Stiff Person Syndrome is a rare autoimmune disorder associated with antibodies against glutamic acid decarboxylase, the key enzyme in γ -aminobutyric acid synthesis. In order to investigate the role of cerebral benzodiazepinereceptor binding in SPS, we performed [ 11 C]flumazenil positron emission tomography in a female patient with SPS compared to nine healthy controls. FMZ is a radioligand to the postsynaptic central benzodiazepine receptor which is co-localized with the GABA-A receptor. In the SPS patient, we found a global reduction of (...) cortical FMZ binding. In addition, distinct local clusters of reduced radiotracer binding were observed. These data provide first in vivo evidence for a reduced postsynaptic GABA-A receptor availability which may reflect the loss of GABAergic neuronal inhibition in SPS. (shrink)

Theories of truth and vagueness are closely connected; in this article, I draw another connection between these areas of research. Gupta and Belnap’s Revision Theory of Truth is converted into an approach to vagueness. I show how revision sequences from a general theory of definitions can be used to understand the nature of vague predicates. The revision sequences show how the meaning of vague predicates are interconnected with each other. The approach is contrasted with the similar supervaluationist approach.

The legal requirements and justifications for collecting patient-identifiable data without patient consent were examined. The impetus for this arose from legal and ethical issues raised during the development of a population-based disease register. Numerous commentaries and case studies have been discussing the impact of the Data Protection Act 1998 and Caldicott principles of good practice on the uses of personal data. But uncertainty still remains about the legal requirements for processing patient-identifiable data without patient consent for research purposes. This is (...) largely owing to ignorance, or misunderstandings of the implications of the common law duty of confidentiality and section 60 of the Health and Social Care Act 2001. The common law duty of confidentiality states that patient-identifiable data should not be provided to third parties, regardless of compliance with the DPA1998. It is an obligation derived from case law, and is open to interpretation. Compliance with section 60 ensures that collection of patient-identifiable data without patient consent is lawful despite the duty of confidentiality. Fears regarding the duty of confidentiality have resulted in a common misconception that section 60 must be complied with. Although this is not the case, section 60 support does provide the most secure basis in law for collecting such data. Using our own experience in developing a disease register as a backdrop, this article will clarify the procedures, risks and potential costs of applying for section 60 support. (shrink)

In recent years there has been increasing concern about the various forms of abuse faced by birthing patients during labour and childbirth. Common examples include being scolded, slapped, pinched, stabbed with scissors or struck with a ruler or other instruments. This mistreatment is collectively termed obstetric violence.A growing body of literature examines legal responses to obstetric violence including the potential use of the criminal law. The present article explores whether, in South Africa, common-law crimes or statutory offences could be used (...) to prosecute healthcare workers for the range of harms falling within the broad definition of obstetric violence. It does not question whether criminal law is an appropriate response in this instance.The article concludes that existing crimes are sufficient to address obstetric violence. It is clear that the common-law crimes of crimen iniuria, assault, assault with intention to commit grievous bodily harm and the statutory offence of involuntary sterilisation, could be used to address both physical and emotional forms of obstetric violence. It is submitted that they cater adequately for the broad range of conduct that potentially falls into the definition of obstetric violence. Further research is required in this area and it may mean that prosecutorial guidelines are needed. (shrink)

In “Aristotle, Empedocles, and the Unity of All Things,” Michael Shaw has provided a novel and intriguing account of Empedocles’ cosmology, wherein he attempts to outline a few ways in which interpreters (beginning with Aristotle) have failed to capture fully Empedocles’ most important ideas. Central to Shaw’s account of Empedoclean cosmology is a distinctive interpretation of the life cycle of the cosmos that presupposes that the four elements that make up everything and the twin forces that govern them—Love and Strife—interact (...) in ways quite different than often is supposed. The upshot of this is that Shaw thinks Empedocles’ theory may not fall prey to the Aristotelian critiques designed to undermine it because they are based on a misunderstanding of his actual view. While I am sympathetic to his impulse to defend Empedocles against Aristotle’s attacks, I am not convinced that Shaw’s reading should be widely adopted, both because I highly doubt that the depiction of Empedoclean cosmology developed by Shaw is Empedocles’ own and because I think the traditional interpretation of Empedocles’ cosmology fares better against Aristotelian critiques than Shaw’s interpretation of it does. It considers the justifications which may be sought when rights are claimed. Particular attention is given to the problem which arises when different rights are seen to be in conflict with each other or with other kinds of moral consideration. Psychologist Eugene C. Goldfield offers an exciting new theoretical framework--based, in part, on the concept of self-organization--that promises to aid researchers in their quest to discover the underlying origins and process of behavioral development.

