Intersex is an umbrella term for variations in sexual characteristics. Although 1.7% to 4% of the population is born outside the framework of sexual binarity, the bodies of intersex people are pathologized and, according to medical specialists, in need of “repair”. This institutional stigmatization takes the form of sex reassignment surgery or hormone treatments, justified by socio-cultural premises based on heterosexist norms. Medical discourse is based less on the health, physical and psychological dangers of intersex patients than on safeguarding gendered (...) bicategorization. At the same time, individuals with intersex variations are demanding their right to self-determination, in particular through respect for their free and informed consent. Sex-assignment surgery, motivated and oriented by historically constructed gender binarity, lead to two types of epistemic injustice. A conceptual philosophical analysis of testimonial injustice and hermeneutic injustice will help us to understand the process of making intersex invisible through the epistemic relationship between intersex people and physicians. Venturing into novel terrain poses physical risks to a female and her offspring. Females have a greater tendency to avoid physical harm, while males tend to have larger range sizes and often outperform females in navigation-related tasks. Given this backdrop, we expected that females would explore a novel environment with more caution than males, and that more-cautious exploration would negatively affect navigation performance. Participants explored a novel, large-scale, virtual environment in search of five objects, pointed in the direction of each (...) object from the origin, and then navigated back to the objects. We found that females demonstrated more caution while exploring as reflected in the increased amounts of pausing and revisiting of previously traversed locations. In addition, more pausing and revisiting behaviors led to degradation in navigation performance. Finally, individual levels of trait harm avoidance were positively associated with the amount of revisiting behavior during exploration. These findings support the idea that the fitness costs associated with long-distance travel may encourage females to take a more cautious approach to spatial exploration, and that this caution may partially explain the sex differences in navigation performance. Moral distress is an experience of profound moral compromise with deeply impactful and potentially long‐term consequences to the individual. Critical care areas are fraught with ethical issues, and end‐of‐life care has been associated with numerous incidences of moral distress among nurses. One such area where the dichotomy of life and death seems to be at its sharpest is in the pediatric intensive care unit. The purpose of this study was to understand the moral distress experiences of pediatric intensive care nurses (...) when caring for pediatric patients at the end of life. A secondary analysis was undertaken of seven transcripts from registered nurses across six Canadian pediatric intensive care units and produced three themes: under prioritization of child patient dignity, burden of insider knowledge, and environmental constraints on nursing roles and responsibilities. When caring for patients at the end of life, nurses experienced moral distress when a dignified death was not realized. Furthermore, despite interprofessional collaboration efforts in Canada, the concept of silo mentality persists and contributes to moral distress. Organizational involvement is needed to address moral distress in pediatric intensive care nurses both to achieve a dignified death for child patients and in addressing silo mentality. (shrink)

This article argues that the misalignment of private profit-maximizing objectives with public health needs causes institutional corruption in the pharmaceutical sector and systematically leads firms to act contrary to public heath. The article analyzes how financial incentives generate a business model promoting harmful practices and explores several means of realigning financial incentives in order to foster therapeutic innovation and promote the rational use of medicines.

Each of the antiretroviral drugs that are currently used to stop the progression of HIV infection causes its own specific side effects. Despite the expansion, multiplication, and simplification of treatment options over the past decade, side effects continue to affect people living with HIV. Yet, we see a clear disconnect between the way side effects are normalized, routinized, and framed in clinical practice and the way they are experienced by people living with HIV. This paper builds on the premise that (...) new approaches are needed to understand side effects in a manner that is more reflective of the subjective accounts of people living with HIV. Drawing on the work of Deleuze and Guattari, it offers an original application of the theory of ‘assemblage’. This theory offers a new way of theorizing side effects, and ultimately the relationship between the body and antiretroviral drugs (as technologies). Combining theory with examples derived from empirical data, we examine the multiple ways in which the body connects not only to the drugs but also to people, things, and systems. Our objective is to illustrate how this theory dares us to think differently about side effects and allows us to originally (re)think the experience of taking antiretroviral drugs. (shrink)

The global response to managing the spread of HIV has recently undergone a significant shift with the advent of ‘treatment as prevention’, a strategy which presumes that scaling-up testing and treatment for people living with HIV will produce a broader preventative benefit. Treatment as prevention includes an array of diagnostic, technological and policy developments that are creating new understandings of how HIV circulates in bodies and spaces. Drawing on the work of Michel Foucault, we contextualize these developments by linking them (...) to systems of governance and discursive subjectivation. The goal of this article is to problematize the growing importance of viral suppression in the management of HIV and the use of related surveillance technologies. For people living with HIV, we demonstrate how treatment-as-prevention’s emphasis on individual and collective viral load is transforming the performative dimensions of embodied risk, affect, subjectivity and sex. Critical notice on Raymond RUYER, Cybernetics and the Origin of Information, translated by Amélie Berger- Soraruff, Andrew Iliadis, Daniel W. Smith, and Ashley Woodward, with an introduction by Ashley Woodward, Lanham, London, Rowman & Littlefield, 2024, XXVII-214 p.

