The purpose of this study was to identify how older Korean people seek information and their desire to participate in decision making about their health care. A total of 165 elderly people living in Seoul, South Korea, participated in the study. Data were collected during individual interviews using the Autonomy Preference Index. The mean information-seeking score was high. The mean score for their desire to participate with a physician in decision making was lower, but this was higher when family members (...) were involved. The study indicates that many older people want to receive information about their health care. Families (or guardians), as well as older people themselves, should be included in the decision-making process. Nurses can encourage older people to express their wishes, while treating each individual with respect. (shrink)

This study aimed to explore the enabling context of the HIV care continuum as perceived by people living with HIV and healthcare professionals. This qualitative study involved in‐depth individual interviews with eight people living with HIV and group interviews with seven nurses and physicians. These interviews took place between March 5, 2021, and July 13, 2022. Thematic analysis was conducted. The main themes that emerged included “network of support and systems,” “personal gains from the care continuum,” “continuity and practicality of (...) healthcare services,” “presence of significant other(s),” and “social belonging.” These themes were structured within the five levels of the socio‐ecological model (system, individual, organizational, interpersonal, and community levels), and 10 subthemes were identified. With the integration of themes, the enabling context was compared to a “care orbit,” indicating that people diagnosed with HIV start treatment at the system level and continue HIV care at other levels. In addition to primary support at the system level, the enabling context also includes individual commitment to care and regular health habits, hospital services, support from significant others, and community culture. Together, the context helps guide people living with HIV into the “orbit” of the care system. (shrink)

This paper elucidates and rationalizes the ethical governance system for healthcare AI research, as outlined in the ‘Research Ethics Guidelines for AI Researchers in Healthcare’ published by the South Korean government in August 2023. In developing the guidelines, a four-phase clinical trial process was expanded to six stages for healthcare AI research: preliminary ethics review (stage 1); creating datasets (stage 2); model development (stage 3); training, validation, and evaluation (stage 4); application (stage 5); and post-deployment monitoring (stage 6). Researchers identified (...) similarities between clinical trials and healthcare AI research, particularly in research subjects, management and regulations, and application of research results. In the step-by-step articulation of ethical requirements, this similarity benefits from a reliable and flexible use of existing research ethics governance resources, research management, and regulatory functions. In contrast to clinical trials, this procedural approach to healthcare AI research governance effectively highlights the distinct characteristics of healthcare AI research in research and development process, evaluation of results, and modifiability of findings. The model exhibits limitations, primarily in its reliance on self-regulation and lack of clear delineation of responsibilities. While formulated through multidisciplinary deliberations, its application in the research field remains untested. To overcome the limitations, the researchers’ ongoing efforts for educating AI researchers and public and the revision of the guidelines are expected to contribute to establish an ethical research governance framework for healthcare AI research in the South Korean context in the future. ObjectivesHostile attribution bias is reportedly common from non-clinical population to those with serious mental illness, such as schizophrenia, and is known to be closely related to theory of mind. This study aimed to investigate whether ToM skills mediate the relationship among neurocognitive ability, personality traits, and attribution bias.MethodsA total of 198 non-clinical youths were recruited. To assess their neurocognitive ability and ToM skills, the participants were asked to complete Raven’s Standard Progressive Matrices and the Korean version of the Reading the (...) Mind in Eyes Test. To determine their personality traits, the Eysenck Personality Questionnaire and interpersonal reactivity index were used. To evaluate hostile attribution bias, the Ambiguous Intentions Hostility Questionnaire was administered. Path analysis and bias-corrected percentile bootstrap methods were used to estimate model fit and the parameters of the mediating effects.ResultsBased on model comparison, the best model characterized two direct pathways from psychoticism and the K-RMET to hostility attribution bias and three indirect pathways, wherein SPM, perspective taking, and psychoticism influenced hostile attribution bias through K-RMET. The final model fit indices were good [x2/df = 1.126; comparative fit index = 0.996; root mean square error of approximation = 0.026; standard root mean square residual = 0.026 and Akaike information criterion = 28.251] and the K-RMET fully mediated the association between SPM, perspective taking, psychoticism, and hostile attribution bias.ConclusionThe main findings suggested that ToM skills, such as the RMET, play an important role in explaining the relationship among neurocognitive ability, personality traits, and hostile attribution bias. ToM skills and a remediation strategy may need to be developed to balance the enhanced hostility bias that underlies the paranoia. (shrink)

