BackgroundThe BelRAI Screener is a short-form assessment consolidating internationally validated interRAI items focusing on physical and psychological aspects of functioning and problems with activities of daily living. It was fully implemented in the Flemish home care setting as of June 2021. In a biopsychosocial model for developing a personalized and effective care plan social and contextual aspects are considered equally important to biomedical ones. Thus, a social supplement to the BelRAI Screener was collaboratively developed with stakeholders and tested to gather (...) additional information on the social context of community-dwelling adults with care needs.Objective and methodsTo examine the interrater reliability of the BelRAI Social Supplement in Flanders, Belgium, an observational study was conducted using a convenience sample. The method of simultaneous rating was used due to strict COVID-19 guidelines at the time and to minimize assessment burden. Fifty two community-dwelling adults requesting home care support were simultaneously assessed by two independent assessors during home visits. Interrater reliability was tested on all 80 items of the BelRAI Social Supplement using observed agreement, kappa coefficients, and intraclass correlation coefficients.ResultsThe kappa mean and median values for nominal items, show substantial agreement, while the kappa mean and median values for ordinal items were 0.81 and 0.90, which correspond to almost perfect agreement. Following the traditional cut-off points for the interpretation of the kappa statistic, reliability was almost perfect for 49% of all items, substantial for 33%, moderate for 8%, and poor for 10%. The majority of items with poor kappa value, showed a high observed agreement, reflecting homogeneity of the sample rather than poor agreement.ConclusionThe strength of kappa agreement for the items in this version of the BelRAI Social Supplement is generally substantial to almost perfect, with high proportions of observed agreement. COVID-19 restrictions had a large impact on the planning and execution of the home visits. A final optimization of the instrument and accompanying manual according to the findings will result in an improved version ready for nation-wide implementation. (shrink)

This article examines the concept of the so-called dialogical self. This theory is based on the pragmatism of George Herbert Mead and the work of Mikhail Bakhtin on dialogicality. This article explains that the dialogical theory view the other not as external to self, but as part of the self and constitutive of it. It also introduces the notion of positioning as a further articulation of the dialogical self situated in time and space. Many individuals with developmental language disorder (DLD) and individuals who are deaf or hard of hearing (D/HH) have social–emotional problems, such as social difficulties, and show signs of aggression, depression, and anxiety. These problems can be partly associated with their executive functions (EFs) and theory of mind (ToM). The difficulties of both groups in EF and ToM may in turn be related to self-directed speech (i.e., overt or covert speech that is directed at the self). Self-directed speech is thought to (...) allow for the construction of non-sensory representations (i.e., representations that do not coincide with direct observation). Such non-sensory representations allow individuals to overcome the limits set upon them by the senses. This ability is constrained by the development of word meaning structure (i.e., the way words are understood). We argue that the greater ability to construct non-sensory representations may result in more enhanced forms of EF and ToM. We conclude that difficulties in EF, ToM, and social–emotional functioning in those with hearing and language problems may be accounted for in terms of word meaning impairments. We propose that word meaning structure and self-directed speech should be considered in assigning EF and ToM treatments to individuals with DLD and those who are D/HH. (shrink)

We discuss findings on evaluative conditioning (EC) that are problematic for the account of learning, namely, dissociations between conscious beliefs and acquired (dis)liking. We next argue that, both for EC and for Pavlovian learning in general, conditioned responding cannot rationally be inferred from propositional knowledge type and that, therefore, performance cannot be explained.

