The present study aims to explore whether the symptoms associated with fibromyalgia are contextually influenced by the area of residence. Furthermore, it is analyzed whether the effect of the acceptance of the disease on the emotional, cognitive and physical symptoms is moderated by the patients’ place of residence. Using a cross-sectional design, a total of 234 women with fibromyalgia were surveyed, of which 55.13% resided in rural areas and 44.87% in urban areas. Self-reported questionnaires were used to assess pain severity, (...) anxiety and depression, functional limitation, physical and mental fatigue and acceptance of the disease. The results show significant differences in acceptance, pain, and physical and mental fatigue between patients from rural and urban areas. The rural area patients presented higher levels of acceptance and pain and lesser levels of physical and mental fatigue compared to the urban area. The moderation analysis add that, only in patients from the rural area, the variables of physical symptoms were significantly and negatively associated with acceptance. This study addresses for the first time the role of the place of residence in suffering from fibromyalgia, suggesting that the rural or urban environment plays a relevant role in the severity and/or management of symptoms in fibromyalgia women. Research on mental health of non-professional caregivers has focused on caregivers of people with specific diseases, especially dementia. Less is known about caregivers of people with other diseases. The aims of this study were (a) to determine the caregivers’ emotional state in a random sample of caregivers of people in situations of dependency, (b) to analyze the association between each disease of the care-recipient (a variety of 23 diseases included in the International Classification of Diseases) and the emotional state of (...) the caregiver, and (c) based on the theoretical model, to analyze the relationship of the different study variables in the appearance of the emotional distress of the caregiver. A sample of 491 non-professional caregivers was selected randomly (89.0% women, average age 55.3 years). Trained psychologists collected sociodemographic and care-related characteristics and evaluated the global emotional distress, somatic symptoms, anxiety-insomnia, social dysfunction, depression, probable mental disorder case, self-esteem and social support. It was found that (a) the caregivers showed moderate emotional distress, and 33.8% presented a probable mental disorder. (b) Caring for a care-recipient with cat's cry syndrome or epilepsy was related to suffering from social dysfunction, and caring for a care-recipient with autism was related to having a probable mental health case. (c) Social support mediated the relationship between social class, daily hours of care, monthly family income, self-esteem and global emotional distress. There is an important impact on the emotional state of the caregivers. This impact was similar in caregivers of care-recipients with different diseases, except in caregivers caring for a care-recipient with cat's cry syndrome or epilepsy (related to social dysfunction), and in caregivers caring for a care-recipient with autism (related to having a probable mental health case). Nowadays, the quality standards of higher education institutions pay special attention to the performance and evaluation of the students. Then, having a complete academic record of each student, such as number of attempts, average grade and so on, plays a key role. In this context, the existence of missing data, which can happen for different reasons, leads to affect adversely interesting future analysis. Therefore, the use of imputation techniques is presented as a helpful tool to estimate the value of missing (...) data. This work deals with the academic records of engineering students, in which imputation techniques are applied. More specifically, it is assessed and compared to the performance of the multivariate imputation by chained equations methodology, the adaptive assignation algorithm based on multivariate adaptive regression splines and a hybridization based on self-organisation maps with Mahalanobis distances and AAA algorithm. The results show that proposed methods obtain successfully results regardless the number of missing values, in general terms. (shrink)

It is sometimes argued that having inconsistent desires is irrational or otherwise bad for an agent. If so, if agents seem to want a and not-a, then either their attitudes are being misdescribed – what they really want is some aspect x of a and some aspect y of not-a – or those desires are somehow 'inconsistent' and thus inappropriate. I argue first that the proper characterization of inconsistency here does not involve logical form, that is, whether the desires involved (...) have the form 'a and not-a', but rather the possibility of fulfilling all one's desires; and secondly, that the 'essential' conflicts involved in such inconsistencies are quite common and no worse for an agent than contingent conflicts. I draw implications concerning moral epistemology, moral realism and the logic of attitudes. (shrink)

After experiments with various economic systems, we appear to have conceded, to misquote Winston Churchill that "free enterprise is the worst economic system, except all the others that have been tried." Affirming that conclusion, I shall argue that in today's expanding global economy, we need to revisit our mind-sets about corporate governance and leadership to fit what will be new kinds of free enterprise. The aim is to develop a values-based model for corporate governance in this age of globalization that (...) will be appropriate in a variety of challenging cultural and economic settings. I shall present an analysis of mental models from a social constructivist perspective. I shall then develop the notion of moral imagination as one way to revisit traditional mind-sets about values-based corporate governance and outline what I mean by systems thinking. I shall conclude with examples for modeling corporate governance in multi-cultural settings and draw tentative conclusions about globalization. (shrink)

