This paper offers a novel, qualitative approach to evaluating the outcomes of integrating humanities and ethics into a newly revised pre-clerkship medical education curriculum. The authors set out to evaluate medical students’ perceptions, learning outcomes, and growth in identity development. Led by a team of interdisciplinary scholars, this qualitative project examines multiple sources of student experience and perception data, including student essays, end-of-year surveys, and semi-structured interviews with students. Data were analyzed using deductive and inductive processes to identify key categories (...) and recurring themes. Results suggest that students not only engaged with the curricular content and met the stated learning objectives but also acknowledged their experience in the humanities and ethics curriculum as an opportunity to reflect, expand their perceptions of medicine (and what it means to be “in” medicine), connect with their classmates, and further cultivate their personal and professional identities. Results of this qualitative study show how and in what ways the ethics and humanities curriculum motivates students past surface-level memorization of factual knowledge and encourages thoughtful analysis and evaluation about how the course material relates to and influences their thinking and how they see themselves as future doctors. The comprehensive qualitative approach reflects a holistic model for evaluating the integration of humanities and ethics into the pre-clerkship medical education curriculum. Future research should examine if this approach provides a protective factor against the demonstrated ethical erosion and empathy decrease during clinical training. (shrink)

Big data technologies are entering the world of ageing computer systems running critical infrastructures. These innovations promise to afford rapid Internet connectivity, remote operations or predictive maintenance. As legacy critical infrastructures were traditionally disconnected from the Internet, the prospect of their modernisation necessitates an inquiry into cyber security and how it intersects with traditional engineering requirements like safety, reliability or resilience. Looking at how the adoption of big data technologies in critical infrastructures shapes understandings of risk management, we focus on (...) a specific case study from the cyber security governance: the EU Network and Information Systems Security Directive. We argue that the implementation of Network and Information Systems Security Directive is the first step in the integration of safety and security through novel risk management practices. Therefore, it is the move towards legitimising the modernisation of critical infrastructures. But we also show that security risk management practices cannot be directly transplanted from the safety realm, as cyber security is grounded in anticipation of the future adversarial behaviours rather than the history of equipment failure rates. Our analysis offers several postulates for the emerging research agenda on big data in complex engineering systems. Building on the conceptualisations of safety and security grounded in the materialist literature across Science and Technology Studies and Organisational Sociology, we call for a better understanding of the ‘making of’ technologies, standardisation processes and engineering knowledge in a quest to build safe and secure critical infrastructures. Neuronal aggregates involved in conscious awareness are not evenly distributed throughout the CNS but comprise key components referred to as the neural network correlates of consciousness (NNCC). A critical node in this network is the posterior cingulate, precuneal, and retrosplenial cortices. The cytological and neurochemical composition of this region is reviewed in relation to the Brodmann map. This region has the highest level of cortical glucose metabolism and cytochrome c oxidase activity. Monkey studies suggest that the anterior thalamic projection likely (...) drives retrosplenial and posterior cingulate cortex metabolism and that the midbrain projection to the anteroventral thalamic nucleus is a key coupling site between the brainstem system for arousal and cortical systems for cognitive processing and awareness. The pivotal role of the posterior cingulate, precuneal, and retrosplenial cortices in consciousness is demonstrated with posterior cingulate epilepsy cases, midcingulate lesions that de-afferent this region and are associated with unilateral sensory neglect, observations from stroke and vegetative state patients, alterations in blood flow during sleep, and the actions of general anesthetics. Since this region is critically involved in self reflection, it is not surprising that it is similarly a site for the NNCC. Interestingly, information processing during complex cognitive tasks and during aversive sensations such as pain induces efforts to terminate self reflection and result in decreased processing in posterior cingulate and precuneal cortices. This article deals with the specific claim that prophylactic male infant circumcision should be employed to prevent HIV transmission in countries in which the prevalence of HIV is relatively low. In a recent editorial, Australian researchers sought to promote the procedure as a ‘surgical vaccine’ against HIV in their country. This raises the question whether it would be reasonable for the UK to adopt a policy of mass infant male circumcision in order to protect individuals from heterosexually acquired infection with (...) HIV. A review of the relevant data and associated commentary indicates that the actual benefits of real-world circumcision policies to prevent HIV transmission are disputed and that circumcision, at best, provides partial protection. In addition, it is uncertain whether infant circumcision confers the same benefits that the adult procedure is proposed to provide. Reasons for performing circumcisions on infants include that the procedure is easier, less complicated and cheaper. However, it is not risk free. Despite arguments to the contrary, this article contends that it is morally problematic to operate on thousands of male infants each year for little benefit to children qua children. It is also argued that the use of the term ‘surgical vaccine’ to describe the procedure is both inaccurate and misleading. (shrink)

We give an example of a theory that strongly maximizes over ZFC and discuss possible consequences of this finding.

Purpose: The purpose of the paper is to provide a constructivist account of the "self as subject" that avoids the need for any metaphysical assumptions. Findings: The thesis developed in this paper is that the human "psychological individual," "self" or "subject" is an emergent within the nexus of human social interaction. With respect to psychological and social wholes (composites) there is no distinction between the form of the elements and the form of the composites they constitute i.e., all elements have (...) the form of composites. Further, recursively, composites may serve as elements within higher order composites. Implications for a rational theory of ethics are discussed. Original Value: The thesis contributes in a fundamental way to the research programme of radical constructivism by demonstrating that metaphysical assumptions about the nature of the "subject" are not an a priori necessity. Although the thesis in itself is not original, the paper offers a useful synthesis of ideas from a number of key thinkers in the disciplines of cybernetics, biology, psychology and philosophy. Mill contends that the fullest publicity and freedom of discussion is essential for all citizens to share in political freedom. Previous commentators have focused on Mill's strictures against censorship; they have paid little attention to his understanding of how freedom of discussion and political will formation are mediated in the public sphere by the distribution of social power in civil society, particularly control of the means of communication. This article shows that Mill's account of the sociology and political economy of (...) how public opinion forms is central to his understanding of freedom of discussion and democratic political freedom. An error in Gruber's paper, “Quantization in Generalised Coordinates,” and the reason for this kind of error are pointed out. A partial answer to the problem posed by the paper is stated.

What is human reason, how did it arise, how is it connected to animal reason? In The Enigma of Reason (ER) Hugo Mercier and Dan Sperber (2017) suggest that reason evolved driven by the need to support communication and coordination in small human groups. They contrast this to the idea that the function of reason is to enable humans to make better decisions and develop more accurate beliefs. After a summation of the ER argument, the theory is critiqued and two (...) of their assumptions -- that human cognition is based on massive modularity and that reason co-evolved with and is dependent upon language -- are challenged. Alternatives to these assumptions are presented from the perspective of pattern matching processes in neural dynamical systems, which it is asserted provides a more realistic approach to the functioning of the brain and mind.
