In this second issue on the scholar and public intellectual Yu Jianrong, we address some of the topics that have been at the core of his academic work and where his research and views have helped shaped awareness of these issues among the general public and the academic community. We focus on the rural-urban divide, land issues, the letters and visits system, and social unrest and the so-called stability issue. To highlight the public intellectual dimension of Yu's work, we have (...) selected some of his articles from traditional media; speeches that he has made and that have been circulated on the Internet; public debates with officials, scholars, and foreigners; and interactions with netizens on Sina Weibo. Introduction: Congenital myasthenic syndromes refer to a heterogenic group of neuromuscular transmission disorders. CMS-subtypes are diverse regarding exercise intolerance and muscular weakness, varying from mild symptoms to life-limiting forms with neonatal onset. Long-term follow-up studies on disease progression and treatment-response in pediatric patients are rare.Patients and Methods: We analyzed retrospective clinical and medication data in a cohort of 32 CMS-patients including the application of a standardized, not yet validated test to examine muscular strength and endurance in 21 patients at the (...) last follow-up. Findings obtained in our cohort were compared with long-term follow-up studies of CMS-cohorts from the literature by considering the underlying molecular mechanisms. Outcomes of CMS-ST were compared to results of normal clinical assessment.Results: Thirty-two pediatric patients with defects in eight different CMS-genes were followed by a median time of 12.8 years. Fifty-nine percentage of patients manifested with first symptoms as neonates, 35% as infants. While 53% of patients presented a reduced walking distance, 34% were wheelchair-bound. Even under adequate therapy with pyridostigmine and 3,4-diaminopyridine, CHAT-mutations led to the progression of muscular weakness partly in combination with persistent respiratory and bulbar symptoms. RAPSN, CHRND, and CHRNB1 patients with neonatal manifestation, early respiratory problems, and bulbar symptoms showed a good and maintained treatment response. CHAT and CHRNE patients required higher PS dosages, whereas RAPSN patients needed a lower mean dosage at the last follow-up. The benefits of short-term medication and long-term progression of symptoms were highly dependent on the specific genetic defect. CMS-ST was carried out in 17/21 patients, determined affected muscle groups including bulbar and ocular symptoms, some of which were not reported by the patients.Conclusions: Our findings and comparison with the literature- suggest a better treatment-response and less severe progression of symptoms present in patients suffering from mutations in CMS-genes directly associated with receptor deficiency, while patients with defects leading to synaptopathy and presynaptic defects tend to have worse outcomes. Assessment of affected muscular groups and clinical symptoms by CMS-ST may be a useful tool for optimal therapeutic management of the patients, especially for future clinical studies. (shrink)

In this essay, we suggest practical ways to shift the framing of crisis standards of care toward disability justice. We elaborate on the vision statement provided in the 2010 Institute of Medicine (National Academy of Medicine) “Summary of Guidance for Establishing Crisis Standards of Care for Use in Disaster Situations,” which emphasizes fairness; equitable processes; community and provider engagement, education, and communication; and the rule of law. We argue that interpreting these elements through disability justice entails a commitment to both (...) distributive and recognitive justice. The disability rights movement's demand “Nothing about us, without us” requires substantive inclusion of disabled people in decision‐making related to their interests, including in crisis planning before, during, and after a pandemic like Covid‐19. Driven by technological innovations, newborn screening (NBS) panels have been expanded and the development of genomic NBS pilot programs is rapidly progressing. Decisions on disease selection for NBS are still based on the Wilson and Jungner (WJ) criteria published in 1968. Despite this uniform reference, interpretation of the WJ criteria and actual disease selection for NBS programs are highly variable. A systematic literature search [PubMED search “Wilson” AND “Jungner”; last search 16.07.22] was performed to evaluate the applicability of the WJ (...) criteria for current and future NBS programs and the need for adaptation. By at least two reviewers, 105 publications (systematic literature search, N = 77; manual search, N = 28) were screened for relevant content and, finally, 38 publications were evaluated. Limited by the study design of qualitative text analysis, no statistical evaluation was performed, but a structured collection of reported aspects of criticism and proposed improvements was instead collated. This revealed a set of general limitations of the WJ criteria, such as imprecise