PurposeContinuous patient-reported outcomes to identify and address patients’ needs represent an important addition to current routine care. The aim of this study was to identify and determine important dimensions of health-related quality of life in routine oncological care.MethodsIn a cross-sectional qualitative study, interviews and focus groups were carried out and recorded. The interviewees were asked for their evaluation on HrQoL in general and specifically regarding cancer treatment. The material was transcribed and analyzed using qualitative content analysis based on Mayring. (...) The results were reviewed in an expert discussion.ResultsInterviews with patients and clinicians, as well as five focus groups with clinicians were conducted. Initially, nine deductive and two inductive categories on HrQoL were built. Four categories were excluded following the qualitative content analysis because they were hardly or not at all mentioned by participants. Following on from the analysis of the expert discussion, one dimension was added, and two further categories were excluded. The resulting system consisted of six dimensions: emotional health, physical ailments, autonomy, social functionality, dignity, and resources.ConclusionThe identified dimensions of HrQoL in routine oncological care were found to differ from those used in existing HrQoL measurements for patients. Further research is needed to test and evaluate the presented structure in a larger sample of cancer patients to further assess its psychometric properties. (shrink)

This brief, accessible introduction shows that philosophy is valuable, practical, and significant for every aspect of Christian life and ministry.

There is no cure or vaccine for HIV/AIDS. The only life-prolonging treatment available is antiretroviral (ARV) therapy. WHO estimates, however, that less than 5 percent of those who require treatment in developing countries currently enjoy access to these medicines. In Africa fewer than 50,000 people–less than 2 percent of the people in need–currently receive ARV therapy. These facts have elicited strongly divergent reactions, and views about the appropriate response to this crisis have varied widely.The intensity of the debate concerning (...) access to life-prolonging medicines for the treatment of HIV/AIDS, and the heated rhetoric with which they are often conducted, suggest that these disagreements may be rooted in deeper disagreements of value. It is not obvious, however, what disagreements of value are at stake in this debate. By analyzing the statements of scholars, public officials, activist organizations, and private sector representatives, each of whom may endorse very different policy recommendations on access to HIV/AIDS drugs, we have identified and created a typology of the different sources of disagreement in the debate. We conclude that the central disagreements concerning access to medicines arise from competing understandings of how responsibilities for bringing remedy to hardships should be allocated to different agents and institutions. A central lesson that emerges from our analysis is that thinking about “health equity” must extend beyond the explanation and justification of goals, values, and ideals, and engage more honestly with the difficult question of how responsibilities for bringing remedy to health crises should be allocated in complex social contexts. The following interview was conducted by Christian Diehm in the home of Arne Naess near Oslo, Norway, in December of 2001. At eighty-nine years of age, Naess was preparing for the English-language release of his latest book, Life’s Philosophy. We are pleased to provide a transcript of a large part of the conversations that spanned two afternoon dialogues.

At the end of his life, Stuart Hall called for the reengagement of Cultural Studies and Marxism. This paper contributes to this task. It analyses Stuart Hall’s works on communication and the media.The goal of the paper is to read Stuart Hall in a manner that can inform the renewal of Marxist Humanism and the development of a Marxist-Humanist theory of communication. This involves reconstructing elements of Hall’s approach, criticising certain aspects of his work, and through this engagement developing (...) new theory elements.The article’s analysis of Stuart Hall’s theory of communication and the media is conducted in four steps. First, the paper reengages and re-evaluates what Hall called the two paradigms of Cultural Studies: Structuralism and “Culturalism”/Humanism. It discusses the role of human agency in society. Second, the paper engages with Hall’s and Althusser’s notions of articulation and sets the notion of articulation in relation to the concept of communication. Third, it discusses the relationship between communication and work in the context of Hall’s works. Fourth, the article revisits and engages with Hall’s encoding/decoding-model in the context of digitalisation.This paper grounds a dialectical concept of communication that is based on the dialectic of articulating and articulatedness, the dialectic of work and communication, as well as the dialectic of communication in the public sphere and society’s power forcefields. It shows how a critical, dialectical theory of communication benefits from engagement with Stuart Hall’s works. The present work argues with, for, against, and beyond Stuart Hall in order or productively draw on ideas that emerge from this engagement. (shrink)

