The doctor's use of deception in appropriate circumstances has commonly been considered a necessity of the medical art. Resistance to full and frank communication is typical of many traditionally Catholic countries, and particularly of Italy, a western country where Catholicism remains particularly influential. The Catholic teaching on truth and lies, and the problem of telling the truth to a severely ill patient is discussed. It is suggested that the contemporary Catholic model of gradually telling a terminal patient the truth, which (...) looks reasonable and feasible in theory, is rarely followed in practice, as in the majority of cases the truth is not told tout court. Problems stem from the way in which medicine is currently practiced in Italy; from the synergism between Catholicism and the medical tradition's grounded paternalism; and from the ambiguity of the term ?hope?. Catholic ethics in fact recommends that the truth must be told without destroying hope, but the Catholic meaning of ?hope? is very different from its meaning in current language. (shrink)

This research paper provides an in-depth examination of how globalization has affected the economic and social standing (...) of both the poor and the rich. The authors highlight that globalization can drive economic growth, creating new opportunities and elevating living standards. For wealthy individuals and developed nations, globalization opens up new markets and fosters technological advancements, increasing their wealth and influence. However, for poorer populations, the benefits are mixed. While globalization can provide access to jobs and technology, it often comes with challenges like low wages, job insecurity, and environmental exploitation, leading to a widening income gap between the rich and poor. The author presents these dynamics to illustrate the uneven distribution of globalization's benefits and the social and economic stratification that has emerged as a result. Beyond economic impacts, the paper also emphasizes how globalization has influenced global security and environmental sustainability. The interconnectedness brought by globalization means that security issues in one region can quickly impact other parts of the world, making global stability a shared concern. This complexity is further compounded by climate change, which has been intensified by globalized industrial activity and resource consumption. The paper balances these concerns by acknowledging both positive developments, such as advancements in global cooperation for environmental action, and the urgent challenges globalization poses to equitable growth and environmental health. Introduction Families of patients with dementia may need support in difficult end-of-life decision making. Such guidance may be culturally sensitive. Methods To support families in Canada, a booklet was developed to aid decision making on palliative care issues. For reasons of cost effectiveness and promising effects, we prepared for its implementation in Italy, the Netherlands and Japan. Local teams translated and adapted the booklet to local ethical, legal and medical standards where needed, retaining guidance on palliative care. Using qualitative content (...) analyses, we grouped and compared adaptations to understand culturally sensitive aspects. Results Three themes emerged: (1) relationships among patient, physician and other professionals—the authority of the physician was more explicit in adapted versions; (2) patient rights and family position—adding detail about local regulations; and (3) typology of treatments and decisions. Considerations underlying palliative care decisions were detailed (Dutch and Italian versions), and the Japanese version frequently referred to professional and legal standards, and life-prolongation was a competing goal. Text on artificial feeding or fluids and euthanasia was revised extensively. Conclusions Providing artificial feeding and fluids and discussing euthanasia may be particularly sensitive topics, and guidance on these subjects needs careful consideration of ethical aspects and possible adaptations to local standards and practice. The findings may promote cross-national debate on sensitive, core issues regarding end-of-life care in dementia. (shrink)

Neuronal aggregates involved in conscious awareness are not evenly distributed throughout the CNS but comprise key components referred to as the neural network correlates of consciousness (NNCC). A critical node in this network is the posterior cingulate, precuneal, and retrosplenial cortices. The cytological and neurochemical composition of this region is reviewed in relation to the Brodmann map. This region has the highest level of cortical glucose metabolism and cytochrome c oxidase activity. Monkey studies suggest that the anterior thalamic projection likely (...) drives retrosplenial and posterior cingulate cortex metabolism and that the midbrain projection to the anteroventral thalamic nucleus is a key coupling site between the brainstem system for arousal and cortical systems for cognitive processing and awareness. The pivotal role of the posterior cingulate, precuneal, and retrosplenial cortices in consciousness is demonstrated with posterior cingulate epilepsy cases, midcingulate lesions that de-afferent this region and are associated with unilateral sensory neglect, observations from stroke and vegetative state patients, alterations in blood flow during sleep, and the actions of general anesthetics. Since this region is critically involved in self reflection, it is not surprising that it is similarly a site for the NNCC. Interestingly, information processing during complex cognitive tasks and during aversive sensations such as pain induces efforts to terminate self reflection and result in decreased processing in posterior cingulate and precuneal cortices. We give an example of a theory that strongly maximizes over ZFC and discuss possible consequences of this finding.

Purpose: The purpose of the paper is to provide a constructivist account of the "self as subject" that avoids the need for any metaphysical assumptions. Findings: The thesis developed in this paper is that the human "psychological individual," "self" or "subject" is an emergent within the nexus of human social interaction. With respect to psychological and social wholes (composites) there is no distinction between the form of the elements and the form of the composites they constitute i.e., all elements have (...) the form of composites. Further, recursively, composites may serve as elements within higher order composites. Implications for a rational theory of ethics are discussed. Original Value: The thesis contributes in a fundamental way to the research programme of radical constructivism by demonstrating that metaphysical assumptions about the nature of the "subject" are not an a priori necessity. Although the thesis in itself is not original, the paper offers a useful synthesis of ideas from a number of key thinkers in the disciplines of cybernetics, biology, psychology and philosophy. Previous commentators have focused on Mill's strictures against censorship; they have paid little attention to his understanding of how freedom of discussion and political will formation are mediated in the public sphere by the distribution of social power in civil society, particularly control of the means of communication. An error in Gruber's paper, “Quantization in Generalised Coordinates,” and the reason for this kind of error are pointed out. A partial answer to the problem posed by the paper is stated.

What is human reason, how did it arise, how is it connected to animal reason? In The Enigma of Reason (ER) Hugo Mercier and Dan Sperber (2017) suggest that reason evolved driven by the need to support communication and coordination in small human groups. They contrast this to the idea that the function of reason is to enable humans to make better decisions and develop more accurate beliefs. After a summation of the ER argument, the theory is critiqued and two (...) of their assumptions -- that human cognition is based on massive modularity and that reason co-evolved with and is dependent upon language -- are challenged. Alternatives to these assumptions are presented from the perspective of pattern matching processes in neural dynamical systems, which it is asserted provides a more realistic approach to the functioning of the brain and mind.
