Objetivo: El siguiente artículo tiene como propósito la revisión de la literatura existente cuyo enfoque sea las herramientas tecnológicas utilizadas para el abordaje del paciente con síntomas de salud mental, de manera virtual, para describir las ventajas, la eficacia, la efectividad y los beneficios de estas herramientas, a partir de la literatura encontrada. Métodos: se realizó una búsqueda en las bases de datos Pubmed, Elsevier, UpToDate, Journal of Medical Internet Research y Google Académico. Se identificó un total de 58 artículos (...) que versan sobre la eficacia y efectividad de las diferentes modalidades digitales (tales como aplicaciones, cuestionarios en línea, mensajes de texto, etc.) que tratan la salud mental. Resultados: la búsqueda realizada arrojó un total de 58 artículos. Tras la eliminación de duplicados, el número fue de 50 artículos. Después de la caracterización de la información basada en la revisión de la totalidad del contenido, fueron elegidos 30 artículos para que sirvieran como base para la construcción de la síntesis de datos. Conclusiones: las herramientas virtuales utilizadas en salud mental ofrecen un sin fin de beneficios para prestar un servicio con un acceso más sencillo a la población, de forma económica y efectiva, sin temor a que los pacientes sean estigmatizados al momento de buscar ayuda, como se pudo evidenciar en los artículos presentados. Además, las herramientas digitales utilizadas en psiquiatría han demostrado ser eficaces y seguras, sin embargo, son de reciente aparición y cuentan con una cantidad limitada de estudios que demuestren objetivamente su eficacia en esta área. The pandemic outbreak in March 2020 and its associated sanitary regulations and restrictions triggered an abrupt and significant change for society in general and for families’ organization in particular. In Chile, the Santiago Metropolitan District was under a strict lockdown that involved the closure of the entire educational system. From a systemic-family stress perspective, the impact of these changes might have consequences not only for each individual family member, but for the parental dynamic and, consequently, for children’s well-being. This paper (...) presents the results of a follow-up study showing changes in self-reported parental depression and the perceived home organization of mothers and fathers assessed at three different moments: before the pandemic, at the initial outbreak, and after 1 month of strict lockdown. Relevant moderators were explored using linear mixed models to understand the within-subject changes in mothers’ and fathers’ self-reports across the different assessment times. Financial strain, personality traits of self-criticism and dependency, previous parent–child quality interaction, recent major stressful events, and number of children are highlighted as relevant factors that moderate changes in home chaos and parental mental health perception. Significant risks and protective factors are described for fathers and mothers. The use of pre-pandemic measures as baseline levels enabled the identification of personal and family characteristics that were related to better outcomes. The results help increase our understanding of the sanitary regulations’ impacts on the family system and identify vulnerability indicators that should be considered. (shrink)

El artículo analiza el caso de científicas negras en universidades colombianas, desde dos cuestiones fundamentales: El lugar que ocupan dichas mujeres dentro del campo científico, tomando en cuenta tanto los sistemas de medición oficiales como las asignaciones que ellas mismas, de modo reflexivo, otorgan a su papel dentro del sistema. Y segundo, analiza cómo a partir de prácticas comunicativas concretas, que asumimos como constitutivas del quehacer científico, estas mujeres se contactan con aquellos que están fuera del campo científico y se (...) constituyen como públicos no expertos. Nuestro análisis parte de la idea de que estas prácticas comunicativas construyen tecnologías del contacto, que pueden comprenderse desde un ethos del cuidado que aboga por un sentido de reparación y repolitización de la producción de conocimiento científico. Presentism is, roughly, the metaphysical doctrine that maintains that whatever exists, exists in the present. The compatibility of presentism with the theories of special and general relativity was much debated in recent years. It has been argued that at least some versions of presentism are consistent with time-orientable models of general relativity. In this paper we confront the thesis of presentism with relativistic physics, in the strong gravitational limit where black holes are formed. We conclude that the presentist position is (...) at odds with the existence of black holes and other compact objects in the universe. A revision of the thesis is necessary, if it is intended to be consistent with the current scientific view of the universe. (shrink)

It is a remarkable fact that all processes occurring in the observable universe are irre- versible, whereas the equations through which the fundamental laws of physics are formu- lated are invariant under time reversal. The emergence of irreversibility from the funda- mental laws has been a topic of consideration by physicists, astronomers and philosophers since Boltzmann's formulation of his famous \H" theorem. In this paper we shall discuss some aspects of this problem and its connection with the dynamics of space-time, (...) within the framework of modern cosmology. We conclude that the existence of cosmological horizons allows a coupling of the global state of the universe with the local events deter- mined through electromagnetic processes. (shrink)