Background Little is known about the attitudes and practices of intensivists working in Lebanon regarding withholding and withdrawing life-sustaining treatments. The objectives of the study were to assess the points of view and practices of intensivists in Lebanon along with the opinions of medical, legal and religious leaders regarding withholding withdrawal of life-sustaining treatments in Lebanese intensive care units. Methods A web-based survey was conducted among intensivists working in Lebanese adult ICUs. Interviews were also done with Lebanese medical, (...) legal and religious leaders. Results Of the 229 survey recipients, 83 intensivists completed it, i.e. a response rate of. Most respondents were between 30 and 49 years old, Catholic Christians, anesthesiologists, working in Beirut. Ninety-two percent of them were familiar with the withholding and withdrawal concepts and 80% applied them. Poor prognosis of the acute and chronic disease and futile therapy were the main reasons to consider withholding and withdrawal of treatments. Ninety-five percent of intensivists agreed with the “Principle of Double Effect”. The main withheld therapies were vasopressors, respiratory assistance and CPR. Most of the respondents reported the decision was often to always multidisciplinary, involving the family, and the patient, or his advance directives or his surrogate and the nurses. The interviewees agreed there was a law governing withholding and withdrawal decisions/practices in Lebanon. Christians and Muslim Sunni leaders declared accepting those practices. Conclusion Withholding and withdrawal of LSTs in the ICU are known concepts among intensivists working in Lebanon and are being practiced. Our results could be used to inform and optimize therapeutic limitation in ICUs in the country. (shrink)

The article questions concepts of ‘democratic peace’ that presuppose an intrinsic relation between pacifism and democracy. This view lacks from both, empirical evidence and historical insight. Instead, pacifism as political and personal virtue can be better linked to the Deweyan idea of democracy as the basic way of life, that is, mutual cooperation and self-realisation. But not only pacifism but also warfare and aggressive conduct often are rooted and result in an ethos of solidarity and cooperation. Therefore, the (...) task for any realistic concept of democratic pacifism is at least two-fold: finding ‘moral equivalents’ (W. James) to war and cultivating an ethos of self-criticism and self-restraint as already the Old Testament’s prophets have proclaimed. (shrink)

Despite the increasing inclusion of ethics and compliance issues in corporate training, the business world remains rife with breaches of responsible management conduct. This situation indicates a knowledge–practice gap among professionals, i.e., a discrepancy between their knowledge of responsible management principles and their behavior in day-to-day business life. With this in mind, this paper addresses the formative, developmental question of how companies’ ethics and compliance training programs should be organized in a manner that enhances their potential to be (...) effective. Drawing on both the qualitative analysis of existing ethics and compliance training and the conceptual literature on behavioral ethics, a framework is proposed that consecutively aligns various types of training into a comprehensive ethics and compliance training program. Adorno's moral philosophy has often been accused of making aporetic prescriptions that are too taxing for moral agents. In this article, I defend his approach in terms of a theory of moral dilemmas. My guideline is Adorno's famous sentence that wrong life cannot be lived rightly. I argue that this claim is not distinctly prescriptive, as most of Adorno's critics believe, but is a claim about moral reality. Emphasizing realist aspects of his moral theory, I suggest that wrong (...) class='Hi'>life is neither inconceivable nor an amoral or skeptical trope. Instead, Adorno's sentence about wrong life can be interpreted as a claim about the salience of particular moral facts. This, I conclude, allows Adorno to envisage moral reasons that motivate moral conduct case by case, although they are blocked overall. (shrink)

