Identities ascribed to research staff in face-to-face encounters with participants have been raised as key ethical challenge in transnational health research. ‘Misattributed’ identities that do not just deviate from researchers' self-image, but obscure unequivocal aspects of researcher identity – e.g. that they are researchers – are a case of such ethical problem. Yet, the reasonable expectation of unconcealed identity can conflict with another ethical premise: confidentiality; this poses challenges to staff visiting participants at home. We explore these around a case (...) study of ‘follow-up’ staff, observed during an ethnographic study of a Kenyan HIV ‘trial community’, which included participant observation, conversations, and interviews with staff (n = 79) and participants (n = 89). We found that because of the need to maintain confidentiality and because of some suspicions towards researchers, research staff drew upon alternative identities – presenting themselves to non-participants as relatives or friends, rather than as researchers. Several staff experienced this as necessary but uncomfortable. Simultaneously, staff and participants forged close relations in line with their fictional identities, which however also posed challenges because they entailed personal responsibilities that were difficult to live up to, due to limited resources, and the trial's limited duration. Similar challenges may arise in transnational HIV treatment programmes and should be explored further in that context. (shrink)

Paul uses the word ἑνότης twice in Ephesians, and quite strangely, those are the only two places where the feminine noun features in the whole of the New Testament. In the two passages where they appear, they both relate to invisible unity, the unity of the Spirit that produces a common faith and knowledge of the Son of God – εἰς τὴν ἑνότητα τῆς πίστεως καὶ τῆς ἐπιγνώσεως τοῦ υἱοῦ τοῦ Θεοῦ. Such unity suggests that ecumenism amongst Christian denominations (...) is not only a possibility, it is also a necessity as far as we all profess one Christ. This unity is however far from ecclesiological unionism. Considering that the church appears weak from the outside when its diverse lines of doctrine, sacraments and ministerial ethics are emphasised. This suggests that a reasonable antidote would be the emphasis on the philosophy of unity amidst our diversity especially to the hearing of non-Christians.Contribution: This study makes firm the belief that Christianity is formed on divergent traditions that produced various strands of practices, which in turn produce different Christian sects and denominations, and a reverse is not possible. It then suggests a bonding in faith through the invisibility of henotic unity, which the pericope suggests. This will help the church to amass a stronger defence politically and structurally against rival religions and social organisations even in the midst of doctrinal differences. (shrink)

This paper introduces a new hypothesis for the rise of the politically correct left via an analysis of Black America. Drawing on Žižekian and psychoanalytical theory, it explores the ideological role of ‘symptom’ within America’s cultural landscape - of that which states that society ‘doesn’t work’ - by way of examining prominent African American figures and how they relate to this ‘symptom’: Will Smith and the ‘hystericization of the symptom’; Barack Obama and the ‘identification with the symptom’; the PC left (...) and the ‘implementation of the symptom as law’; the far right and the ‘fear of loss of the symptom’; Kanye West, Candace Owens and the ‘rejection of clinging to the symptom’. Additionally, the 90s sit-com The Fresh Prince of Bel-Air and the film Black Panther are analyzed and juxtaposed in order to further illustrate the process by which the recent ideological shift in our cultural and political discourse towards political correctness and identity politics has occurred. At one point, Krafel discovers a world in a one-inch-square patch of ordinary ground. Inspiring for parents and teachers seeking to encourage excitement about the positive role of people in nature, Krafel's work (...) harkens to St. Exupery's The Little Prince, Annie Dillard's Pilgrim at Tinker Creek, and Jean Giono's The Man Who Planted Trees. In a country characterized by a natural inclination to question authority, fighting the status quo of the ineffective, corrupt and disorganized [End Page 89] Haitian health system often appears to be the right thing to do.In 2002, (...) I played an active role in a large protest movement for better working conditions at the largest health facility in Port-au-Prince: the General Hospital (GH). Port-au-Prince is the capital and economic center of Haiti. It is home to close to a third of Haiti's ten million people. At that time, the situation at the GH facility was dire. All of the 400 beds at the GH were dirty. The staff was underpaid and unhappy. Drug and medical supply stock-outs were routine. Medical equipment was rarely maintained and only one of the six operating theatres in the surgery block was working. Unsupervised physicians in training (like