Tumor necrosis is a common histological feature and poor prognostic predictor in various cancers. Despite its significant clinical implications, the mechanism underlying tumor necrosis remains largely unclear due to lack of appropriate pre‐clinical modeling. We propose that tumor necrosis is a synergistic consequence of metabolic stress and inflammation, which lead to oxidative stress‐induced cell death, such as ferroptosis. As a natural consequence of tumor expansion, tumor cells are inevitably stripped of vascular supply, resulting in deprivation of oxygen and nutrients. The (...) resulting metabolic stress has commonly been considered the cause of tumor necrosis. Recent studies found that immune cells, such as neutrophils, when recruited to tumors, can directly trigger ferroptosis in tumor cells, suggesting that immune cells can be involved in amplifying tumor necrosis. This article will discuss potential mechanisms underlying tumor necrosis development and its impact on tumor progression as well as the immune response to tumors. (shrink)

The purpose of this research is the analysis of a specific cultural and political tension, through two decisive concepts: imagocracy and imagomaquia. Our aim is to define the political battlefield in the arena of audiovisual communication. In the case of Latin America, we have identified two significant moments, in which the popular audiovisual strategies had crucial importance. Our material of research has been both the New Latin American Cinema and the sociology of the decolonial image. With the demise of Marxism and socialism, the United States is becoming a model not merely for free enterprise, but also for employment practices worldwide. I believe that free enterprise is the least worst economic system, given the alternatives, a position I shall assume, but not defend, here. However, I shall argue, a successful free enterprise political economy does not entail mimicking US employment practices. I find even today in 1998, as I shall outline in more detail, these practices, when (...) consistently carried out, by and large erode trust in the workplace, they are, on balance unfair to workers and managers, and, if Jeffrey Pfeffer is correct, they do not maximize long-term corporate earnings or growth. Getting clear on US employment practices and their weaknesses may help to shape other models for employment that neither contravene free enterprise nor are degrading to workers. (shrink)

Sport is an emotional experience. Studies have shown that high emotional intelligence (EI) is associated with better sports performance, though different aspects of sports experience and their relationship with EI are still unclear. This study examined the possible relationships between sports experience and EI dimensions of undergraduate athletes. Likewise, according to the differences described in the literature between men and women, the secondary aim was to identify the possible relationship between EI and sports experience in both subgroups. A total of (...) 1784 [712 men (39.9%), 1072 women (60.1%); mean age = 21.3 years, SD = 4.2)] undergraduate athletes completed the Trait Meta Mood Scale and a sports experience questionnaire. Comparisons between groups were performed using Mann–Whitney-Uand H-Kruskal–Wallis tests and correlations between variables were analyzed using Spearman correlation. We found that the number of different sports practiced and the number of years practicing sports were positively associated with emotional repair (ER). However, the number of years practicing sports was negatively associated with emotional attention (EA). Male athletes who trained more and had a higher competitive level were more likely to show higher ER. In any case, it is necessary to take into account that all the associations were weak. Our study suggested that athletes tend to attend to and value their feelings and use positive thinking to repair their negative moods. (shrink)

From the perspective of African American women firefighters, the authors examine the social interactions that make them excluded “outsiders within” their firehouses and different from not only dominant white men but also other subordinated groups of Black men and white women firefighters. Drawing on extensive survey data from 24 Black women career firefighters nationwide and detailed interviews with 22 of these, the authors found persistent and pervasive patterns of subordination through the exclusion of Black women, reflected in insufficient instruction, coworker (...) hostility, silence, close supervision, lack of support, and stereotyping. Perceived differences of Black women from white and Black men as well as white women created strained relations, especially when Black men and white women gained some acceptance by virtue of their gender and race, respectively, and thus reportedly distanced themselves from Black women. The experiences of African American women firefighters highlight the omnirelevance and intertwining of race and gender. (shrink)

