The emphasis on evidence based medicine (EBM) has placed increased focus on finding timely answers to clinical questions in presence of patients. Using a combination of natural language processing for the generation of clinical excerpts and information theoretic distance based clustering, we evaluated multiple approaches for the efficient presentation of context-sensitive EBM excerpts.

This paper is a study about Philadelphia’s comprehensive user engagement sites (CUESs) as the authors address and examine issues related to the upcoming implementation of a CUES while seeking solutions for its disputed questions and plans. Beginning with the federal drug schedules, the authors visit some of the medical and public health issues vis-à-vis safe injection facilities (SIFs). Insite, a successful Canadian SIF, has been thoroughly researched as it represents a paradigm for which a Philadelphia CUES can expand upon. Also, (...) the existing criticisms against SIFs are revisited while critically unpackaged and responded to in favor of the establishment. In the main section, the authors propose the layout and services of the upcoming CUES, much of which would be in congruent to Vancouver’s Insite. On the other hand, the CUES would be distinct from Insite, as the authors emphasize, in that it will offer an information center run by individuals in recovery and place additional emphasis on early education for young healthcare professionals by providing them a platform to work at the site. The paper will also briefly investigate the implementation of a CUES site under an ethical scope of the Harm Reduction Theory. Lastly, the authors recommend some strategic plans that the Philadelphia City government may consider employing at this crucial stage. (shrink)

Cheating in the undergraduate classroom is not a new problem, and it is recognized as one that is endemic to the education system. This paper examines the highly normative behavior of using unauthorized assistance (e.g., a solutions manual or a friend) on an individual assignment within the context of an upper division undergraduate course in engineering mechanics. The findings indicate that there are varying levels of accepting responsibility among the students (from denial to tempered to full) and that acceptance of (...) responsibility can lead to identification of learning and necessary behavioral changes. The findings have implications for institutions and engineering faculty, in particular the need for consistent academic integrity education and the teaching of professional integrity and ethics. Rationale: Deep brain stimulation (DBS) of the hippocampus is proposed for enhancement of memory impaired by injury or disease. Many pre-clinical DBS paradigms can be addressed in epilepsy patients undergoing intracranial monitoring for seizure localization, since they already have electrodes implanted in brain areas of interest. Even though epilepsy is usually not a memory disorder targeted by DBS, the studies can nevertheless model other memory-impacting disorders, such as Traumatic Brain Injury (TBI). Methods: Human patients undergoing Phase II invasive monitoring for (...) intractable epilepsy were implanted with depth electrodes capable of recording neurophysiological signals. Subjects performed a delayed-match-to-sample (DMS) memory task while hippocampal ensembles from CA1 and CA3 cell layers were recorded to estimate a multi-input, multi-output (MIMO) model of CA3-to-CA1 neural encoding and a memory decoding model (MDM) to decode memory information from CA3 and CA1 neuronal signals. After model estimation, subjects again performed the DMS task while either MIMO-based or MDM-based patterned stimulation was delivered to CA1 electrode sites during the encoding phase of the DMS trials. Each subject was sorted (post hoc) by prior experience of repeated and/or mild-to-moderate brain injury (RMBI), TBI, or no history (control) and scored for percentage successful delayed recognition (DR) recall on stimulated vs. non-stimulated DMS trials. The subject’s medical history was unknown to the experimenters until after individual subject memory retention results were scored. Results: When examined compared to control subjects, both TBI and RMBI subjects showed increased memory retention in response to both MIMO and MDM-based hippocampal stimulation. Furthermore, effects of stimulation were also greater in subjects who were evaluated as having pre-existing mild-to-moderate memory impairment. Conclusion: These results show that hippocampal stimulation for memory facilitation was more beneficial for subjects who had previously suffered a brain injury (other than epilepsy), compared to control (epilepsy) subjects who had not suffered a brain injury. This study demonstrates that the epilepsy/intracranial recording model can be extended to test the ability of DBS to restore memory function in subjects who previously suffered a brain injury other than epilepsy, and support further investigation into the beneficial effect of DBS in TBI patients. ABSTRACT Our goal in this paper is to extend counterfactual accounts of scientific explanation to mathematics. Our focus, in particular, is on intra-mathematical explanations: explanations of one mathematical fact in terms of another. We offer a basic counterfactual theory of intra-mathematical explanations, before modelling the explanatory structure of a test case using counterfactual machinery. We finish by considering the application of counterpossibles to mathematical explanation, and explore a second test case along these lines.