terminology, lack of measurability and objectivity, missing pediatric focus, and absent guidance on program management. Furthermore, it unraveled specific aspects of criticism on clinical, diagnostic, therapeutic, and economical aspects. A major obstacle was found to be the incompletely understood natural history and phenotypic diversity of rare diseases prior to NBS implementation, resulting in uncertainty about case definition, risk stratification, and indications for treatment. This gap could be closed through the systematic collection and evaluation of real-world evidence on the quality, safety, and (cost-)effectiveness of NBS, as well as the long-term benefits experienced by screened individuals. An integrated NBS public health program that is designed to continuously learn would fulfil these requirements, and a multi-dimensional framework for future NBS programs integrating medical, ethical, legal, and societal perspectives is overdue. The annual Kronos Philosophical Journal was established in Warsaw in 2012. The papers presented in the annual might be of interest to the readers from outside Poland, allowing them to familiarize themselves with the dynamic thought of contemporary Polish authors, as well as entirely new topics, rarely discussed by English speaking authors. Volume V/2016 comprises articles problematizing Russian phlosophy and literature as well as Ancient Greek philosophy and culture.

Challenging many of the currently accepted conceptions of autonomy and of how it is valued, Oshana develops a social-relational account of autonomy that is constituted by a person's relations with others and by the absence of certain social relations. She denies that command over one's motives and the freedom to realize one's will are sufficient to secure the kind of command over one's life that autonomy requires, and argues against psychological, procedural, and content neutral accounts of autonomy.

In 1809--the year of Charles Darwin's birth--Jean-Baptiste Lamarck published Philosophie zoologique, the first comprehensive and systematic theory of biological evolution. The Lamarckian approach emphasizes the generation of developmental variations; Darwinism stresses selection. Lamarck's ideas were eventually eclipsed by Darwinian concepts, especially after the emergence of the Modern Synthesis in the twentieth century. The different approaches--which can be seen as complementary rather than mutually exclusive--have important implications for the kinds of questions biologists ask and for the type of research they conduct. (...) Lamarckism has been evolving--or, in Lamarckian terminology, transforming--since Philosophie zoologique's description of biological processes mediated by "subtle fluids." Essays in this book focus on new developments in biology that make Lamarck's ideas relevant not only to modern empirical and theoretical research but also to problems in the philosophy of biology. Contributors discuss the historical transformations of Lamarckism from the 1820s to the 1940s, and the different understandings of Lamarck and Lamarckism; the Modern Synthesis and its emphasis on Mendelian genetics; theoretical and experimental research on such "Lamarckian" topics as plasticity, soft (epigenetic) inheritance, and individuality; and the importance of a developmental approach to evolution in the philosophy of biology. The book shows the advantages of a "Lamarckian" perspective on evolution. Indeed, the development-oriented approach it presents is becoming central to current evolutionary studies--as can be seen in the burgeoning field of Evo-Devo. This paper maintains that objectivism about practical reasons should be combined with pluralism both about the nature of practical reasons and about action explanations. We argue for an ‘expanding circle of practical reasons’, starting out from an open-minded monist objectivism. On this view, practical reasons are not limited to actual facts, but consist in states of affairs, possible facts that may or may not obtain. Going beyond such ‘that-ish’ reasons, we argue that goals are also bona fide practical reasons. This (...) makes for a genuine pluralism about practical reasons. Furthermore, the facts or states of affairs that function as practical reasons are not exclusively natural or descriptive, but include normative facts. That normative facts can be reasons justifies a pluralism about reason explanations, one which allows for what we call enkratic explanations in addition to teleological ones. (shrink)

Current knowledge of the genetic, epigenetic, behavioural and symbolic systems of inheritance requires a revision and extension of the mid-twentieth-century, gene-based, 'Modern Synthesis' version of Darwinian evolutionary theory. We present the case for this by first outlining the history that led to the neo-Darwinian view of evolution. In the second section we describe and compare different types of inheritance, and in the third discuss the implications of a broad view of heredity for various aspects of evolutionary theory. We end with (...) an examination of the philosophical and conceptual ramifications of evolutionary thinking that incorporates multiple inheritance systems. (shrink)