In Study 1, 37 British Muslims reported incidents in which they were devalued as Muslims. In Study 2, 108 British Muslims were presented with objective evidence of their in-group's devaluation: the controversial cartoons about Prophet Muhammad The appraisal of harm to social-image predicted anger and shame, whereas the appraisal of offense only predicted anger. Anger was a more empowering response (...) than shame, as anger predicted willingness for public confrontation, institutional punishment, and written disapproval. In contrast, shame only predicted written disapproval. From Embodied Cognition to the Cognitivised Body. The construction of meaning, before being a linguistic or neuronal phenomenon, is a sensitive phenomenon, indebted to the bodily experience of the world, the lived body. Varela’s neurophenomenological approach, which is inspired by the intertwining of the subject and the world as proposed by Merleau-Ponty, can only take in charge an ordinary production of meaning. What about when one produces a concept or a work of art? In other words, how does the body-mind (...) relationship function in the act of creation? If the construction of meaning starts from the subject, in the sense that it is the subject who by his embodied cognitive activity produces meaning, the construction of a concept or a work of art solicits a super-personal force that engenders the subject himself: a heccéité, in the sense of Deleuze. What does this engendering of the subject mean and how does it intervene in the act of creation? In other words, why must the subject be somehow “recreated” in order to create? It is only when thought is destabilized by a point of crisis that it becomes a creative device that plays out between the chaotic intensities from which it tears itself away and the composition of a consistency. The starting point of the creative thought is the stopping of the thought and its continuation on another plane: a thought that leaves the field of cognition and recognition and derails, carried away by a sensitive line of flight, produced in the body, towards the inorganic and impersonal plane of a super-personal power. With the act of creation, the embodied cognition swings towards a de-subjectivation: the cognition be-comes then a “chaognition”, an impersonal faculty mobilizing the power of passivity. (shrink)

Research involving human participants has been conducted in the Philippines since the beginning of the Spanish colonial period. Such studies are expected to adhere to internationally accepted ethical guidelines. This paper discusses trends in clinical research ethics in the Philippines during the American colonial period (1898‐1946). Specifically, studies were assessed on: 1) their observance of ethical protocols, including review; 2) identification of inclusion and exclusion criteria in the selection of participants; 3) use of vulnerable subjects; and 4) practice of the (...) informed consent process. Only the informed consent process had a significant logistic correlation with progression of years. Recruitment of vulnerable groups was common during this period; children and prisoners were the most common participants. Trends in medical ethics in the Philippines reflected those in the United States prior to the publication of the Nuremberg Code, which served as a milestone in the protection of human welfare in clinical research. This paper examines the implications of the General Agreement on Trade in Services (GATS), the World Trade Organization’s agreement governing trade in health-related services, for health policy and healthcare reform in the United States. The paper describes the nature and scope of US obligations under the GATS, the ways in which the trade agreement intersects with domestic health policy, and the institutional factors that mediate trade-offs between health and trade policy. The analysis suggests that the GATS provisions on market access, (...) national treatment and domestic regulation, which are designed to eliminate ‘regulatory barriers’ to global trade in health services, limit the range of options that state and federal regulators and legislative bodies can employ to regulate the health sector and implement healthcare reforms. In June 1992, a national mail survey was directed to 204 state inpatient psychiatric institutions. This study was implemented following the 1992 Joint Commission on Accreditation of Healthcare Organizations (JCAHO) requirement that hospitals put in place some means with which to address ethical issues. The goals of the study were: 1. to examine state mental hospital characteristics and their response to the JCAHO requirements; 2. to describe healthcare ethics committee (HEC) composition, function, and role; 3. to study patient and family (...) access to a HEC; and 4. to discover ethical issues most frequently addressed. The survey response rate was 71%. Of the 145 responders, 62 had HECs in place, and 53 were in the process of implementing HECs. There were no differences between JCAHO accredited institutions and non-accredited facilities with regard to having HECs. Physicians accounted for 22% of HEC membership, followed by nurses (15%), and administrators (12%). Twenty-six percent of facilities systematically notified patients about existence of the HEC; 71% of facilities had no patient request for HEC services in the last year. A patient's danger to others, and resuscitation policy were issues most frequently (23%) heard by HECs. (shrink)

Over the last decade, and culminating in the 2008 global financial meltdown, there has been an erosion of trust and a concomitant rise of distrust in domestic companies, multinational enterprises, and political economies.In response to this attrition, this article presents three arguments. First, we suggest that trust is the “glue” of any viable political economy, and we propose that the stakes of violating public trust are particularly high in light of the asymmetry between trust and distrust. Second, we identify a (...) constellation of key barriers to overcoming distrust that companies face in the current environment: (1) corporate mind-sets that promote a preoccupation with quantification, hierarchical leadership models, and “blind trust” in authority; (2) the anonymity of core stakeholders; (3) the agency of the media as a driver of the political economy; and (4) firm-centric models of stakeholder relationships.Third, we argue that, notwithstanding these challenges, these phenomena are not fatal and can be addressed through a holistic transformation in corporate culture. Such a transformation might include a shift to collaborative leadership models and replacing authority models with responsibility, a “names and faces” approach to stakeholders through cases and stories, more egalitarian communication exchanges with external stakeholders, and a reprioritization of the firm as a vital element among others within a system rather than the central core of a network.We conclude that the value of trust at the individual, institutional, national, and global levels cannot be overstated. Without a reinvigoration of trust in our political economies, at all levels, the future of an economically vibrant planet is indeed bleak. (shrink)