Work in quantum gravity suggests that spacetime is not fundamental. Rather, spacetime emerges from an underlying, non-spatiotemporal reality. After clarifying the type of emergence at issue, I argue that standard conceptions of emergence available in metaphysics won’t work for the emergence of spacetime. I go on to consider spacetime functionalism as a way to make sense of spacetime emergence. I argue that a functionalist approach to spacetime modelled on mental state functionalism is not a viable alternative to the standard conception (...) of emergence in metaphysics. I go on to consider an alternative: ‘partial’ functionalism, whereby certain aspects of spacetime are functionalised, rather than spacetime as a whole. (shrink)

This paper motivates and develops a new theory of time: priority presentism. Priority presentism is the view according to which (i) only present entities exist fundamentally and (ii) past and future entities exist, but they are grounded in the present. The articulation of priority presentism is an exercise in applied grounding: it draws on concepts from the recent literature on ontological dependence and applies those concepts in a new way, to the philosophy of time. The result, as I will argue, (...) is an attractive position that can do much of the same work in satisfying our intuitions about time as presentism, but without the ontological cost. (shrink)

Rappaport's comment includes several errors. First, he conflates manipulation and deceit. Second, he confuses the rationalism of the evolutionary biological analysis of organisms with the rationalism (or lack thereof) of the motivational and cognitive structures of the organisms under study. Third, his moralistic judgment of my focus on manipulation implies that scientists should not only not explore but should also suppress such unsettling ideas. We will make little progress in understanding morality and in fostering truly moral behavior if we refuse (...) to acknowledge that moralistic statements may sometimes, and perhaps even often, be used in a manipulative way. This article provides a non-technical overview of the conflict between the special theory of relativity and the dynamic theories of time. The chief argument against dynamic theories of time from relativistic mechanics is presented. The space of current responses to that argument is subsequently mapped.

The standard philosophical view of time travel has it that time travelers cannot change the past. It has been argued by some that the standard view is false, and that this can be shown using a two-dimensional model of time. I defend the standard view against this attack. I show, first, that the addition of a second temporal dimension does not provide a model of changing the past and, second, that neither does the addition of n temporal dimensions for any (...) n > 1. Enhanced indispensability arguments seek to establish realism about mathematics based on the explanatory role that mathematics plays in science. Idealizations pose a problem for such arguments. Idealizations, in a similar way to mathematics, boost the explanatory credentials of our best scientific theories. And yet, idealizations are not the sorts of things that are supposed to attract a realist attitude. I argue that the explanatory symmetry between idealizations and mathematics can potentially be broken as follows: although idealizations contribute to the explanatory (...) power of our best theories, they do not carry the explanatory load. It is at least open however that mathematics is load-carrying. To give this idea substance, I offer an analysis of what it is to carry the explanatory load in terms of difference-making and counterfactuals. (shrink)