For both Spinoza and Zhuang-zi, the goal of life is attaining freedom, namely, human freedom. What do they mean by “human freedom”? What are the necessary conditions for humans to attain freedom? Seeking for answers to these questions, I’ve noted that Spinoza and Zhuang-zi suggest remarkably similar answers to these questions. First, both Spinoza and Zhuang-zi understand human freedom as a form of complete independence from externals: self-determination in Spinoza and union with the Dao in Zhuang-zi. Zhuang-zi assert that human freedom is attainable when one sees things truly or adequately: in Spinoza, when one sees things from the viewpoint of eternity, and in Zhuang-zi, when one sees things from the viewpoint of the Dao. Background Nurses providing care to patients with end-of-life or terminal illnesses often encounter ethically challenging situations leading to moral distress. However, existing quantitative studies have examined moral distress using instruments that address general clinical situations rather than those specific to end-of-life care. Furthermore, qualitative studies have often been limited to participants from a single unit or those experiencing moral distress-induced circumstances. A comprehensive and integrated understanding of the overarching process of moral distress is vital to discern the unique circumstances surrounding (...) end-of-life care and its consequential impacts. Research objectives To explore the moral distress experiences of nurses who are frequently involved in caring for patients with end-of-life or terminal illnesses and apply it to two existing theories: the model of moral distress and the ecological model. Research design A qualitative descriptive approach was employed. Participants and research context Seven focus group interviews involving 30 nurses were performed. The subsequent transcriptions underwent rigorous content analysis. Ethical considerations We obtained Institutional Review Board approval from a university. Focus group interviews were conducted with nurses who agreed to participate and signed the consent form. Findings The moral distress-inducing factors and nurses’ perceived impact of moral distress were identified and categorized based on moral distress theories and ecological models. A total of 15 categories and 30 subcategories across the following 4 domains were derived: (1) intrapersonal, (2) interpersonal, (3) organizational, and (4) structural factors. Conclusions End-of-life-specific circumstances induced moral distress among nurses, with both negative and positive impacts identified. Effective organizational and policy support is essential to manage conflicts, form a healthy organizational culture, provide training, and prevent unnecessary expenses due to the negative consequences of moral distress. The population of international students in South Korea is growing. During the career development phase, international students face unique challenges related to their bicultural identity and acculturation experiences. The present study examined the role of bicultural self-efficacy on mediating the relationship between acculturation and career decision-making self-efficacy for international students in South Korea. Responses from 120 international students in South Korea were analyzed using structural equation modeling. The results showed that bicultural self-efficacy fully mediated the relationship between acculturation to mainstream (...) culture and career decision-making self-efficacy but did not mediate the relationship between acculturation to heritage culture and career decision-making self-efficacy. Mon étude s’inspire de la thèse suivante : que la philosophie construit son objet seulement à partir d’une certaine perspective normative. La question philosophique du cosmopolitisme – comment établir par le biais de la morale universelle l’association des « citoyens du monde » libres et égaux – ne fonde qu’un horizon normatif d’attentes qui attire le regard sur une réalité irrationnelle. La réalité irrationnelle se caractérise par la notion de « société mondiale du risque », introduite par Ulrich Beck pour (...) faire le diagnostic de notre temps frappé par toutes les catastrophes envisageables. Celles-ci montrent à quel point les fondations sur lesquelles nous avons bâti l’État national et la démocratie sont instables. Plus les risques sont globalisés, plus nous sommes contraints à une « conscience civile mondiale ». Malgré la connaissance des risques globalisés, nous sommes en premier lieu « citoyens d’État » et non « citoyens du monde ». Il est néanmoins indéniable que les frontières de l’État national perdent leur importance. Partant de l’hypothèse que la société mondiale est plus qu’un marché mondial, je souhaiterais souligner le « moment cosmopolite » de la délimitation transnationale afin de développer la morale universaliste de la société civile mondiale à l’égard de l’avenir de la démocratie. This chapter explores Rainer Forst’s justification-centric view of nondomination toleration. This view places an idea of equal respect and a corresponding requirement of reciprocal and general justification at the core of non-domination toleration. After reconstructing this view, this chapter addresses two issues. First, even if this idea of equal respect requires the limits of non-domination toleration to be drawn in a manner that is equally justifiable to all affected people, equal justifiability should not be understood in terms of Forst’s requirement (...) of reciprocal and general acceptability. Second, for the equal justifiability of relevant constraints to ensure non-domination outcomes, discursive equality must be understood in substantive, purchase-sensitive terms. This means that a justification-centric view of non-domination toleration stands or falls with the participation value of what it regards as the standards of justification. This places reasonably contested matters of value at the heart of such views. (shrink)