The language of evolutionary biology and psychology is built on concepts applicable in the first instance to individual strategic rationality but extended to the level of genetic explanation. Current discussions of mental disorders as evolutionary adaptations would apply that extended language back to the individual level, with potentially problematic moral/political implications as well as possibilities of confusion. This paper focuses on one particularly problematic area: the explanation of women's greater tendency to depression. The suggestion that there are "good evolutionary reasons" (...) for depression makes sense, and might be helpful to note in therapy, as implying that the tendency is not a defect. However, evolutionary adaptiveness should not be confused with individual or psychological adaptiveness. Besides making reference to an earlier environment, it presupposes a strategic standpoint that may not accord with the legitimate interests of the individual, as this example makes vivid. (shrink)

This article will defend a very simple thesis. In a diverse globalized “flat” world with expanding economic opportunities and risks, we will need to revisit and revise our mindsets about free enterprise, corporate governance, and leadership. That we can change our mindsets and world view is illustrated by studies of primate behavior, and the kind of leadership necessary in a global economy is, interestingly, exemplified by women.

In this paper we defend that elective ventilation (EV), even if conceived as the instrument to maximise the chances of organ recovery, is mainly the means to provide the patient who is dying with a dignified death in several ways, one of them being the possibility of becoming an organ donor. Because EV does not harm the patient and permits the medical team a better assessment of the patient's clinical trajectory and a better management of the dying process by the (...) family, EV does not violate the principle of non-beneficence nor the principle of autonomy if we restrict the initiation of EV to those cases in which it is not known what the previous wishes of the patient were as regards to his or her care at the end of life. This essay reviews the documents of the pontifical magisterium of the Church from the encyclical Mater et magistra (1961) to the exhortation Evangelii gaudium (2013), in order to show the Church’s historical commitment to the defense of the environment. It then argues that Laudato si’ elevates the theological status of the environmental crisis to that of a new social issue, much as Leo XIII did for the industrial crisis with his encyclical letter Rerum novarum (1891).

Background Ethical conflicts generate difficulties in daily clinical activity. Which methods of ethical advice are most frequently used to resolve them among Spanish doctors has not been studied. The objective of this study is to describe what methods hospital internal medicine physicians in Spain use to resolve their ethical doubts and which they consider most useful. Design A cross-sectional observational study was conducted through a voluntary and anonymous survey and distributed through an ad hoc platform of the Spanish Society of (...) Internal Medicine. Measures We measured methods by which to resolve doubts, types of tools sought, frequency of consulting the Clinical Ethics Committees, and satisfaction with resolution of ethical issues. Results Of 261 internists surveyed, 86 per cent resolve their ethical doubts with assistance, the most frequently used method being consultation with colleagues (58.6 per cent), followed by using specific protocols or guides (11.8 per cent) and consultation with experts in bioethics (9.6 per cent). The most preferred tools are the creation of protocols (30.3 per cent) and the establishment of a consultant/expert in bioethics (27.8 per cent). Conclusions Internists in Spain usually seek assistance to resolve their ethical doubts. Consulting colleagues is the most frequently adopted method. The majority regard tools to resolve ethical conflicts as necessary, seeking above all protocols and consultants/experts in bioethics. (shrink)

I argue that because bluffing, puffing, and spinning are features of corporate life, they are likely to characterize the doctor-patient relationship in managed care medicine. I show that managed-care organizations (MCOs) and the physicians who contract with them make liberal use of puffing and spinning. In this way, they create a context in which it is likely that patients will also use deceptive mechanisms. Unfortunately, patients risk their health when they deceive their doctors. Using the warranty theory of truth I (...) argue that although bluffing may be ethical in business because all participants agree to it and business has not warranted truth-telling, it is not ethical in a medical context because physicians and MCOs have warranted truth-telling and the quality of medical care depends on it. El artículo presenta, reconstruye y contextualiza la discusión filosófica a la cual Michel Henry somete el planteamiento de Martin Heidegger acerca de la disposición afectiva (Befindlichkeit). Henry toma como hilo conductor el concepto de cooriginariedad (Gleichursprünglichkeit) entre las estructuras existenciales. Sostendremos que la lectura de Henry no solo afecta a una herramienta metodológica central de Ser y tiempo; también nos aproxima, con una economía filosófica nada desdeñable, al núcleo argumentativo de la obra principal de Heidegger, núcleo que se entreteje a (...) partir de las estrechas relaciones que guardan ontología, fenomenología y analítica existencial. Sin embargo, el planteamiento de Henry ha sido encasillado dentro del «giro teológico» de la fenomenología. Con lo cual será necesario mostrar que la crítica a Heidegger puede sostenerse con independencia de la deriva teológica de la filosofía henriniana. (shrink)

