Background Physical morbidity is rife among patients with serious mental illness. When they are involuntarily hospitalized and even treated, they may still refuse treatment for physical illness leading clinicians to wonder about the ethics of coercing such treatments. Research aim This survey study explored psychiatric caregivers’ perceptions on whether compulsory treatment of physical illness is legal and whether it is justifiable in patients with serious mental illness and under what circumstances. Research design A questionnaire that included two case vignettes of (...) an involuntarily hospitalized psychiatric patient with diabetes refusing treatment with insulin for various reasons. The cases differed in terms of diabetes severity. Participants answered questions regarding the appropriateness of involuntary treatment. Participants and research context Psychiatric medical doctors and nurses working in a mental health center. ( N = 89, 50 female, ages 26–66). Ethical considerations The study was approved by the Medical Centre Institutional Review Board (IRB) and the University Ethics Committee. The respondents’ anonymity was kept. Participation was voluntary and consent was obtained. Results The severity of the patient’s medical condition and their reason for refusing treatment were associated with participants’ willingness to give insulin despite patient objection [( F(1, 87) = 49.41, p <.01; ( F(1, 87) = 33.44, p <.01), respectively]. Participants were more inclined to support compulsory treatment if the patient’s refusal was “illness-oriented” (i.e. directly related to psychiatric illness). Participants presented diverse views regarding the perceived legality of compulsory treatment of physical illness in such situations (illegal 63.09%; legal 23.8%; 13% unsure). The majority (53.5%–55.3%) of those who thought it was illegal supported compulsory treatment in high-severity, illness-oriented refusal situations. Conclusions The severity of the medical condition and the reason for treatment refusal influence psychiatric caregivers’ willingness to provide compulsory treatment for physical illness in involuntary hospitalized psychiatric patients. Beyond the legal framework, ethical guidelines for these situations are warranted, while decisions should be made on a case-by-case basis. (shrink)

This paper investigates the spatial dimensions of olfactory perception, challenging philosophical views that marginalize smell in spatial navigation and cognition compared to visual phenomenology. I argue that both olfactory and visual perceptions—despite smell often being considered non-spatial or minimally spatial—involve intricate spatial structuring when processed through unconscious cognitive processes. An information-theoretical approach shows that cognitive inferences turn spatially deficient sensory data into spatialized perceptual content to generate spatial perception across sensory modalities. This challenges the idea that spatial perception is tied (...) to external features. My argument is supported by two lines of evidence: (i) figure-ground segregation across sensory modalities, suggesting a general mechanism in perceptual processing; (ii) recent neuroscientific evidence demonstrating how spatial information is processed in olfaction for navigation and environmental mapping. My analysis calls for a reevaluation of assumptions about the non-spatial nature of smell, highlighting significant cognitive and spatial capabilities in olfaction. (shrink)

Legal battles continue in the UK over the Government’s plans to transport asylum seekers arriving on British shores to Rwanda in East Africa. Originally announced as a system for ‘processing’ asylum seekers, the Government has subsequently made it clear that there would not be an option for asylum seekers to return to the UK. The arrangement forms part of a deal between the UK and Rwanda, with the UK promising to invest £120 m in economic growth and development in Rwanda, (...) along with financing the cost of the ‘offshored’ asylum operation.1 The Government states that the policy is designed to break the business model of people traffickers involved in facilitating hazardous trips across the Channel in small, overcrowded and unseaworthy craft, leading to multiple drownings. Critics argue that the policy will do nothing to stop desperate people from seeking refuge in the UK, that transferring already traumatised people to Rwanda – which has been criticised for its human rights record – is inhumane and potentially outwith international law. Medical bodies, including the BMA, along with refugee organisations have expressed serious concerns about the health impact of such a system of ‘offshoring’. An equivalent system in Australia has been notorious for the devastating health impacts on those ‘offshored’ to the Pacific islands of Manus and Naura. Unusually in the UK the offshoring proposals have been greeted by unanimous condemnation by the Church of England. The lawfulness of the process is also subject to question. On the 14 July, a plane carrying a small number of asylum seekers to Rwanda was halted shortly before take-off following an intervention by the European Court of Human Rights. We investigate the rural resistance of one of the world’s largest social movements, La Via Campesina, as a powerful enactment of radical democracy in practice. More than this, the paper describes how the movement challenges the framework of radical democracy by pointing towards the ethical importance of recognizing the relationship of human dignity with nature and considering ethico-political values inherent in the peasants’ way of living. Their resistance is a rejection of depoliticizing silencing, and their everyday life is a commitment (...) to a “more than human” radical democracy in its most radical sense, as they are always already “in parliament with land”. We conclude by outlining a perspective which is both more than radical and more democratic, considering those who have not yet been heard but also that which, in the light of radical democracy, has never been counted as part of the political body at all: nature. Lowe, whose powerful and influential work was still developing at the time of his death in 2015. Leading philosophers present new essays on topics to do with ontology, necessity, existence, and mental causation.

