the anti-aging medicine movement as well as in the media can be identified. The paper’s methods and sources are those of contemporary history and ethnography. The three approaches to anti-aging medicine are drawn from publications of spokespersons from the three presented anti-aging medicine organizations and from participant observation of anti-aging medicine conferences in Germany and Europe during the period from 2005 to 2008. The media analysis is based on the study of about 300 articles that appeared between 1990 and 2009 in US and German dailies and weeklies such as Newsweek, The New York Times, Der Spiegel, and Die Welt. Our analysis shows that, against the backdrop of pessimistic demographic apprehensions, the leitmotiv of both nations’ medical journalism between 1990 and 2009 was overwhelmingly pro anti-aging medicine. It is criticized that medical journalism on anti-aging medicine refrains from own investigations on potential risks. Scrutinizing activities of the media in terms of a fourth estate require stimulation from science itself. Hence, we argue for sensitization of medical journalism regarding ageism. (shrink)

Anti-ageing medicines are products intended to extend lifespan and healthspan in humans that have a good potential use in public health policies. In the European Union, their development, production and consumption are dependent on regulatory science performed by the European Medicines Agency and its associated epistemic policies. They impose, among other things, an unfavourable burden of proof, a strict standard of proof and meta-methodological constrictions related to some theoretical issues. This results in a distribution of errors that tends to (...) reduce false positives while increasing false negatives, leading to a set of social consequences that are generally accepted when the focus is placed on conventional medicines. However, when the same epistemic policies are applied to anti-ageing medicines, the distribution of errors is imbalanced, and undesirable outcomes like research discouragement and waiting time extensions appear. Three possible strategies that policymakers could implement to unblock the situation are presented for future reflection: the consideration of ageing as a disease, the application of methodological asymmetry and the use of biomarkers during clinical research. Healthcare counts as a morally relevant good whose distribution should neither be left to the free market nor be simply imposed by governmental decisions without further justification. This problem is particularly prevalent in the current boom of anti-ageing medicine. While the public demand for medical interventions which promise a longer, healthier and more active and attractive life has been increasing, public healthcare systems usually do not cover these products and services, thus leaving their allocation to the mechanisms of supply (...) and demand on the free market. This situation raises the question on which basis the underlying preferences for and claims to a longer, healthier life should be evaluated. What makes anti-ageing medicine eligible for public funding? In this article, we discuss the role of anti-ageing medicine with regard to the scope and limits of public healthcare. We will first briefly sketch the basic problem of justifying a particular healthcare scheme within the framework of a modern liberal democracy, focusing on the challenge anti-ageing interventions pose in this regard. In the next section, we will present and discuss three possible solutions to the problem, essentialistic, transcendental, and procedural strategies of defining the scope of public healthcare. We will suggest a procedural solution adopting essentialistic and transcendental elements and discuss its theoretical and practical implications with regard to anti-ageing medicine. (shrink)

Interventions aiming to slow, stop, or reverse the aging process are starting to enter clinical trials. Though this line of research is nascent, it has the potential to not only prevent prolonged human suffering, but also to extend human well-being. As this line of research develops, it is important to understand the ethical constraints of conducting such research. This paper discusses some of these constraints. In particular, it discusses the ethical difficulties of conducting this research in a way that (...) would produce reliable data regarding the effectiveness of an anti-aging intervention. Clinical trials of such interventions, I argue, will be faced with a dilemma between two confounding variables. Eliminating the variables requires introducing ethically problematic research practices. Thus, researchers must either perform research in ethically problematic ways, or forego the conduct of high-impact clinical research on anti-aging interventions. (shrink)

Biosemiotics and the analysis of biopower have not yet been explicitly brought together. This article attempts to find their connecting points from the perspective of biosemiotics. It uses the biosemiotic understanding of the different types of semiosis in order to approach the practices of biopower and biopolitics. The central concept of the paper is that of the ‘semiotic threshold’. We can speak of (1) the lower semiotic threshold, signifying the dividing line between non-semiosis and semiosis; and (2) the secondary semiotic (...) thresholds, signifying the borders between different types (iconic, indexical, symbolic) of semiosis. Speaking in terms of types of semiosis means speaking in terms of different capabilities for normativity, which is why the article uses the approaches of Michel Foucault on normalization in biopower and of Georges Canguilhem on organismic normativity. As an example on which biopolitics and biosemiotics could connect, the discourse of regenerative and anti-aging medicine is used. (shrink)