We present a formal analysis of the Cosmological Argument in its two main forms: that due to Aquinas, and the revised version of the Kalam Cosmological Argument more recently advocated by William Lane Craig. We formulate these two arguments in such a way that each conclusion follows in first-order logic from the corresponding assumptions. Our analysis shows that the conclusion which follows for Aquinas is considerably weaker than what his aims demand. With formalizations that are logically valid in hand, we (...) reinterpret the natural language versions of the premises and conclusions in terms of concepts of causality consistent with (and used in) recent work in cosmology done by physicists. In brief: the Kalam argument commits the fallacy of equivocation in a way that seems beyond repair; two of the premises adopted by Aquinas seem dubious when the terms ‘cause’ and ‘causality’ are interpreted in the context of contemporary empirical science. Thus, while there are no problems with whether the conclusions follow logically from their assumptions, the Kalam argument is not viable, and the Aquinas argument does not imply a caused origination of the universe. The assumptions of the latter are at best less than obvious relative to recent work in the sciences. We conclude with mention of a new argument that makes some positive modifications to an alternative variation on Aquinas by Le Poidevin, which nonetheless seems rather weak. (shrink)

El presente artículo pone en diálogo dos teorías sobre la justicia frente al fenómeno migratorio, específicamente, aquel que corresponde a la movilización masiva de ciudadanos venezolanos a Colombia desde el caso particular de la ciudad de Bucaramanga. Lo anterior tiene por objetivo proponer alternativas frente a los principales conflictos que han surgido con el choque cultural, político y económico que conlleva una movilización de esta envergadura en un país poco capacitado. Para conseguir esta meta, se proponen tres momentos; primero, se (...) exponen en conjunto tanto la teoría de Jürgen Habermas como la de Jacques Derrida, el primero de estos al respecto de la relación justicia-tolerancia, mientras que el segundo lo hace desde la dupla justicia-hospitalidad. Seguidamente, se exponen algunos puntos principales para ser analizados sobre la ola migratoria en Colombia, de modo que, finalmente, se pueda hacer una lectura desde el aparato conceptual de ambos autores a la problemática migratoria en la ciudad de Bucaramanga. The strong division that the author establishes between the real city and the literate city does not allow one to think of the role of literature outside the field of institutionalized power, at the same time that he relegates the ontological reality of the latter to a merely instrumental one. Our proposal is to read this opposition in the light of the notion of transculturation, establishing a dialectic that allows us to consider the very policy of the real city and its active role in the expression of a political resistance literature. IntroductionRheumatologists are the primary healthcare professionals responsible for patients with rheumatic diseases and should acquire medical ethical competencies, such as the informed consent process (ICP). The objective clinical structured examination is a valuable tool for assessing clinical competencies. We report the performance of 90 rheumatologist trainees participating in a station designed to evaluate the ICP during the 2018 and 2019 national accreditations.MethodsThe station was validated and represented a medical encounter in which the rheumatologist informed a patient with systemic lupus erythematosus (...) with clinically active nephritis about renal biopsy. A trained patient–actor and an evaluator were instructed to assess ICP skills (with a focus on kidney biopsy benefits, how the biopsy is done and potential complications) in obtaining formal informed consent, delivering bad news and overall communication with patients. The evaluator used a tailored checklist and form.ResultsCandidate performance varied with ICP content and was superior for potential benefit information (achieved by 98.9% of the candidates) but significantly reduced for potential complications (37.8%) and biopsy description (42.2%). Only 17.8% of the candidates mentioned the legal perspective of ICP. Death (as a potential complication) was omitted by the majority of the candidates (93.3%); after the patient–actor challenged candidates, only 57.1% of them gave a clear and positive answer. Evaluators frequently rated candidate communications skills as superior (≥80%), but ≥1 negative aspect was identified in 69% of the candidates.ConclusionsEthical competencies are mandatory for professional rheumatologists. It seems necessary to include an ethics competency framework in the curriculum throughout the rheumatology residency. (shrink)

The voices of children and young people have been largely neglected in discussions of the extent to which the internet takes into account their needs and concerns. This paper aims to highlight young people’s lived experiences of being online.