While cancer survivors commonly experience fear and anxiety, a substantial minority experience an enduring and debilitating fear that their cancer will return; a condition commonly referred to as fear of cancer recurrence. Despite recent advances in this area, little is known about FCR among people from Indigenous or other ethnic and racial minority populations. Given the high prevalence and poor outcomes of cancer among people from these populations, a robust understanding of FCR among people from these groups is critical. The (...) current review identified and aggregated existing literature on FCR amongst adult cancer survivors from Indigenous and minority populations. The protocol of this review was registered with PROSPERO in July 2020. A systematic search of bibliographic databases was conducted for relevant articles published from 1997 to November 2019. Data from eligible articles were extracted and appraised for quality by two independent reviewers. Nineteen articles from four countries met the inclusion criteria, including 14 quantitative, 4 qualitative and 1 mixed-methods study. Only one article reported on an Indigenous population. Few studies reported on FCR prevalence or severity. While the variation in tools used to measure FCR hindered a robust estimate of severity, results suggested some differences in FCR severity between minority and dominant populations, although these may have been due to study metholodological differences. Few factors were reported as being associated with FCR in minorities across multiple studies. The qualitative synthesis found five themes associated with the lived experience of FCR: variations in the lived experience of FCR; spirituality and worldview impacting on FCR; the importance of staying positive; complexities around support; and increasing cancer knowledge. The findings of this review highlight differences in FCR across cultures and contexts, which reinforces the need for culturally-specific approaches to this condition. The dearth of research in this area is of concern given the significant burden of cancer in these populations. A deeper understanding of this condition among Indigenous and minority populations is critical to developing and delivering appropriate and effective psychosocial care for cancer survivors from these groups. While intending to reproduce this heritage, how do they reinvest the gendered dimension in the context of verbal interaction with their teen-agers, sending them back or not to their cultural “origins”? Do the mothers go beyond the traditional binary model of sex and gender to reach an agentive model? Through the continuum that goes from normativity to agentivity, the idea of an in-between stage makes sense: in what way does the linguistic imaginary constitute an area to be forged, a crossing space? In mid-2018, following a survey of lesbian, gay, bisexual and transgender groups, the UK government issued a consultation on the proposed reform of the Gender Recognition Act for England and Wales.1 When it was first introduced in 2004, the GRA was considered innovative, even world-leading legislation.2 The act enables any adult to seek to change their legal gender provided several criteria are met. These include: If the applicant is successful, he or she is issued with a ‘gender recognition certificate’, their (...) birth certificate is changed and ‘for all purposes’ they become, legally, their acquired or affirmed gender. Since 2004, cultural change in relation to gender identity has been rapid. During the government’s preliminary survey, several concerns were raised about the GRA, particularly among those who have changed, or who may be seeking to change, their gender. These included: The government has stated that being trans is not a mental illness—more a fact of human diversity. We support Carruthers' rejection of models 1 (independent systems) and 3 (metacognition before mindreading), and provide evidence to strengthen his critique. However, we also present evidence from autism that we believe supports model 2 (one mechanism, two modes of access) over model 4 (mindreading is prior).

