Amit Sharma, Rajesh Ramanathan, Marc Posner, Robert A Fisher Hume-Lee Transplant Center, Virginia Commonwealth University, Richmond, VA, USA: Pediatric kidney transplantation is the preferred treatment for children with end-stage renal disease. The most common indications for transplantation in children are renal developmental anomalies, obstructive uropathy, and focal segmental glomerulosclerosis. Living donor kidney transplants are often performed pre-emptively and offer excellent graft function. Policy changes in deceased-donor kidney allocation have increased the proportion of such transplants in pediatric recipients. Adequate (...) pretransplant workup along with evaluation of urologic abnormalities is imperative in achieving good outcomes. Overall, patient and graft outcomes after kidney transplantation have improved, with five-year deceased donor and living donor graft survivals of 78.8% and 84.3%, respectively. Improvements in induction and maintenance immunosuppression have contributed to the gradual improvement in outcomes. Unique challenges in pediatric recipients include increased graft thrombosis, adverse growth, and abnormal development relating to immunosuppression, increased rejection due to nonadherence, increased susceptibility to opportunistic infections, and post-transplant malignancy. This review focuses on the current practices and outcomes in pediatric kidney transplantation in North America. We discuss the indications for transplantation, the evaluation process, some key surgical and immunologic considerations, and the common risk factors for graft dysfunction. It has often been argued that Humean accounts of natural law cannot account for the role played by laws in scientific explanations. Loewer (Philosophical Studies 2012) has offered a new reply to this argument on behalf of Humean accounts—a reply that distinguishes between grounding (which Loewer portrays as underwriting a kind of metaphysical explanation) and scientific explanation. I will argue that Loewer’s reply fails because it cannot accommodate the relation between metaphysical and scientific explanation. This relation also resolves a puzzle (...) about scientific explanation that Hempel and Oppenheim (Philosophy of Science 15:135–75, 1948) encountered. (shrink)

How should we define ‘health’ and ‘disease’? There are three main positions in the literature. Naturalists desire value-free definitions based on scientific theories. Normativists believe that our uses of ‘health’ and ‘disease’ reflect value judgments. Hybrid theorists offer definitions containing both normativist and naturalist elements. This paper discusses the problems with these views and offers an alternative approach to the debate over ‘health’ and ‘disease’. Instead of trying to find the correct definitions of ‘health’ and ‘disease’ we should explicitly talk (...) about the considerations that are central in medical discussions, namely state descriptions and normative claims . This distinction avoids the problems facing the major approaches to defining ‘health’ and ‘disease’, and it more clearly captures what matters in medical discussions. (shrink)

In this paper I outline the main features of Karen Bennett’s (Australasian Journal of Philosophy 1–21, 2011) non-classical mereology, and identify its methodological costs. I argue that Bennett’s mereology cannot account for the composition of structural universals because it cannot explain the mereological difference between isomeric universals, such as being butane and being isobutane. I consider responses, which come at costs to the view.

Hempel and Giere contend that the existence of provisos poses grave difficulties for any regularity account of physical law. However, Hempel and Giere rely upon a mistaken conception of the way in which statements acquire their content. By correcting this mistake, I remove the problem Hempel and Giere identify but reveal a different problem that provisos pose for a regularity account — indeed, for any account of physical law according to which the state of affairs described by a law-statement presupposes (...) a Humean regularity. These considerations suggest a normative analysis of law-statements. On this view, law-statements are not distinguished from accidental generalizations by the kind of Humean regularities they describe because a law-statement need not describe any Humean regularity. Rather, a law-statement says that in certain contexts, one ought to regard the assertion of a given type of claim, if made with justification, as a proper way to justify a claim of a certain other kind. Homeostatic Property Cluster (HPC) theory suggests that species and other biological taxa consist of organisms that share certain similarities. HPC theory acknowledges the existence of Darwinian variation within biological taxa. The claim is that “homeostatic mechanisms” acting on the members of such taxa nonetheless ensure a significant cluster of similarities. The HPC theorist’s focus on individual similarities is inadequate to account for stable polymorphism within taxa, and fails properly to capture their historical nature. A better approach is to treat distributions (...) of traits in species populations as irreducible facts, explained in terms of selection pressures, genealogy, and other evolutionary factors. We call this view Population Structure Theory (PST). PST accommodates the view, implicit in biological systematics, that species are identified by reference to particular historical populations. Science policy mandates across the industrialized world insinuate more active roles for publics, their earlier participation in policy decisions, and expanded notions of science and technology governance. In response to these policies, engaged scholars in science studies have sought to design and conduct exercises aimed at better attuning science to its public contexts. As demand increases for innovative and potentially democratic forms of public engagement with science and technology, so also do the prospects for insights from science studies to contribute (...) to policy agendas and institutional capabilities. Philosophers who regard some mathematical proofs as explaining why theorems hold, and others as merely proving that they do hold, disagree sharply about the explanatory value of proofs by mathematical induction. I offer an argument that aims to resolve this conflict of intuitions without making any controversial presuppositions about what mathematical explanations would be.