me) were the main health care providers. Attending physicians were busy running their own private clinics, and most of them would come to the hospital solely to "recruit" the less poor patients for their clinics. As physicians-in-training, we had the responsibility of caring for the poor, but we had no means. We grew frustrated and angry witnessing our powerless patients losing legs, arms and their lives for lack of care. On many occasions, I had the parents of my patients buy the gloves that I would use to examine their sick relatives. I've seen patients fighting for the inheritance of medications left by others who had just passed away. There was no way for this hospital to fulfill its mission, and no way for its staff to deliver anything other than mediocre care. We could not accept that situation at such an early stage in our medical development, and so we refused to continue to work. We went on a strike, which completely paralyzed the delivery of care at the hospital for nearly two months. After intense negotiation, the strike ended with a signed agreement with the Minister of Health that addressed, more or less, the reasons of our movement. However, the implementation of the agreement was permanently delayed by other competing "state emergencies" including a general insurrection.At the time of our movement, I thought we were making history. After witnessing 11 "social movements" that similarly paralyzed the GH in the following decade for reasons identical to ours, I am now convinced that we were not. We were temporary actors performing in the cynical play of scarcity on permanent display in Port-au-Prince. The social movement that we started ended up hurting the very same people that we were trying to help.Extending access to heath care to poor people seemed to me at that time to be too complicated and too expensive for an unorganized state like Haiti. I realized that local protests could, at best, influence the allocation of local resources. But so-called local resources are so limited in "resource-limited" countries. Haiti spends only $40 U.S. dollars per capita for health care. This is not enough for good care. The options we were provided were not real ones—since all of them would end in more suffering. The case of providing health care to poor people needed to be approached differently.I was born in Port-au-Prince at the same general hospital in which I would later work. I am the third of four children, and the first male child in my family. My father made me understand very clearly that my first responsibility was for my siblings. My sisters made me understand equally clearly that I had no authority over any of them. I ended up with a lot of responsibility but with very little authority. By blending multiple strands of thought into one ideology, Chinese Syncretists of the pre-imperial period created an essential guide to contemporary ideas about self, society, and government. Merging traditions such as Ruism, Mohism, Daoism, Legalism, and Yin-Yang naturalism into their work, Syncretists created an integrated intellectual approach that contrasts with other, more specific philosophies. Presenting the first full English translation of the earliest example of a Syncretist text, this volume introduces Western scholars to both the brilliance of the syncretic method (...) and a critical work of Chinese leadership. Written by Shi Jiao, China's first syncretic thinker, during the Warring States Period of 481 to 221 BCE, _Shizi_ is similar to Machiavelli's _The Prince_ in that it dispenses wisdom to would-be rulers. It stresses the need for leaders to be detached and objective. It further encourages self-cultivation and effective government, recommending that rulers maintain self-discipline, hire reliable people, delegate power transparently, and promote others in an orderly fashion. The people, it is argued, will emulate their leader's wisdom and virtue, and a just and peaceful state will result. Paul Fischer provides an extensive introduction and a chapter-by-chapter summary and analysis of the text--outlining the importance of syncretism in Chinese culture--and explores the text's particular features, authorship, transmission, loss, and reconstruction over time. The _Shizi_ set the stage for a long history of syncretic endeavor in China, and its study provides insight into the vital traditions of early Chinese philosophy. The article scrutinizes Michel Foucault’s interpretation of Machiavelli in his famous lecture on governmentality. Foucault is slightly misguided in his search for the origins of governmentality, the article asserts. Foucault gives credit for the development of what he calls a new art of government to anti-Machiavellian treatises, but also follows those treatises in their distorted interpretation of Machiavelli. Consequently, Foucault’s analysis gets confused and regards as novel those arguments and developments that were essentially of ancient pedigree compared with Machiavelli’s ideas. (...) The article discusses especially two points in Foucault’s interpretation of Machiavelli: Foucault’s insistence on the singularity of the prince in Machiavelli and the importance