In a new and interesting book entitled Medical Nihilism (2018), Jacob Stegenga attempts to convince us that modern medical therapies are less effective than we think. Given the heterogeneity of hypotheses in medicine and the evidence for or against them, I argue that such a decontextualised critique cannot be made unless substantially weakened. Instead, I put forward an alternative, more nuanced and defensible epistemic view of medicine. According to this view, evaluating medical evidence requires analysis of both the methods of (...) research e.g. randomised controlled trial (RCT), and context-specific information. This is because the way a trial (even an RCT) is conducted e.g. the population recruited and how it is intervened on, will vary and will have significant effects on the likelihood of a positive outcome. Moreover, the relationship between the positive outcome of a trial and the actual effectiveness of an intervention (the trial validity) will depend on these context specific factors. I argue for this position against nihilism by showing how each of Stegenga's individual claims about medical trials (trials are biased in favour of positive outcomes etc) can be questioned by taking the context into consideration. Defined as patients who ‘lack decision-making capacity and a surrogate decision-maker’, the unrepresented (sometimes referred to as the ‘unbefriended’, ‘isolated patients’ and/or ‘patients without surrogates’) present a major quandary to clinicians and ethicists, especially in handling errors made in their care. A novel concern presented in the care of the unrepresented is how to address an error when there is seemingly no one to whom it can be disclosed. Given that the number of unrepresented Americans is expected to rise in (...) the coming decades, and some fraction of them will experience a medical error, creating protocols that answer this troubling question is of the utmost importance. This paper attempts to begin that conversation, first arguing that the precarious position of unrepresented patients, particularly in regards to errors made in their care, demands their recognition as a vulnerable patient population. Next, it asserts that the ethical obligation to disclose error still exists for the unrepresented because the moral status of error does not change with the presence or absence of surrogate decision-makers. Finally, this paper concludes that in outwardly acknowledging wrongdoing, a clinician or team leader can alleviate significant moral distress, satisfy the standards of a genuine apology, and validate the inherent and equivalent moral worth of the unrepresented patient. Although the impact of job demands and work–family conflict (WFC) on burnout has been extensively discussed and analysed in the past literature, the role of WFC as a generative mechanism has been neglected. Therefore, the purpose of this study is to examine the mediating effects of WFC between job demands and burnout. The studied sample consisted of 713 Indian primary school government teachers who completed a self-report questionnaire assessing job demands, WFC and burnout. The results confirmed that WFC partially mediates (...) the adverse association of job demands with burnout. Primary schools should, therefore, be encouraged to provide effective practices to manage work–family interfaces. (shrink)

Thriving is a psychological state in which individuals experience a sense of vitality and a sense of learning. Thriving come from relational connections with others, and is deeply rooted in social systems. Theoretical literature suggests that thriving occurs in the presence of decision-making discretion, broad information sharing, and a climate of trust. However, no study has investigated these environmental factors empirically. Using a multiple-studies approach, we established valid and reliable scale for each of these environmental factors using experimental vignettes, confirmed (...) the association of decision-making discretion, broad information sharing and climate of trust with thriving, and identified the role of self-determination theory in determining these relationships. Our analysis was based on data collected from 512 Indian management students across five studies. The results indicate significant difference in thriving for high vs. low level for decision-making discretion, broad information sharing and climate of trust. The relationship of these environmental factors with thriving is mainly due to the fulfillment of the need for competence. Competence partially mediates the relationship of decision-making discretion with thriving, and fully mediates the relationship of broad information sharing and climate of trust with thriving. Autonomy, although positively related with the environmental factors, does not lead to thriving. Practical implications, limitations and research avenues are discussed. (shrink)

