In this paper, we outline a framework for understanding the different kinds of knowledge required for medical practice and use this framework to show how scientism undermines aspects of this knowledge. The framework is based on Michael Polanyi’s claim that knowledge is primarily the product of the contemplations and convictions of persons and yet at the same time carries a sense of universality because it grasps at reality. Building on Polanyi’s ideas, we propose that knowledge can be described along two (...) intersecting “dimensions”: the tacit–explicit and the particular–general. These dimensions supersede the familiar “objective−subjective” dichotomy, as they more accurately describe the relationship between medical science and medical practice. Scientism, we argue, excludes tacit and particular knowledge and thereby distorts “clinical reality” and impairs medical practice and medical ethics. (shrink)

People often have a strong intuitive sense that we ought to rescue those in serious need, even in cases where we could produce better outcomes by acting in other ways. It has become common in such cases to refer to this as the Rule of Rescue. Within the medical field this rule has predominantly been discussed in relation to decisions about whether to fund particular treatments. Whilst in this setting the arguments in favour of the Rule of Rescue have generally (...) been found to be unconvincing, there are some reasons for thinking that it may have more of a role to play at the clinical level. In this article we examine three lines that such reasoning might take. In each case we argue that the reasons given do not support the adoption of a Rule of Rescue in clinical practice. (shrink)

This paper describes the UK Research Ethics Committee’s (REC) preparations and review of the global first SARS-CoV-2 human infection challenge studies. To frame our review, we used the WHO guidance and our UK Health Research Authority ethical review framework. The WHO criteria covered most issues we were concerned about, but we would recommend one further criterion directing RECs to consider alternative research designs. Could research questions be equally well answered by less intrusive studies? The committee met virtually, ensuring broad representation (...) across the UK nations and also ensuring applicants could attend easily. We worked in collaboration with the applicants but while we recognise that such proximity might raise the accusation of ‘collusion’, we made every effort to maintain ‘moral distance’ and all decisions were made by the committee alone. Prior existing processes and policy facilitated training and review but even with this preparation, review took time and this could have hindered a rapid response to the emergency. Review for the various follow-on studies will now be speedier and once the pandemic has subsided, our group could be reconvened in future emergencies. In conclusion, we have tried to make decisions in good faith. We know there is controversy and disagreement and reasonable people may feel we have made the wrong decision. Research consent processes must provide potential participants with the necessary information to help them decide if they wish to join a study. On the Oxford ‘A’ Research Ethics Committee we’ve found that current research proposals mostly provide adequate detail (even if not in an easily comprehensible format), but often fail to support decision making, a view supported by published evidence. In a previous paper, we described how consent might be structured, and here we develop the concept of an Information and (...) Decision Aid (IDA) that can support decision making and be used to guide the dialogue between researcher and potential participant. Our proposal requires limited changes to current processes or paperwork and would provide an easily accessible document for others that the potential participant might approach for advice. It could later be integrated with the Informed Consent Form to ensure all matters of concern to the individual participant have been addressed before consent is formally signed off. (shrink)

This essay offers a critical examination of Neil Ormerod’s treatment of the debate between Lonergan and Gilson on the question of being. Although this debate concerns a highly technical issue of metaphysics and epistemology, it remains germane and relevant, especially within the field of Christian thought. In Ormerod’s careful and for the most part generous examination of this debate, he argues that being for Gilson is perceived through the senses, whereas for Lonergan being is intended in the questions that arise (...) from the relevant sense data. Where Gilson’s philosophy gives priority to the metaphysics of being, Lonergan gives priority to epistemology and cognitional theory. In arguing for the superiority of Lonergan’s approach to the question of being, Ormerod relies on a fundamental misunderstanding of Gilson’s metaphysics. By appeal to the more recent work of Kenneth Schmitz, this essay proposes a proper understanding of Gilson’s metaphysics as a basis for a more conciliatory relationship between these two giants in modern Christian philosophy who too often are pitted against one another. I argue that the reference for “God” is determined by the definite description “the being that is worthy of our worship.” I describe two desiderata for rival theories of the reference of “God” to meet: accessibility and scope. I explain the deficiencies of a view where God is dubbed “God” and the name passed down by causal chains and a view where “God” picks out the unique satisfier of a traditional definite description. After articulating the “Worship-Worthiness” view, I show how (...) it best satisfies the desiderata. I then respond to some putative counterexamples to the view. (shrink)

Looking back we often take the Second Vatican Council as the point where change began. However, the seeds of what flowered at the council and have continued to bear (...) fruit after it were planted well before it. The future is not clear, yet we can expect that significant change will be part of it. As we reflect on our changing situation, it would be helpful to keep in mind another comment of Dean Inge: 'There are two kinds of fools: one says, "This is old, therefore it is good"; the other says, "This is new, therefore it is better" '. This article explores the problem of justice between age-groups. Specifically, it presents a challenge to a leading theory in this field, Norman Daniels' Prudential Lifespan Account. The challenge relates to a key assumption that underlies this theory, namely the assumption that all individuals live complete lives of equal length. Having identified the roles that this assumption plays, the article argues that the justifications Daniels offers for it are unsatisfactory and that this threatens the foundation of his position, undermining his claim (...) that ‘the fact that we all age’ makes age a special problem of distributive justice. This shows that the problem of justice between age-groups is not special in the way Daniels proposes; rather it involves the same irreducibly interpersonal distributive decisions as other problems of justice. The consequences of this argument are several-fold. Most importantly, it shows that the Rawlsian account of justice to which Daniels hopes to attach his theory to requires significantly greater benefits to be conferred on those in earlier age-groups relative to those in later age-group, not a distribution similar to simultaneous equality as Daniels proposes. This Symposium provides an important opportunity to reflect on the current state of scholarship positioning alternative foods against mainstream agri-food systems. Symposia of this kind have a long tradition as marking particular turning points in agrifood debates. This collection provides an opportunity to examine the current positioning of scholarship around the theoretical and methodological fracture line between successor theories to classical political economy and more post-structuralist approaches to alternative economic activities around food and agriculture. In the current collection, despite clear (...) evidence of theoretical positioning around the structural: post-structural divide, I argue that both aspects of agri-food theorizing share similar political intent and are positioned within the same wider political project of agrifood critique. Therefore, despite theoretical fracture, there is a unity of political intention that continues to bind together this particular field of research and practice. (shrink)

Successful tutoring depends in part on child tutors’ ability to recognise and interpret accurately signals of misunderstanding by their tutees. Age- and gender-related differences were investigated in a study which exposed 80 children to a video-recorded episode involving a target child receiving ambiguous instructions in her attempts to move a model car along a designated route on a playmat roadway from one destination to another. The results showed that explicit, general and facial modes of displaying puzzlement by the target child (...) were judged as equally powerful expressions of degree of puzzlement, but 8-10 year olds judged the target child as significantly more puzzled than did 7 year olds. Older children were also more likely to attribute the puzzlement of the target child to the ambiguous instructions than to her verbal or facial expression of puzzlement. Girls were generally more accurate than boys in judging facial expressions and less likely than boys to give wrong or alternatively ambiguous responses when given the opportunity to correct the video-recorded ambiguous instructions. The results throw some light on the developing sensitivities of boys and girls in recognising and attributing failures of understanding by other children. (shrink)