of territory to Machiavelli. In both of these points Foucault is beside the mark. Foucault’s interpretation inverts the development of an art of government and regards as new those ideas that were fundamentally reactionary vis-à-vis Machiavelli’s ideas. The article suggests that a more viable lead in searching for an art of government might be found from Machiavelli’s writings and the republican experience of the late medieval Italian city-states rather than from the birth of administrative monarchies of the 16th and 17th centuries. Therefore, the article concludes that Foucault is somewhat misled in contextualizing the birth of governmentality, a view which also has some wider implications for the whole framework of governmentality Foucault is trying to develop. In this article, I survey the integrated connectionist/symbolic (ICS) cognitive architecture in which higher cognition must be formally characterized on two levels of description. At the microlevel, parallel distributed processing (PDP) characterizes mental processing; this PDP system has special organization in virtue of which it can be characterized at the macrolevel as a kind of symbolic computational system. The symbolic system inherits certain properties from its PDP substrate; the symbolic functions computed constitute optimization of a well-formedness measure called Harmony. The (...) most important outgrowth of the ICS research program is optimality theory (Prince & Smolensky, 1993/2004), an optimization based grammatical theory that provides a formal theory of cross-linguistic typology. Linguistically, Harmony maximization corresponds to minimization of markedness or structural ill-formedness. Cognitive explanation in ICS requires the collaboration of symbolic and connectionist principles. Two commentaries by esteemed scholars Renee Fox and Byron and Mary-Jo Good accompany the narratives, helping situate the lived experiences of global health practitioners within the frameworks of sociology and (...) medical anthropology respectively.The burgeoning interest in global health among students, health science trainees, clinical practitioners, social entrepreneurs, philanthropists, and government officials is often linked with substantial moral claims. People working in global health often start with the rhetorical premise that each and every human being, regardless of economic, social, or political circumstances that lie beyond his or her control, deserves equal access to quality health care services. This is a bold position at risk of trivialization, in part because the sentiment is so commonplace among global health equity activists.Despite this relatively recent global outpouring of solidarity and concern, billions of poor people still lack access to basic health services. All too often, interventions that perpetuate existing trade practices and market economics are promoted, usually to the detriment of the poor. As our good friend and colleague Arthur Kleinman warns:The irrelevance of ethics can be seen when considering universal ethical formulations of justice and equity that do not begin with the local moral conditions of poor people, those experiencing the systematic injustice of higher disease rates and fewer health-care resources because of their positioning at the bottom of local social structures of power.(1999, p.72)If we are serious about reducing health disparities globally, we must be prepared to mobilize resources in Africa just as we would in the United States or in Europe. If we fetishize cost-effective (read: low-cost) interventions for the poor, we must ask whether we use the same metrics in other situations. In other words, we must always strive to address the fundamental structural and social causes of health inequity.We believe that global health must avoid the "iron cage of rationality," to use sociologist Max Weber's words. One unanticipated consequence of the growth of global health as a field is that the "audit culture"—which encourages accountability and effectiveness—can at times reinforce power differentials between donors (whether government agencies or multilateral foundations) and their intended beneficiaries. Agendas are often set not by community members but by global health leaders who rarely demonstrate sustained commitments to a local community. Narratives, even when presented as raw and unrefined as many within this issue, remind us of the immense challenges—both programmatic and moral—involved in this work. The profound scarcity of resources available to health providers in poor countries forces ethical questions on doctors, nurses, pharmacists, social workers, and other health care workers, who make difficult choices every day about what to do with the few resources they have. These are the ethical dilemmas of mortal dramas at their most dire. Global health work demands of its practitioners an alternate mode of audit than academic methodologies can provide. Narratives return us to the basic human commitments that led many of us to this work, and they remind us to use words like equality and justice meaningfully.The narratives that follow offer unmitigated perspectives on the working