Although a majority of the presentations concerned the relevance of the Church's teaching on artificial contraception, the membership did devote the second part of the Congress to a sustained discussion of some new issues raised by Donum vitae (1987), the recent instruction of the Congregation for the Doctrine of the Faith on " Respect for Human Life in its Origin and on the Dignity of Procreation." To be sure, both documents emphasize the Church's constant teaching concerning the providence of God in human life and the dignity of the human person. Nevertheless, procedures such as GIFT (gamete intra.-fallopian transfer), LTOT (low tubal ovum transfer ), TOT (tubal ovum transfer) provide the moral theologian with new challenges to interpret the Church's position which insists on identifying the procrea.tive and unitive aspects of Christian marriage. The following article discusses the papal documents from the perspective of a realist moral theology. First of all, I would like to recall a principle which Paul VI, however obliquely, enunciated in the course of Humanae Vitae, for I consider the principle an axiomatic one in the present discussion. At the beginning of section 8 entitled "Pastoral 178 174 ROMANUS CESSARIO, O.P. Directives," we find a statement of theological purpose which should guide all Catholic moral practice. Hence, I would like to quote this text as a way of introducing the meaning of virtue in the Christian moral life. The Church, in fact, cannot have a different conduct toward men [and women] than that of the Redeemer: she knows their weaknesses, has compassion on the crowd, receives sinners; but she cannot renounce the teaching of the law which is, in reality, that law proper to a human Zife restored to its original truth and conducted by the Spirit of God.1 I am especially struck by the phrase " that law proper to a human life restored to its original truth," for I believe it actually describes the kind of life which develops in those who practice the virtues. The sharing economy has transformed economic transactions, created new organizational forms, and contributed to changes in consumer culture. Started as a movement with promises of a more sustainable, democratic, and inclusive economy, the sharing economy, and its impact on issues such as privacy, discrimination, worker rights, and regulation, is now the subject of heated debate. Many of these issues root in the changes that digital technologies have brought and the unresolved moral and ethical questions emerging therefrom. This special issue contributes (...) to this ongoing debate with five articles that develop theoretical frameworks and conduct empirical investigations, providing fine-grained analyses of urgent issues in the sharing economy. A leading ethicist offers a theological guide to thinking Christianly about the ordinary nature of everyday life.

Today, there are different understandings of the first article on the content of the Christian faith, for which an analysis from a theological perspective is necessary. This research sought to reveal the meaning of the first article on the content of the Christian faith in recent theological works that have been produced, through the use of a hermeneutic exercise, conducting a bibliometric and categorical analysis and using NVivo software to analyse the qualitative data. We concluded that the recent theological literature (...) addresses the meaning of the first article of the Christian faith based on the existence, concept and definition of God and his attributes, as well as his power, behaviour, creation, revelation and reign. Although there is little extant material compared to other topics or areas of knowledge that can be explored using these tools, this fact is explained because, normally, theologians are not interested in publishing in high-impact journals. The way of approaching God in contemporary literature can be enriched by taking into account specific contexts, such as poverty, discrimination, suffering, violence, human trafficking, immigration and political realities, in such a way that it is not only a metaphysical reflection of God but also a historical and immanent and even an interreligious research that broadens the concept of the Christian God and answers questions that contemporary society asks in daily life. Unlike (especially) the various Protestantisms, Orthodox Christianity recognizes no fundamentally different problems in the development and (future) application of human germ line genetic engineering (HGGE) than those raised by more traditional medicine. The particular challenges which frame the life of a traditional Christian arise not only in view of “groundbreaking” technological progress and its attendant increase in human power over nature, but permeate already his most simple daily routines. The diverse post-traditional Christianities have ceased confronting such liturgical–ascetical challenges. The (...) quite appropriately pious desire, among those committed to such Christianities, for some restraint in the face of such increase of power therefore gets refocused onto substitute concerns. The new challenge then arises to establish the universal validity of the moral norms underlying such concerns in order to ground the desired prohibition. The discussions of several such norms offered in this issue of Christian Bioethics illustrate the embarrassment presented by the unavailability of either theological or rational foundations needed for such a project. The attempt to prove that there is something in principle morally wrong about HGGE is thus not only unnecessary for the Christian conduct of life (and presents a distraction), but also doomed to fail. Background The significance of medical implants goes beyond technical functioning and reaches into everyday life, with consequences for individuals as well as society. Ethical aspects associated with the everyday use of implants are relevant for individuals’ lifeworlds and need to be considered in implant care and in the course of technical developments. Methods This scoping review aimed to provide a synthesis of the existing evidence regarding ethically relevant psychosocial and cultural aspects in cochlear, glaucoma and cardiovascular implants in patient-centered (...) empirical research. Systematic literature searches were conducted in EBSCOhost, Philpapers, PsycNET, Pubmed, Web of Science and BELIT databases. Eligible studies were articles in German or English language published since 2000 dealing with ethically relevant aspects of cochlear, glaucoma and passive cardiovascular implants based on empirical findings from the perspective of (prospective) implant-wearers and their significant others. Following a descriptive-analytical approach, a data extraction form was developed and relevant data were extracted accordingly. We combined a basic numerical analysis of study characteristics with a thematically organized narrative synthesis of the data. Results Sixty-nine studies were included in the present analysis. Fifty were in the field of cochlear implants, sixteen in the field of passive cardiovascular implants and three in the field of glaucoma implants. Implant-related aspects were mainly found in connection with autonomy, freedom, identity, participation and justice, whereas little to no data was found with regards to ethical principles of privacy, safety or sustainability. Conclusions Empirical research on ethical aspects of implant use in everyday life is highly relevant, but marked by ambiguity and unclarity in the operationalization of ethical terms and contextualization. A transparent orientation framework for the exploration and acknowledgment of ethical aspects in “lived experiences” may contribute to the improvement of individual care, healthcare programs and research quality in this area. Ethics-sensitive care requires creating awareness for cultural and identity-related issues, promoting health literacy to strengthen patient autonomy as well as adjusting healthcare programs accordingly. More consideration needs to be given to sustainability issues in implant development and care according to an approach of ethics-by-design. (shrink)