The Aleksanteri Institute of the University of Helsinki organized in 25–26 of September 2009 a special symposium Northern Lights — Facets of Enlightenment Culture with the aim to discuss form of Enlightenment thought in Sweden/Finland and Russia. The symposium, which was opened by Prof. Emeritus Matti Klinge, a renowned historian of 18th- and 19th-century Finland, had four participants from Russia, five from Finland and one from Germany; thus, it was yet a quite small event, but we hope that with it (...) the foundations of a fruitful co-operation, with annual symposia on questions of Enlightenment culture, will be laid. Of the speakers, Prof. Tatiana Artemyeva, Dr. Oili Pulkkinen and Prof. Vesa Oittinen focused on more general problems of study of Enlightenment ideas, while the other contributions dealt with different ―case studies‖ either in Sweden/Finland (Dr. Kimmo Sarje, Dr. Johan Sten), Russia (Prof. Mikhail Mikeshin, Dr. Johannes Remy, Dr. Larisa Agamalian), in France (Dr. Alla Zlatopolskaya) or Germany (Dr. Carola Häntsch). This brings us not only to the conclusion of my list of eight features proposed as being common to all or most value experience, but also to a reminder of its purpose. First, I hope that, in the spirit of Husserl's dictum, “to the things themselves,” this proposal will initiate a discussion of a “basic research” type of question, namely: What are the main features of value experience? Second, I hope that the fruits of such a discussion might eventually contribute (...) to the working out of a metatheory for value inquiry: a theory about criteria of adequacy for any general theory of value.In a paper of this scope it is obvious that one cannot even begin to discuss all the points and subpoints made in the manner in which they deserve. Much less can one defend all, or even any, of its claims with the kind of logical rigor and linguistic fastidiousness which have come to be the norm in most Anglo-American philosophy today. As a counterbalance to a tendency in some professional circles to making adherence to this norm the end of, rather than merely a means for, philosophic understanding, I have offered this paper after the spirit of Socrates and Plato, who certainly appreciated appropriate logical rigor and linguistic tidiness. Philosophy was nevertheless always for them first and foremost a mutual adventure of exploration into value through significant dialogue. Thus, rather than submitting one more piece of logic or language to the microscope, it has been my purpose to go exploring this time with a telescope. I cordially invite other seekers to join me and to challenge my findings. (shrink)

In recent years, some political realists have argued that there is a “distinctively political normativity” which should be used when construing and justifying political theories. Among realists focusing on a distinctively political normativity, one can identify two approaches. On the “moral view,” it is explicitly acknowledged that moral norms have a role to play in political normativity. On the “non-moral view,” distinctively political normativity is understood in terms of a non-moral kind of practical normativity. The non-moral view has received severe (...) criticism, not least pertaining to its instrumental versions. Recently, however, Carlo Burelli has attempted to develop a realist account that is faithful to the non-moral view, but which is said to avoid the criticism directed against non-moral accounts in general, and the purely instrumental ones, in particular. Burelli offers a functional account of distinctively political normativity, according to which the function of providing binding collective decisions generates a normative standard that is independent of morality. Despite its many innovative features, however, we argue that it fails with regard to the most pressing concern, which is not whether functional normativity is genuine normativity, but whether it is the right normativity for its assigned role. The paper discusses the origin of dark matter and dark energy from the concepts of time and the totality in the final analysis. Though both seem to be rather philosophical, nonetheless they are postulated axiomatically and interpreted physically, and the corresponding philosophical transcendentalism serves heuristically. The exposition of the article means to outline the “forest for the trees”, however, in an absolutely rigorous mathematical way, which to be explicated in detail in a future paper. The “two deductions” are two successive (...) stage of a single conclusion mentioned above. The concept of “transcendental invariance” meaning ontologically and physically interpreting the mathematical equivalence of the axiom of choice and the well-ordering “theorem” is utilized again. Then, time arrow is a corollary from that transcendental invariance, and in turn, it implies quantum information conservation as the Noether correlate of the linear “increase of time” after time arrow. Quantum information conservation implies a few fundamental corollaries such as the “conservation of energy conservation” in