Macroscopic irreversible processes emerge from fundamental physical laws of reversible character. The source of the local irreversibility seems to be not in the laws themselves but in the initial and boundary conditions of the equations that represent the laws. In this work we propose that the screening of currents by black hole event horizons determines, locally, a preferred direction for the flux of electromagnetic energy. We study the growth of black hole event horizons due to the cosmological expansion and accretion (...) of cosmic microwave background radiation, for different cosmological models. We propose generalized McVittie co-moving metrics and integrate the rate of accretion of cosmic microwave background radiation onto a supermassive black hole over cosmic time. We find that for flat, open, and closed Friedmann cosmological models, the ratio of the total area of the black hole event horizons with respect to the area of a radial co-moving space-like hypersurface always increases. Since accretion of cosmic radiation sets an absolute lower limit to the total matter accreted by black holes, this implies that the causal past and future are not mirror symmetric for any spacetime event. The asymmetry causes a net Poynting flux in the global future direction; the latter is in turn related to the ever increasing thermodynamic entropy. Thus, we expose a connection between four different “time arrows”: cosmological, electromagnetic, gravitational, and thermodynamic. (shrink)

Parental burnout is a unique and context-specific syndrome resulting from a chronic imbalance of risks over resources in the parenting domain. The current research aims to evaluate the psychometric properties of the Spanish version of the Parental Burnout Assessment across Spanish-speaking countries with two consecutive studies. In Study 1, we analyzed the data through a bifactor model within an Exploratory Structural Equation Modeling on the pooled sample of participants obtaining good fit indices. We then attained measurement invariance across both gender (...) and countries in a set of nested models with gradually increasing parameter constraints. Latent means comparisons across countries showed that among the participants’ countries, Chile had the highest parental burnout score, likewise, comparisons across gender evidenced that mothers displayed higher scores than fathers, as shown in previous studies. Reliability coefficients were high. In Study 2, we tested the relations between parental burnout and three specific consequences, i.e., escape and suicidal ideations, parental neglect, and parental violence toward one’s children. The medium to large associations found provided support for the PBA’s predictive validity. Overall, we concluded that the Spanish version of the PBA has good psychometric properties. The results support its relevance for the assessment of parental burnout among Spanish-speaking parents, offering new opportunities for cross-cultural research in the parenting domain. (shrink)