This chapter focuses on documenting the changes in life expectancy and human lifespan and reviewing current understandings of demographic and epidemiological transitions. It allows assumption of shared understandings of the meaning of human aging and old age. A key policy indicator of changes in health and life expectancy is the concept of healthy active life expectancy and associated concept of disability‐free life expectancy. From a social gerontological perspective, aging is a concept that is less contested than the idea (...) of old age. The biomedicalization of aging has been a major influence across social spaces. Vincent has described four categories of anti‐aging medicine: symptom alleviation; life expectancy extension; lifespan extension; and abolition. The development of anti‐aging technology, science, and practice challenges us to think about the enhancement of human aging and the future of later life. Attitudes to aging remain essentially negative with little evidence to suggest that institutionalized ageism will disappear. (shrink)

The aim of the article is to examine the representation of ageing in selected issues of the Polish women’s magazine Twój Styl. With reference to Wolf’s concept of the “beauty myth,” the article argues that ageing is presented as a threat to women’s psychological integrity. Although the theme of old age is rarely directly addressed in the magazines, its presence is implied in the advertised anti-age beauty products. Based on semiotic theory and Cognitive Metaphor Theory, the paper demonstrates that (...) the advertising of anti-ageing beauty products serves the function of controlling ageing female bodies, which are positioned outside of the mainstream concept of femininity. This article examines the rise of the anti-vaccination movement, the proliferation of laws allowing parental exemptions to mandatory school vaccines, and the impact of the movement on immunization rates for all vaccines. It uses the ongoing debate about the Human Papillomavirus vaccine as an example to highlight the ripple effect and consequences of the anti-vaccine movement despite robust evidence of the vaccine's safety and efficacy. The article scrutinizes how state legislatures ironically promote vaccination while simultaneously deferring to the (...) opposition by promulgating broad opt-outs from mandatory vaccine laws. This article concludes by offering an alternative legislative approach to specifically combat the anti-vaccine movement's impact on HPV vaccination rates. Lowering the age of consent has not been widely attempted or proposed and provides an alternative statutory mechanism to push back against vaccine resistance. (shrink)