quantum mechanics from reasons quite different from those in classical mechanics and physics as well as the “absence of hidden variables” (versus Einstein’s conjecture) in it. However, the paper is concentrated only into the inference of another corollary from quantum information conservation, namely, dark matter and dark energy being due to entanglement, and thus and in the final analysis, to the conservation of quantum information, however observed experimentally only on the “cognitive screen” of “Mach’s principle” in Einstein’s general relativity. therefore excluding any other source of gravitational field than mass and gravity. Then, if quantum information by itself would generate a certain nonzero gravitational field, it will be depicted on the same screen as certain masses and energies distributed in space-time, and most presumably, observable as those dark energy and dark matter predominating in the universe as about 96% of its energy and matter quite unexpectedly for physics and the scientific worldview nowadays. Besides on the cognitive screen of general relativity, entanglement is available necessarily on still one “cognitive screen” (namely, that of quantum mechanics), being furthermore “flat”. Most probably, that projection is confinement, a mysterious and ad hoc added interaction along with the fundamental tree ones of the Standard model being even inconsistent to them conceptually, as far as it need differ the local space from the global space being definable only as a relation between them (similar to entanglement). So, entanglement is able to link the gravity of general relativity to the confinement of the Standard model as its projections of the “cognitive screens” of those two fundamental physical theories. During the course of this article, we examine the use of membership categorisation practices by a high-profile celebrity public social media account that has been understood to generate interest, attention and controversy across the UK media ecology. We utilise a data set of harvested tweets gathered from a high-profile public ‘celebrity antagonist’ in order to systematically identify types of antagonistic formulation that have generated different levels of interest within the social media community and beyond. Drawing from classic ethnomethodological studies of (...) banner headlines and other means of generating public interest and ‘making sense’, we respecify high-profile antagonistic tweets as category formulations that exhibit particular and regular membership category features that are reflexively bound to potential antagonistic readings, interest and controversy. In conclusion, we consider how such formulations may be understood to represent resources that constitute ignition points within antagonistic flows of communication and information that can be metaphorically understood as ‘digital wildfires’. In recent years, a number of authors have defended the coherence and philosophical utility of the notion of metaphysical indeterminacy. Concurrently, the idea that reality can be stratified into more or less fundamental ‘levels’ has gained significant traction in the literature. Here, I examine the relationship between these two notions. Specifically, I consider the question of what metaphysical determinacy at one level of reality tells us about the possibility of metaphysical determinacy at other more or less fundamental levels. Towards this (...) end, I propose a novel conception of the way in which fundamental states of affairs determine derivative states of affairs in the presence of indeterminacy and construct a corresponding formal model of multilevel systems that demonstrates the compatibility of determinacy at the fundamental level with indeterminacy at higher levels, thereby rebutting Barnes' suggestion that indeterminacy at any level of reality implies indeterminacy at the fundamental level. The GOOGLE and XPRIZE $5,000,000 for the practical and socially useful utilization of the quantum computer is the starting point for ontomathematical reflections for what it can really serve. Its “output by measurement” is opposed to the conjecture for a coherent ray able alternatively to deliver the ultimate result of any quantum calculation immediately as a Dirac -function therefore accomplishing the transition of the sequence of increasingly narrow probability density distributions to their limit. The GOOGLE and XPRIZE problem’s solution needs (...) the initial understanding that the result of any quantum calculation is a wave function, or respectively, a probability (density or not) distribution unlike the Turing machine one, and only a “calculating ray” is able to transform the former into the later without any “curving” disturbances. Thus, the unique capability of the quantum computer due to its inherent quantum parallelism can be conserved for all Gödel unresolvable problems, only on the fast “NP” track of which the quantum computer “Achilles” is able practically to overrun the Turing machine “Tortoise” since the latter can “sprint” only by any “P” calculating speed even on it and unlike “Achilles” himself able for “NP” velocities as well. The way for