Medicine has traditionally been regarded as a rewarding career both financially and socially. How true, however, is that tradition in today's world of rising costs and decreasing revenues? The educational debt of the physician-in-training is steadily increasing, and currently does not affect specialty choice. As the cost of medical education continues to rise, the applicant pool begins to shrink, thereby possibly affecting the quality of future physicians. Once the physician has completed training however, the majority enjoy a positive return on (...) investment. Their incomes generally fail to remain ahead of inflation, and therefore, have remained within a narrow band of $40,000 in 1970 dollars. Finally, the demand for physician services cannot be attributed solely to either the consumer (patient) or to the supplier (physician). Rather, the demand for medical services appears to be a unique combination of the two. In conclusion, medicine still is an attractive career path, but the choices and consequences are becoming much more demanding. (shrink)

The past few decades have seen an explosion of research on causal reasoning in philosophy, computer science, and statistics, as well as descriptive work in psychology. In Causation with a Human Face, James Woodward integrates these lines of research and argues for an understanding of how each can inform the other: normative ideas can suggest interesting experiments, while descriptive results can suggest important normative concepts. Woodward's overall framework builds on the interventionist treatment of causation that he developed in (...) Making Things Happen. Normative ideas discussed include proposals about the role of invariant or stable relationships in successful causal reasoning and the notion of proportionality. He argues that these normative ideas are reflected in the causal judgments that people actually make as a descriptive matter. Woodward also discusses the common philosophical practice-particularly salient in philosophical accounts of causation--of appealing to intuitions or judgments about cases in support of philosophical theses. He explores how, properly understood, such appeals are not different in principle from appeals to results from empirical research, and demonstrates how they may serve as a useful source of information about causal cognition. This paper explores some issues concerning how we should think about interventions (in the sense of unconfounded manipulations) of "upper-level" variables in contexts in which these supervene on but are not identical with lower-level realizers. It is argued that we should reject the demand that interventions on upper-level variables must leave their lower-level realizers unchanged– a requirement that within an interventionist framework would imply that upper-level variables are causally inert. Instead an intervention on an upper-level variable at the same time (...) changes its lower-level realizer in a way that is consistent with the change in the upper- level variable. The lower-level realizer should not be regarded as a potential "confounder" of upper-level causal relations, as in "exclusionist" arguments. Several proposals for making this precise are considered and several pro-exclusionist arguments are criticized. (shrink)

Two versions of global supervenience have recently been distinguished from each other. I introduce a third version, which is more likely what people had in mind all along. However, I argue that one of the three versions is equivalent to strong supervenience in every sense that matters, and that neither of the other two versions counts as a genuine determination relation. I conclude that global supervenience has little metaphysically distinctive value.

We argue that Koch’s postulates are best understood within an interventionist account of causation, in the sense described in Woodward. We show how this treatment helps to resolve interpretive puzzles associated with Koch’s work and how it clarifies the different roles the postulates play in providing useful, yet not universal criteria for disease causation. Our paper is an effort at rational reconstruction; we attempt to show how Koch’s postulates and reasoning make sense and are normatively justified within an interventionist (...) framework and more difficult to understand within alternative frameworks for thinking about causation. To date, research into socially responsible investment (SRI), and in particular the socially responsible investment funds industry, has focused on whether investing in SRI assets has any differential impact on investor returns. Prior findings generally suggest that, on a risk-adjusted basis, there is no difference in performance between SRI and conventional funds. This result has led to questions about whether SRI funds are really any different from conventional funds. This paper examines whether the portfolio allocation across industry sectors and the (...) stock-picking ability of SRI managers are different when compared to conventional fund managers. The study finds that SRI funds exhibit different industry betas consistent with different portfolio positions, but that these differences vary from year to year. It is also found that there is little difference in stock-picking ability between the two groups of fund managers. (shrink)

Interventionist Theories of Causation in Psychological Perspective.