Radical democratic thinking is becoming intrigued by the material situatedness of its political agents and by the role of nonhuman participants in political interaction. At stake here is the displacement of narrow anthropocentrism that currently guides democratic theory and practice, and its repositioning into what we call ‘the nonhuman condition’. This Critical Exchange explores the nonhuman condition. It asks: What are the implications of decentering the human subject via a new materialist reading of radical democracy? Does this reading dilute political (...) agency? Or should this be seen, on the contrary, as an invitation for new voices and demands to enter into democratic assemblages? How might engagement with the more-than-human disrupt or extend theories of radical democracy? Hans Asenbaum and Amanda Machin discuss the human democratic subject both through radical democratic and new materialist lenses. They suggest that a new materialist decentred subject does not lose agency but further gains political responsibility in radical democracy. Jean-Paul Gagnon highlights the possibility of becoming through loss. In losing our anthropocentric arrogance—our understanding of being other and better than animal—we become more connected and discover our place in human-nonhuman assemblages. Melissa Orlie agrees that something can be gained through renouncing dominant human fantasies and presents a ‘radical democratic naturalism’. By renouncing land violence and acknowledging nature’s subjectivity, we can further nonexploitative radical democratic politics. This is where the notion of ‘tidalectic’ processes introduced by James L. Smith is helpful. Focusing on water and the Indigenous knowledges around rivers, he considers how democracy and community can be reimagined as plural, cyclical and attentive to the nonhuman with which the human is inevitably entangled. Diana Leong offers a different approach. Considering the nonhuman from the perspective of Black Studies demands an awareness to how blackness has long been constituted as non- or partially human. La seconde partie analyse les arguments et precédés utilisés par Newton pour identifier des mouvements absolus, et prouver ainsi que de tels mouvements existent. Animated by a passion without limits for the world and the extraordinary deployment of all the riches it contains, without falling into some simplistic optimism, Teilhard de Chardin offers us one of the most beautiful expressions of Christian hope ever. Background Over the past few decades, moral distress has been examined in the nursing literature. It is thought to occur when an individual has made a moral decision but is unable to act on it, often attributable to constraints, internal or external. Varying definitions can be found throughout the healthcare literature. This lack of cohesion has led to complications for study of the phenomenon, along with its effects to nursing practice, education and targeted policy development. Objectives The aim of this (...) analysis was to uncover unique definitions of moral distress as found in the nursing literature and to examine the relationship between these definitions. Research Design and Context Morse’s method of concept clarification was applied given the large body of literature which includes definitions, descriptions and measurements of the concept in research. The steps include (a) conducting a literature review; (b) analysing the literature; and (c) identifying, describing, comparing, and contrasting attributes, antecedents and consequences of each category. Findings Each of the 18 included studies described constraints in their definition of moral distress, whether implied or explicitly stated. External constraints are widely described as obstacles outside of the individual, whether institutional, systemic or situational, while internal constraints are located within the individuals themselves and are described as personal limitations, failings or weakness of will. Conclusion Upon reviewing these definitions, we determined that the term ‘internal constraints’ is problematic due to the emphasis of responsibility on the individual experiencing moral distress. We propose an alteration to ‘internal characteristics’ that will assume less responsibility of change from the individual to place a heavier onus on systemic and institutional constraints. (shrink)

Research has shown that context influences how sincere a smile appears to observers. That said, most studies on this topic have focused exclusively on situational cues (e.g. smiling while at a party versus smiling during a job interview) and few have examined other elements of context. One important element concerns any knowledge an observer might have about the smiler as an individual (e.g. their habitual behaviours, traits or attitudes). In this manuscript, we present three experiments that explored the influence of (...) such knowledge on ratings of smile sincerity. In Experiments 1 and 2, participants rated the sincerity of Duchenne and non-Duchenne smiles after having been exposed to cues about the smiler’s tendency to reciprocate (this person always, never or occasionally returns favours). In Experiment 3 they performed the same task but with cues about the smiler’s love of learning (this person always, never or occasionally enjoys learning new tasks). The results show that cues about the smiler’s reciprocity tendency influenced participants’ ratings of smile sincerity and did so in a stronger manner than cues about the smiler’s love of learning. Overall, these results both strengthen and broaden the literature on the role of context on judgements of smile sincerity. (shrink)