lives of global health practitioners. They highlight the translation of the moral and programmatic challenges of health care delivery into real choices: for example, between a visiting surgeon's desire to treat a patient and his capacity (or really incapacity) to provide follow-up care. Hesiod’s fable of the hawk and the nightingale, addressed to kings, notoriously has no moral. Its depiction of a hawk carrying off a nightingale, preaching the futility of either resistance or pleading, appears to communicate the counsel, commonly designated as “Machiavellian,” that a ruler must know how to imitate a beast as well as a man. Such instruction—which advises that unjust actions are justifiable and necessary for a ruler—is clearly at odds with Hesiod’s explicit exhortations to his brother Perses to (...) work hard and avoid hubris, and his caution that unjust kings or lords will be punished by Zeus. I argue that Hesiod’s addressing the fable to kings “who themselves have understanding” explains the lack of a moral. To substantiate my claim I compare Hesiod’s and Machiavelli’s ranking of intellects, and illuminate Hesiod’s position with particular reference to and comparison with Machiavelli’s Prince, and examples drawn from the Old Testament and Old Irish law. The article discusses the self-portrayal of the ‘Royal Dictatorship’ of Carol II of Romania and analyses four theories of monarchy produced or published under his regime. It shows that the Romanian ‘Royal Dictatorship’ relied on leitmotifs targeting the multiparty system, territorial revisionism, and the Iron Guard, but that it lacked a coherent official doctrine. The article argues that this void allowed for Romanian theorists of monarchy to draw divergent, Western or (pseudo-)autochthonous genealogies for the regime. The lingering image is one of a Renaissance holy prince lying in state beneath twisted Bernini columns, his aquiline Medici profile turned toward heaven. One of the latest volumes in the "Modern Masters" series edited by Frank Kermode, this small, introductory study exhibits that admixture of philosophical acuity, wit, and style which we have come to associate with the work of Arthur Danto. That a thinker noted for his significant contributions to the analytic tradition should focus his attention on the prince of existentialists is itself something of a Wunder. That he does not approach Sartre like a silhouette-maker appraising an impressionist painting reveals (...) Danto’s own catholicity as well as the clarity and rigor of Sartre’s underlying insights and arguments. After acknowledging a professional debt to Sartre: "I have quarried Sartre’s work like a Barbarini over the years, taken fragments of his thought which I would never, I am certain, have been able to think of by myself," Danto employs a structural and synchronic approach to elucidate "the Sartrian system..., the most important and least accessible and most widely misappreciated of all that he has done." The elements of that system which Danto examines are five, treated in as many chapters. Characteristic of the book’s irenic spirit, each chapter bears two titles, one the canonized Sartrian term and the other Danto’s translation of the same into current analytic discourse. So we are offered: "Ambiguity: or, Language and Existence" ; "Nothingness: or, Consciousness and Ontology" ; "Engagement: or, Knowledge, Action, and the World" ; "Shame: or, The Problem of Other Minds" ; and "Anguish: or, Factual Beliefs and Moral Attitudes". In their highly topical paper, Grahamet alargued that Trusted Research Environments (TREs) are not actually about trust because they reduce or remove ‘…the need for trust in the use and sharing of patient health data’. We believe this is fundamentally mistaken. TREs mitigate or remove some risks, but they do not address all public concerns. In this regard, TREs provide evidence for people to decide whether the bodies holding and using their data can be trusted. This symposium begins with a critique by Paul Hirst of Wilfred Carr's ‘Philosophy and Education’(Journal of Philosophy of Education, 2004, 38.1), where Carr argues that philosophy of education should be concerned with ‘practical philosophy’ rather than ‘theoretical philosophy’. Hirst argues that the philosophy of education is best understood as a distinctive area of academic philosophy, in which the exercise of theoretical reason contributes critically to the development of rational educational practices and their discourse. While he acknowledges that these practices (...) and their discourse must of their nature be directly developed in the exercise of practical reason, or phronesis, the notion of ‘practical philosophy’ is rejected as ultimately incoherent and illusory. In his reply to Hirst's critique, Carr identifies three central claims in Hirst's argument and takes issue with each of these. He reaffirms the need to draw upon the resources afforded by the Aristotelian tradition of practical philosophy in order to identify inadequacies in our present understanding of how philosophy is related to education. He suggests that it is only through bringing their own ‘prejudices’ into critical confrontation with this tradition that philosophers of education will be able to assess whether practical philosophy is incoherent and illusory, as Hirst claims, or whether it is indispensable to the future development of their discipline. In a rejoinder to Carr, Hirst, defends the claim that philosophy of education is a social practice concerned with developing justifiable propositional accounts of the conceptual relations, justificatory procedures and presuppositions of educational practices. He rejects the argument that this ‘theoretical philosophy’ approach must be replaced by that of a new ‘practical philosophy’. (shrink)

