Contrary to the widely-held view that our conscious states are necessarily private (in that only one person can ever experience them directly), in this paper I argue that it is possible for a person to directly experience the conscious states of another. This possibility removes an obstacle to thinking of conscious states as physical, since their apparent privacy makes them different from all other physical states. A separation can be made in the brain between our conscious mental representations and the (...) executive processes that manipulate them and are guided by them in planning and executing behaviour. I argue here that these executive processes are also largely responsible for producing our sense of self in the moment. Our conscious perceptual representations themselves reside primarily in the posterior portions of the brain's cortex, in the temporal and parietal lobes, while the executive processes reside primarily in the prefrontal lobes. We can imagine an experiment in which we sever the association fibers that connect the posterior regions with these prefrontal regions and, instead, connect the posterior regions to the prefrontal regions of another person. According to my hypothesis, this would produce in the latter person the direct experience of the conscious perceptual states of the first person. (shrink)

BackgroundSubjective cognitive decline, non-amnestic mild cognitive impairment, and amnestic mild cognitive impairment are regarded to be at high risk of converting to Alzheimer’s disease. Amplitude of low-frequency fluctuations can reflect functional deterioration while diffusion tensor imaging is capable of detecting white matter integrity. Our study aimed to investigate the structural and functional alterations to further reveal convergence and divergence among SCD, naMCI, and aMCI and how these contribute to cognitive deterioration.MethodsWe analyzed ALFF under slow-4 and slow-5 bands and white matter (...) fiber integrity among normal controls, SCD, naMCI, and aMCI groups. Correlation analyses were further utilized among paired DTI alteration, ALFF deterioration, and cognitive decline.ResultsFor ALFF calculation, ascended ALFF values were detected in the lingual gyrus and superior frontal gyrus within SCD and naMCI patients, respectively. Descended ALFF values were presented mainly in the LING, SFG, middle frontal gyrus, and precuneus in aMCI patients compared to CN, SCD, and naMCI groups. For DTI analyses, white matter alterations were detected within the uncinate fasciculus in aMCI patients and within the superior longitudinal fasciculus in naMCI patients. SCD patients presented alterations in both fasciculi. Correlation analyses revealed that the majority of these structural and functional alterations were associated with complicated cognitive decline. Besides, UF alterations were correlated with ALFF deterioration in the SFG within aMCI patients.ConclusionsSCD shares structurally and functionally deteriorative characteristics with aMCI and naMCI, and tends to convert to either of them. Furthermore, abnormalities in white matter fibers may be the structural basis of abnormal brain activation in preclinical AD stages. Objective: We aimed to evaluate the activity of the human glymphatic system in type 2 diabetes mellitus using diffusion tensor image analysis along with the perivascular space.Methods: Diffusion tensor images were acquired to calculate the diffusivities in the direction of the x-axis, y-axis, and z-axis of the plane of the lateral ventricle body in 20 patients with type 2 diabetes and 10 people in a control group. We evaluated the diffusivity along with the perivascular spaces, as well as the projection (...) fibers and association fibers, separately. The analysis along the perivascular space was defined as the mean /mean, where the Dxpro and Dxasc were the Dx values in the projection and association fiber areas, respectively.Results: There were significant differences in diffusivity along the projection fibers and the association fibers among the groups. The significant differences among the groups along the perivascular spaces, shown as the ALPS-index and medical history of T2DM, indicating lower water diffusivity along the perivascular space concerning type 2 diabetes severity, was also observed.Conclusion: Lower diffusivity along the perivascular space on DTI-APLS can reflect impairment of the glymphatic system in T2DM. This study showed that the activity of the glymphatic system could be evaluated by diffusion tensor image analysis. (shrink)

