The progress of electronic health technologies and biobanks holds enormous promise for efficient research. Evidence shows that studies based on sharing and secondary use of data/samples have the potential to significantly advance medical knowledge. However, sharing of such resources for international collaboration is hampered by the lack of clarity about ethical and legal requirements for transfer of data and samples across international borders. Here, the International Clinical Trial Center Network reports the legal and ethical requirements governing data and sample exchange (...) across four continents. The most recurring requirement is ethical approval, whereas only in specific conditions approval of national health authorities is required. Informed consent is not required in all sharing situations. However, waiver of informed consent is only allowed in certain countries/regions and under certain circumstances. The current legal and ethical landscape appears to be very complex and under constant evolution. Regulations differ between countries/regions and are often incomplete, leading to uncertainty. With this work, ICN illuminates the unmet need for a single international collaborative framework to facilitate DSE. Harmonising requirements for global DSE will reduce inefficiency and waste in research. There are many challenges to realising this ambitious vision, including inconsistent terminology and definitions, and heterogeneous and dynamic legal constraints. Here, we identify areas of agreement and significant difference as a necessary first step towards facilitating international collaboration. We propose the establishment of a working group to continue the comparison across jurisdictions, create a standardised glossary and define a set of basic principles and fundamental requirements for DSE. It is often debated whether what we ought, politically, to do is determined by standards that are independent of any actual political process or whether, by contrast, judgments reached in actual democratic processes have constitutive importance in determining what we should do. This paper argues that this is not an exclusive disjunction and that, consistently with there being independent standards, constitutively authoritative judgments can enter into the truth-conditions pertaining to claims about what we ought, politically, to do. The crucial objection (...) to constitutive judgment is that it involves an unacceptable form of bootstrapping, according to which reasons arise out of nothing. To circumvent this objection, the paper deploys John Broome's notion of a wide-scope 'normative requirement'. Normative requirements affect what ought to be done without altering the balance of reasons and defuse the bootstrapping objection by blocking the derivation of all-things-considered conclusions. To show that politics involves constitutive authority of this kind, the paper defends two illustrative normative requirements applicable to the political process that give constitutive roles to political judgments of various kinds. Throughout, the discussion is enlivened by comparisons to the judgments of a baseball umpire, which have been illuminatingly discussed by Robert Brandom. (shrink)

This book weaves together concerns from three different literatures in philosophy: the logical analysis of propositional attitude reports, the semantics and metaphysics of cognition, and issues of causation and externalism in epistemology. The topics these literatures deal with fit naturally together, but the literatures do not. Often ideas are developed in isolation from the others; sometimes authors in one area see the relevance of the others, but do not have the patience to get things straight. This work is based on (...) the version of Philip R. Henry's dissertation that was left behind after his untimely death in 1987. Professor Julius Moravscik, Henry's advisor, and Professor John Perry, a member of Henry's committee, felt it worthwhile to make the ideas of this serious and talented philosopher available to the philosophy community. Non‐invasive microanalytical methods have been devised to study the energy metabolism of single human preimplantation embryos. Psyruvate, which is added routinely to all media used to culture human embryos, is consumed throughout the preimplantation period, with glucose assuming an increasing role at embryo compaction and blastocyst formation. All of the glucose consumed may be accounted for by the appearance of lactate in the incubation medium. The enzyme hexokinase my be involved in regulating this aerobic glycolysis. There is cosiderable indirect evidence (...) for the utilisation of endogenous as opposed to exogenous energy substrates, the most likely candidate being protein. Information on early human embryo metabolismis likely to find application in a number of areas: these include the improvement of techniques for assisted human conception, notably in the selection of embryos for transfer following In Vitro Fertilisation; the diagnosis of gentic defects at the preimplantation stage; increased undersding of the causes of implantation failure and miscarriage, and the development of novel post‐coital contraceptives. To this branch of dialectics the present paper does not attempt to contribute, chiefly because it is written from the conviction that no part of science, nor any philosophy, can safely predict what may be feasible or knowable. Scientific uncertainty puzzles many people. The puzzlement arises when scientists have more than one answer, and disagree among themselves. This book helps people find their way through this maze of scientific contradiction and uncertainty. By acquainting them with the ways that uncertainty arises in science, how scientists accommodate and make use of uncertainty, and how they reach conclusions in the face of uncertainty, the book enables readers to confidently evaluate uncertainty from their own perspectives, in terms of their own experiences.

The negative consequences of physicians' failure to establish and maintain personal relationships with patients are at the heart of the humanistic crisis in medicine. To resolve this crisis, a new model of doctor-patient interaction is proposed, based on the ideas of Martin Buber's philosophy of dialogue. This model shows how the physican may successfully combine the personal (I-Thou) and impersonal (I-It) aspects of medicine in three stages. These Three Stages of Medical Dialogue include:1. An Initial Personal Meeting stage, which initiates (...) the doctor-patient relationship and involves mutual confirmation; 2. An Examination stage, which requires a shift from a personal to an impersonal style of interaction; 3. An Integration Through Dialogue or Healing Through Meeting Stage, which involves the integration of the impersonal medical data into the ongoing dialogue between doctor and patient, as a basis for shared decision-making. Erwin Schrödinger and Werner Heisenberg were the originators of two approaches, known respectively as “wave mechanics” and “matrix mechanics”, to what is now called “quantum mechanics” or “quantum theory”. The two approaches appear to be extremely different, both in their technical forms, and in their philosophical underpinnings. Heisenberg arrived at his theory by effectively renouncing the idea of trying to represent a physical system, such as a hydrogen Bohr's atom model for example, as a structure in space—time, but instead, following (...) the lead of Einstein's 1905 theory of relativity, representing only empirically observable properties, such as the transition amplitudes between the stationary states of the atom. These amplitudes can be arranged in square arrays of numbers. In Heisenberg's scheme these arrays, and other like them, are combined according to certain rules that were later recognized by Max Born to be the rules of matrix multiplication. The whole scheme is abstract and mathematical, and avoids using any space—time picture of what is going on at the atomic level. Schrödinger, on the other hand, represented the electron in an atom by a cloudlike wave surrounding the nucleus. This is a space—time structure that, superficially at least, is more in line with the classical physical theories of the eighteenth and nineteenth centuries. -/- Niels Bohr invited Schrödinger to come to Copenhagen to present his ideas, and to discuss this subject with himself, Heisenberg, and others. Schrödinger arrived in Copenhagen on October 1st, 1926, and was immediately intensively engaged by Bohr and the others in a “debate” that lasted for days. Eventually, Schrödinger become ill, and was confined in Bohr's home to a bed, upon which Bohr sat, continuing the discussion. Schrödinger finally exclaimed “If all the quantum jumping is here to stay, then I am sorry that I ever became involved in quantum mechanics”. Bohr replied, “But we are glad that you did!”. (shrink)