The historical emphasis of medical ethics, based on substantive frameworks and principles derived from them, is no longer seen as sufficiently sensitive to the moral pluralism characteristic of our current era. We argue that moral decision-making in clinical situations is more properly derived from a process of dialogic consensus. This process entails an inclusive, noncoercive, and self-reflective dialogue within the community affected. In order to justify this approach, we make two claims—the first epistemic, and the second normative. The epistemic claim (...) follows from Habermas’ “ways” of knowing. We argue that the dialogue emanating from this episteme focuses on the relevant facts about the patient, as well as on the values attached to these facts, thus allowing facts and their meanings to serve as the basis for reflective action appropriate to the circumstances. The normative claim follows from Habermas’ discourse theory of morality and communicative action. It determines that clinical decision-making should be approached from the perspective of the actual reality of people in their situation of illness, set in their socio-cultural background, and hence be grounded in the second-person perspective. Relocating ethical decision-making from a monological space into one characterized by dialogue is especially appropriate to the clinical encounter. (shrink)

Chiefly through Coleridge's influence, eighteenth-century rationalist suppositions gave place in certain men to a fundamentally opposed, 'Romantic' philosophy, and so to a new kind of History. Mr. Forbes is particularly concerned with the part played in this revolution by the (...) liberal Anglicans: Thomas Arnold, Headmaster of Rugby and Regius Professsor of Modern History at Oxford; Richard Whitely, Professor of Political Economy at Oxford and Archbishop of Dublin; Julius Charles Hare, disciple of Coleridge and translator (with Thirlwall) of Niebuhr's History of Rome; Connop Thirlwall, Bishop of St David's and author of the History of Greece; Henry Hart Milman, Professor of Poetry and Oxford and Dean of St Paul's; Arthur Penrhyn Stanley, pupil and biographer of Thomas Arnold, and Dean of Westminster. They have elsewhere been studied in the compartments of 'classical' and 'ecclesiastical' history. But it is fundamental to their outlook on Church and State that for them no such compartments existed, and their idea of History as a whole has hitherto lacked an English historian. This essay does much more than clarify technical problems in one of the various ideas of History embraced in Professor Toynbee's system. China has been labeled an “anti-imperial empire” that forcefully challenges previous Euro-American imperial legacies—all while it verges on reenacting them.1 A critical way it resolves this geopolitical tension is through the articulation of a mission for global China, and since 2017, the heroic filmic fictional figure of the “Wolf Warrior” anchors such an approach. The Wolf Warrior is a gorgeous, muscled, Chinese male rogue soldier in exile who adventures through the archipelago of the PRC’s shipping, mining, and medical projects across the Global South to battle white mercenaries, corrupt Asian businessmen, and local pirates—all while saving Africa from a ravaging pandemic. Extending a hugely popular film series, the release in 2017 of the blockbuster Wolf Warrior 2 (abbreviated below as Wolf 2) unleashed a worldwide social media frenzy as well as a revolution in diplomatic tone and protocol for China. In these ways, Wolf Warrior 2 came to mark the debut of the global PRC as a hegemonic structure 1. These fused into a global cultural megaproject with the PRC presenting itself as a leader in responding to the COVID-19 pandemic in the Global South—a time the PRC has officially branded as its “Rejuvenation Era” of “Stepping Out into the World.” This essay shows how exactly this Wolf Warrior imagination has legitimized a new global regime, but I also reveal how, paradoxically, the Wolf Warrior regime has offered a distinct, more subversive worlding project. I argue here that the utopia immanent in this project can be revealed by interpretively turning this Wolf universe inside out through a deimperial queer analysis that honors and combines a Third World feminist lens and the “Asia as Method” approach with Danmei/BL and Slash queer-feminist fanfiction hacks. Danmei is a Chinese (originally mainly Tawainese) fanfiction genre that subverts mainstream heteronormative narratives by erotically