Family income is associated with gray matter morphometry in children, but little is known about the relationship between family income and white matter structure. In this paper, using Tract-Based Spatial Statistics (TBSS), a whole brain, voxel-wise approach, we examined the relationship between family income (assessed by income-to-needs ratio) and white matter organization in middle childhood (N = 27, M = 8.66 years). Results from a nonparametric, voxel-wise, multiple regression (threshold-free cluster enhancement, p < 0.05, FWE corrected) indicated that lower family (...) income was associated with lower white matter organization (assessed by fractional anisotropy) for several clusters in white matter tracts involved in cognitive and emotional functions including fronto-limbic circuitry (uncinate fasciculus and cingulum bundle), association fibers (inferior longitudinal fasciculus, superior longitudinal fasciculus), and corticospinal tracts. Further, we examined the possibility that cumulative risk exposure might function as one of the potential pathways by which family income influences neural outcomes. Using multiple regressions, we found lower fractional anisotropy in portions of these tracts, including those found in the left cingulum bundle and left superior longitudinal fasciculus, was significantly related to greater exposure to cumulative risk (β = -0.47, p < 0.05 and β = -0.45, p < 0.05). Patients with Wilson’s disease suffer from prospective memory impairment, and some of patients develop cognitive impairment. However, very little is known about how brain structure and function changes effect PM in WD. Here, we employed multimodal neuroimaging data acquired from 22 WD patients and 26 healthy controls who underwent three-dimensional T1-weighted, diffusion tensor imaging, and resting state functional magnetic resonance imaging. We investigated gray matter volumes with voxel-based morphometry, DTI metrics using the fiber tractography method, and RS-fMRI using the seed-based (...) functional connectivity method. Compared with HC, WD patients showed GM volume reductions in the basal ganglia and occipital fusiform gyrus, as well as volume increase in the visual association cortex. Moreover, whiter matter tracks of WD were widely impaired in association and limbic fibers. WM tracks in association fibers are significant related to PM in WD patients. Relative to HC, WD patients showed that the visual association cortex functionally connects to the thalamus and hippocampus, which is associated with global cognitive function in patients with WD. Together, these findings suggested that PM impairment in WD may be modulated by aberrant WM in association fibers, and that GM volume changes in the association cortex has no direct effect on cognitive status, but indirectly affect global cognitive function by its aberrant functional connectivity in patients with WD. Our findings may provide a new window to further study how WD develops into cognitive impairment, and deepen our understanding of the cognitive status and neuropathology of WD. (shrink)

Phenotypic plasticity is a crucial feature of aggressive cancer, providing the means for cancer progression. Stochastic changes in tumor cell transcriptional programs increase the chances of survival under any condition. I hypothesize that unstable chromatin permits stochastic transitions between transcriptional programs in aggressive cancers and supports non‐genetic heterogeneity of tumor cells as a basis for their adaptability. I present a mechanistic model for unstable chromatin which includes destabilized nucleosomes, mobile chromatin fibers and random enhancer‐promoter contacts, resulting in stochastic transcription. (...) I suggest potential markers for “unsettled” chromatin in tumors associated with poor prognosis. Although many of the characteristics of unstable chromatin have been described, they were mostly used to explain changes in the transcription of individual genes. I discuss approaches to evaluate the role of unstable chromatin in non‐genetic tumor cell heterogeneity and suggest using the degree of chromatin instability and transcriptional noise in tumor cells to predict cancer aggressiveness. To further comprehend how synthesis and assembly of myofibrillar components is regulated, several laboratories have undertaken genetic studies of muscle development in Drosophila melanogaster. This small fly lends itself well to classical and molecular genetic approaches, and possesses a set of muscle fibers, termed indirect flight muscles (IFM), which is particularly advantageous for such investigations. Structural and functional analyses of cloned Drosophila contractile protein genes have revealed that protein isoforms can be specified either by multigene families or by differentially (...) splicing primary transcripts of a single gene. Characterization of mutations which disrupt flight muscle development has so far revealed that profound abnormalities are caused by defective alleles of actin, tropomyosin, and myosin heavy‐chain genes. Muscle defects associated with particular alleles can be alleviated by integration of corresponding wild‐type gene copies into germ‐line chromosomes. Strategies for further applying genetic techniques to investigations of Drosophila muscle development are discussed. (shrink)