¿En qué consistiría una filosofía de la universidad? ¿Cómo esbozar el modo en que ésta puede proceder? ¿Qué puede decir hoy la filosofía para quienes trabajan en esta institución? Para responder estas preguntas se propone revisar algunos de los textos en los que el pensador de origen argelino habla de manera directa sobre la universidad: Las pupilas de la Universidad, La Universidad sin condición y Cátedra vacante: censura, maestría y magistralidad. Los tres textos fueron escritos para ser leídos en público (...) como lecciones inaugurales o conferencias. La tesis de este artículo es que una filosofía de la universidad debe asumir que esta institución es el lugar en el que todo debe ser cuestionado y debe reflexionar críticamente sobre las metáforas que se usan para referirse a su razón de ser, su esencia y fundamento. Para desarrollar la idea central el artículo se organiza en cuatro partes: primero, se revisa el modo de proceder y las preocupaciones generales, a partir de la relación que se puede establecer con elementos de dos textos de Martin Heidegger; segundo, se examina las metáforas sobre la visión y la luz; tercero, se reflexionará sobre la destinación y razón de ser de la universidad; por último, se tratará la pregunta ¿quiénes somos nosotros en esta corporación de profesores y alumnos? The primary purpose behind effectiveness research is to determine whether a treatment with demonstrated efficacy has utility when administered to the general population. The main questions these studies are meant to answer are these: Can the typical patient respond to treatment? Is the treatment acceptable to the typical patient? Can the treatment be administered safely and in its entirety in the typical treatment setting? Is the treatment under study significantly better than the community standard of care both from a cost (...) and outcome perspective? Answering these questions is meant to provide sufficient information to providers and policymakers so that effective interventions can be adopted and become the new community standard. For this research to make a meaningful impact on a provider and policymaker's decision to change the status quo, study interventions should be compared to the existing community standard of treatment, often referred to as treatment as usual (TAU). From an ethical perspective, this decision may not always be the safest choice. In some populations, TAU may mean no treatment at all, and in others TAU may be worse than withholding treatment. The effectiveness researcher is then caught between the pull to do no harm and the need for research to have an impact on change. The purpose of this article is to highlight certain conditions when TAU is ethically acceptable and to discuss alternatives when TAU may be an unethical treatment condition. For purposes of precision, we focus exclusively on psychotherapy effectiveness research rather than system-intervention research or medication-intervention research. (shrink)

This work addresses limitations in current approaches to rhetorical historiography and provides fresh philosophical ground that responds to these limitations. By integrating philosophical ideas, a philosophy of communicative engagement is formed and illustrated with descriptions of three women’s successful efforts to change the face of society.

Mark Wilson, in his 1985 paper entitled "What Is This Thing Called 'Pain'?: The Philosophy of Science Behind the Contemporary Debate," proposed an account of physicalism that departs significantly from standard approaches. One of the main points of his paper was to explain the flaws in arguments claiming that psychological properties cannot be shown to be physical because of their functional nature. However, the positive proposal that Wilson makes in this article bears further examination. I argue that it not only (...) resolves many problems that have grown up around the topic of physicalism, but that the proposal itself should make us radically rethink some important philosophical questions, especially those concerning explanation and property identification. El duelo de una persona adulta por la muerte de un animal no humano suele ser percibido por quienes la rodean como ridículo, frívolo o patológico. A estas actitudes reprobatorias típicamente subyacen creencias relativas a las limitadas facultades de los animales no humanos y/o los supuestos requerimientos de una vida humana plena. Consideraré varios argumentos en contra de tales creencias y espero que eso desactive algunas de las objeciones que podrían plantearse a la siguiente tesis que quiero defender: El duelo (...) por un animal no humano puede entenderse adecuadamente en términos de la frustración de los deseos categóricos que pierden sentido tras su muerte. Para sostener esta tesis argumentaré que las nociones de vida plena y de identidad práctica son constitutivas de las concepciones tanto de deseo categórico como de duelo. (shrink)