any material body to “calculate” the certain trajectory of least action also resolves the “traveling salesman problem” in fact, therefore illustrating furthermore what the “NP” output of a quantum computer by a “calculating ray” should mean. Another example is the problem of the number of all prime numbers less than “N”, which is suggested to visualize once again the quantum computer capability to resolve problems by a “NP” speed and thus qualitatively to overrun any competing Turing machine. The technically implemented idea of laser is now reinterpreted to “radiate a wave function” in order to explain the “calculating ray” option for a quantum computer “NP” output. The generalization of an entanglement “trigger ray” described by any non-Hermitian operator (unlike a laser) is suggested as well as those “sci-fi” horizons which it reveals. One of them is meant for elucidating the practical meaning of the “Yang-Mills existence and mass gap problem”, one more of the CMI “Millennium Problems” after that of “P vs NP”. -/- . (shrink)

This article aims to explore how epistemic status is negotiated during talk about the life memories of one speaker. Direct questions which foreground ‘remembering’ can lead to troubled sequences of talk. However, interlocutors sometimes frame their first parts as ‘co-rememberings’, and the sequential positioning of these can be crucial to the outcome of the talk. We draw on almost 10 hours of video data from dementia settings, where memory is a talked-about matter. Our focus is on 30 sequences which are (...) initiated with a question or other first part taking a K-stance, selecting one person as next speaker, and topically relating to the recipient’s past life. We show how type 2 knowables can be used alongside markers of tentativeness, to jointly construct the recipient’s epistemic primacy. (shrink)

The practice-dependent approach to justice has received a lot of attention in post-millennium political philosophy. It has been developed in different directions and its normative implications have been criticized, but little attention has been directed to the very distinction between practice-dependence and practice-independence and the question of what theoretically differentiates a practice-dependent account from mainstream practice-independent accounts. The core premises of the practice-dependent approach, proponents argue, are meta-normative and methodological. A key feature is the presumption that a concept of justice (...) is dependent on the function or aim of the social practices to which it is supposed to be applied. Closely related to this meta-normative thesis is an interpretive methodology for deriving principles of justice from facts about existing practices, in particular regarding their point and purpose. These two premises, practice-dependent theorists claim, differentiate their account since they are not accepted by practice-independent accounts and they justify different principles of justice than practice-independent accounts. Our aim in this article is to refute both and, demonstrating that practice-independent accounts may indeed accept the meta-normative and methodological premises of the practice-dependent accounts, and that we are given no theoretical reason to think that practice-dependent accounts justify other principles of justice for a practice than do practice-independent accounts. In other words, practice-dependent theorists have not substantiated their claim that practice-dependence is theoretically differentiated from mainstream accounts. When practice-dependent proponents argue for other principles of justice than mainstream theorists, it will be for the usual reason in normative theory: their first-order normative arguments differ. Rodney G. Peffer argues that Karl Marx cannot be considered a utilitarian, a consequentialist, or a nonutilitarian consequentialist. Based on ethics of social consequences as one of the versions of nonutilitarian consequentialism, the author examines Marx’s early journalistic articles concerning the common good published mainly in the Rheinische Zeitung. The author verifies the hypothesis that Marx was a nonutilitarian consequentialist in the given period with regard to the common good. By examining Marx’s views on freedom of the press and censorship, (...) as well as his criticism of the feudal experiences of Prussia, the author came to the conclusion that Marx’s views expressed in early journalism are close to ethics of social consequences and thus to nonutilitarian consequentialism. The main idea consists in researching the existence of certain characteristics of nature similar to human reasonability and purposeful actions, originating and rigorously inferable from the postulates of quantum mechanics as well as from those of special and general relativity. The pathway of the “free-will theorems” proved by Conway and Kochen in 2006 and 2009 is followed and pioneered further. Those natural reasonability and teleology are identified as a special subject called “God” and studyable by “quantum theology”, a scientific counterpart (...) of classical theology as far as the latter endeavors to justify