Ageing has become a central topic of medical ethical debates, e.g. regarding autonomy and care in geriatric practice, the just distribution of healthcare or the implications of anti-ageing medicine. In all these debates, however, particular conceptions of ageing are tacitly presupposed. The aim of my research is to develop an explicit understanding of the relevance of ageing for ethical reasoning, providing a conceptual framework for discussing concrete medical ethical problems in a more comprehensive and reflected manner. I proceed from (...) the life course approach, assuming that our life is shaped by a time regime defined and implemented by social institutions and structures, reflected in cultural images, and internalized as an “inner clock” directing individual decisions. From the ethical viewpoint, two concepts appear particularly significant: In a deontological perspective, the idea of “age norms”, social standards of age-appropriate behavior, becomes relevant. In a teleological perspective focusing on individual flourishing and good life, the concept of life plans, comprising age-related self-images and expectations, is important. I will briefly sketch the current medical ethical debates around ageing, then delineate the life course perspective and its implications for ethical reasoning, and finally exemplify its theoretical potential regarding medical ethical controversies about long time care, resource allocation and life extension. This Article argues that a universal approach to age discrimination promotes justice (including intergenerational justice) and efficiency. As explained herein, legal regimes regulate age discrimination in employment in various ways. While some regimes create specific anti–age discrimination legislation, others ban most kinds of employment discrimination, including age discrimination, in a general way. These latter promote a universal approach to age discrimination. The current Article explores the theoretical justifications for either a particularistic or a universal approach to age discrimination. While the Battle of Seattle immortalized a certain image of anti-globalization resistance, processes and agents of contestation remain sociologically underdeveloped. Even with the time-space compression afforded by new information technologies, how can a global civil society emerge among multi-cultured, multi-tongued peoples divided by miles of space and oceans of inequality? This article examines two cases that confronted the U.S. model of global corporate rule: the defeat of the Multilateral Agreement on Investment (MAI), and the Zapatista challenge to the North (...) American Free Trade Agreement (NAFTA). Evaluating cross-border solidarity in these cases encourages critical evaluation of claims about global civil society, the role of the Internet, and the eclipse of traditional politics in a supposedly post-national age. Contrary to orthodox globalization narratives, our analysis suggests that states, nations, and nationalisms remain key elements in contestation processes, at least in the kinds of cases examined. At the same time, transnational networks played an important role in bypassing unfavorable political opportunity structures at the domestic level, and nurtured incipient processes of framing resistance to neo-liberal globalism across national boundaries. Debates in fields studying the biological aspects of aging and longevity, such as biogerontology, are often split between ‘anti-aging’ approaches aimed largely at treating diseases and those focusing more on maintaining, promoting, and even enhancing health. However, it is far from clear what this ‘health’ is that would be maintained, promoted, or enhanced. Interestingly, what few have yet to fully reflect on is that there is still no theory of health within the health or aging sciences (...) that would provide an integrative explanatory framework akin to other scientific theories. After clarifying why such a theory could be useful, I discuss five general features of medical theories that could be used to evaluate the utility of a given proposal. With these features in hand, I suggest that philosophers and scientists work together on analyzing actual medical research (experimental analysis), and the ways in which a theoretical construct of ‘health’ is being progressively identified and measured therein. I conclude by suggesting that research fields studying stress and aging might be particularly helpful in developing candidates for theory construction due to their broad scope, increasing specificity, and potential for providing integrative explanations. Biological studies have demonstrated that it is possible to slow the ageing process and extend lifespan in a wide variety of organisms, perhaps including humans. Making use of the findings of these studies, this article examines two problems concerning the effect of life extension on population size and welfare. The first—the problem of overpopulation—is that as a result of life extension too many people will co-exist at the same time, resulting in decreases in average welfare. The second—the problem of underpopulation—is (...) that life extension will result in too few people existing across time, resulting in decreases in total welfare. I argue that overpopulation is highly unlikely to result from technologies that slow ageing. Moreover, I claim that the problem of underpopulation relies on claims about life extension that are false in the case of life extension by slowed ageing. Of the many topics worthy of discussion regarding older adults and bioethics, two seem to provide an especially pointed opportunity for reflection on our aging society. First, is aging itself something that biomedical researchers should focus on as a deficit to be overcome through eventual anti-aging treatments? While aging may not fall neatly into the disease category, it is clearly the primary susceptibility factor for the innumerable diseases of older adults, and therefore its potential deceleration (...) consistent with the compression of morbidity might constitute a salutary biomedical goal. The aging society is no panacea to those who suffer from a host of chronic illnesses and feel overwhelmed by the burden of years. Second, we must concentrate on the most challenging problematic of our current aging society, assuming that anti-aging technologies will only become available in future decades. One immense problem is the harsh reality of irreversible progressive dementia, which will serve here as an example of the rise of chronic illness, for which age itself is the primary risk factor. The importance of “brand dilution” is changing with the rise in internet-mediated consumer power and increasing consumer involvement in the brand identity and message creation processes. In light of recent legal rulings, this study re-conceptualizes brand dilution as a matter of counter-posed brand meanings and associations in digital markets. Anti-branding dilution cases from both a blurring and a tarnishment dilution basis are examined through consumer interviews. The results show that consumer anti-branding has less potential for brand dilution, and (...) more potential for brand identity collusion. By addressing both legal and marketing views of the meaning systems associated with the dilution versus collusion perspectives, this study provides an approach for understanding anti-branding dilution discussions and a way to develop better functioning branding exchange systems for digital markets. Where does the aspiration to retard human ageing fit in the ‘big picture’ of medical necessities and the requirements of just healthcare? Is there a duty to retard human ageing? And if so, how much should we invest in the basic science that studies the biology of ageing and could lead to interventions that modify the biological processes of human ageing? I consider two prominent accounts of equality and just healthcare – Norman Daniels's application of the principle of fair equality (...) of opportunity and Ronald Dworkin's account of equality of resources – and conclude that, once suitably amended and revised, both actually support the conclusion that anti-ageing research is important and could lead to interventions that ought to be considered ‘medical necessities’.