This is a big-picture book, 2 written with a breadth of focus which I find admirable. It exhibits what's come to be known as the ‘intersubdiscplinary’ approach to philosophy, which is not restricted by traditional boundaries within the discipline but rather proceeds with an eye to all sorts of areas of philosophy where relevant arguments, results, analogies and strategies might be lurking. I approve of this way of doing philosophy; it seems to me that all too often that wheels are (...) reinvented, or crucial points missed, because of a lack of this sort of intersubdisciplinarity.To help guide us through such a broad landscape, this book needed to be well-signposted and clearly written, and it is. It cannot be denied, however, that the ambitious scale of The Nature of Normativity won't be to every taste. For one thing, not everyone likes big pictures. For another, this is a deep, dense and in many ways difficult book, which can only really be appreciated by sitting down with it for hours on end. It will not repay the casual skim-reader, nor is it likely to appeal to those who prefer dipping into philosophy books over reading them wholesale.The book is also long . The view under construction has a great many interlocking elements, including non-reductive normative naturalism, normative realism, a priorist intuitionism, normative internalism, the normativity of intentionality, conceptual-role semantics, contextualism, possible-worlds semantics, and certain ideas about ‘planning’ derived from the recent work of Allan Gibbard. It will be a tough read for specialists who do not share Wedgwood's familiarity with a wide range of subdisciplines, who will either have to content themselves with a superficial reading of many passages or else hunt down the necessary background material to …. Despite the prominence of healthcare-relatedconcerns in public debate, the ground remainsinfertile for the idea of conscripting citizensinto medical research. Reluctance to entertainthe thought of a system where nearly everyonecould be selected for service might reflectuncertainty about what the project wouldinvolve. There might also be a fear that themore crucial issue is how to protect researchsubjects within current, voluntary systems. Nodoubt reluctance to explore a system ofuniversal service results from the common hopethat each of us might avoid research in anycapacity besides (...) researcher. A system of fullcivic participation might, however, avoid manyof the usual objections. Ethics regulations,including informed-consent guidelines, couldfor the most part remain in force. Though thesystem would compel people to serve, it couldremain responsive to principles of autonomy andjustice if it centered on broad publiceducation, community representation, and alottery-type selection process. The systemcould draw from the largest possiblecross-section of society, and offer conscriptsthe widest possible range of service. An appropriate combination of Quine’s ontology and Frege’s semantics is useful to emphasize the empirical entities “ethnic group” and “nation” expressed through an axiomatic system. If “cultural diversity” is taken as a primitive because it is good ethically and it is beautiful esthetically, then “internationalist nationalism” is the correct order of human society from a political point of view.

De generationewas the last of the three works published by William Harvey during his lifetime. Although this work on generation was most ambitious, being the product of prolonged and detailed researches, it has received relatively little attention from modern writers. It is generally felt that this work, like William Gilbert'sDe mundo, departs significantly from the more pronounced empirical approach to science which characterized Harvey's first publication,De motu cordis. De generationeshows that Harvey regarded reference to teleological and vitalistic principles as necessary (...) for the solution of crucial problems in biology. In this respect he differed from his contemporaries, the iatrochemical and iatromechanical physiologists, whose non-teleological approach seems, at least superficially, to be in sympathy with the modern biological tradition. The structure and content ofDe generationeare so evidently determined by Aristotle's biological writings, that the work is used to illustrate Harvey's failure to emancipate himself from the philosophical encumbrances of antiquity. This paper sets out to assess the internal coherence of Roy Bhaskar's transcendental realist account of science. Whilst fully supporting his transcendental derivation of a stratified ontology of structures and generative mechanisms from the scientific practice of experimentation, I argue that Bhaskar's adoption of the stance of epistemic relativism results in his inability to defend the generalizability of this ontology. My argument against his epistemic stance turns on the fact that it rests on a false dichotomy between epistemic relativism and (...) epistemic absolutism, and, moreover, that it is quite unnecessary for his efforts to avoid the excesses of epistemological foundationalism. The net result of this stance, in my view, is an inability to sustain the normative force of the concepts of truth and scientific explanation precisely at a time when postmodern authors are denying their privileged status. In Defense of Utilitarianism, C.L. Sheng provides a more intensive study of the Unified Utilitarian Theory , which he proposed in his previous work A New Approach to Utilitarianism . Sheng defends utilitarianism, particularly UUT, against the objections and attacks raised by nonutilitarians, showing it to be a viable ethical theory.

The elegiac poem of eight lines discovered in the Ceramicus and published by by W. Peek is of considerable interest for the historian. Peek is surely right in maintaining that it was composed for the Athenians who fell under Tolmides at Coronea in 447 B.C., and his general exposition of the poem's meaning is convincing. The aim of this paper is to make some comments and supplements to his interpretation and then to consider some peculiarities in the thought and technique (...) of the poem. I Want to discuss and illustrate in this paper certain fallacies which we are all very liable to commit in our thinking about political and social questions. Perhaps “thinking” is rather too high-sounding a name to attach to the mental processes which lie behind most political talk. It is at any rate thinking of a very low grade, for a considerable proportion of such discussion in Press and Parliament and private conversation hardly rises above the intellectual level of disputes between (...) boys at a preparatory school.