rationally God (without quotation marks) postulated by religion, but meaning in the present study, first of all, Christianity. The suggested “quantum theology” is situated in the historical opposition and conflict of science and religion in Modernity and its reflection in Heidegger’s conception of “ontotheology”. The conception of “ontomathematics” introduced in previous papers to unite and unify physics, mathematics, and philosophy is reinterpreted in the present context to be conservatively generalized in a way to include “quantum theology”. Two conceptions, “locality” and “nonlocality”, originating initially from physics and especially, from theory of entanglement and quantum information, are also generalized ontomathematically, and thus in relation to the newly introduced “quantum theology”. The “quantum theology” subject of “God” is juxtaposed with God of religion and billions of believers all over the world. Husserl’s conception of “philosophy as a rigorous science” is considered to be generalized now to theology as quantum theology, particularly by the phenomenon (in the sense of his “phenomenology”) of “God”, or by an “epoché” to whether God exists or not. The research of the phenomenon of “God” is extended to the conception of the “totality” being inherent for philosophy, especially for classical German philosophy preceding Husserl’s phenomenology. Those sequences for philosophy implied by the introduction of quantum theology are investigated further, on the pathway of ontomathematics, also in relation to what “God” should mean for today’s physics and mathematics. The option and research field of “experimental quantum theology” is discussed as a direct corollary from quantum theology though being sacrilegious as for classical theology. The concluding section reflects in a more loose way on the relation of “God” for quantum theology to God for religion and billions of believers all over the world. (shrink)

A case study of quantum mechanics is investigated in the framework of the philosophical opposition “mathematical model – reality”. All classical science obeys the postulate about the fundamental difference of model and reality, and thus distinguishing epistemology from ontology fundamentally. The theorems about the absence of hidden variables in quantum mechanics imply for it to be “complete” (versus Einstein’s opinion). That consistent completeness (unlike arithmetic to set theory in the foundations of mathematics in Gödel’s opinion) can be interpreted furthermore as (...) the coincidence of model and reality. The paper discusses the option and fact of that coincidence it its base: the fundamental postulate formulated by Niels Bohr about what quantum mechanics studies (unlike all classical science). Quantum mechanics involves and develops further both identification and disjunctive distinction of the global space of the apparatus and the local space of the investigated quantum entity as complementary to each other. This results into the analogical complementarity of model and reality in quantum mechanics. The apparatus turns out to be both absolutely “transparent” and identically coinciding simultaneously with the reflected quantum reality. Thus, the coincidence of model and reality is postulated as necessary condition for cognition in quantum mechanics by Bohr’s postulate and further, embodied in its formalism of the separable complex Hilbert space, in turn, implying the theorems of the absence of hidden variables (or the equivalent to them “conservation of energy conservation” in quantum mechanics). What the apparatus and measured entity exchange cannot be energy (for the different exponents of energy), but quantum information (as a certain, unambiguously determined wave function) therefore a generalized law of conservation, from which the conservation of energy conservation is a corollary. Particularly, the local and global space (rigorously justified in the Standard model) share the complementarity isomorphic to that of model and reality in the foundation of quantum mechanics. On that background, one can think of the troubles of “quantum gravity” as fundamental, direct corollaries from the postulates of quantum mechanics. Gravity can be defined only as a relation or by a pair of non-orthogonal separable complex Hilbert space attachable whether to two “parts” or to a whole and its parts. On the contrary, all the three fundamental interactions in the Standard model are “flat” and only “properties”: they need only a single separable complex Hilbert space to be defined. (shrink)

