Advance care directives for health care have been promoted as a way to improve end-of-life decision making. These documents allow a patient to state, in advance of incapacity, the types of medical treatments they would like to receive, to name a surrogate to make those decisions, or to do both. Although studies have shown that both physicians and patients generally have positive attitudes about the use of these documents, relatively few individuals have actually completed one.What underlies this discrepancy between attitudes (...) and behavior with regard to advance care directives? One obvious explanation is lack of access. Emanuel et al. estimated that approximately 90 percent of the population desire an advance care directive, and they pointed to access as the major barrier. Yet interventions that increase accessibility have typically failed to yield more than a 20 percent completion rate. Thus, it appears that access is not the sole determinant of advance care directive completion. (shrink)

BackgroundThe time that children and young people spend in nature and outdoor spaces has decreased significantly over the past 30 years. This was exacerbated with a further 60% decline post-COVID-19. Research demonstrating that natural environments have a positive impact on health and wellbeing has led to prescription of nature-based health interventions and green prescribing, although evidence for its use is predominantly limited to adults. Growing evidence also shows the impact of arts on all aspects of health and wellbeing. However, what (...) has received scant attention in literature is the interconnection between the two: arts and nature.AimsThis review synthesizes the literature surrounding the interconnectedness between arts and nature, and their impact on the health and wellbeing of children and young people.MethodsEight major electronic databases were systematically searched, while hand-searching included 20 journals, six books, and contact with experts. The review was conducted using the Cochrane handbook for systematic reviews, PRISMA guidelines and TIDieR template. All stages were conducted independently by two researchers and the protocol was published on PROSPERO.ResultsAlthough 9,314 records were identified, only 11 records were included as most studies focused either on arts or nature, but not both. Studies were conducted in United Kingdom, United States, Ireland, Australia, and Hong Kong, in a range of spaces such as forests, woodlands, beaches, parks, fields, gardens, and school playgrounds. The review encompasses data from 602 participants in total.DiscussionArts-in-nature offered an inclusive medium to engage all children and young people, especially those who might otherwise remain disinterested about environmental issues and disengaged with educational programs. Further, arts-in-nature provided stimuli to increase nature connectivity, understand environmental issues and explore ways to prevent environmental disasters. This led to higher environmental awareness and pro-environmental behaviors, and potential decrease in eco-anxiety.ConclusionAlthough the quality of qualitative studies was high, the quality of quantitative studies was low or unclear, thus quantitative evidence is still at its infancy. Implications for research, policy, and practice are discussed, such as methods and activities to strengthen future interventions. Scaling-up existing interventions may lead to wider recognition and inclusion of arts-in-nature in future health guidelines, including green prescribing. (shrink)

This article explores the development and creative outcomes of the Yijala Yala Project. This project was created in the Pilbara region as a long-term, inter-generational, multi-platform arts project that set out to highlight cultural heritage as living, continually evolving and in the ‘here and now’ rather than something static. The project name Yijala Yala means ‘now’ in the two main regional languages of Yindjibarndi and Ngarluma. Yijala Yala works with members of the local Aboriginal community to create content that reflects (...) cultural heritage in new ways, and is also created using new methods of teaching and skill-building. Yijala Yala has created content in the following media: films, music, recordings, photographs, books, animation and apps. A major artistic outcome of the project is the beautiful operatic, cross-cultural, multi-media performance work Hipbone Sticking Out, which has played in the Centenary of Canberra Festival and the Melbourne Festival, as well as in Roebourne and Perth. Concern about high-profile ethical lapses by business managers has led to an increasing emphasis on ethics instruction in business schools. Various pedagogical methods are used to expose business students to real-world ethical dilemmas, yet students may not readily grasp the linkages between ethical theories and dilemmas to identify possible ethical solutions. Venn diagrams are a valuable instructional tool in business ethics classes when used with other teaching methodologies such as case studies. We describe how the use of Venn diagrams assists (...) students in visualizing the key points of and the connections between ethical theories and dilemmas to shed light on possible ethical solutions. Examples of teaching exercises are provided along with ideas for future research in the use of Venn diagrams in activating moral imagination and improving ethical reasoning. Overall, positive student reactions to the introduction of Venn diagrams in business ethics classrooms support the use of this methodology. (shrink)

Amazon Book Review Series of “Wild Wise Weird”.