aestheticizing and renarrating male-male intimacies within mainstream plotlines from feminist perspectives. BL (short for “boylove ”) is a similar Japanese fanfiction genre, and Slash is the term used in English for the queer/feminist hacking and excavation of sublimated and particularly utopian gender and sexuality plots from within seemingly conventional heteronormative or patriarchal popular-cultural texts. The everted or inside-out epistemology given expression here uses a set of non-phallogocentric lenses and decolonial feminist decoding practices. At stake is the urgency of not repeating the epistemic violence of conventional “anti-imperial” arguments, which tend to be heteronormative, statist, macho, and monolithic. Instead, the form of critique elaborated here brings questions of gender, race, and sexuality to the fore through a three-part approach. First, I identify the homoerotic register of the techno-infrastructural elements in the film. I generate close readings that detail the visual and genre codes of battle scenes, thug subjects, and piracy plots that forge a homoerotic model of rogue or “wolf” governmentality and that act as a metaphor for China’s techno -infrastructural rule. Second, this essay explores the gendered hauntings of supremacist humanism. I deploy a decolonial feminist lens to map the configuration of African women, female doctors, Maoist women insurgents, and feminized “flowerboys” as a rebel protectorate overseen by China’s phallic prerogative that is itself haunted by the specters of Euro-American humanist betrayal and white supremacy. Despite this fact he has received little attention from scholars of the French Renaissance. Perhaps the problem is that Seyssel does not seem to fit (...) neatly into any category. He was neither French nor Italian but a Savoyard, not truly noble but the illegitimate son of a noble family, not truly a historian but a writer of history-based propaganda and a translator of histories. He began his career as a jurist, became a royal councillor, then a bishop, and in the end died as the Archbishop of Turin. Before his death Seyssel left numerous works, his most famous being a political treatise entitled La Monarchie de France, a work contemporaneous with The Prince by Machiavelli, with whom he shared a similar view of the political world. 1Much like Seyssel himself the genre of history translation has been rather neglected by scholars. A notable exception is a recent article by A.C. Dionisotti, which also examines the translations of Seyssel. The article's intention is to continue a project proposed by Arnaldo Momigliano, to collect evidence that "would help us to understand where and on what occasions and by what people books of history were written and read." 2 This essay attempts to continue this project by asking the following questions: Why did Seyssel undertake the translations? For whom were they intended? How did Seyssel conceive his task as translator? Finally, what can be inferred from his technique of translation? [End Page 561]First, the translations must be put in the context of Seyssel's life. The translations were a sideline in his busy career as royal councillor. One of many itinerant scholar-diplomats hired by the states of Europe at that time, Seyssel had been teaching law at the University of Turin when he was called into the service of France by the newly crowned Louis XII in 1498. Seyssel was of value to the court because of his reputation as a jurist as well as his connections with the court of Savoy, the state which held the mountain passes leading to Italy. This was especially important because the conquest of Milan was the main goal of Louis XII throughout his reign. Seyssel played an active role in the French occupational government in several Italian cities. Although Seyssel spent much of his time in Italy, he often followed the king around France or was in transit on diplomatic missions to England, to Flanders, and to the Swiss cantons.It is amazing that Seyssel even found the time to write, but by the end of his life, he had left an astounding variety of written works: legal commentaries, translations, works of propaganda for the French monarchy, a political treatise, and at the end of his life several religious tracts. Despite the copiousness of his written works, however, Seyssel remained outside the circle of writers at the French court. He had no connections either with Budé or with the Grands Rhetoriqueurs, such as Jean Lemaire de Belges, nor did he leave any friendly and eloquent correspondence to other humanists. If Seyssel lived during the budding years of the French renaissance, he showed few signs of appreciating it. As a Savoyard educated in Italy he probably thought of the French in the same way as did many Italians of his age: they were militarily powerful but culturally backward. 