An ethics of bewilderment, which differs dramatically from the more familiar ethics of ease, is best understood through poetic presentations. Using examples drawn from Chinese and Western sources—notably Du Fu and Dante—this inquiry treats bewilderment as both an emotion and a virtue. Both these forms of bewilderment involve an acknowledgment of how minimal is the ethical confidence we have, given the feelings we have and the judgments we must make, but they also extend in productive ways the implications of that (...) acknowledgment. The convergence of increasingly efficient high throughput sequencing technology and ubiquitous Internet use by the public has fueled the proliferation of companies that provide personal genetic information direct-to-consumers. Companies such as 23andme and Navigenics are emblematic of a growing market for PGI that some argue represents a paradigm shift in how the public values this information and incorporates it into how they behave and plan for their futures. This new class of social networking business ventures that market the science of (...) the personal genome illustrates the new trend in collaborative science. In addition to fostering a consumer empowerment movement, it promotes the trend of democratizing information—openly sharing of data with all interested parties, not just the biomedical researcher—for the purposes of pooling data and escalating the innovation process. This target article discusses the need for new approaches to studying DTC genomics using social network analysis to identify the impact of obtaining, sharing, and using PGI. As a locus of biosociality, DTC personal genomics forges social relationships based on beliefs of common genetic susceptibility that links risk, disease, and group identity. Ethical issues related to the reframing of DTC personal genomic consumers as advocates and research subjects and the creation of new social formations around health research may be identified through social network analysis. Recently, nominalists have made a case against the Quine–Putnam indispensability argument for mathematical Platonism by taking issue with Quine’s criterion of ontological commitment. In this paper I propose and defend an indispensability argument founded on an alternative criterion of ontological commitment: that advocated by David Armstrong. By defending such an argument I place the burden back onto the nominalist to defend her favourite criterion of ontological commitment and, furthermore, show that criterion cannot be used to formulate a plausible form of (...) the indispensability argument. In this paper I consider the aboutness objection against standard truth-preserving presentism (STP). According to STP: (1) past-directed propositions (propositions that seem to be about the past) like , are sometimes true (2) truth supervenes on being and (3) the truth of past-directed propositions does not supervene on how things were, in the past. According to the aboutness objection (3) is implausible, given (1) and (2): for any proposition, P, P ought to be true in virtue of what P is (...) about, and so it is upon the past that the truth of past-directed propositions ought to supervene. Although an objection along these lines has been offered previously, I press the objection in two ways. First, by providing needed support for the view that propositions ought to be true in virtue of what they are about and, second, by arguing that the two responses available to the proponent of STP fail to be compelling. Tim travels back in time and tries to kill his grandfather before his father was born. Tim fails. But why? Lewis's response was to cite "coincidences": Tim is the unlucky subject of gun jammings, banana peels, sudden changes of heart, and so on. A number of challenges have been raised against Lewis's response. The latest of these focuses on explanation. This paper diagnoses the source of this new disgruntlement and offers an alternative explanation for Tim's failure, one that Lewis would (...) not have liked. The explanation is an obvious one but controversial, so it is defended against all the objections that can be mustered. (shrink)

In 1976, John Immerwahr published a classroom simulation designed to illustrate Hobbes’ model of the mutual transfer of rights in the formation of the social contract. The game is fruitfully seized upon in classrooms from a broad range of disciplines (economics, psychology, sociology, etc.) because the lesson of Hobbes’ state of nature and Immerwahr’s game can both be represented and elucidated by principles of game theory. This paper reintroduces a new generation of teachers to what the author calls “one of (...) the finest philosophy simulations ever designed”, offers suggestions on conducting the game and on how Immerwahr’s implementation of it can be improved, and discusses its game-theoretic implications. Whereas Immerwahr’s version of the game approximates the Prisoner’s Dilemma, the version presented in this paper more closely resembles the “Assurance Game”. Accordingly, the results of the author’s game indicate that, if given the chance to cooperate, students do so, and the average student finishes the game better off than in the Prisoner’s Dilemma. The author relates the results of his game to the theories of Hobbes, Locke, and Rousseau, and concludes by underscoring the richness of the game for philosophical analysis. (shrink)

ABSTRACTPresentism and endurantism are natural bedfellows: arguments have been mounted from endurantism to presentism and vice versa. I generalise an argument against the compatibility between presentism and endurantism offered recently by Tallant. I then show how to reformulate endurantism so that it is compatible with presentism. I demonstrate that this reformulated version of endurantism can do the same work with respect to the problem of temporary intrinsics as can standard definitions.

