The push by some bioethicists to excise religion from the clinical ethics consultative process has received institutional support from the American Society for Bioethics and the Humanities. Their certification program, Healthcare Ethics Consultant-Certified, is intended to identify and assess “a national standard for the professional practice of clinical healthcare ethics consulting” devoid of religious content. As Christian ethicists who wish to preserve the morally evaluative nature of healthcare ethics, we must pause and theologically reflect on the meaning of such a (...) program. The five articles in this issue offer rich theological responses to the religion-free standardized methodology endorsed by the ASBH and reified in the HEC-C certification program. They offer a depth of theological reflection we see previously lacking in the literature, attending to the real possibilities of a “terraformed bioethics” effecting metaphysical harm, severing of clinical ethics from its sources, eliminating possibilities for conversion, and confusing the meaning of moral expertise. Honor walks are ceremonies that purportedly honor organ donors as they make their final journey from the ICU to the OR. In this paper, we draw on Ronald Grimes’ work in ritual studies to examine honor walks as ceremonial rituals that display medico-technological power in a symbolic social drama (Grimes, 1982). We argue that while honor walks claim to honor organ donors, ceremonies cannot primarily honor donors, but can only honor donation itself. Honor walks promote the quasi-religious idea of donation (...) as a “good death,” and mask the ambiguity and discomfort inherent in organ donation to promote greater acceptance by the medical community. While some goods may be achieved through honor walks, particularly for donor families, it is still important to examine the negative work done by this practice. (shrink)

Here we identify approximately 40,000 healthy human volunteers who were intentionally exposed to infectious pathogens in clinical research studies dating from late World War II to the early 2000s. Microbial challenge experiments continue today under contemporary human subject research requirements. In fact, we estimated 4,000 additional volunteers who were experimentally infected between 2010 and the present day. We examine the risks and benefits of these experiments and present areas for improvement in protections of participants with respect to safety. These are (...) the absence of maximum limits to risk and the potential for institutional review boards to include questionable benefits to subjects and society when weighing the risks and benefits of research protocols. The lack of a duty of medical care by physician–investigators to research subjects is likewise of concern. The transparency of microbial challenge experiments and the safety concerns raised in this work may stimulate further dialogue on the risks to participants of human experimentation. (shrink)

Jeffrey A. Mason has written an informative, accessible guide to today's most popular form of philosophical writing, the journal-length essay. The Philosopher's Address does what no other book on the market has attempted: it takes the reader behind the scenes of the writing process to expose the rhetorical underpinnings of philosophical texts. Mason argues that readers need to understand why philosophical writing is constructed as it is, and to be aware of the rhetorical devices by which authors (...) seek to persuade them if they are to engage fully with these texts. One must technologize bodies to conceive of organ transplantation. Organs must be envisioned as replaceable parts, serving mechanical functions for the workings of the body. In this way, it becomes possible to imagine exchanging someone’s organs without changing anything essential about the selfhood of the person. But to envision organs as mechanical parts is phenomenologically uncomfortable; thus, the terminology used to describe the practice of organ retrieval seems to attempt other, less technological ways of viewing the human body. In this (...) paper, I analyze three common metaphors that currently contextualize the process of organ retrieval in English-speaking communities: harvesting the agrarian body, procuring the commodified body, and receiving the gifted body. These powerful images constrain the gaze toward the body in important ways. Every gaze both obscures and reveals. While each of these three metaphors makes sense of some aspects of organ retrieval, each of them is ultimately subject to being overtaken by what Jeffrey Bishop calls the technological imaginary. This imaginary deploys a gaze that obscures important elements of what it means to be human and does violence to parts of the phenomenological experience of transplantation and bodily existence. I argue that no matter how hard one tries to avoid the technological aspect of transplantation practices by embracing nonviolent metaphors—even the metaphor of gifting, which seems the most promising—it will never be possible to fully resist organ transplantation’s violence toward our phenomenological sense of embodiment. David Lewis (1969) introduced sender-receiver games as a way of investigating how meaningful language might evolve from initially random signals. In this report I investigate the conditions under which Lewis signaling games evolve to perfect signaling systems under various learning dynamics. While the 2-state/2- term Lewis signaling game with basic urn learning always approaches a signaling system, I will show that with more than two states suboptimal pooling equilibria can evolve. Inhomogeneous state distributions increase the likelihood of pooling equilibria, but (...) learning strategies with negative reinforcement or certain sorts of mutation can decrease the likelihood of, and even eliminate, pooling equilibria. Both Moran and APR learning strategies (Bereby-Meyer and Erev 1998) are shown to promote successful convergence to signaling systems. A model is presented that illustrates how a language that codes state-act pairs in an order-based grammar might evolve in the context of a Lewis signaling game. The terms, grammar, and the corresponding partitions of the state space co-evolve to generate a language whose structure appears to reflect canonical natural kinds. The evolution of these apparent natural kinds, however, is entirely in service of the rewards that accompany successful distinctions between the sender and receiver. Any metaphysics grounded on the structure of a natural language that evolved in this way would track arbitrary, but pragmatically useful distinctions. (shrink)