In amblyopia, abnormal visual experience during development leads to an enduring loss of visual acuity in adulthood. Physiological studies in animal models suggest that intracortical GABAergic inhibition may mediate visual deficits in amblyopia. To better understand the relationship between visual cortical γ-aminobutyric acid (GABA) and perceptual suppression in persons with amblyopia (PWA), we employed magnetic resonance spectroscopy (MRS) to quantify GABA levels in both PWA and normally-sighted persons (NSP). In the same individuals, we obtained psychophysical measures of perceptual suppression for (...) a variety of ocular configurations. In PWA, we found a robust negative correlation between the depth of amblyopia (the difference in visual acuity between the amblyopic and non-amblyopic eyes) and GABA concentration that was specific to visual cortex and was not observed in a sensorimotor cortical control region. Moreover, lower levels of visual cortical GABA were associated with weaker perceptual suppression of the fellow eye by the amblyopic eye and stronger suppression of the amblyopic eye by the fellow eye. Taken together, our findings provide evidence that intracortical GABAergic inhibition is an important component of the pathology of human amblyopia and suggest possible therapeutic interventions to restore vision in the amblyopic eye through enhancement of visual cortical GABAergic signaling in PWA. This essay draws a parallel between Macaulay’s stint as the “lawgiver” of India under the East India Company and the Anglicists-Orientalists debate that he brought to a decisive end on the one hand and on the other the culture/canon wars of the 1980s, and the neoconservative ascendancy that followed it and remained influential during the second Iraq War. Although neo-conservativism’s fierce resistance to a more inclusive liberal arts curriculum in the 1980s and its towering role during the militarization of the (...) US foreign policy in the last decade has established it as the most Eurocentric and logocentric intellectual movement at present, post-colonial studies is yet to take a serious reckoning of this ideological movement. This essay tries to fill this lacuna with an attempt to first establish Macaulayite Whiggism as a precursor of the American neo-conservatism and then to focus on Macaulay’s oedipal hostility with Sir William Temple, who was not just the most accomplished statesman of English Restoration Era (that Macaulay himself considered the golden period of English history and became its historian laureate), but also stood on the exact opposite pole of Macaulay’s Whiggish Eurocentrism. Finally, the essay will briefly consider Temple’s little-known essay “On the Ancient and Modern Learning” and its genuine and uniquely anti-logocentric outlook on non-European cultures and their histories. The traditional conception of immune function is that of a system which differentiates the organism’s own tissues (the self) from any foreign invaders (nonself), preserving the former by rejecting the latter. In a mammalian pregnancy, however, the immunologically foreign foetus is accepted by the gestator’s immune system. This presents a serious challenge to the self–nonself theory which has sometimes been called the immunological paradox of pregnancy. In this paper I shall defend the self–nonself theory against the critique posed by Thomas (...) Pradeu. In addition I critically examine the alternative he proposes—the continuity theory. The main problem I will raise for any theory of immunology is that the phenomenon to be explained in pregnancy is the non-rejection of the foetus despite the triggering of the gestator’s immune system. I will argue that contemporary modified versions of the self–nonself theory can rise to this challenge but that continuity theory cannot. As such, the immunology of pregnancy gives us a good reason to favour contemporary self–nonself theory over the continuity theory. (shrink)