The curriculum design, faculty characteristics, and experience of implementing masters' level international research ethics training programs supported by the Fogarty International Center was investigated. Multiple pedagogical approaches were employed to adapt to the learning needs of the trainees. While no generally agreed set of core competencies exists for advanced research ethics training, more than 75% of the curricula examined included international issues in research ethics, responsible conduct of research, human rights, philosophical foundations of research ethics, and research regulation and ethical (...) review process. Common skills taught included critical thinking, research methodology and statistics, writing, and presentation proficiency. Curricula also addressed the cultural, social, and religious context of the trainees related to research ethics. Programs surveyed noted trainee interest in Western concepts of research ethics and the value of the transnational exchange of ideas. Similar faculty expertise profiles existed in all programs. Approximately 40% of faculty were female. Collaboration between faculty from low- and middleincome countries (LMICs) and high-income countries (HICs) occurred in most programs and at least 50% of HIC faculty had previous LMIC experience. One of the major difficulties facing presentism is the problem of causation. In this paper, I propose a new solution to that problem, one that is compatible with intrinsic, fundamental causal relations. Accommodating relations of this kind is important because (i) according to David Lewis (2004), such relations are needed to account for causation in our world and worlds relevantly similar to our own, (ii) there is no other strategy currently available that successfully reconciles presentism with relations of this kind (...) and (iii) resolving the problem of causation by accommodating intrinsic, fundamental causal relations provides the presentist with a far more general solution to the problem of causation than those currently on offer. Teaching poetry presents difficulties of various kinds, and these can be illustrative of wider curricular and pedagogical challenges. This chapter considers questions of knowledge and remembrance as these arise in two poems, placing these in relation to Jean‐Francois Lyotard's conception of the immemorial: Lyotard is concerned with a kind of repression in dominant conceptions of knowing and thinking, and with the insulation from experience that this effects. Celan addresses extremity of experience, entwined with massive and devastating historical upheaval; Heaney addresses (...) a desperately sad situation, but it is one that can arise in any community at any time, and is in this sense part of the ordinary human condition. Celan's poem is unthinkable without the moral political context, whereas Heaney's, however traced with Northern Ireland's Troubles, does not seem overly political; nor does it give us moral lessons in any obvious way. This article examines the ways in which the language of legal rights is invoked by those seeking to improve the treatment of animals. Drawing from a range of analytical, realist, and critical legal and social theorists, it argues that certain argumentative techniques commonly employed to justify the extension of legal rights to animals may serve to strengthen and reproduce the very forms of exploitation they seek to challenge. The article begins by identifying and critiquing the binary characterisation of rights/welfare and (...) property/personality in liberal animal law scholarship. It then employs the insights of Theodor Adorno and Walter Benjamin to expose and critique various appeals to an ‘exterior’ or ‘extra-legal’ domain which functions to stabilise the meaning of these doctrinal categories. In doing so, it explores the strategic viability of rights discourse in the animal advocacy movement with a view to highlighting the limitations of liberal constructions of animal rights. (shrink)

This paper aims at proposing a criterion to analyze the concept of subjectivity by surveying and classifying the theories of some major figures in the history of the western philosophy: Plato, Aristotle, Descartes, Hobbes, Bentham, Kant, and Hegel. As proceeding in this work, I reveal two approaches which confront each other, self-centered viewpoint and system-centered viewpoint, and arrange Descartes, Hobbes, and Bentham into the former, and Aristotle and Kant into the latter. Also, I assign Plato and Hegel to an alternative, (...) say, unified viewpoint. In this project, the subjectivity is defined as the fundamental status of a man. When dealing with it, two kinds of viewpoint present and distinguish themselves from each other by two factors as follows: one is whether or not it allows mediation with others to be involved in the configuration of subjectivity, and the other is whether it is determined by its formal features or by its contents. The self-centered viewpoint gets married with the no allowance of mediation and the contents-determining, and the system-centered one does with the allowance and theformality-determining. And the unified viewpoint is suggested as an overcoming account of those two. The significance of this attempt would be these three: one is that it can provide a clue to create new spectrum through which theories in the history of the philosophy are looked out over. Secondly, it can be groundwork to seek for a source of normative ethics by a way of re-determining personal identity. Finally, criticizing the self-centered viewpoint, a dominant account in present moral discourse, it can propose a footstone for an alternative theory of it. (shrink)

This paper examines the relationship between North American beef consumption and deforestation in South and Central America. Some writers have argued that consumption of hamburgers in North America, particularly hamburgers consumed in fast food restaurants, contributes to the depletion of the rainforest in South and Central America. We survey the published policy literature on the causes of rainforest depletion in the region. We also review the published estimates of the rate and extent of clearing of rainforest that has occurred in (...) South and Central America since 1970. Finally, we review the data on beef imports and consumption in Canada and the United States in a effort to assess the importance of South and Central America as suppliers of beef to the North American market. We conclude that the relationship between beef consumption in North America should not be considered an important cause of forest depletion in South and Central America. Domestic policies and market forces in the countries where rainforests are located are the leading causes of rainforest depletion in this region. This lesson seems to have been lost on some popular and even some textbook writers on this subject. (shrink)