‘Exclusivity’ is the claim that when deliberating about whether to believe that p one can only be consciously motivated to reach one's conclusion by considerations one takes to pertain to the truth of p. The pragmatist tradition has long offered inspiration to those who doubt this claim. Recently, a neo-pragmatist movement, Keith Frankish (), and Conor McHugh ()) has given rise to a serious challenge to exclusivity. In this article, I defend exclusivity in the face of this challenge. First, I (...) dispute a crucial assumption underlying the challenge, namely, that one can have evidence sufficient to enable but not compel belief. Secondly, I examine several cases that McHugh in particular offers to call exclusivity into question and argue on independent grounds that these cases do not threaten exclusivity. Whether or not exclusivity holds, in addition to being of intrinsic interest, has a decisive consequence for the contemporary debate over Bernard Williams’ () claim that “beliefs aim at truth”. If exclusivity is true, as David Owens () has argued, the deliberator cannot be said to literally aim at truth. So, in defending exclusivity, I thereby show that the notion of ‘aiming’ fails to illuminate the nature of belief. (shrink)

This book develops a unique phenomenology of plurality by introducing Hannah Arendt’s work into current debates taking place in the phenomenological tradition. Loidolt offers a systematic treatment of plurality that unites the fields of phenomenology, political theory, social ontology, and Arendt studies to offer new perspectives on key concepts such as intersubjectivity, selfhood, personhood, sociality, community, and conceptions of the "we." _Phenomenology of Plurality_ is an in-depth, phenomenological analysis of Arendt that represents a viable third way between the "modernist" and (...) "postmodernist" camps in Arendt scholarship. It also introduces a number of political and ethical insights that can be drawn from a phenomenology of plurality. The philosophical difficulties presented by self-deception are vexed and multifaceted. One such difficulty is what I call the ‘doxastic problem’ of self-deception. Solving the doxastic problem involves determining whether someone in a state of self-deception that ∼p both believes that p and believes that ∼p, simply holds one or the other belief, or, as I will argue, holds neither. This final option, which has been almost entirely overlooked to-date, is what I call ‘ nondoxasticism ’ about self-deception. In this article, (...) I present a negative case for nondoxasticism according to which, in the paradigm case of self-deception, there is no explanatory need to attribute the self-deceived person either their undesired belief that p, or their desired belief that ∼p. Folk psychology is replete with concepts other than belief, and if we bear this in mind, it becomes clear that the explanatory roles for which the self-deceived person's purported beliefs have traditionally been enlisted can be comfortably filled without recourse to belief. Background Moral distress is reported to be a critical force contributing to intensifying rates of anxiety, depression and burnout experienced by healthcare workers. In this paper, we examine the moral dilemmas and ensuing distress personally and collectively experienced by healthcare workers while caring for patients during the pandemic. Methods Data are drawn from free-text responses from a cross-sectional national online survey of Australian healthcare workers about the patient care challenges they faced. Results Three themes were derived from qualitative content analysis (...) that illuminated the ways in which moral dilemmas and distress were relationally experienced by healthcare workers: (1) the moral ambiguity of how to care well for patients amid a rapidly changing work environment; (2) the distress of witnessing suffering shared _between_ healthcare workers and patients; and (3) the distress of performing new forms of invisible work in the absence of institutional recognition. These findings reveal that moral distress was a strongly shared experience. Conclusions Findings advance understandings of moral distress as a relational experience, collectively felt, constituted, and experienced by healthcare workers. Considering how to harness collective solidarity in effectively responding to moral distress experienced across the frontline healthcare workforce is critical. (shrink)

Many views rely on the idea that it can never be rational to have high confidence in something like, “P, but my evidence doesn’t support P.” Call this idea the “Non-Akrasia Constraint”. Just as an akratic agent acts in a way she believes she ought not act, an epistemically akratic agent believes something that she believes is unsupported by her evidence. The Non-Akrasia Constraint says that ideally rational agents will never be epistemically akratic. In a number of recent papers, the (...) Non-Akrasia Constraint has been called into question. Epiphanies is a philosophical exploration of epiphanies, peak experiences, 'wow moments', or ecstasies as they are sometimes called. What are epiphanies, and why do so many people so frequently experience them? Are they just transient phenomena in our brains, or are they the revelations of objective value that they very often seem to be? What do they tell us about the world, and about ourselves? How, if at all, do epiphanies fit in with our moral systems and our theories of (...) how to live? And how do epiphanic experiences fit in with the rest of our lives? These are Sophie Grace Chappell's questions in this ground-breaking new study of an area of inquiry that has always been right under our noses, but remains surprisingly under-explored in contemporary philosophy. Proponents of the subset account of property realization commonly make the assumption that the summing of properties entails the summing of their forward-looking causal features. This paper seeks to establish that this assumption is false. Moreover, it aims to demonstrate that without this assumption the fact that the subset account captures an entailment relation—which it must if it is to be of any use to non-reductive physicalism—becomes questionable.