El presente artículo ahonda en la vulnerabilidad de las y los migrantes en México, ya sean en tránsito o de destino, haciendo énfasis en la falta de políticas públicas al respecto, las cuales si las hay, no contemplan la perspectiva de género, categoría de análisis imprescindible. La llamada “feminización de las migraciones” está aumentando, adquiriendo especial importancia en la Frontera Sur. La violación sistemática de los derechos humanos hacia las mujeres migrantes y su situación de indefensión, hace que sean objeto (...) de vejaciones que ni si quiera pueden denunciar por su condición irregular, así como por sus factores sociales y culturales (género, clase, etnia…), lo que les hace estar desprovistas de los recursos necesarios de empoderamiento y acción. Se analiza la desprotección de las migrantes centroamericanas en Chiapas así como los Convenios nacionales e internacionales en defensa de sus derechos, acabando el artículo con propuestas específicas, relativas a las políticas públicas para el caso estudiado. Resumen:La reflexión sobre la exigencia de evitar la guerra y asegurar la paz mundial es hoy, quizá más que nunca, una de las principales tareas de la filosofía política y de la filosofía del derecho. El trabajo aborda la aportación de Virginia Woolf a esta reflexión en varios de sus escritos y, en especial, en su ensayo Tres guineas, considerado uno de los referentes más lúcidos de la genealogía femenina del pensamiento pacifista. El discurso antifascista, antimilitarista y antipatriarcal de (...) Woolf da cuenta de los vínculos existentes entre estas tres raíces profundas del belicismo. Frente a las relaciones de fuerza, el hecho de vivir fuera del imaginario del poder se presenta aquí como oportunidad de acometer tareas civilizatorias imprevisibles. La experiencia y el saber de las mujeres, marginados a lo largo de la historia, pueden ser rescatados subversiva y creativamente para generas prácticas políticas no-violentas y aspirar a una redefinición global de la vida en común.Abstract: Today, one of the main tasks of political philosophy and the philosophy of law is, perhaps, to reflect on the need to avoid war and ensure world peace. This paper discusses the contribution made by Virginia Woolf to this reflection in several of her writings and especially in her essay Three Guineas, which is regarded as one of the most striking examples of pacifist thinking by a female writer. Woolf’s anti-fascist, anti-militarist and anti-patriarchal discourse recognises the links between these three deep roots of warmongering. In the face of struggles for power, the reality of living outside the imaginary of power is presented here as an opportunity to carry out unforeseeable civilizing tasks. Women’s experience and knowledge, which has been marginalised throughout history, can be set free subversively and creatively to generate non-violent political practices and aspire to a global redefinition of coexistence. El cuidado suele mostrar una distribución desigual, donde estas responsabilidades recaen comúnmente en los hogares y, específicamente, en las mujeres. En este texto abordaré tres problemas del cuidado si no se aborda desde una perspectiva feminista.6 En primer lugar, analizaré la trampa de esencializar a las mujeres en las tareas de cuidado y, con esto, reproducir el orden heteronormado. Una segunda trampa radica en considerar el cuidado como el único valor fundamental porque, si así fuera, ¿qué pasa, entonces, con la (...) justicia? Nos podríamos preguntar, como cuestiona Virginia Held, «¿el cuidado y la justicia son maneras alternativas e incompatibles de entender las mismas situaciones morales y políticas, y si es así, cuál nos debería guiar?» (1995, 1). Considerar que el cuidado está por sobre la justicia, por ejemplo, nos trae nuevos dilemas tanto en el ámbito privado como en el ámbito público. Finalmente, analizaré la trampa de considerar que estaría bien cuidar pero solamente a cercanos o a quienes consideramos que son como nosotros. Una moralidad y una política que limita el cuidado a un grupo específico de personas conlleva la peligrosa idea de que hay otro grupo de la población que no merece nuestra atención y cuidado. Los lenguajes del cine y el teatro comparten elementos tanto de contenido narrativo y verbal como visual, debido a la cercanía de sus propósitos como géneros artísticos, ya que ambos producen relatos dramatizados. Tienen, sin embargo, intereses dispares y ponen énfasis en aspectos diferentes del ambiente, de los sucesos o de los personajes. El análisis de la adaptación cinematográfica de obras teatrales permite observar las similitudes y diferencias de ambos medios, y enriquecer ese análisis por medio del contraste. En una (...) primera parte, este artículo hace un repaso a la literatura sobre el tema para observar las principales implicaciones teóricas de la adaptación cinematográfica. Posteriormente se centra en el estudio de dos secuencias de las adaptaciones de Who’s Afraid of Virginia Woolf (Edward Albee) y Closer (Patrick Marber). Estas dos secuencias ilustran los puntos de intersección entre los dos lenguajes en lo referente al diálogo y a la puesta en escena respectivamente. (shrink)

Este estudio de relaciones médico-paciente en Mexico involucró entrevistas realizadas a 1700 pacientes inmediatamente de.\pués de salir de su consulta clínica. Se llevó a cabo entre los años 2004-05 en León, More ha, Guanajuato y en Charleston, West Virginia, USA. Los propósitos fueron: 1) establecer un base de datos como punto de referencia para investigaciones futuras en instituciones del cuidado de la salud con referencia a la comunicación médico-paciente, a la satisfacción de los pacientes y al consentimiento infórmado y, (...) 2) hacer una comparación entre las relaciones medico-paciente en Mexico y los Estados Unidos de America. Hicimos varias conclusiones sobre la comunicación médico-paciente, participación del paciente en decisiones con respecto a su cuidado de salud y el ambiente social de la consulta médica. Ofi-ecemos los datos y los cuestionarios completos por internet a cualquier investigador interesado para estudios futuros de acuerdo con sus propios intereses. Además, encontramos que no huho ninguna diferencia importante en las relaciones médico-pacientes con respecto al consentimiento infórmado y la satisfáccián del paciente entre México y los EEUU. Contemporary society is in the grip of contractual thinking. Realities are interpreted in contractual terms, and goals are formulated in terms of rational contracts. The leading current conceptions of rationality begin with assumptions that human beings are independent, self-interested or mutually disinterested, individuals; they then typically argue that it is often rational for human beings to enter into contractual relationships with each other.