The purpose of this study is to assess the efficacy of transcranial direct current stimulation in patients with treatment-refractory trigeminal neuralgia and examine the utility of neuroimaging methods in identifying markers of such efficacy. Six patients with classical TN refractory to maximal medical treatment, underwent tDCS. All patients underwent pre- and posttreatment functional magnetic resonance imaging during block-design tasks as well as single-shell diffusion MRI acquisition. The precise locations of tDCS electrodes were identified by neuronavigation. Five therapeutic tDCS sessions were (...) carried out for each patient with either anodic or cathodic applications. The Numeric Rating Scale of pain and the Headache Disability Index were used to score the subjective efficacy of treatment. Altered activity of regional sites was identified by fMRI and associated changes in the spinothalamocortical sensory tract were measured by the dMRI indices of fractional anisotropy and mean diffusivity. Fiber counts of the bilateral trigeminal root entry zone were performed as an added measure of fiber loss or recovery. All patients experienced a significant reduction in pain scores with a substantial decline in HDI in the number of contralateral sensory fibers in the STCT with either anodic or cathodic tDCS treatment in four of the six patients. A significant reduction in FA was observed in the ipsilateral REZ in the posttreatment phase in five of the six patients. Preliminary evidence suggests that navigated tDCS presents a promising method for alleviating the pain of TN. Different patterns of activation manifested by anodic and cathodic stimulation require further elaboration to understand their implication. Activation and attenuation of responses at various sites may provide further avenues for condition treatment. (shrink)

Parvalbumins (PVs) are acidic, intracellular Ca2+‐binding proteins of low molecular weight. They are associated with several Ca2+‐mediated cellular activities and physiological processes. It has been suggested that PV might function as a “Ca2+ shuttle” transporting Ca2+ from troponin‐C (TnC) to the sarcoplasmic reticulum (SR) Ca2+ pump during muscle relaxation. Thus, PV may contribute to the performance of rapid, phasic movements by accelerating the contraction–relaxation cycle of fast‐twitch muscle fibers. Interestingly, PVs promote the generation of power stroke in fish by (...) speeding up the rate of relaxation and thus provide impetus to attain maximal sustainable speeds. However, immunological monitoring of diverse tissues demonstrated that PVs are also present in non‐muscle cells. The axoplasmic transport and various intracellular secretory mechanisms including the endocrine secretions seem to be controlled by the Ca2+ regulation machinery. Any defect in the Ca2+ handling apparatus may cause several clinical problems; for instance, PV deficiency alters the neuronal activity, a key mechanism leading to epileptic seizures. Moreover, atypical relaxation of the heart results in diastolic dysfunction, which is a major cause of heart failure predominantly among the aged people. PV may offer a unique potential to correct defective relaxation in energetically compromised failing hearts through PV gene transfer. Consequently, PV gene transfer may present a new therapeutic approach to correct cellular disturbances in Ca2+ signaling pathways of diseased organs. Hence, PVs appear to be amazingly useful candidate proteins regulating a variety of cellular functions through action on Ca2+ flux management. (shrink)

A grafted frame is a new kind of frame which combines a modal frame and some relevance frames. A grafted model consists of a grafted frame and a truth-value assignment. In this paper, the grafted frame and the grafted model are constructed and used to show the completeness of S1. The implications of S1-completeness are discussed. A grafted frame does not combine two kinds of frames simply by putting relations defined in the components together. That is, the resulting grafted frame (...) is not in the form of $\langle W, R, R'\rangle$ , or more generally, in the form of $\langle W, R, R', R'',...\rangle$ , which consists of a non-empty set with several relations defined on it. Rather, it resembles the construction of fibering proposed by D. M. Gabbay and M. Finger (see [4] and [3]). On a grafted frame, some modal worlds, which belong to the initial modal frame, are attached by some relevance frames. However, these two semantics have important differences. Consider the combined semantics involving semantics of relevance logic and modal logic. A fibred model and a grafted model proposed in this paper differ in the following respects. First, a fibred model is constructed from a class of modal models and a class of relevance models. A grafted model consists of a grafted frame and a truth-value assignment, where the grafted frame is constructed from a modal frame and some relevance frames, and the assignment is a union of a modal truth-value assignment V M and some relevance truth-value assignments V R . V M (V R ) defined in this paper is not the same as the assignment contained in a modal (relevance) model. Second, in a fibred model each relevance world is associated (or fibred) with a modal model and each modal world with a relevance model. To be the grafted frame on which a grafted model is based, it is enough to have some modal worlds attached by some relevance frames. Moreover, no relevance world is associated with a modal frame in the grafted frame. Third, fibred models are intended to provide an appropriate semantics to combined logics. Grafted frames and grafted models are inspired to characterize S1, which, containing only one modality □, is not a combined logic. It is shown in this paper that S1 is sort of a meta-logic of the intersection of S0.4 and F, where S0.4, a new system proposed in this paper, is in turn a meta-logic of the relevance logic. (shrink)