Recent calls to address racism in bioethics reflect a sense of urgency to mitigate the lethal effects of a lack of action. While the field was catalyzed largely in response to pivotal events deeply rooted in racism and other structures of oppression embedded in research and health care, it has failed to center racial justice in its scholarship, pedagogy, advocacy, and practice, and neglected to integrate anti-racism as a central consideration. Academic bioethics programs play a key role in determining the (...) field’s norms and practices, including methodologies, funding priorities, and professional networks that bear on equity, inclusion, and epistemic justice. This article describes recommendations from the Racial Equity, Diversity, and Inclusion (REDI) Task Force commissioned by the Association of Bioethics Program Directors to prioritize and strengthen anti-racist practices in bioethics programmatic endeavors and to evaluate and develop specific goals to advance REDI. In this article we build on the program of research in well-being marketing by further conceptualizing and refining the conceptual domain of the concept of consumer well-being (CWB). We then argue that well-being marketing is a business philosophy grounded in business ethics. We show how this philosophy is an ethical extension of relationship marketing (stakeholder theory in business ethics) and is superior to transactional marketing (a business philosophy grounded in the principles of consumer sovereignty). Additionally, we argue that well-being marketing (...) is based on duty ethics concepts, specifically the duty of beneficence and non-maleficence. The convergence of increasingly efficient high throughput sequencing technology and ubiquitous Internet use by the public has fueled the proliferation of companies that provide personal genetic information direct-to-consumers. Companies such as 23andme and Navigenics are emblematic of a growing market for PGI that some argue represents a paradigm shift in how the public values this information and incorporates it into how they behave and plan for their futures. This new class of social networking business ventures that market the science of (...) the personal genome illustrates the new trend in collaborative science. In addition to fostering a consumer empowerment movement, it promotes the trend of democratizing information—openly sharing of data with all interested parties, not just the biomedical researcher—for the purposes of pooling data and escalating the innovation process. This target article discusses the need for new approaches to studying DTC genomics using social network analysis to identify the impact of obtaining, sharing, and using PGI. As a locus of biosociality, DTC personal genomics forges social relationships based on beliefs of common genetic susceptibility that links risk, disease, and group identity. Ethical issues related to the reframing of DTC personal genomic consumers as advocates and research subjects and the creation of new social formations around health research may be identified through social network analysis. According to the Interface Hypothesis in the field of bilingualism, the interface connecting a linguistic module with a language-external domain (e.g., syntax-discourse) will present prolonged difficulties for adult bilingual learners, as compared with the interface connecting language-internal modules (e.g., syntax-semantics). This study tested whether the Interface Hypothesis is applicable to the acquisition of Mandarin Chinese as a heritage language. An internet-based acceptability judgment task (AJT) was administered to 58 advanced and intermediate adult Chinese heritage speakers to collect data in accuracy (...) and reaction time to investigate the adult heritage speakers’ mastery of referential nominal expressions regulated at the syntax-semantics and syntax-discourse interfaces, respectively, in Mandarin Chinese. The target linguistic phenomena involved three nominal expressions (i.e., the bare N(oun), the [Cl(assifier)-N], and the [Num(eral)-Cl-N]) under four interface-regulated referential readings (i.e., type-denoting, quantity-denoting, indefinite individual-denoting, and definite individual-denoting). In terms of accuracy, the results showed that (i) for the N and the [Num-Cl-N], regardless of the interface type, the advanced group acquired the target phenomena to a nativelike level, who significantly outperformed the intermediate group; (ii) for the [Cl-N], the advanced group exhibited nativelike attainment at the syntax-discourse interface but not at the syntax-semantics interface, and performed significantly better than the intermediate group at both interfaces. Regarding reaction time, no significant differences were reported between the advanced group and the native group for the target structures at either the syntax-semantics or the syntax-discourse interface, while the advanced group performed significantly better than the intermediate group, regardless of the interface type and the structure type. The findings suggest that the nature of the language interface, i.e., whether it pertains to language-external domains (i.e., the external interface) or not (i.e., the internal interface), should not be a reliable factor for predicting the (im)possibility of nativelike attainment of bilingual grammar knowledge, contra the predictions of the Interface Hypothesis. The present study provides new empirical evidence to show that language-external interface properties are not necessarily destined for prolonged difficulties in heritage language acquisition, and that it is possible for adult heritage speakers to make developmental progress in both accuracy and processing efficiency at different types of interfaces. Research on delinquency reduction often highlights the importance of identifying and sanctioning antisocial and illegal activities so as to reduce the likelihood of future offending. The rise of digital technology complicates the process of detecting cybercrimes and technology enabled offenses, as individuals can use devices from anywhere to engage in various harmful activities that may appear benign to an observer. Despite the growth of cybercrime research, limited studies have examined the extent to which technology enabled offenses are detected, or the (...) behavioral and attitudinal factors associated with being unobserved or caught for one’s actions. The current study addresses this gap in the literature by estimating a multinomial regression model for self-reported computer hacking behavior and the likelihood of those actions being detected in a large international sample of juveniles (N = 51,059). The findings demonstrate significant differences between youth who hack without detection compared to those who are caught. The implications of this analysis for our understanding of cybercrime and its relationship to traditional delinquency are explored in depth. (shrink)

