BackgroundSarcomas are a diverse group of neoplasms that vary greatly in clinical presentation and responsiveness to treatment. Given the differences in the sites of involvement, rarity, and treatment modality, a multidisciplinary approach is required. Previous literature suggests patients with sarcoma suffer from poorer quality of life especially physical and functional wellbeing. Adolescent and young adult patients are an underrepresented population in cancer research and have differing factors influencing QoL.MethodsRetrospective analysis of Young Adult patients enrolled in the Sarcoma Tissue Repository at (...) University of Iowa. QoL was assessed using the self-report FACT-G questionnaire at enrollment and 12 months post-diagnosis; overall scores and the 4 wellbeing subscales were calculated. Linear mixed effects models were used to measure the association between the rate of change in FACT-G subscale scores and baseline clinical, comorbidity, and treatment characteristics.Results49 patients were identified. 57.1% of patients had a malignancy involving an extremity. Mean FACT-G scores of overall wellbeing improved from baseline to 12 months. Social and emotional wellbeing did not differ significantly between baseline and 12 months. Physical wellbeing and functional wellbeing scores improved from baseline to 12 months. No difference was seen for FACT-G overall scores for age, sex, laterality, marital status, performance status, having children, clinical stage, limb surgery, chemotherapy, or tumor size. A difference was demonstrated in physical wellbeing scores for patients with baseline limitation compared to those with no baseline limitation. A difference was demonstrated in social wellbeing based on anatomical site.ConclusionYoung adults with sarcoma treated at a tertiary center had improvements in overall reported QoL at 12 months from diagnosis. Overall baseline QoL scores on FACT-G were lower than the general adult population for YA patients with sarcoma but at 12 months became in line with general population norms. The improvements seen merit further investigation to evaluate how these change over the continuum of care. Quality of life changes may be useful outcomes of interest in sarcoma trials. (shrink)

A thinker as sinuous as Renan was bound to appreciate the power of subtle reasoning in such (...) a work as the admirable Recherche de la V~rit6. In a certain sense Renan is even the heir of a spirit to which Malebranche gave fresh impetus. Like Malebranche, he inveighed against pedants, dilettantes, and idolators of antiquity; he turned his back on tradition to favor modem thought over ancient, and placed his faith in a world governed by knowledge and reason in which progress and human perfectibility promise man happiness. In his basic optimism, in his belief that man is capable of knowing truth, in his somewhat pantheistic mysticism, Renan joins hands with Malebranche across the eighteenth century. How well did Renan know Malebranche? What did he think of him? Did his opinion of him undergo any evolution? To find the answers to these questions one must go back to about 1840, when Renan, a.t seventeen, began his study of Malebranche. By the time he was twenty, Renan, under the guidance of M. Gosselin, who little suspected how great a contribution Malebranche would make to Renan's decision to leave the Church, was reading the philosopher with enthusiastic regularity. Indeed, the Oratorian had become "l'objet perp6tuel" of his reflections, he tells us. The speculative terminology of the Entretiens sur la M6taphysique and the M6ditations chrgtiennes echoed through his class compositions and notebooks. His ideas on the general action of Providence in the universe were already similar to those of this philosopher, who, "illustre" and "saint," soon became for him a kind of spiritual hero, a hero whose exemplary intellectual zeal drew Renan instinctively ever more deeply into the contemplative life, into.the fervent and determined quest for "Truth." The attempt of a Christian to satisfy the intelligence through vigorous application of Cartesian rationalism to theology and science fascinated the young student in 1843 as he read the Conversations chr~tiennes and the Recherche de la Vdritd. He eagerly believed that reason leads one to knowledge of God, that the search for Truth is the search for God. Hermeneutical injustice occurs when there is a gap in the interpretive resources available to members of a society due to the marginalization of members of a social group from sense‐making practices. In this paper, I address two questions about hermeneutical injustice that are undertheorized in the recent literature: (1) what do we mean when we say that someone lacks the interpretive resources for making sense of an experience? and (2) how do marginalized individuals develop interpretive resources? In response to (1), (...) I argue that to lack interpretive resources is to lack conceptual skill or know how. In response to (2), I draw on resources from Gilbert Ryle and Andy Clark and provide a model of how marginalized individuals develop new conceptual skills by naming their shared experience and using it as a tool for scaffolding each other's conceptual performance. At the same time, I draw on the work of Gaile Pohlhaus and Kristie Dotson to show how these practices succeed only through the redistribution of epistemic power across differently situated social groups. In this short paper, I outline a non-standard account of what it feels like to have an intuition. According to this account, intuitive contents are ontologically ambiguous. Because intuition alone is liable to persuade us of both motivated inferences and necessary truths, it is not a reliable source of evidence. However, we would not be able to grasp the concept of necessity without intuitions. Hence, I do not think it is any good to ignore or quarantine our intuitions when forming (...) judgments. (shrink)

The paper begins by contrasting the unitary nature of conscious experience with the demonstrable localization of neural events. Philosophers and neuroscientists have developed models to account for this paradox, but they have yet to be tested empirically. The author proposes a `Conscious Mental Field', which is produced by, but is phenomenologically distinct from, brain activity. The hypothesis is, in principle, open to experimental verification. This article aims to stimulate dispute about the justification of Paul Ricœur’s hermeneutic reading of John Rawls. Offering a close, methodically point-for-point textual engagement, I shall propose that Ricœur’s misreading of certain hermeneutic circularities in Rawls is owed to some confusion about the role of the procedural nature of Rawls’ theory. Generally speaking, the problems with Ricœur’s interpretations center on the question of whether there is something “pre-understood” within the formal theoretical understanding of the procedural theory of justice and the (...) substantive convictions and judgments that figure within the reflective equilibrium of deliberations about the terms of justice. Arguably, Ricœur has not made a satisfactory case that the difference and liberty principles are considered convictions that anticipate their discovery and establishment. Ultimately, Ricœur has not demonstrated that there is a single presuppositional form that renders Rawls’ procedure self-defeating. Instead, he has proposed to us several potential forms of damaging presupposition, each of which is based on a questionable reading of Rawls’ text. (shrink)

Challenging the assumption that that the capitalist transformation includes a radical break with the past, this edited volume traces how historically older forms of social inequality are transformed but persist in the present to shape the social structure of contemporary societies in the global South. Each society comprises an interpretation of itself - including the meaning of life, the concept of a human being and the notion of a collective. This volume studies the interpretation that various societies have of themselves. (...) This interpretation is referred to as social ontology. All chapters of the edited volume focus on the relation between social ontology and structures of inequality. They argue that each society comprises several historical layers of social ontology that correspond to layers of inequality, which are referred to as sociocultures. Thereby, the volume explains why and how structures of inequality differ between contemporary collectives in the global South, even though all of them seem to have similar structures, institutions, and economies.