3Undoubtedly, Seyssel believed the translations would serve his own interests at court. They were a way to cultivate patronage and to enhance his prestige by building up a reputation as an important scholar. Good and Mary-Jo DelVecchio GoodIf, as Arthur Frank (2002) writes, "moral life, for better and worse, takes place in storytelling," this collection of narratives written by physicians working in field settings in global medicine gives us a glimpse of some aspects of moral experience, practice, and dilemmas in settings of poverty and low health care resources. These essays are written (...) from the midst of practice, in diverse settings in Africa, South Asia, Latin America, the Caribbean, and the United States. The majority of the writers are practitioners of contemporary 'global medicine'—physicians from North America or Europe who spend part of their lives flying into settings of poverty or special need to practice medicine and work with on-going collaborative clinical and public health systems—while a smaller number are working in the societies in which they were born. Although a few work for classic humanitarian organizations or have developed their own local NGO's, the great majority are engaged in the 'hybrid life' described by Robert Riviello and noted by Renée Fox, moving between academic medical centers and distant health care systems that are supported by local governments, major international organizations and funding agencies, and international NGO's like Partners in Health. These diverse forms of practice have become a common model for contemporary clinicians engaged in what has emerged as a new discipline of global health or global medicine, and these narratives give us a glimpse of some of the enduring, as well as newly emerging, forms of moral practice and ethical dilemmas characteristic of today's global medicine.Many of these stories are filled with the tension and pathos that makes clinical narratives powerful to read. Physicians are forced to make unfathomable decisions about whether to resuscitate and keep a baby alive, using all means available, or to let the infant die, whether to carry out a procedure that will save an infant in birth but would put the mother at risk, about how to deal with a cancer found in a very advanced state. However, virtually all of the stories transcend these classical stories of the decisions physicians are forced to make by being placed in settings of great scarcity of resources, which stand in particular contrast to the academic medical settings in North America or Europe where many of these global medicine specialists have trained and many continue to practice. The unspeakable inequality of resources emerges throughout these narratives as a kind of master theme, as the underlying moral, as opposed to plot, which gives meaning and sense to the stories. For those who practice medicine in such settings, global inequalities and scarcity of medical resources are deeply felt. They produce the rage that Jeffrey Deal describes. ("I learned to despise American churches during that time, a time when my world encompassed a dying newborn in Sudan, as well as the air conditioned padded seats [End Page 121] of our affluent suburban church.") They produce the distinctive experiences of helplessness and pain felt by clinicians who cannot separate how they would be able to treat such a patient at home from how they have to practice in these settings. For those clinicians working in their own societies, scarcity of resources often produces deep anger at the corrupt governments that fail to support basic human services for the great majority of their population. ("We were temporary actors performing in the cynical play of scarcity on permanent display in Port-au-Prince," writes Haitian physician Paul Pierre about his experience prior to joining Partners in Health.)But for those who regularly travel back and forth across the divide between rich and poor, whether across national boundaries or within their own society, the challenge to live a moral life is unendingly complicated. While becoming a part of a struggle for global justice with colleagues and advocates from both sides of the divide, few of the narrators can avoid personal feelings of ambivalence and guilt at being able to easily move from one side of that divide to the other back home. In 1990, the Federal Trade Commission (FTC) issued a consent order to the American Institute of Certified Public Accountants (AICPA). The order decreed the AICPA to lessen its longstanding ethics code which had until then banned the receipts of commissions, referral fees and contingent fees. The FTC alleged that the AICPA banned receipt of the fees as an attempt to restrain trade (FTC, 1990).In the present study, we sought to determine if CPAs'' preference for bans on commissions, referral fees and (...) contingent fees is related to their moral reasoning whereby CPAs perceive the bans to serve as a means of resolving ethical issues. While determining this matter cannot prove whether the bans did or did not actually result in restrained trade, it can offer insight into the perceived ethical importance to CPAs of the overturned rules. Based on a random sample of AICPA members and using Rest''s Defining Issues Test (DIT) to measure moral reasoning, we did not find a CPA''s moral reasoning to be related to his/her preference for ethics rules which ban commissions, referral fees or contingent fees. However, our results did indicate that most CPAs prefer banning commissions, referral fees and contingent fees, with those CPAs holding a higher financial stake in public accounting, namely partners, favoring banning referral fees and contingent fees significantly less than CPAs with a lesser stake. Further, we noted a significant negative relationship between financial stake and moral reasoning. These results seem to suggest that self-interest among CPAs may influence their moral reasoning.Further study is needed to examine the relationship between self-interest of CPAs and their moral reasoning. If self-interest clouds moral judgments made by CPAs, capital markets are in danger. Rendering an independent audit opinion must exclude self-interest. (shrink)