Lieberman for taking the time to read and respond to our article, “Making Up Monsters, Redirecting Blame: An Examination of Excited Delirium,” so thoughtfully. They each offered us an opportunity to consider dimensions of excited delirium that we had not encountered as (...) of yet and encouraged us to think more carefully about them. In our response, we consider each of their commentaries in turn, given that, as a philosopher and a psychiatrist, they each speak to different aspects of our argument.First, we turn to Kathryn Petrozzo’s commentary “Excited Delirium: The Self-Fulfilling Prophecy of Police Brutality.” We are grateful for her generous comments and careful engagement with our arguments. While much of the commentary is supportive of our piece, Petrozzo makes three critical and insightful points that we hope to respond to here.The first point Petrozzo makes is that our assertion that excited delirium is non-falsifiable is not supported. She suggests that those “seeking to exonerate the police/first responders responsible for these deaths, are not accepting this evidence leading them to reject the excited delirium diagnosis, but rather, are crafting an incorrect causal narrative. This is deeply problematic, but it does not necessarily render excited delirium as nonfalsifiable.”Although we are intrigued by this challenge, we would like to hear more about what it would look like to falsify the phenomenon of excited delirium. The way that the causal theories related to it are structured are such that if one causal factor is not present (e.g., drug use), then another causal factor can be located (e.g., mental health), and if that is not found, another may be simply made up (e.g., genetic predisposition), making excited delirium multiply realizable in a seemingly endless fashion. This is precisely what took place in the case of Anthany Dawson, who was initially said to have been experiencing a mental health crisis, but was later said to have died from excited delirium related to a genetic condition never identified before (Hon, 2004; Razack, 2015). [End Page 361]In practice, confirmations of excited delirium are found on the basis of various unrelated factors, such that after a death involving police force, there is almost always the option to reach for excited delirium as an explanation/exoneration. According to proponents, there is no factor that could be present that could rule out the diagnosis of excited delirium as it is currently constructed. Indeed, even in the presence of video evidence showing police officers choking an individual until they stop breathing, these deaths are still attributed to excited delirium. This seems like a clear instance of non-falsifiability.Although Petrozzo might maintain that this is simply “an incorrect causal narrative,” it is a causal narrative that aligns with the current theoretical scaffolding surrounding excited delirium.The second point Petrozzo makes is regarding widespread criticisms of Popper’s demarcation criterion of falsifiability. Indeed, this criterion has been called too inclusive, in that any empirical claim can be called falsifiable in principle—even if it refers to a future apocalypse or a UFO landing. It has also been criticized for being too exclusive, in that it ignores the “remarkable tenacity of scientific theories,” as Lakatos puts it (2009, p. 516). In practice, scientists do not merely abandon a theory if it is not supported by facts; they tend to find other explanations (e.g., the measurement was faulty) rather than accept an instance of falsification.Despite this, we believe it is still worth discussing falsifiability in the context of excited delirium for two reasons. First, while falsifiability may not be successful as a criterion that draws a line between science and pseudoscience, in this case, we argue, it still works well as a signifier of pseudoscience. Recently, many interested in the demarcation question have turned towards pluralism as a way to identify pseudoscientific practices (Pigliucci & Boudry, 2013). And excited delirium offers an excellent example of a phenomenon that bumps up against many of the features that might indicate a pseudoscientific practice... (shrink)

While American physicians have traditionally practised as non-unionised professionals, there has been increasing debate in recent years over whether physicians in training (known also as interns, residents or house staff) are justified in unionising and using collective action. This paper examines specific ethical criteria that would permit union action, including a desire to ameliorate patient care as well as the goal of improving the conditions of working physicians. We posit that traditional rebuttals to physician unionisation often lean on an infinite (...) conception of a doctor’s energies and obligations, one that promotes burnout and serves to advance the financial motives of hospital management and administration. Furthermore, this paper explores the empirical justifications for collective action, which include substantial reductions in medical error. Finally, we address the free-rider problem posed by non-union physicians who might benefit from working improvements garnered through union action. We conclude that in order to maintain a notion of justice as fairness, resident physicians who benefit from union deliberations are impelled to acquire union membership or make a commensurate donation and that the healthcare organisations for which they work ought to share in the responsibility to improve patient care. Non-dualistic Vedanta insists on the importance of knowledge rather than action for attaining liberation. Knowledge is regarded as the direct and immediate means of liberation. The cause of the bondage is ignorance and the latter can be removed by knowledge. Non-dualistic Vedānta advocates that the individual is ignorant of the truth that his Self is none other than the Brahman and gets involved in the chain of transmigration. Knowledge of the Self leads to liberation. However, this position leads to a (...) debate between Non-dualistic Vedānta and Mīmāmsā School of thought with reference to understanding whether knowledge itself is a mental action. The development of democracy and the development of science are not in a simple causal relationship. Rather, history shows that science can also develop in non-democratic and autocratic societies. Given the production conditions of scientific knowledge, the natural and technical sciences, for example, need well-equipped laboratories and technical equipment. Scientists in many disciplines can only do their work in institutions that provide them with access to the facilities necessary for their research. The freedom of scientific research and its protection from (...) political interference can only be guaranteed by democratic conditions. Universities continue tobe among the most important sites of knowledge production. In the humanities and social sciences in particular, the freedom of research is often threatened. The importance of freedom in the sciences can only be understood if it is related to the fundamental value of scientific freedom. It is the responsibility of universities to safeguard this value and to protect the freedom of research. They can only fulfill their task of generating and developing knowledge if the freedom of critical research is guaranteed. (shrink)