Lewis signaling games illustrate how language might evolve from random behavior. The probability of evolving an optimal signaling language is, in part, a function of what learning strategy the agents use. Here we investigate three learning strategies, each of which allows agents to forget old experience. In each case, we find that forgetting increases the probability of evolving an optimal language. It does this by making it less likely that past partial success will continue to reinforce suboptimal practice. From the early 1960s until his death, French philosopher Gilles Deleuze wrote many influential works on philosophy, literature, film, and fine art. One of Deleuze's main philosophical projects was a systematic inversion of the traditional relationship between identity and difference. This Deleuzian philosophy of difference is the subject of Jeffrey A. Bell's Philosophy at the Edge of Chaos. Bell argues that Deleuze's efforts to develop a philosophy of difference are best understood by exploring both Deleuze's claim to be a (...) Spinozist, and Nietzsche's claim to have found in Spinoza an important precursor. Beginning with an analysis of these claims, Bell shows how Deleuze extends and transforms concepts at work in Spinoza and Nietzsche to produce a philosophy of difference that promotes and, in fact, exemplifies the notions of dynamic systems and complexity theory. With these concepts at work, Deleuze constructs a philosophical approach that avoids many of the difficulties that linger in other attempts to think about difference. Bell uses close readings of Plato, Aristotle, Spinoza, Nietzsche, Heidegger, Derrida, and Whitehead to illustrate how Deleuze's philosophy is successful in this regard and to demonstrate the importance of the historical tradition for Deleuze. Far from being a philosopher who turns his back on what is taken to be a mistaken metaphysical tradition, Bell argues that Deleuze is best understood as a thinker who endeavoured to continue the work of traditional metaphysics and philosophy. We consider the efficacy of various forms of reinforcement learning with punishment in evolving linguistic conventions in the context of Lewis-Skyrms signalling games. We show that the learning strategy of reinforcement with iterative punishment is highly effective at evolving optimal conventions in even complex signalling games. It is also robust and can be easily extended to a self-tuning variety of reinforcement learning. We briefly discuss some of the virtues of reinforcement with iterative punishment and how it may be related to (...) learning in nature. (shrink)

The use of deception in medical care is highly suspect in this country. Yet there is one condition for which deception is often used as a diagnostic tool. Nonepileptic seizures, a psychiatric condition in which emotional or psychological conflicts manifest themselves unconsciously through bodily symptoms, are currently diagnosed by a procedure called “provocative saline infusion.” The test is fundamentally deceptive, requiring the physician to intentionally and directly lie to the patient, causing the patient to believe that the administered solution caused (...) his seizures. This work introduces the ACTWith model of moral cognition. This is a model of conscience and conscientious agency, inspired by Socratic philosophy, neurology and artificial intelligence. The ACTWith model is a synthesis across these disciplines, integrating ancient and contemporary insights into the human condition, while distilling this synthesis into a practicable dynamic simplified via architectural paradigms imported from theories of computational models of human learning. It was developed in response to the need in these fields for a clear articulation of (...) conscience. In the world at large, conscience is often referenced, yet hardly understood. Although there is a long-standing international debate concerning the privatization and corporatization of health services, there has been relatively little systematic analysis of the ways these types of reform manifest. We examine the impact of privatization and corporatization on public hospitals, and in particular on hospitals’ autonomy and accountability, with two aims: to uncover the key themes in the literature, and to consider implementation issues. The review of 2,319 articles was conducted using content analysis and a discussion of selected key (...) issues. Several major themes appear in the privatization and corporatization literature, including their use as tools in health systems reform, and the role of governments in sponsoring the processes. We show that much of the underlying argument is ideological rather than evidence based. Those who promote versions of privatization or corporatization claim that decreased government involvement in the management of hospitals leads inter alia to benefits such as greater efficiency, better quality services, and increased choice for patients. Those who argue against say that increased privatization leads to deleterious outcomes such as decreased equity, compromised efficiency and poorer quality of care. The evidence is often weak and at times conflicting. Privatization and corporatization are difficult to implement, and at best produce mixed results, and their impact seems to depend more on the motivation of the evaluator than the standard of the results. These debates are of a type that is to a large extent only resolvable ideologically. (shrink)

This chapter considers two types of fine‐tuning, those dealing with the initial conditions of the universe and those based on fixed parameters. Three approaches have been taken to argue that fine‐tuning does not need any special explanation. The first is an appeal to coincidence. The second is that the data are biased by our own observations. The third has to do with the nature of probability itself. The chapter assesses each of these objections in detail. Many naturalistic explanations have been (...) offered for fine‐tuning. The chapter considers the three most important ones. The first is that future discoveries might show that fine‐tuning was not improbable after all. The second is that other life‐permitting ranges for the fine‐tuned constants (FTCs) might exist, allowing for exotic forms of life unlike ours. The third is that we might live in a multiverse of many different universes all with different values of FTCs. (shrink)