There are many different forms and interpretations of rewilding: the concept and its practice vary from country to country, with distinct interpretations according to its geographical location. Despite the term rewilding having been present in the lexicon for three decades, the concept of rewilding in England has experienced a prolonged developmental stage. This paper argues that a unique form of English rewilding is now emerging, which is distinct from rewilding in other parts of the world. Compared to other locations rewilding (...) in England operates at smaller spatial scales; its ambitions to increase biodiversity, restore ecosystem functioning and increase natural autonomy are somewhat curtailed; it involves higher levels of human intervention; and, perhaps most tellingly of all, it goes by another name – ‘wilding’, ‘wild’ or ‘wilder’ with little mention of the much-maligned prefix ‘re’. This conclusion has been developed following a comparative case study of two English ‘rewilding’ sites (the Avalon Marshes and Wild Ennerdale) involving 49 semi-structured interviews: twelve expert interviews and nineteen and eighteen stakeholder/practitioner interviews at the Avalon Mashes and Wild Ennerdale respectively. Rethinking the in/visibility of race within the Australian nursing workplaceThis article presents an analysis of data from a critical qualitative study with 14 skilled black African migrant nurses, which document their experiences of nurse‐to‐nurse racism and racial prejudice in Australian nursing workplaces. Racism generally and nurse‐to‐nurse racism specifically, continues to be under‐researched in explorations of these workplaces; when racism (...) is researched, the focus is nurse‐to‐patient racism and racial prejudice. Similarly, research on the experiences of migrant nurses from a variety of ethnicities in Australia has tended to neglect their experiences of the social dynamics of the workplace, thus reinforcing their racialisation. When racialised, the migrant nurse becomes ‘the problem’ through a focus on English language competency and ensuing communication barriers. This paper applies Essed’s framework of ‘everyday racism’ to theorise narratives of racism by black African migrant nurses in Australia. In so doing, it not only brings to the fore silenced discussions of nurse‐to‐nurse racism in Australia, but also exposes the subtle, mundane nature of contemporary racism. For this reason, while the data we present must be read within their context, that is, the Australian nursing workplace, it has significance for advancing a critical analysis of racialised minority groups’ experiences of racism within seemingly ‘race‐less’ nursing workplaces internationally. Nursing professionalization is both ongoing and global, being significant not only for the nursing workforce but also for patients and healthcare systems. For this reason, it is important to have an in‐depth understanding of this process and the factors that could affect it. This literature review utilizes a welfare state approach to examine macrolevel structural determinants of nursing professionalization, addressing a previously identified gap in this literature, and synthesizes research on the relevance of studying nursing professionalization. The use of a (...) welfare state framework facilitates the understanding that the wider social, economic, and political system exercises significant power over the distribution of resources in a society, providing a glimpse into the complex politics of health and health care. The findings shed light on structural factors outside of nursing, such as country‐level education, health, labor market, and gender policies that could impact the process of professionalization and thus could be utilized to strengthen nursing through facilitating increased professionalization levels. Addressing gender inequalities and other structural determinants of nursing professionalization could contribute to achieving health equity and could benefit health systems through enhanced availability, skill‐level, and sustainability of nursing human resources, improved and efficient access to care, improved patient outcomes, and cost savings. Past research (Lawson, 2004; Nonis & Swift, 2001) has revealed a correlation between academic and business ethics. Using a sample survey, this study extends this inquiry by examining the role of dispositional variables (neuroticism, extraversion, and conscientiousness) and academic honesty on business ethics perceptions. Results indicate that (1) neuroticism and conscientiousness were positively related to more ethical perceptions in a work context, and (2) academic honesty partially mediated the relationship between conscientiousness and business ethics. Two studies investigated the development of infants' visual preferences for the human body shape. In Study 1, infants of 12,15 and 18 months were tested in a standard preferential looking experiment, in which they were shown paired line drawings of typical and scrambled bodies. Results indicated that the 18-month-olds had a reliable preference for the scrambled body shapes over typical body shapes, while the younger infants did not show differential responding. In Study 2, 12- and 18-month-olds were tested with the (...) same procedure, except that the typical and scrambled body stimuli were photographic images. The results of Study 2 again indicated that only the 18-month-olds had a reliable preference for the scrambled body shapes. This finding contrasts sharply with infants' precocious preferences for human faces, suggesting that infants' learning about human faces and human bodies follow different developmental trajectories. The productive use of English past tense morphology in school-aged children (N= 74; 3 years, 8 months to 13 years, 5 months) is explored using on elicited production task. Errors represented 20% of the responses overall. Virtually all of the children demonstrated productivity with regular (e.g., good) and irregular patterns (zero-marking, e.g., sit + sit; vowel-change, e.g., ride -+ rid). Overall frequency of errors decreased with age, yet the tendency for certain types of irregularizations increased in the older groups. Analyses (...) across items indicated that all error types were predicted by combinations of item frequency, phonological chamcteristics of stems and past tense forms, and aspects of phonological past tense “neighborhoods.” Contrary to “hybrid” or dual-mechanism models incorporating a phonologically-insensitive default mechanism (e.g., Prasada & Pinker, 1993), these results suggest that children’s productivity with regular and irregular patterns is consistent with a phonologically-based constraint satisfaction system similar to that implemented in connectionist models (Daugherty & Seidenberg, 1992; Plunkett 81 Marchman, 1991,1993). Many ideas grow better when transplanted into another mind than the one where they sprang up.