This paper critically examines Deleuze’s treatment of the Nietzschean problem of nihilism. Of all the major figures in contemporary continental thought, Deleuze is at once one of the most luminous, and practically a lone voice in suggesting that nihilism may successfully be overcome. Whether or not he is correct on this point is thus a commanding question in relation to our understanding of the issue. Many commentators on Nietzsche have argued that his project of overcoming nihilism is destined to failure (...) because of the affinity between the problem of nihilism and the logic of negation. While Nietzsche wants an absolute affirmation of life, Spinoza’s principle that “all determination is negation,” as well as Hegel’s dialectical conception of negation, suggest that affirmation free of negation is not possible. However, some commentators indicate that Deleuze successfully shows how overcoming nihilism is possible because his “logic of difference” allows for an affirmation which is not dialectically reappropriated by negation. This paper argues that beyond such logical considerations, there are metaphysical and existential reasons why Deleuze’s interpretation of nihilism fails to show that it can be overcome. For Deleuze, the overcoming of nihilism hinges not just on a logic of difference, but on a radical interpretation of Nietzsche’s doctrine of eternal return as “selective being.” Drawing on recent scholarship and on Nietzsche’s own writings I argue that this is not a tenable interpretation, and also, more importantly, that the metaphysical and existential implications of this understanding of eternal return reinstate nihilism at the very point where it is supposedly overcome. Moreover, I argue that there are attendant ethical and political dangers to Deleuze’s position on nihilism. (shrink)

This article explores the theme of nihilism from the perspective of post-continental philosophy by focusing on semiotics and information theory and the question of “meaning” at stake between them. Nihilism is characterised here as an avatar of the counter-Enlightenment tradition. Post-continental philosophy is defined by a positive revaluation of reason, science, and technology, which were critiqued for their nihilistic effects by key continental philosophers. Rather than critiquing nihilism, then, post-continental philosophers have tended to affirm it. This article argues that, despite (...) appearances, such developments in fact allow a deepened response to nihilism, considered as an existential problem. It does so by using Lyotard’s critique of semiotics to show how the kind of linguistic and cultural meaning associated with continental philosophy is itself a kind of nihilism. It then examines Meillassoux’s theory of the meaningless sign and Laruelle’s idea of the secret truth of Hermes to argue that this new paradigm of post-continental philosophy allows a response to nihilism by offering an alternative to semiotic meaning. Thus freed, this new paradigm allows the embrace of information theory as a plural articulation of meanings grounded in meaningless data, which enables a superior response to nihilism in the information age. This paper explores some general questions about the sorts of abilities that are involved in tool use and “causal cognition”, both in humans and in non-human primates. An attempt is made to relate the empirical literature on these topics to various philosophical theories of causation.

This innovative critical study reinterprets Ludwig Wittgenstein's philosophy for the study of modernist and contemporary literature and brings Wittgenstein into literary conversations around problems of difficulty, ethical instruction, and the yearning for transformation. Central to Karen Zumhagen-Yekple͹'s book are her critical readings of key modernist texts by Franz Kafka, Virginia Woolf, and James Joyce. Throughout, Zumhagen-Yekplé brings to bear an interpretive framework that she derives from Wittgenstein's gnomic "Tractatus Logico-Philosophicus" (first published in English in 1922, the "annus mirabilis" of (...) modernism), which she treats not as a theory of logic or metaphysics but as a complex mock-theoretical puzzle. The book's final chapter turns to recent fiction by J. M. Coetzee, a living author conscious of his debts both to Wittgenstein and his modernist literary precursors. This book will interest students of literary modernism, Wittgenstein, and the interconnections between fiction and ordinary language philosophy. (shrink)