The famous Cartesian Nicolas Malebranche (1638-1715) espoused the occasionalist doctrine that ‘there is only one true cause because there is only one true God; that the nature or power of each thing is nothing but the will of God; that all natural causes are not true causes but only occasional causes’ (LO, 448, original italics). One of Malebranche’s well-known arguments for occasionalism, known as, the ‘no necessary connection’ argument (or, NNC ) stems from the principle that ‘a true cause… is (...) one such that the mind perceives a necessary connection between it and its effect’ (LO, 450). The outline of this paper is as follows. I explicitly layout NNC and articulate some of its prima facie strengths (§1). I then critically discuss, what I take to be, the two main arguments against NNC of the Lee-Pyle interpretation (§2). The main conclusion from (§2) is that Malebranche did not abandon NNC in his later works given textual evidence from the Dialogues, contrary to the Lee-Pyle interpretation. In (§3) I discuss in what ways Suárez, Leibniz, Régis and Spinoza all accepted the main premise of NNC. Then, I rebut Steven Nadler’s influential and unchallenged criticism that Malebranche conflated causal and logical necessity, and provide a more accurate interpretation of Malebranche that only commits him to a partial reduction of causal to logical necessity (§4). Bernard Williams briefly discusses agent regret in his broader account of moral luck. The present paper first outlines one way to develop Williams’s notion with reference to the unintended harm; it then suggests that agent regret can be counteracted by externalizing the action that caused unintended harm, in Harry Frankfurt’s sense of externalization; and then the present paper argues that apology is a mechanism by which a person can externalize an offending action/effect—in that way counteracting agent regret. This function for (...) apology—self-repair—is different from others described in the literature, which address the role of apology in repairing relationships. The present paper describes a nonfictional example, that of a veteran of the U.S. war in Iraq who contacts a family gravely injured by him and his combat unit; the veteran is motivated to contact the family to apologize. The example serves as a prototypical case of agent regret. The example is taken from a recent literature on the clinical psychology of self-inflicted “moral injury”; categorizing this example of moral injury as agent regret also helps broaden our understanding of moral injury as a philosophical category and as a psychological phenomenon. considers what I call free-floating chances—objective chances that obtain at a given time despite the fact that their values are not determined by the laws of nature together with the full history of non-chancy facts up to that time. I offer an intuitive example of this phenomenon, and use it to argue that free-floating chances are indeed possible. Their possibility violates three quite widely held principles about chances: the lawful magnitude principle, the principle that chances evolve by conditionalization and a (...) version of David Lewis' principal principle. I argue that we should reject common formulations of each of these principles, though I offer revised understandings of each which retain much of the intuitive attractiveness of the originals and are consistent with the possibility of free-floating chances. Following recent work by Don Ross (Ross, 2000; Ross & Spurrett, 2004), I contrast the influential theories of Daniel Dennett and Paul Churchland in information-theoretic terms. Dennett makes much of the fact that the morphological shorthand which emerges before a witness as she looks upon cohesive aggregates of matter commands some measure of predictive power. This, for him, speaks against eliminating recourse to an intentional vocabulary. By contrast, the eliminative materialism defended by Churchland does not gloss such informational compressibility as (...) an explanatory desideratum, and thus regards the informational noise which accrues at higher levels of description as patently unacceptable. Yet, since it is unlikely, as Ross, Ladyman, and Collier (2007) have recently suggested, that anything remains once we subtract the appeal to patterns, I argue that the ubiquity of informational compression in scientific explanation seriously undermines the claim that talk of the mental could be eliminated. I offer an argument regarding chances that appears to yield a dilemma: either the chances at time t must be determined by the natural laws and the history through t of instantiations of categorical properties, or the function ch(•) assigning chances need not satisfy the axioms of probability. The dilemma's first horn might seem like a remnant of determinism. On the other hand, this horn might be inspired by our best scientific theories. In addition, it is entailed by the familiar (...) view that facts about chances at t are ontologically reducible to facts about the laws and the categorical history through t. However, that laws are ontologically prior to chances stands in some tension with the view that chances are governed by laws just as categorical-property instantiations are. The dilemma's second horn entails that if chances are in fact probabilities, then this is a matter of natural law rather than logical or conceptual necessity. I conclude with a suggestion for going between the horns of the dilemma. This suggestion involves a generalization of the notion that chances evolve by conditionalization. Generation and heredity theories before early modern mechanist accounts might be faulted for numerous deficits. One might cite in this regard the failure to even attempt to explain how the inheritance of traits could occur, given what is known about the generation of new individuals. On the other hand, it would be hard to allow this as a true failure against the backdrop of a generation theory that poses form, and not matter, as the key to understanding the emergence of (...) new structures in the offspring of two organisms. Hence the signal contribution of the mechanist accounts in this sphere was simply the suggestion that theories of generation and heredity might look to matter and its behavior in order to explain how new individuals are created and retain or discard features of the individuals whence they sprung. Among these accounts, one early important set of views was presented by Pierre Gassendi, eventually as a full-blown atomist conjecture but even beforehand, and early on, as a thoroughgoing materialist theory. Gassendi's materialist mechanism for the transmission of traits is imperfect (and by the common sense of our day, implausible), not always or exclusively deployed in generation, and not clearly the physical bearer of information, as conformity to a materialist model would suggest. Nonetheless, his proposed mechanism represents a valuable attempt to provide an account of inheritance phenomena in materialist and ultimately atomist terms and rooted in an account of generation. An analysis of survey data on membership, pillarisation, voting behaviour and attitudes.Belgian society is traditionally portrayed as heavily pillarised, i.e. having a system of exclusive linkages between voluntary associations and political parties, resulting in the formation of a catholic, a socialist and a liberal 'pillar' within society. Recently, several authors have questioned the validity oft his model. Our survey of the Flemish population, however, shows that pillarisation is an enduring feature of Flemish society. Membership of voluntary associations, trade unions and (...) health insurance organisations remains ideologically motivated, and shows a high degree ofconsistency in this respect. Integration into a 'pillar' exerts a strong influence on voting behaviour, although this effect weakens in younger generations. Pillars also have significant, but weaker effects on attitudes like individualism, trust and solidarity. In the social capital research tradition, these differential effects of membership are often neglected. Although there are signs that pillarisation weakens in Fiemish society, the system certainly has not disappeared. (shrink)