Traditionally, cognitive, motivational, and volitional determinants have been used to explain and predict health behaviors such as physical activity. Recently, the role of affect in influencing and regulating health behaviors received more attention. Affects as internal cues may automatically activate unconscious processes of behavior regulation. The aim of our study was to examine the association between affect and physical activity in daily life. In addition, we studied the influence of the habit of being physically active on this relationship. An (...) ambulatory assessment study in 89 persons (33.7% male, 25 to 65 years, M = 45.2, SD = 8.1) was conducted. Affect was assessed in the afternoon on 5 weekdays using smartphones. Physical activity was measured continuously objectively using accelerometers and subjectively using smartphones in the evening. Habit strength was assessed at the beginning of the diary period. The outcomes were objectively and subjectively measured moderate-to-vigorous physical activity (MVPA) performed after work. Multilevel regression models were used to analyze the association between affect and after work MVPA. In addition, the cross-level interaction of habit strength and affect on after work MVPA was tested. Positive affect was positively related to objectively measured and self-reported after work MVPA: the greater the positive affect the more time persons subsequently spent on MVPA. An inverse relationship was found for negative affect: the greater the negative affect the less time persons spent on MVPA. The cross-level interaction effect was significant only for objectively measured MVPA. A strong habit seems to strengthen both the positive influence of positive affect and the negative influence of negative affect. The results of this study confirm previous results and indicate that affect plays an important role for the regulation of physical activity behavior in daily life. The results for positive affect were consistent. However, in contrast to previous reports of no or an inverse association, negative affect decreased subsequent MVPA. These inconsistencies may be—in part—explained by the different measurements of affect in our and other studies. Therefore, further research is warranted to gain more insight into the association between affect and physical activity. (shrink)