Recent theoretical and philosophical movements within the study of material culture are more carefully attending to the variety of ways in which human artefacts, institutions, and cultural developments extend, shape and alter human cognition over time. Material Engagement Theory in particular has set out to map, explore and understand the relational nature of mind and material world as can be read through cultural artefacts. Within the context of MET, the neurological concept of metaplasticity has been expanded to include the affective (...) domains of technology, materials, and things in the neurological development and architecture of the plastic human mind; a ‘transactional’ relationship between a plastic mind and a plastic material world that are correlated at the ontological level. The challenges of mapping this metaplasticity of mind lie in understanding how the mind and material culture should be understood in relation to the constantly changing lifeworlds of humans over time; the ecological, social, technological and environmental contexts that form the historical specificity of cognitive development. This paper explores how the historical specificity of metaplasticity can be made tangible through the study of material culture, focusing upon the particular activity of oil painting. It will be argued that paintings can provide clues to the historical specificity of the mind that crosses the lifeworld of human action; the technological, phenomenological, philosophical, material, and social conditions underpinning the creation of a painted mark. Drawing from a range of sources that have a root within a Deleuzian process philosophy, the paper builds an account of painting that can be read as expressing the encultured and historical manipulation of paint as an expressive material in itself; an action rendered visible that express the historical emergence of mind. Roughly 20 percent of organic cropland wasdevoted to produce compared to only 3 percent forconventional agriculture in 1995. At the otherextreme, only 6 percent of organic cropland was incorn production while 25 percent of all croplandproduced corn. Only 30 percent of all organicfarmland was in pasture and rangeland compared to 66percent of all farmland. Clearly, these differencesreflect the greater importance of meat and dairyproduction in agriculture overall than in the organicsubsector. In recent years, the organic industry hasgrown not only in (...) volume but in products offered,moving well beyond fresh produce into dairy, snacks,and frozen foods. The estimated growth in retail salesof organic foods averaged over 20 percent a year forthe last eight years compared to only two percent inthe food industry overall, reaching an estimated $4.5billion in 1998. The mix of commodities produced atthe farm level have and will continue to change inresponse to several dynamic forces, including consumerdemand, regulation, and consolidations, mergers, andmainstream entrants at the farm, manufacturing, andretail levels. Consumer demand for organic foods hasbeen spurred by a number of factors including concernsover pesticide residues on foods, food produced usinggenetically modified organisms (GMOs), and hormones indairy and meat products. These concerns coupled withthe overall increase in demand for convenience foodsexplain the phenomenal recent growth of over 70 percent in sales of organic snacks, candy, and frozenfoods. The result has been increased demand forGMO-free grains for snacks, cereals, soy products, andfeed for dairy cows. The recent allowance by USDA forthe labeling of meat and poultry as organic willaccelerate already heightened demand for organic grainand feed from the organic dairy industry. Although thefastest growth in organic foods has been in categoriesother than produce, clearly produce plays an importantrole in the consumers decision on where to shop, anddemand should increase with the overall increase inthe number of organic consumers. Market outlets nowinclude sophisticated natural food store chains,gourmet specialty stores, and e-commerce, allowing fora broadened array of products. Mainstream agriculturalproducers, manufacturers, and supermarket chains haveentered the organic marketplace at a time of lowprices for commodities at the farm gate, fiercecompetition for retail shelf space, and increasinglysophisticated targeting of consumers. Inevitably,these changes in food products offered and in retailoutlets will lead to an organic agriculture thatincreasingly resembles the conventional foodindustry. (shrink)

Nihilism in Postmodernity is an exploration of the nature of the problem of meaninglessness in the contemporary world through the philosophical traditions of nihilism and postmodernism. The author traces the advent of modern nihilism in the works of Nietzsche, Sartre, and Heidegger, before detailing the postmodern transformation of nihilism in the works of three major postmodern thinkers: Lyotard, Baudrillard, and Vattimo. He presents a qualified defense of their positions, arguing that while there is much under-appreciated value in their responses to (...) nihilism, they fail to address adequately the problem of contingency in contemporary life. Drawing on the critical encounters with nihilism in both existentialist and postmodern traditions, the author concludes by staking out future directions for combating meaninglessness. (shrink)

This brief position paper is stimulated from the continued need to define and redefine the area of transpersonal psychology. Understandably, being able to articulate what ‘transpersonal psychology’ is enables discussions within the wider academic and public community, yet all existing definitions are complex, conveying a number of inherent meanings in their definition, which in turn, can cloud others’ perceptions on the area.

Background Moral distress is reported to be a critical force contributing to intensifying rates of anxiety, depression and burnout experienced by healthcare workers. In this paper, we examine the moral dilemmas and ensuing distress personally and collectively experienced by healthcare workers while caring for patients during the pandemic. Methods Data are drawn from free-text responses from a cross-sectional national online survey of Australian healthcare workers about the patient care challenges they faced. Results Three themes were derived from qualitative content analysis (...) that illuminated the ways in which moral dilemmas and distress were relationally experienced by healthcare workers: (1) the moral ambiguity of how to care well for patients amid a rapidly changing work environment; (2) the distress of witnessing suffering shared _between_ healthcare workers and patients; and (3) the distress of performing new forms of invisible work in the absence of institutional recognition. These findings reveal that moral distress was a strongly shared experience. Conclusions Findings advance understandings of moral distress as a relational experience, collectively felt, constituted, and experienced by healthcare workers. Considering how to harness collective solidarity in effectively responding to moral distress experienced across the frontline healthcare workforce is critical. (shrink)