Comparative study of Hinduism and Sikhism.

This paper chronicles the cycles of scientism and romanticism that structure the discourse on science and technology in India since 1850. However, it does not promise a detailed review of this enormous archive. On the contrary, it aspires to identify the principle concerns, the important interlocutors, the prevalent frameworks and contextualizes them socio-politically, in both their local and global embodiments. In historical time, as has been suggested elsewhere, the scientism-romanticism dialectic acquires diversified formulations. This paper analyses how agricultural policy and science deal with the problem of increasing exploitation of low quality irrigation water and consequent deterioration of water quality in the States of Punjab and Haryana in India. In these cereal growing tracts the policy objective of food security is translated into production technologies, price protection and subsidies. Deterioration of water quality is countered with technocentric solutions. The paper argues that the response of science to the complexities involved in natural resource problems or (...) in the scientific understanding of farmers partial response to technological solutions recommended to improve degraded resources, is due to the existing “administrative rationalism” of natural resource bureaucracies. This administrative rationalism, “the problem-solving discourse which emphasizes the role of the expert rather than the citizen” allows policy and science to maintain their hierarchy in determining policy goals and technological solutions with scant ecological or democratic concerns. Sustainable use of water demands institutional reform in agricultural policy and the agricultural sciences. (shrink)

This paper examines the controversial diagnosis of excited delirium, which is often employed after individuals die during an encounter with the police. Rather than asking the important, and widely explored, question of whether the diagnosis is real or not, here, we consider how it operates in the world and why it seems to stick around, despite growing controversy and resistance to its use. First, we consider the question of what kinds of people are made up through the diagnosis of excited (...) delirium, exploring how racial stereotypes are exploited and certain behaviors are engendered through the illness script offered by excited delirium. Next, we ask what work excited delirium doing is doing in the world, unpacking how it tends to justify force and redirect responsibility, as well as who is it serving, and who is it not benefitting. Finally, we interrogate the slippery logic of excited delirium, examining how the way it is employed renders it non-falsifiable, and therefore impossible to uncover as real or unreal. (shrink)

In a recent article on the interpretation of probability in evidence-based medical practice, Jonathan Fuller argues that we should interpret probabilities as credences in individual cases because this avoids some important problems. In this article, I argue that Fuller misidentifies the real issue and so fails to offer a meaningful solution to it. The real problem with making probability judgments in individual cases is deciding which objective considerations ought to constrain our formation of credences. This leads us to the reference (...) class problem, which, as Alan Hajek argues, is a problem for any interpretation of probability. (shrink)