The central ideas coming out of the so-called pragmatic turn in philosophy have set in motion what may be described as a pragmatic turn in normative political theory. It has become commonplace among political theorists to draw on theories of language and meaning in theorising democracy, pluralism, justice, etc. The aim of this paper is to explore attempts by political theorists to use theories of language and meaning for such normative purposes. Focusing on Wittgenstein's account, it is argued that these (...) attempts are unsuccessful. It is shown that pragmatically influenced political theorists draw faulty epistemological, ontological and semantic conclusions from Wittgenstein's view in their normative theorising, and it is argued that pragmatically influenced theories of language and meaning, however full of insight, cannot be put to substantial normative use in political theory. The general scope of the thesis is motivated by pointing to the general form of the argument and by moving beyond Wittgenstein to other philosophers of mind and language, illustrating how similar overextensions are made with regard to Robert Brandom's theory of language and meaning. Contingent valuation surveys generally elicit stated preferences by asking how much a respondent would be willing to pay for an environmental improvement. By drawing on linguistic theory, we propose that the modal phrasing of this question establishes a particular type of commitment towards a hypothetical payment, namely a subjective want or desire. Based on the idea that beyond subjective desires, considerations about what is morally adequate may guide expressed values and that elicitation of these can be linguistically facilitated, we employ (...) an experimental framework to investigate the effects of different modals ( willing, should and appropriate) in the elicitation question on stated preferences. We find that elicited amounts with appropriate are higher than those elicited with willing and should for environmental improvements more associated with use values, while differences are non-significant for environmental improvements more associated with non-use values. We discuss the implications of our findings for stated preference studies, as well as the potential broader theoretical implications that our study entails regarding linguistic representations of the moral entrenchment of environmental values. (...) They appear in your mind when you walk by the hospital room they occupied or when someone mentions their name. There is [End Page 19] something about the narrative of their life and illness that makes you hold on to them. Lily is one such patient for me.Lily was a 45-year-old paraplegic woman who was well known to many providers in the hospital. She had a complex trauma history (childhood physical and sexual abuse, intimate partner violence, domestic violence), homelessness, and multiple psychiatric admissions. In addition, she had a complex medical history, including chronic pain issues and substance use disorder. Many providers considered her to be a "very difficult" or "challenging" patient because she would "dictate" her care, had low distress tolerance and would have explosive angry outbursts requiring frequent visits from Police and Security. To top it off, after months of a challenging stay on a unit, she would eventually leave against medical advice. Everyone involved in her care felt that discharge to a safe setting such as a skilled nursing facility would be her best option. Yet, she was a very "hard" sell to any facility because of her behavioral issues, relatively young age, extensive history of drug use, and medication non-compliance. Even though the unit I worked in was familiar with "difficult" patients like Lily, the floor staff was anxious and concerned about the effect Lily would have on patients, staff, and providers. The combined anticipatory anxiety worsened after we were told that Lily left against medical advice three days before her admission to our unit as a reaction to the staff's attempt to develop a behavioral plan.I remember the day I met Lily. I was working as the clinical social worker on the floor, helping patients with adjustment to coping with hospitalization, addressing mental health issues that usually exacerbate in the acute care setting, and providing support to the staff caring for a psychosocially complex patient population. It was the second day after Lily arrived on the floor, and Police and Security had already come up three times. As I walked out of another patient's room, a bedside nurse stopped me in the hallway. "Can you please talk to her? … Please, just talk to her, or sit with her. I just need a break. I have been in that room since I started my shift. I am exhausted." The nurse had an exasperated and pleading look, and even though I was rushing to a meeting, I decided to meet with Lily instead. When I walked into her room, I saw a thin, small-framed woman with brown hair pulled back into a tight bun. She had freckles peppered all over her round face, neck, and chest. Her features were delicate and small, and her almond-shaped green eyes were her most prominent feature. She was wearing a white tee and jeans. She was sitting in her wheelchair; her makeup bag was lying on the table next to her. I introduced myself and my role on the team, trying not to stumble under a very heavy glare. "Tell them to leave me alone. That nurse is always in my room. Spying. I might not be able to walk, but I can still use my hands. I can call if I need help."And so I did. And when I came back, Lily and I talked a little about her life. And the next day, we talked a little more. And then again. Lily was giving me small glimpses of who she was and the events in her life.