We say a countable model A has a d-basis if the types realized in A are all computable and the Turing degree d can list $\Delta _{0}^{0}$ -indices for all types realized in A. We say A has a d-decidable copy if there exists a model B ≅ A such that the elementary (...) diagram of B is d-computable. Goncharov, Millar, and Peretyat'kin independently showed there exists a homogeneous A with a 0-basis but no decidable copy. We prove that any homogeneous A with a 0'-basis has a low decidable copy. This implies Csima's analogous result for prime models. In the case where all types of the theory T are computable and A is a homogeneous model with a 0-basis, we show A has copies decidable in every nonzero degree. A degree d is 0-homogeneous bounding if any automorphically nontrivial homogeneous A with a 0-basis has a d-decidable copy. Stephen Yablo (2001) argues that traditional fictionalist strategies run into trouble due to a mismatch between the modal status of a claim like ‘2 + 3 = 5’ and the modal status of its fictionalist paraphrase. I argue here that Yablo is best seen as confronting the fictionalist with a dilemma, and then go on to show how this dilemma can be resolved.

This article pursues two central goals. First, I seek to advance the sustained study of occupational identity as a pivotal mechanism for organizing work and, thus, as a productive means of integrating the aims of two scholarly movements: 1) the ‘dislocation’ of organization and 2) the renewed emphasis on work in organization studies. Specifically, I propose the study of evolving relations between occupational image discourse and role communication, and my analysis of US commercial airline pilots enacts the potential of such (...) research. The analysis demonstrates how contemporary pilots draw on the deliberate historical construction of airline pilots as elite, fatherly professionals to make sense of their work, explicitly invoking gender discourse as a rational and emotional warrant for their labor and, in so doing, implicitly articulating sexuality, race, and class discourses. Accordingly, the second major goal of the article is to promote inquiry into how discourses of difference function to organize occupational identity and, thus, to produce the division and hierarchy of labor. With this move, I seek to bring a more constitutive view of discourse and communication, as well as a focus on intersection and interplay among discourses of difference, to the study of occupational identity and segregation. This paper responds to criticisms of my account of the role of proportionality and stability in causal reasoning. It reformulates the notion of proportionality in response to objections.

ObjectivesThis pilot study sought to develop and evaluate a novel online group-based intervention to help breast cancer survivors make healthy lifestyle changes intended to yield not only beneficial physical outcomes but also greater behavioral, and psychosocial well-being.MethodsAn exploratory single-arm, mixed-method triangulation design was employed to evaluate the feasibility and preliminary effectiveness of the HLM-ABC intervention for overweight BCSs. Fourteen women participated in the 10-week intervention and completed quantitative measures of the above-mentioned outcomes at baseline, post-treatment, 6-month, and 12-month follow-up time (...) points. Qualitative data were obtained post-treatment via semi-structured interviews and a treatment satisfaction questionnaire.ResultsParticipants lost an average of 2.83% of their baseline weight by 1-year follow-up, equal to a small effect size. Despite achieving only modest weight loss, participants achieved meaningful gains in the form of increased physical activity, discovery of gratifying movement, more intuitive eating habits, greater bodily and emotional awareness, and positive shifts in beliefs about being able to make healthy choices regarding food and physical activity. Furthermore, they demonstrated a slight improvement in body image and described feeling more self-compassionate, empowered, and acknowledging of variables beyond control that can present barriers to change.ConclusionAfter completing a 10-week online program, participants achieved meaningful and lasting changes on a number of healthful indicators, even when this did not correspond with a significant reduction in weight. Findings highlight the complex, multifaceted nature of “health” and lend support for promotion of healthier lifestyle following cancer treatment that encompasses not only physical weight, but also behavior, psychosocial well-being, and circumstances such as life-preserving hormonal treatments. Emerging systems using artificial intelligence (AI) including the complexities of deep learning leading to decision-making outcomes pose challenge, risk alongside opportunities to revolutionize business sectors and thus, human life. Building AI that impact on critical decision-making must be entwined with ethical questioning from the initial conception of design. As academics educating future technologists, we must lead on embedding the importance of ethical thinking for equitable designed systems. Currently, it is unclear across UK Higher Education how widely ethics is taught across (...) programmes of study within computer science disciplines, their importance within the curriculum plus the confidence of academics for its delivery. This paper carries out a survey analysis to identify gaps, measures of engagement, expertise and knowledgebase of topics through both quantitative and qualitative questioning. The survey was distributed to 15 member universities of the Council for Professors and Heads of Computing (CPHC) randomly selected across the UK for which 32 academics engaged in the study. Results show ethics was noted as being important although one-third reported it was not widely embedded across the curriculum. Just less than half of participants schedule the teaching of ethics regularly either across one or several modules. Interestingly, 91% of respondents stated that they have not received any instruction or training and that 62% felt that they require more training on the subject matter. This paper is inspired by wider research from European studies however, this study provides a specific novel focus on a broad range of UK universities, providing comparative analysis and unique findings from the UK perspective. The significance of these findings suggest academics perspectives vary with specific training needs. This demonstrates that this early-stage research requires further exploration of the wider computer science curriculum, sharing of good practice and collaborative working. (shrink)