The “centre-periphery” relationship historically structured scientific exchanges between metropolis and province, between the fount of empire and its outposts. But the exchange, if regarded merely as a one-way flow of scientific information, ignores both the politics of knowledge and the nature of its appropriation. Arguably, imperial structures do not entirely determine scientific practices and the exchange of knowledge. Several factors neutralise the over-determining influence of politics—and possibly also the normative values of science—on scientific practice.In examining these four examples of Indian (...) scientists in encounters with their peers at the centre, exceptional scientists are seen in a social context where the epistemology of science supposedly describes its practice. Imperialism imposes practices and patronage, which moderate the exchange of scientific knowledge. But, at Level Two, the politics of knowledge and the patterns of patronage within it mediate exchanges between the centre and the periphery.The first step in reconfiguring exchanges between centre and periphery —in this case, between Europe and India during the period 1850 to 1930— is to recognise the relation between the acquisition of resources and the maintenance of legitimacy and identity.67 Political life is not confined to the core of political institutions.68 Second, in examining science as practised in the colonies, it is necessary to see stages of scientific institutions, whose development structures the exchange.From the encounter of Ramchandra and De Morgan, it is evident that the centre-periphery framework should be separated from the models of transmission embedded within it. The notion of “translation” helps to suggest that scientists bring personal motives and meanings to each encounter. Ramchandra, for example, sought a novel method of teaching Indians calculus, while De Morgan's interest lay in finding a place for algebra in a liberal education.The hierarchy inherent in the centre-periphery framework compels the conclusion that, at Level Two, the autodidact outside the institutions of science must have his work presented to scientists at the centre by authoritative figures from the centre. This is not mainly a question of imperialism, but rather of patronage. The peripheral scientist could not be granted direct entry into the collegial circle until his efforts at the periphery could be translated into the language and concerns of the central community. Ramanujan's enigmatic formulas were translated into the language of analysis by Hardy, which enabled the creation of a field to which Hardy was committed. Scientists from the periphery who were already part of the circle by virtue of their training, were not necessarily subject to the same degree of attestation as other scientists from the periphery. P.C. Ray, with his DSc from Edinburgh, and his position at Calcutta University, had less difficulty in winning the trust of colleagues at the centre, even when he returned to India. On the contrary, remaining at the periphery, he moved from a context of patronage to a sphere of competition. In addition, Ray's collegiality, even at Level Two, was more comprehensive, and connected him with Level One.Eventually, the professional Indian science graduate found collegiality within the international community of scientists. Saha's self-imposed progressive nationalism constrained his identification with the centre and made him a potential competitor instead. Once having achieved eminence in the world of science, C.V. Raman and Saha shifted their work to journals of physics published in India in order to further the cause of physics research in their own country.69To go beyond the limitations of the centre-periphery model, it is necessary not merely to examine exchanges between scientists functioning in a “shared epistemological universe”,70 but also to recognise the part played by institutions, the experience of colonialism, and the forms of patronage characterising both colonialism and science. Put another way, although there is relative epistemological autonomy within the disciplinary research communities of science, the interplay between knowledge and power structures this exchange.The scientific links between colonial India and Britain at the turn of the century were mediated by structures which prefigured change. Does structure determine all? If it does, we are left with an Orientalist reconstruction of the docile native, and a passive cultural medium into which science percolates. But this neglects the role of scientists in creating new structures within which they worked. A middle position—one more sensitive to the exigencies of colonial scientific life—would be one where the participants are seen not as the dupes of “structure nor the potentates of action”, but as occupying a ground between the two.71. In this essay, we offer a preliminary account of why and how to consciously cultivate curiosity in contemporary learning environments. First, we begin by discussing some of the educational theory upon which curiosity-centric classrooms might be built: experiential learning pedagogy, feminist pedagogy, critical pedagogy, and abolitionist pedagogy. Second, recognizing that our social, cultural, political, and economic processes all shape who can be curious, about what, and when, we then formulate what we call a critically curious pedagogy. Critically curious pedagogy aims (...) to stay accountable to the complex sociopolitical processes in and against which curiosity is either cultivated or suppressed. Such pedagogy relies on the affective practices of reflexivity, mindfulness, empathy, uncertainty, and transformative questioning. Third, we identify several key elements of curiosity-based assignments by which teacher–learners from all disciplinary backgrounds—whether they be mathematicians, engineers, anthropologists, psychologists, or philosophers—can facilitate the growth of critical curiosity in their students. These elements include student leadership, a research mindset, collaborative environments, multimodal outputs, real-life applications, and community engagement. Finally, we reflect on future directions in the theory and praxis of curiosity-centric learning environments. It is our hope that this chapter provides a framework for members of teacher–learner communities of all sorts to become aware of and cultivate their own curiosity with one another. (shrink)