Originally published 1867. This volume describes not only the basic tenets of the Sufis but also the _Ahl i wahdat_ which was a branch of Sufism. The author’s use of a Persian manuscript treatise by ‘Aziz bin Mohammed Nafasi’ is an indispensable tool, particularly because the author did not merely translate it but gave a clearer and more succinct account of the system. The volume contains an Appendix containing a glossary of allegorical and technical terms in use among Sufiistic writers.

Background Moral distress was first described by Jameton in 1984, and has been defined as distress experienced by an individual when they are unable to carry out what they believe to be the right course of action because of real or perceived constraints on that action. This complex phenomenon has been studied extensively among healthcare providers, and intensive care professionals in particular report high levels of moral distress. This distress has been associated with provider burnout and associated consequences such as (...) job attrition, with potential impacts on patient and family care. There is a paucity of literature exploring how middle and late career healthcare providers experience and cope with moral distress. Objectives We explore the experience of moral distress and the strategies and resources invoked to mitigate that distress in mid- and late-career healthcare providers practicing in paediatric intensive care, in order to identify ways in which the work environment can build a culture of moral resilience. Research design An exploratory, qualitative quality improvement project utilizing focus group and semi-structured interviews with pediatric intensive care front-line providers. Participants Mid-and-later career (10 + years in practice) pediatric intensive care front line providers in a tertiary pediatric hospital. Research context This work focuses on paediatric intensive care providers in a single critical care unit, in order to explore the site-specific perspectives of health care providers in that context with respect to moral distress coping strategies. Ethical considerations The study was approved by the Quality Management Office at the institution; consent was obtained from participants, and no identifying data was included in this project. Findings Participants endorsed perspective-building and described strategies for positive adaptation including; active, reflective and structured supports. Participants articulated interest in enhanced and accessible formal supports. Discussion Findings in this study resonate with the current literature in healthcare provider moral distress, and exposed ways in which the work environment could support a culture of moral resilience. Avenues are described for the management and mitigation of moral distress in this setting. Conclusion This exploratory work lays the groundwork for interventions that facilitate personal growth and meaning in the midst of moral crises in critical care practice. (shrink)

Statisticians in medicine can disagree on appropriate methodology applicable to the design and analysis of clinical trials. So called Bayesians and frequentists both claim ethical superiority. This paper, by defining and then linking together various dichotomies, argues there is a place for both statistical camps. The choice between them depends on the phase of clinical trial, disease prevalence and severity, but supremely on the ethics underlying the particular trial. There is always a tension present between physicians primarily obligated to their (...) own patients (the weight of 'individual ethics') and ethical committees responsible for the scientific merit of the trial and its long-term implications ('collective ethics'). Individual ethics, it is proposed, favour the Bayesian approach; collective ethics, the frequentist. Though in some situations the choice appears clear-cut, there remain other where both methodologies can be appropriate. (shrink)

Like most other sciences, behavior analysis adopts an assumption of uniformity, namely that principles discovered under controlled conditions apply outside the laboratory as well. Since the boundary between public and private depends on the vantage point of the observer, observability is not an inherent property of behavior. From this perspective, private events are assumed to enter into the same orderly relations as public behavior, and the distinction between public and private events is merely a practical one. Private events play no (...) role in the experimental analysis of behavior, but they permit us to make sense of many commonplace phenomena where controlled observation is impossible but unsystematic data are available. Such interpretive exercises serve both to guide research and to displace occult explanations. (shrink)