Traditional Informed Consent is becoming increasingly inadequate, especially in the context of research biobanks. How much information is needed by patients for their consent to be truly informed? How does the quality of the information they receive match up to the quality of the information they ought to receive? How can information be conveyed fairly about future, non-predictable lines of research? To circumvent these difficulties, some scholars have proposed that current consent guidelines should be reassessed, with trust being used as (...) a guiding principle instead of information. Here, we analyse one of these proposals, based on a Participation Pact, which is already being offered to patients at the Istituto Europeo di Oncologia, a comprehensive cancer hospital in Milan, Italy. We review major developmental evidence on the continuity from action to gesture to word and sign in human children, highlighting the important role of caregivers in the development of multimodal communication. In particular, the basic issues considered here and contributing to the current debate on the origins and development of the language-ready brain are: (1) links between early actions, gestures and words and similarities in representational strategies; (2) importance of multimodal communication and the interplay between gestures and spoken words; (3) (...) interconnections between early actions, gestures and signs. The innovation of this report is in connecting these themes together to relevant findings from studies on children between 6 and 36 months of age and highlighting interesting parallels in studies on ape communicative behavior. We can usefully draw an analogy between ethics and science, despite the significant differences between them. We can then see the ways in which moral theories can indeed be “tested,” not by empirical experience but by moral experience. This can be expected to lead to rival moral theories, but in science also we have rival theories. I argue that we should demand more than coherence of our moral theories, as we do of our scientific theories. The Russo Williamson thesis (RWT) states that a causal claim can be established only if it can be established that there is a difference-making relationship between the cause and the effect, and that there is a mechanism linking the cause and the effect that is responsible for such a difference-making relationship (Russo & Williamson, 2007). The applicability of Russo and Williamson’s idea was hugely debated in relation to biomedical research, and recently it has been applied to the social sciences (Shan (...) & Williamson, 2021). While many philosophers and social scientists have advocated the use of different kinds of evidence for causal discoveries, others have criticised this approach. With this paper, I aim to defend RWT from criticisms and to show its importance in the social sciences. The paper is structured as follows. After a brief introduction, in Sect. 2, I will summarise RWT, and in Sect. 3 I will describe how this approach can be applied to the social sciences. In Sect. 4, I will reconstruct two main criticisms of this thesis proposed in the philosophy of the social sciences literature: namely (i) RWT is not used in the social sciences, (ii) RWT does not work. For each criticism I will provide a defence of RWT. My defence will be based on two general considerations: (i) RWT appears perfectly in line with the research methods used in the social sciences and (ii) RWT can be applied successfully to establish causal claims. In Sect. 5, moreover, I will examine the causal accounts that have motivated such criticisms and I will argue that they should be rejected to endorse RWT and a causal account able to accommodate the current use of mechanistic and difference-making evidence in the social sciences. Section 6 will conclude with a note on the relevance of RWT in both its descriptive and normative form. (shrink)

Despite increased attention to the question of how best to evaluate clinical ethics consultations and emphasis on external evaluation, there has been little sustained focus on how we, as clinicians, make sense of and learn from our own experiences in the midst of any one consultation. Questions of how we evaluate the request for, unfolding of, and conclusion of any specific ethics consultation are often overlooked, along with the underlying question of whether it is possible to give an accurate account (...) of clinical ethics consultants’ experience as experienced by ethics consultants. Before the challenge of submitting one’s accounts or case reports for review and evaluation from others, there is an underlying challenge of understanding and evaluating our own accounts. To highlight this crucial and deeply challenging dimension of actual clinical ethics practice, we present an account of a complex consultation, explicitly constructed to engage the reader in the unfolding experience of the consultant by emphasizing the multiple perspectives unfolding within the consultant’s experience. Written in script format, the three perspectives presented—prototypical clinically descriptive account; didactically reflective and self-evidentiary account often seen in journal presentations; highly self-critical reflective account emphasizing uncertainties inherent to clinical ethics practice—reflect different manners for responding to the ways actual clinical involvement in ethics consultation practice accentuates and refocuses the question of how to understand and evaluate our own work, as well as that of our colleagues.