When he was elected as a member of the Académie Française in 1946 at nearly 80 years of age he quipped, "For an academic, that is puberty." He died at 87, having lived large in two centuries and on three continents.Following an adolescence marked with indifference toward religion, Claudel experienced a profound conversion to Catholicism during a Christmas night service at Notre Dame Cathedral in Paris at the age of 18. With Christianity at the center of his universe and under the influence of Symbolist poets Rimbaud and Mallarmé, he developed a response to the world that was at once poetic and spiritual. Through his poetry, plays, and essays, he insisted on a fundamental unity between matter and spirit, a harmonious and vast universe into which the human mind could enter. And, at least for him, the point of entry was the portal of the Church. He struggled at times, but never lost sight of his faith. For all this, he is often credited with the revival of French Catholicism in the wake of WWI. After all, if Paul Claudel could take religion seriously and find meaning in it, pourquoi pas the rest of us?The facts of his extraordinary biography form a peculiar background to the recent translation by James Lawler of Claudel's collection of prose poems titled Knowing [End Page 229] the East. First published in 1946 as Connaissance de l'est, these sixty-one texts were written between 1895 and 1905 during Claudel's first visits to China and Japan as a young vice-consul stationed in Shanghai. He was twenty-six and still finding his poetic voice. And though his stance in the world is decidedly Christian, the content of this intriguing work reveals an inner dialogue with Buddhist ideas of interconnection, harmony, the ongoing quest for a higher life condition, and the overwhelming impact of nature in the lives of mere humans.In "Pagoda," Claudel describes a seven-story Chinese pagoda as it "amplifies and multiplies the roof, exaggerates the horned corners that rise with an elegant surge, and turns their curves upward to the sky." This form that "does away with walls" and concentrates on the uplifting roof creates a space where "Buddha, Prince of Peace, dwells inside with all the gods." In this architecture he reads a structuring of physical and spiritual space, a kind of never ending upward movement divided into "the seven mystical heavens." The culminating effect is the heavenly achievement of the unknowable. And so, after going as far as he can go into the world of the spirit, he ends this prose poem with an abrupt return to temporal reality. "I have no more to say of the Pagoda. I do not know its name."Elsewhere, in "The Wanderer," he turns his sense of awe toward nature. "Every tree has its personality, every small beast its role, every voice its part in the symphony," he writes. "As one understands music, so I understand nature, like a detailed narrative of proper names; as my walk and the day advance, so does the elaboration of the doctrine." The "doctrine," it seems, is the idea of dependent origination, the interrelation of all life that springs from a single source. In the last line, he states, "I understand the world's harmony. When will I grasp its melody?"In "The Temple of Consciousness" Claudel climbs "the black rock's vertical wall, and only at this late afternoon hour do I know myself to be on the right track." The ‘universal cell reaction’ (UCR), a coordinated biphasic response to external (noxious and other) stimuli observed in all living cells, was described by Nasonov and his colleagues in the mid‐20th century. This work has received no attention from cell biologists in the West, but the UCR merits serious consideration. Although it is non‐specific, it is likely to be underpinned by precise mechanisms and, if these mechanisms were characterized and their relationship to the UCR elucidated, then our understanding of the integration (...) of cellular function could be improved. As a step towards identifying such mechanisms, I review some recent advances in understanding cell mechanics and the stress response and I suggest potentially testable hypotheses. There is a particular need for time‐course studies of cellular responses to different stimulus doses or intensities. I also suggest a correspondence with hormesis; re‐investigation of the UCR using modern biophysical and molecular‐biological techniques might throw light on this much‐discussed phenomenon.