Epistemologists often assume that rationality bears an important connection to the truth. In this paper I examine the implications of this commitment for permissivism: if rationality is a guide to the truth, can it also allow some leeway in how we should respond to our evidence? I first discuss a particular strategy for connecting permissive rationality and the truth, developed in a recent paper by Miriam Schoenfield. I argue that this limited truth-connection is unsatisfying, and the version of permissivism that (...) supports it faces serious challenges; so, for mainstream permissivism, the truth problem is still unsolved. I then discuss a strategy available to impermissivists, according to which rationality bears a quite strong connection to truth. I argue that this second strategy is successful. (shrink)

People experiencing mental distress and illness are frequently on the receiving end of stigma, epistemic injustice, and social isolation. A range of strategies are required to alleviate the subsequent marginalisation. We ran a series ‘philosophy of mind’ workshops, in partnership with a third-sector mental health organisation with the aim of using philosophical techniques to challenge mental health stigma and build resources for self-understanding and advocacy. Participants were those with lived experience of mental distress, or unusual beliefs and experiences; mental health (...) advocates; and mental health service providers. We draw on a shared perspective as a participant and facilitator of the workshop series to assess their impact. We discuss the following benefits: the opportunity for structured discussion of experiences and models; dialogue across different mental health backgrounds; the potential to reduce self-stigma and to increase self-understanding and advocacy; and the potential to alleviate epistemic injustice. We invite researchers and mental health practitioners to consider further opportunities to investigate the potential benefits of philosophy groups in mental health settings to establish whether they generalise. This article explores how George Lapassade’s institutional pedagogy meets the definition of ‘praxis’ formulated by Cornelius Castoriadis, as the activity creating reflective and deliberative subjects. Lapassade applies Castoriadis’s criticism of bureaucracy to transform the teacher-learners’ relationship and emphasises how self-governance group dynamics among learners facilitates learning in general and access to critical thinking in particular. Castoriadis’s concept of democracy as individual and collective autonomy demands an interpretation of equality as a dynamic process instead of as a state of social relations, (...) both in politics and in classrooms. His understanding of politics as a matter of opinions rather than applied knowledge necessitates a questioning of how the relationship between politics and knowledge is presented in classrooms. The argument articulates two main themes: authority and power both between adults and children and within class groups, and the relationship between knowledge and politics, problematised by Castoriadis’s concept of truth. I propose a programme of research in ethical philosophy, into the peak-experiences or wow-moments that I, following James Joyce and others, call epiphanies. As a first pass, I characterize an epiphany as an (1) overwhelming (2) existentially significant manifestation of (3) value, (4) often sudden and surprising, (5) which feels like it “comes from outside” – it is something given, relative to which I am a passive perceiver – which (6) teaches us something new, which (7) “takes us out of (...) ourselves”, and which (8) demands a response. Often the correct response is love, often it is pity, or again creativity. It might also be anger or reverence or awe or a hunger to put things right – a hunger for justice; or many other things. It may be something that leads directly to action, but it may also be something that prompts contemplation; or other responses again. Since epiphanies are what I call a focal-case category, not all of the conditions listed above have to be fulfilled by all instances of epiphanies. In order to allow the reader to get a better grip on which range of phenomena may count as an epiphany, I examine in some detail several examples from literature, in particular from works by Murdoch, Hopkins, Wordsworth, C.S. Lewis, and by James Joyce. (shrink)

Believing rationally is epistemically valuable, or so we tend to think. It’s something we strive for in our own beliefs, and we criticize others for falling short of it. We theorize about rationality, in part, because we want to be rational. But why? I argue that how we answer this question depends on how permissive our theory of rationality is. Impermissive and extremely permissive views can give good answers; moderately permissive views cannot.