Previous research suggests that implicit sequence learning is superior for believers in the paranormal and individuals with increased cerebral dopamine. Thirty-five healthy participants performed feedback-guided anticipations of four arrow directions. A 100-trial random sequence preceded two 100-trial biased sequences in which visual targets on trial t tended to be displaced 90° clockwise or counter-clockwise from those on t − 1. ISL was defined as a positive change during the course of the biased run in the difference between pro-bias and counter-bias (...) responses. It was hypothesized that this difference would be greater for believers in the paranormal than for skeptics, for those who received dopamine than for those who received placebo, and for believers who received dopamine than for the other groups. None of the hypotheses were supported by the data. It is suggested that a simple binary guessing task with a focus on prediction accuracy during early trials should be considered for future explorations. (shrink)

Positivists have always objected to people talking about God. Their objection rests on quite general logical grounds. They have discovered a simple, formal test by which to tell significant remarks from nonsensical collections of words. There must, they say, be some minimal conditions of intelligibility. From these we can construct a Principle of Meaning. Now religiousremarks, on any positivist Principle, are demonstrable nonsense. What are these conditions of intelligibility ? Speaker and hearer, we suppose, must use the same language; must (...) possess a rather similar background of information; must both have some interest or other in communication.… Such matters of ‘more and less’ will vary with the topic, the parties, and the occasion. People used to think that the items referred to by one word, e.g. , should all have certain features in common, which would jointly constitute : and that this should enable us to decide whether any given item is a triangle or not.

The old ritual word macte was only vaguely understood even in Republican times. As is well known, the ancient critics connected the word with magis, magnus, and explained it as magis auctus. A glance at Walde's Wörterbuch reveals that many attempts have been made in modern times to solve the mystery; but the formidable equipment of the modern philologist has yielded little better results than the popular etymology of the ancients, the most favoured view to-day seeing in mactus the PPP (...) of a verb mago with macto as the frequentative which replaces it. The supposed semantic development of this verb, originally meaning ‘to increase’, has been set forth by Warde Fowler in a well-known passage, to which recent authorities have accorded their assent and praise: ‘the vitality of the deity … was really increased by placing on the altar the organs of life of the victim’. But this is a solution which ignores half the problem: macte and mactare are used not only of the deities but also of the victim. One can perhaps understand lovem mactare as ‘to magnify J.’, but not even by the most superficial analogy can we find a transition from here to the meaning typified in mactare vinum. Yet this is one of the most frequent usages of the word. So deep a gulf lies between the two semantic spheres of the verb that some scholars have sought to find in it two different verbs. But this is a counsel of despair which the editor of the Thesaurus dismisses with a non recte. We must agree, then, with Hey that the modern explanation is ‘sicher eine unrichtige’ and with Meillet-Ernout that there exists ‘aucune étymologie claire’. It is, therefore, surprising that so ill-founded a preconception should lead investigators to ignore or even dismiss important evidence which the ancient authorities offer for the solution of this problem. Not only did it divide Jews and Christians, it also caused divisions among Jews and among Christians as well. In the 20th century, philosophers and political leaders attached increasing importance to it in a more secular form. The authors of the articles collected in this volume deal with the subject of messianism in the 20th century in various contexts. The articles are written from the standpoint of different (...) disciplines and provide a multifaceted perspective of a spiritual and intellectual phenomenon that deserves further research. German description: Die Verhaltnisse von religiosen und sakularen Ordnungen stehen neu zur Debatte. Sah es lange so aus, als wurden die sakularen Ordnungen sich nach und nach gegen die religiosen durchsetzen, so hat sich dieses Bild entschieden verkehrt: Die neue Hinwendung zu Religionen ist ein politisch relevantes Thema geworden. Das Phanomen des Messianismus hat in allen Umbruchzeiten eine schillernde Zwischenstellung gehalten. So konnten einerseits die christlichen Theologien von aussen als der Versuch beschrieben werden, einen durch seinen Tod den meisten als afalsch erwiesenen Messias zum richtigen Messias umzudeuten.