The effect of specificity of autobiographical memory (AM) retrieval on the affective impact of an emotional event was examined. In Study 1 (N = 90) the impact of a negative and positive experience was compared between student participants who habitually retrieve autobiographical memories (AMs) in a specific way and participants who generally retrieve less specific memories. In Study 2 (N = 48) the effect of an experimentally induced (specific vs. overgeneral) retrieval style on the impact of a negative experience was (...) studied in student participants who habitually retrieve less specific memories. Study 1 replicated the finding of Raes, Hermans, de Decker, Eelen, & Williams (Citation2003) that a negative event leads to less subjective distress in low‐specific participants as compared with high‐specific participants. However, both groups did not differ in their affective reaction to a positive event. Important, reduced memory specificity was associated with “repressive coping”, providing further evidence for the idea that reduced memory specificity is used as an avoidant or repressive‐defensive mechanism to regulate negative affect (CitationWilliams, 1996). In Study 2, participants who were induced to retrieve memories in an overgeneral way experienced more distress following a negative event as compared with participants who were induced to retrieve memories in a specific way. Results are discussed in the context of recent findings concerning AM specificity and emotion regulation (CitationPhilippot, Schaefer, & Herbette, 2003). In this paper we introduce the ‘New Substitution Problem’ which, on its face, presents a problem for adaptation proposals that are justified by appeal to obligations of reparation. In contrast to the standard view, which is that obligations of reparation require that one restore lost value, we propose instead that obligations to aid and assist species and ecosystems in adaptation, in particular, follow from a failure to adequately justify – either by absence, neglect, omission or malice – actions that caused, (...) or coalesced to cause, climatic change. Because this position suggests a different reason for reparation – namely, it does not rely on the notion that an obligation to repair is contingent upon a lost good – it permits moving forward with assisted colonisation and migration, but does so without falling subject to the complications of the New Substitution Problem. Should we restore degraded nature, and if so, why? Environmental theorists often approach the problem of restoration from perspectives couched in much broader debates, particularly regarding the intrinsic value and moral status of natural entities. Unfortunately, such approaches are susceptible to concerns such as the baseline problem, which is both a philosophical and technical issue related to identifying an appropriate restoration baseline. Insofar as restoration ostensibly aims to return an ecosystem to a particular baseline state, and depends upon clearly identifying (...) this baseline for success, the very project of restoration appears impossible to get off the ground. Recasting environmental restoration in terms of obligations, instead of status, value, or worth, can avoid this and other classic challenges. If obligations to restore nature follow from intersubjectively validated reasons to justify our actions, we can salvage restoration from the threat of the baseline problem. (shrink)

Public trust in business, defined as the degree to which the public—meaning society at large—trusts business in general, is largely understudied. This article suggests four domains of existing trust research from which scholars of public trust in business can draw. The authors then propose four main hypotheses, which aim to predict the determinants of public trust, and test these hypotheses using a factorial vignette methodology. These results will provide scholars with more direction as this article is, to the authors’ knowledge, (...) one of the first empirical studies of public trust. In this article we will address the issue of obtaining insight into the way in which teachers deal with the normative side of their profession. We outline the problem that forms the context of our question (the difference in the meaning of good teaching in the process–product model and in ethical models) and we discuss Oser's discourse approach as a solution for that problem. His discourse approach appears to be a step forward, but at the same time leaves questions unanswered. (...) As a contribution we introduce the concepts of pragmatic, ethical and moral. We show in what sense this conceptual framework provides a refinement of the discourse approach. (shrink)

Journalists differ in the degree to which they have adopted the Internet professionally. While earlier studies were predominantly descriptive, this study explains why journalists differ in the amount and nature of their use of the Internet. Based on a random sample of members of the Dutch Association of Journalists, results indicate that the digital divide in terms of demographic characteristics is absent. The perceived functionality of the Internet as a professional tool is the most important explanatory factor for the use (...) of the Internet. Surprisingly, professional attitudes towards journalistic quality are not reflected in journalists' use of the Internet. (shrink)