According to what I call the ‘Vagueness Thesis’ about belief, ‘believes’ is a vague predicate. On this view, our concept of belief admits of borderline cases: one can ‘half-believe’ something or be ‘in-between believing’ it. In this article, I argue that VT is false and present an alternative picture of belief. I begin by considering a case—held up as a central example of vague belief—in which someone sincerely claims something to be true and yet behaves in a variety of other (...) ways as if she believes that it is not. I argue that, even from the third-person perspective prioritised by proponents of VT, the case does not motivate VT. I present an alternative understanding of the case according to which the person in question believes as they say they do yet also has a belief-discordant implicit attitude otherwise. Moreover, I argue that, independently of the interpretation of any particular case, VT fails to accommodate the first-person perspective on belief. Belief is not only an item of one’s psychology that helps explain one’s behaviour; it is what one takes to be true. This fact about belief manifests itself in the nature of deliberation concerning whether to believe something and that of introspection regarding whether one believes something. Attending to these phenomena reveals that VT is not merely unmotivated, but untenable. (shrink)

In his insightful review, Eugene V. Koonin discusses various aspects of CRISPR-Cas systems with a strong focus on their qualities as "adaptive immune systems". The CRISPR-Cas system is most famous for its application as a gene-editing tool. Koonin provides a deeper insight into its biological function in bacteria, which is to immunize the cell against parasite DNA. I shall comment on one issue discussed in the text, in two steps. First, I shall elaborate on CRISPR-Cas systems and their supposed Lamarckian (...) character. Criteria for calling biological phenomena genuinely Lamarckian will be narrowed down and then applied to the CRISPR-Cas system, considering interference-driven spacer acquisition as an instantiation of a truly Lamarckian paradigm. Second, I shall consider whether Lamarckian and "canonical" instances of inheritance are a case of theoretical pluralism, being two non-reducible, yet interconnected paradigms. (shrink)

Anorexia nervosa is an eating disorder characterized by intense fear of gaining weight and a distorted body image which usually leads to low caloric intake and hyperactivity. The underlying mechanism and pathogenesis of anorexia nervosa is still poorly understood. In order to learn more about the underlying pathophysiology of anorexia nervosa and to find further possible treatment options, several animal models mimicking anorexia nervosa have been developed. The aim of this review is to systematically search different databases and provide an (...) overview of existing animal models and to discuss the current knowledge gained from animal models of anorexia nervosa. For the systematic data search, the Pubmed—Medline database, Embase database, and Web of Science database were searched. After removal of duplicates and the systematic process of selection, 108 original research papers were included in this systematic review. One hundred and six studies were performed with rodents and 2 on monkeys. Eighteen different animal models for anorexia nervosa were used in these studies. Parameters assessed in many studies were body weight, food intake, physical activity, cessation of the estrous cycle in female animals, behavioral changes, metabolic and hormonal alterations. The most commonly used animal model is the activity-based anorexia model in which typically young rodents are exposed to time-reduced access to food with unrestricted access to a running wheel. Of the genetic animal models, one that is of particular interest is the anx/anx mice model. Animal models have so far contributed many findings to the understanding of mechanisms of hunger and satiety, physical activity and cognition in an underweight state and other mechanisms relevant for anorexia nervosa in humans. The aim of this research is to explore the process linking participative leadership to organizational identification. The study examines the relationship between participative leadership and internal CSR perceptions of employees and also investigates the role that pride in membership plays in the affiliation of CSR perceptions with organizational identification. By studying these relationships, the paper aspires to contemplate new presumed mediators in the association of participative leadership with organizational identification as well as determine a possible novel antecedent of employee CSR (...) perceptions. Empirical evidence is provided from data that was collected through a survey distributed to employees working for small- and medium-sized enterprises in three countries in the Middle East and North Africa regions, particularly the United Arab Emirates, Lebanon, and Tunisia. Findings show that participative leadership leads to positive internal CSR perceptions of employees and that these CSR perceptions lead to pride in membership which, in turn, results in organizational identification.