At first, he values an acceptation of suffering through reconciliation with time, and puts the onus on the divine to refute the dismembering of the oneness of meaning and unity of the soul’s quest for joy in eternity. Then confrontingChristianity, he sees its (...) refusal to sacrifice anyone, at the cost of making all sick with a unique healer, and rejects it as incompatible with his ideal of plenitude. Inthe absence of an ontology of the person, the affirmation of the individual and his value, opposed to the antagonistic affirmation of the many put in front of theone God and destroyed by him, ends up dislocating the reality of the self. The Nietzschean option resisted any leveling down—this is its merit—yet the mysteryof the Trinity needs to be brought into the reflection to respect Nietzsche’s own terms in defining the final problem which is also the one option: Dionysus orthe Crucified? A summary is provided of Ruyer's important contribution, also a reversal from some conclusions held in his secondary doctoral dissertation, about the limits inherent in technological progress, and an attempt is made to show the coherence of this position to Ruyer's metaphysics. Simondon's response is also presented, and subsequently analyzed especially as it culminates in a concept of concretizations. As Simondon indicated, and with a displacement in Ruyer's limitating framework on unconditional growth, we end up searching for what represents the (...) category of the ultimate for those two philosophers of the cyberworld. I present first the challenge for epistemology when it faces the dilemma between rationalism and empiricism, followed by a presentation of the ideas introduced by Ruyer in order to ask if they can be articulated to the "third way" in epistemology. I explore the consequences of Ruyer's inversion of our understanding of space which can be looked upon as psychic. I consider Ruyer's refusal to locate in pure immanence the scheme of eupraxic resolution of successful aggregates–as living forms–in our experience. (...) I then highlight the major principles of Gaston Bachelard's and Michel Serres's respective epistemologies, and in a lesser measure those of Bergon and Deleuze as well, in order to underscore Ruyer's refusal to limit philosophy to what science allows one to say, but also in order to finally situate the verticalism of his position. In this public debate with Philippe Deterre (research director in immunology at the CNRS) – held at l'Enclos Rey in Paris' 15th district during the biennial Conference of the Réseau Blaise Pascal in March 2017 –, I defended the usefulness of natural theology. I first clarify theology's nature and understanding, then I speak about a tradition that upheld the public and exterior knowledge of God, and make an effort to show the presence of a theme reminiscent of natural theology behind (...) attempts at the good life. I then ask whether natural theology would only exist for the Christian. In the reply to my opponent's own reaction, I insist on the incongruity of separating our knowledge of God from our knowledge of science's wonderful discoveries, I ask whether nature could be said to be crafty and "ingenious," and I conclude by building a case for the return of God in public conversation, as part of an effort that our world needs in terms of finding back its compass, and restoring an ideal of living rationally. Close attention to levels of organization leads one to doubt the random character of the regulations of the cosmos as a whole. Scientific knowledge seems able, after all, to bring into focus the enigma of the individual histories that have shaped the world. Religious consciousness of a personal destiny should be analogically linked to the destiny of the universe in which it is rooted.

This is a critical notice/review essay on *L'embryogenèse du monde et le Dieu silencieux*, a manuscript completed by Raymond Ruyer in the early 1980s. It came out as a monograph in November 2013, with the Éditions Klincksieck in Paris. It offers a presentation in an organized fashion of many aspects of his thought. Ruyer considered that a book about God could only be churned into a series of chapters on the unachievable character of our knowledge in different domains of human (...) inquiry. The nature of this final solution on God's relationship to the world and to natural forms is here assessed critically. (shrink)

The myth that sexual violence is perpetrated by strangers in dark alleyways has long since been debunked; the vast majority of sexual assaults are committed by someone known to the victim. This article examines how individuals and communities nevertheless experience epistemic and moral barriers when reacting to reports of sexual violence levelled against community members, friends, and loved ones. Love can, for example, cloud our moral and epistemic oughts when responding to sexual violence, through such avenues as mutual identity constitution (...) and epistemic partiality, which then contribute to discourse and practices that normalize sexual violence. I draw on Miriam Toews’ 2018 novel, Women Talking, to demonstrate that this social framework can be inverted through the very emotions that in part generate it, namely love and care. I argue that we can catalyze the drive to preserve our relationships with loved ones and community members who perpetrate harm by foregrounding moral accountability and transformation, atonement and amends, as preconditions of continued relationships. The upshot of this is the development of new moral norms and practices that deconstruct the widespread and casual nature of sexual violence. Importantly, this article and the course of action it explores is aimed at individuals whose wellbeing is not jeopardized through accountability projects. (shrink)

Dans ce texte, je suggère que les personnes souffrant d’anorexie mentale s’infligent une forme particulière de violence épistémique lorsqu’elles ignorent volontairement les symptômes de leur maladie. Pour ce faire, dans une démarche inspirée par celle de Havi Carel, je présente d’abord le rapport particulier qu’entretiennent les personnes souffrant d’anorexie avec leur corps, et ce, à l’aide de trois axes d’analyse empruntés à Susan Bordo, soit le dualisme Corps/Esprit, les idéaux de contrôle et d’autonomie, et les rapports de genre. Je propose (...) ensuite le concept de violence épistémique auto-infligée en montrant que l’ignorance volontaire de la faim a pour objectif de préserver l’identité des personnes vivant avec cette maladie comme autonomes et en contrôle, autrement dit comme n’étant pas « malades ».