The Malabo Declaration places the transformation of agriculture and food systems at the centre of regional and national policy priorities across Africa. Transformative change in the way that food is produced, processed and consumed is seen as not only necessary for addressing the complex challenges of food security and poverty alleviation, but also as a driver of new employment opportunities and economic development. As pointed out within the recent UN Food Systems Summit, essential elements of food system transformations include digital (...) transitions and the empowerment of women and youth. However, there are few empirical examples demonstrating how these agendas come together to affect food system change. Here we focus on an enterprising group of young farmers referred to as Malawi’s _new achikumbe elite_, who are urban based, educated and engaging in agriculture on a commercial basis. The aim is to characterise this emergent group of agriculturalists and to understand the role that they have within the transformation of Malawi-s agricultural sector. We explore how digital platforms are supporting the emergence of this new category of farmer and positioning young people as agents of change in food systems transformation. Based on interviews and ethnographic research with 32 young farmers between 2018 and 2022 combined with interviews with representatives of service providers and agricultural organisations, we argue that this group is characterised by a higher level of education, self-dependency and use of digital platforms, enabling them to adapt their context to sourcing production resources and engaging in commercial agriculture. We present evidence that digital platforms are supporting the new achikumbe elite (NAE) to engage flexibly with new commercial markets, contracts and access a wider range of training and advice. However, while digital platforms can offer more equitable access to information and market opportunities, they also represent potential avenues for food system transformations that are inequitable. Supervised injectable opioid assisted treament prescribes injectable opioids to individuals for whom other forms of addiction treatment have been ineffective. In this article, we examine arguments that opioid-dependent people should be assumed incompetent to voluntarily consent to clinical research on siOAT unless proven otherwise. We agree that concerns about competence and voluntary consent deserve careful attention in this context. But we oppose framing the issue solely as a matter of the competence of opioid-dependent people and emphasize that it should be (...) considered in the context of inequities in access to siOAT as a medical treatment. Consequently, we suggest that bioethics literature on nonexploitation, which focuses on clinical research in low-income countries, is helpful due to locating ethical issues within systemic social conditions. Finally, we consider the implications of our argument for the ethics of clinical research on siOAT. (shrink)

Standard views of good death in human and veterinary medicine considerably differ from one another. Whereas the good death ideal in palliative medicine emphasizes the positive aspects of non-induced dying, veterinarians typically promote a quick and painless killing with the aim to end suffering. Recent developments suggest a convergence of both professions and professional attitudes, however. Palliative physicians are confronted with patients wishing to be ‘put to sleep’, while veterinarians have begun to integrate principles and practices from hospice care. We (...) will argue that the discourses on good human and animal deaths are not distinct, but that they interact and influence each other. On the one hand, veterinary medicine adapts techniques like chemotherapy or sedation from palliative end-of-life care. On the other hand, philosophers, veterinarians, pet owners, patients and the general public alike make certain assumptions about the analogy of human and animal dying or killing. Unfortunately, these interactions have only scarcely been reflected normatively, especially on the part of human medicine. Conflicts and misattributions with potential serious negative consequences for the patients’ wellbeing are provoked. For these reasons, palliative physicians and veterinarians are invited to engage in the debate around human and animal end-of-life care. (shrink)

Auditory scene analysis is an elementary aspect of music perception, yet only little research has scrutinized auditory scene analysis under realistic musical conditions with diverse samples of listeners. This study probed the ability of younger normal-hearing listeners and older hearing-aid users in tracking individual musical voices or lines in JS Bach's The Art of the Fugue. Five-second excerpts with homogeneous or heterogenous instrumentation of 2–4 musical voices were presented from spatially separated loudspeakers and preceded by a short cue for signaling (...) the target voice. Listeners tracked the cued voice and detected whether an amplitude modulation was imposed on the cued voice or a distractor voice. Results indicated superior performance of young normal-hearing listeners compared to older hearing-aid users. Performance was generally better in conditions with fewer voices. For young normal-hearing listeners, there was interaction between the number of voices and the instrumentation: performance degraded less drastically with an increase in the number of voices for timbrally heterogeneous mixtures compared to homogeneous mixtures. Older hearing-aid users generally showed smaller effects of the number of voices and instrumentation, but no interaction between the two factors. Moreover, tracking performance of older hearing aid users did not differ when these participants did or did not wear hearing aids. These results shed light on the role of timbral differentiation in musical scene analysis and suggest reduced musical scene analysis abilities of older hearing-impaired listeners in a realistic musical scenario.