Explicitly considering human values in the design process of socio-technical systems has become a responsibility of designers. It is, however, challenging to design for values because (1) relevant values must be identified and communicated between all stakeholders and designers and (2) stakeholders’ values differ and trade-offs must be made. We focus on the first aspect, which requires elicitation of stakeholders’ situated values , i.e. values relevant to a specific real life context. Available techniques to elicit knowledge and requirements from (...) stakeholders lack in providing the context and means for reflection needed to elicit situated values as well as an explicit concept of value. In this paper we present our design of a tool to support active elicitation of stakeholders’ values and communication between stakeholders and designers. We conducted an exploratory user study in which we compared the suitability of methods used in social sciences for (1) eliciting situated values, (2) supporting people’s expressions of values and (3) being implemented in value elicitation tool. Based on the outcomes we propose a design for a value elicitation tool that consists of a mobile application used by stakeholders for data collection and in-situ self-reflection, and a website used collaboratively by designers and stakeholders to analyse and communicate values. This study seeks to gain knowledge about key conditions in the process of digitalization using a socio-technical systems design as a theoretical framework and a case-study approach. Semi-structured interviews with 15 relevant stakeholders are conducted to learn about barriers to and enablers of the development and implementation process in a manufacturing company. After conducting a thematic analysis, eight higher-ranked themes relevant to the digitalization process are identified. These are grouped to describe the overarching phenomena, resulting in four enablers and four (...) barriers. The study takes a holistic view of digitalization in its investigation of the development and subsequent implementation processes. The findings contribute to the literature via three key takeaways for stakeholders of a digitalization process in manufacturing. First, it is vital to understand and strengthen the role of the key enablers identified in this paper. Second, managers should observe and identify barriers in their own organizations, related both to technical and social aspects. Third, some of the enablers described in this paper can serve managers as helpful tools to approach the expected barriers to digitalization. This paper also challenges the research literature by arguing that research conducted on digitalization that sets new standards and premises for the working life in industry needs to use up-to-date socio-technical design concepts and theories. In this paper, a personalist ethical perspective on end-of-life care of severely ill newborns is presented by posing two questions. (1) Is it ethically justified to decide not to start or to withdraw life-sustaining treatment in severely ill newborns? (2) Is it ethically justified, in exceptional cases, to actively terminate the life of severely ill newborns? Based on five values—respect for life and for the dignity of the human person, quality of life, respect for the (...) process of dying, relational autonomy, and justice—an ethical assessment is conducted that brings us answers to the two ethical questions. (1) Noninitiation or withdrawal of life-sustaining medical treatment in severely ill newborns is ethically acceptable, and might even be a moral duty, when initiation or continuation of medical treatment can be considered futile or even harmful. (2) However, according to the personalist approach, it is not ethically acceptable to actively terminate the life of a severely ill newborn. (shrink)