Over several years, genetic studies in the model system, Drosophila melanogastor, have uncovered genes that when mutated, lead to a block in myoblast fusion. Analyses of these gene products have suggested that Arp2/3‐mediated regulation of the actin cytoskeleton is crucial to myoblast fusion in the fly. Recent advances in imaging in Drosophila embryos, both in fixed and live preparations, have led to a new appreciation of both the three‐dimensional organization of the somatic mesoderm and the cell biology underlying myoblast fusion. This article concerns the emergence of psychological constructs of personal power and control in the United States during the 1960s and 1970s and the ways in which they contributed to contemporary political explanations of social unrest. While social scientists and politicians at the time saw this unrest as a social problem that posed threats to social cohesion and stability, they located its cause not in the power structure of society but in the individual’s sense of his or her own powerlessness. (...) The article discusses ‘locus of control’ as the central construct in new psychological explanations of powerlessness which drew on personality theory and behavioural psychology. The first half of the article traces the rise of the self-managing subject in behavioural psychology, identifying a key shift in conceptual, strategic and technical emphases, away from using behavioural approaches to modify the behaviour of others and towards developing ways of enabling people to manage their own behaviour. In the second half it examines the ways in which locus of control and related constructs were used to account for the educational under-achievement and political militancy of poor, black people in the United States. These explanations implicated individual helplessness and a sense of powerlessness in black people as a major social problem in the USA during this period: as a threat not only to personal development but, in particular, to social stability. In the process of this analysis I aim to demonstrate that the deployment of these constructs did more than reformulate old social problems in new ways; it enabled new social problems to be identified for which these constructs could offer explanations and solutions which both appealed to political authorities and helped to shape their conceptions of the ‘problem’. For Tomasello's proposed ontology of the human sense of moral obligation, observations of early moral language may provide useful evidence complementary to that afforded by experimental research. Extant reports of children's everyday moral talk reveal patterns of participation and content that accord with the proposal and hint at extensions addressing individual differences.

Nursing staff of many specialities are taking on and developing their roles in new and advanced practice areas. Patients will be offered new services from highly skilled advanced nurse practitioners. Such nurses need guidance, direction and information to assist them in their new roles. This book will offer insight and guidance on a variety of issues that are likely to be encountered by the Nurse Practitioner in everyday practice.

This paper argues that spatial contingencies, defined by the relationship between where historical actors are in the narrative and what they say, are crucial for understanding the political and ideological effects of Thucydides' History. A comprehensive approach to these contingencies is linked to two related premises. First, that the city of Athens is the principal spatial referent in the History and, second, that Athens refers both to a set of “real” topographical features and to a transcendent and trans-historical ideal that (...) exceeds those features. The dynamic between these two is mediated by Athens' inevitable defeat and by the related conflict between the History as the presentation of facts about the past, on the one hand, and as the source of future predictions, on the other. Framing this analysis are the distinctive characteristics ascribed to the Athenians as a collective and to the positions of Thucydides and Alcibiades as Athenians in exile. (shrink)