What defence does the Narrative Practice Hypothesis have against the charge that it is a covert form of externalist theory theory ? I discuss and reject Dan Hutto's own strategies and argue that the NPH remains vulnerable to a threat of collapse into externalist TT as long as narrative folk-psychological explanation is differentiated from simple belief-desire explanation merely by a degree of complexity, subtlety and/or context-sensitivity. It is entirely plausible, however, that there is a more principled distinction between these two (...) types of explanation of human behaviour. They are part of nature in the sense that, like other forms of life, they were brought into existence by natural processes, and, like other forms of life, they are dependent on their environment for survival and success. Darwin offered an intriguing answer to the species problem. He doubted the existence of the species category as a real category in nature, but he did not doubt the existence of those taxa called ‘‘species’’. And despite his scepticism of the species category, Darwin continued using the word ‘‘species’’. Many have said that Darwin did not understand the nature of species. Yet his answer to the species problem is both theoretically sound and practical. On the theoretical side, DarwinÕs answer is (...) confirmed by contemporary biology, and it offers a more satisfactory answer to the species problem than recent attempts to save the species category. On the practical side, DarwinÕs answer frees us from the search for the correct theoretical definition of ‘‘species’’. The goal of this article is twofold. First, it revises the historiographic partition proposed by John Deely in Four Ages of Understanding (2001) by arguing that the moment marking the beginning of philosophical Modernity has been vividly recorded in Descartes’ Meditations on First Philosophy with the experiment with the wax. Second, an upshot of this historical study is that it helps make sense of Deely’s somewhat iconoclastic use of the words “subject” and “subjectivity” to designate mind-independent worldly things.