In Improvisation, Samuel Wells defines improvisation in the theater as "a practice through which actors seek to develop trust in themselves and one another in order that they may conduct unscripted dramas without fear." Sounds a lot like life, doesn't it? Building trust, overcoming fear, conducting relationships, and making choices--all without a script. Wells establishes theatrical improvisation as a model for Christian ethics, a matter of "faithfully improvising on the Christian tradition." He views the Bible not as a (...) "script" but as a "training school" that shapes the habits and practices of the Christian community. Drawing on scriptural narratives and church history, Wells explains six practices that characterize both improvisation and Christian ethics. It seems to be important to recollect that if there were no names in the history of philosophy except those belonging to the creators of new systems, this would mean the extermination of culture, and thereby the death of philosophy itself. The very word "culture" and the inherent meaning in philosophy presuppose a continuity. For this reason they evoke disciples, imitators and followers who weave a living and indestructible chain. In other words, a tradition is sown, the fruits of which (...) are perpetually gathered in values, reassuring man of the ultimate and eternal harmony of all being, reaffirming the truth that nothing finally that is contingent or fleeting, nothing that is exclusively material or functional within life can sustain life, and reestablishing that only from that which lies beyond the phenomena can mankind gain access to all the abundance of spiritual resources indispensable for dealing with life. And it is tradition that constitutes an everlasting re-dedication to the life and the immortality of the spirit. The Dalai Lama once wrote that the object of human existence was to be happy. This sounds extremely glib as happiness in the popular imagination is a feeling and in the words of the song 'the greatest gift that we possess'. On the other hand, von Hugel wrote 'Religion has never made me happy;it's no use shutting your eyes to the fact that the deeper you go, the more alone you will find yourself' This small masterpiece by the late Fr (...) Herbert McCabe of the Dominican order steers a steady courss between these two extremes. We feels instinctively that human beings are designed to enjoy themselves and to be happy and yet we are told that suffering is good for the soul. But in the Catholic tradition the true object of human existence is the vision of God and nothing less than this will ever make us truly happy. But Fr McCabe explores much deeper issues. The papers given at the conference led to this new book, Josiah Royce: Pragmatist, Ethicist, Philosopher (...) of Religion. Edited by the German scholars Christoph Seibert and Christian Polke, the volume includes eleven essays, nine delivered at the 2015 conference, together with additional contributions by Seibert and Polke, the conference organizers.It is valuable to see contemporary German scholars engaging with Royce and to compare their approach with that of American scholars. Americans tend to approach Royce through the tradition of pragmatism in the company of William James, Charles Peirce, and John Dewey, and to minimize the metaphysics in Royce's thought and its idealistic influences. [End Page 116]The book has the goal of developing Royce's thought to show how its insights and concerns may be valuable for contemporary life, not only for scholars of differing philosophical traditions. In their Introduction, Seibert and Polke describe Royce as a "pragmatist of his own kind" (3) and identify three themes in his philosophy. The first is community, which was central to Royce from the beginning and, following his serious study of Peirce, "became the ultimate principle of both his metaphysics and his philosophy of life" (3). The second theme is religion, which, for Royce, involved the "search for the ultimate meaning of his own life, as well as that of his fellows' lives" (3). The third broad theme in Royce is ethics and a lifelong concern with the nature of the examined life. Royce developed his ethical philosophy from an original critique of moral skepticism to the "never-ending task of building communities within and through mankind by way of a critical spirit of true and mutual loyalty" (3). Among these three, the editors consider religion as the most important. They state: "[R]eligion is a very important theme of Royce's philosophy: one may even assume that it is the central subject, which implies that his philosophy may be properly understood only if it is regarded as a philosophy of religion" (8). The centrality of religion is apparent in the essays. Consideration here must be brief.The book's first section, "Bridging Two Continents: Royce between Pragmatism and Idealism," includes three broad-ranging essays that work toward a synthesis between pragmatism and idealism in the thought of Royce. Ludwig Nagl's contribution, "Toward a Global Philosophical Discourse on Religion," discusses Royce's posthumously published "Lectures on Modern Idealism." Nagl uses this work, together with Royce's adaption of Peirce, to help create a bridge between pragmatism and idealism. He further explores Royce's interest in Eastern religion to suggest how Royce's thought might support a broad philosophical study of religion, not restricted to Christianity. Co-editor Seibert's article, "The Ethical Nucleus of Reality," focuses on Royce's epistemology and his ethics, while touching briefly on religion. The essay shows how Royce combined pragmatic and idealistic threads from his earliest writings, with particular attention to his book The Spirit of Modern Philosophy (SM). Royce's SM is also central to the American philosopher Douglas Anderson's essay "What Philosophy Meant to Josiah Royce." In SM, Royce argues that philosophy constitutes a critical reflection on experience that may be practiced by every person, not only by professional philosophers. Anderson, however, also identifies and criticizes a more elitist, professional aspect in Royce's conception of philosophy. For Anderson, Royce's thought had a pragmatist dimension in that philosophy was a "voluntarily chosen attitude toward conducting our lives" (80). In Emerson and the Conduct of Life, David M. Robinson describes Ralph Waldo Emerson's evolution from mystic to pragmatist, stressing the importance of Emerson's undervalued later writing. Emerson's reputation has rested on the addresses and essays of the 1830s and 1840s, in which he propounded a version of transcendental idealism, and memorably portrayed moments of mystical insight. But Emerson's later writings suggest an increasing concern over the elusiveness of mysticism, and an increasing stress on ethical choice and practical (...) power. These works reveal Emerson as an ethical philosopher who stressed the spiritual value of human relations, work and social action. (shrink)

The shape of the moral life is determined for Christians at least in part by beliefs peculiar to Christians, and a perennial problem for Christian ethics is relating that peculiar understanding to more general claims about moral knowledge. Since the problem is perennial, I propose not to solve it but to think about it. I do so by considering first the sense in which Christian ethics may be a kind of "insider's" ethic-the shared language of believers. Despite the strengths (...) of such a view, it may too greatly restrict both human and divine freedom. Having considered its difficulties from both those angles, I return to the sense of Christian life as a tradition of conduct in order to illustrate the importance of such a vision in our cultural setting.