Do liberals’ and conservatives’ brain processes differ in moral reasoning? This research explains these groups’ dissimilar moral stances when they face ethical transgressions in business. Research that explores the effects of ideological asymmetry on moral reasoning processes through moral foundations has been limited. We hypothesize two different moral reasoning processes and test them in the South Korean culture. Study 1 uses the neuroscientific method of event-related potentials to explore the dissociable neural mechanisms that underlie Korean liberals’ and conservatives’ moral reasoning (...) processes in business ethical transgressions. Liberals’ early frontal negative-going brain waves showed that they are quick to pass negative judgment by intuitively detecting violations of fairness, while conservatives’ temporoparietal positive-going brain waves showed that they have a higher motivation to respect authority. Both liberals’ and conservatives’ ERP components occur within the first second of the decision-making phase, suggesting the rapid and intuitive nature of moral reasoning processes. Study 2 tests a mediating process and confirms that Korean liberals exhibit the moral engagement strategy, through the fairness foundation. These findings from our interdisciplinary research deepen the knowledge of the complexity of human morality in business ethics research. (shrink)

From personalised medicine to big data and social media, these platforms facilitate new kinds of interactions, challenge longstanding understandings of privacy and consent, and raise fundamental questions about how the translational patient pathway should be organised.This editorial introduces the cross-journal article collection "Translation in healthcare: ethical, legal, and social implications", briefly outlining the genesis of the (...) collection in the 2015 Translation in healthcare conference in Oxford, UK and providing an introduction to the contemporary ethical challenges of translational research in biology and medicine accompanied by a summary of the papers included in this collection. Um es gleich zu sagen: Freiheit ist mit dem Politischen identisch und gleichursprünglich. „Der Sinn von Politik ist Freiheit.” Das heißt, die genuin menschliche Freiheit in dieser Welt ist nur politisch vollziehbar, während die Freiheit der einzige Existenz- und Rechtfertigungsgrund der Politik ist. Hannah Arendts Überlegungen über das Kriterium politischer Freiheit konzentrieren sich vor allem auf drei Kategorien: „Identität”, „Pluralität” und „Spontaneität”.

Toleration is one of many responses toward diversity and difference. With the growing diversity, the theme of toleration has often taken center stage in discussions of multiculturalism and social pluralism. Nonetheless, it has not received much attention in the social work profession. Social workers often encounter situations in which they face a choice between tolerating and not tolerating. We argue that toleration is a legitimate and relevant topic in social work discourse. To make this point, first, this paper discusses different (...) conceptions of toleration. Then, it demonstrates its relevance to social work and explores a potential benefit of including the idea of toleration in social work discourse. Social work code of ethics implicitly supports toleration, or at least respect-toleration and esteem-toleration. Incorporating toleration in social work discourse may help social workers to better cope with or reduce ethical stress and disjuncture. (shrink)