Studies have shown that in the mammalian neuromuscular system stretch reflexes are localized within individual muscles. Neuromuscular compartmentalization, the partitioning of sensory output from muscles, and the partitioning of segmental pathways to motor nuclei have also been demonstrated. This evidence indicates that individual motor nuclei and the muscles they innervate are not homogeneous functional units. An analysis of the functional significance of reflex localization and partitioning suggests that segmental control mechanisms are based on subdivisions of motor nuclei–muscle complexes. A partitioned (...) organization of segmental control mechanisms could utilize (1) the potential functional diversity of muscle fiber types, (2) the variety of mechanical actions of individual muscles arising from their distributed origins and insertions, and (3) diverse architectural features such as intramuscular variations in pinnation and complex in-series and in-parallel arrangements of muscle fibers. The differentiated activity observed in some muscles during natural movements also calls for localized segmental control mechanisms. Partitioning may also play a role in mechanical interactions between contracting motor units and in increasing the stability of neuromuscular systems. The functional advantages of reflex localization and partitioning suggest they are probably common features of segmental systems, whose organization reflects the structure and function of their associated neuromuscular systems. Just to take a recent example, James Cornman and Keith Lehrer, trying to get (...) around the objection to the identity theory that the language of neural processes and the language of feeling pains belong to different linguistic categories, suggest that the materialist could devise a new term to refer to the same entity across categories: ‘fibain.’They write,He would then claim that there are fibains, that is, entities with those properties usually associated with pains, and also with the properties usually associated with firing C-fibers. To make a business case for corporate sustainability, firms must be able to sell their sustainable products. The influence that firm engagement with non-governmental organizations may have on consumer adoption of sustainable products has been neglected in previous research. We address this by embedding corporate sustainability in a cosmopolitan framework that connects firms, consumers, and civil society organizations based on the understanding of responsibility for global humanity that underlies both the sustainability and cosmopolitanism concepts. We hypothesize that firms’ sustainability engagement (...) and their NGO engagement influence consumer adoption of sustainable products. Empirically, we investigate the adoption of sustainable Eco-circle products made from recycled fibers marketed by Li Ning, a China-based global sportswear brand. We apply a stepwise regression approach to test our hypotheses with paper-and-pencil survey data from 217 Chinese consumers. We find adoption to be positively associated with consumers’ sustainability attitude but not with firms’ sustainability engagement. For firm–NGO engagement, these relationships are reversed: Adoption is positively associated with firm–NGO engagement, but not with consumers’ related attitude. Our results present a picture of the Chinese context in which there is a business case for corporate sustainability if firms’ words about sustainable product strategies are supported by signals from civil society about firm deeds. The results imply that in a Chinese context, firms need to be particularly aware of the role of NGOs when hoping to be rewarded for sustainability. (shrink)

Recent years have witnessed an explosion of the single-cell biochemical toolbox including chromosome conformation capture -based methods that provide novel insights into chromatin spatial organization in individual cells. The observations made with these techniques revealed that topologically associating domains emerge from cell population averages and do not exist as static structures in individual cells. Stochastic nature of the genome folding is likely to be biologically relevant and may reflect the ability of chromatin fibers to adopt a number of alternative (...) configurations, some of which could be transiently stabilized and serve regulatory purposes. Single-cell Hi-C approaches provide an opportunity to analyze chromatin folding in rare cell types such as stem cells, tumor progenitors, oocytes, and totipotent cells, contributing to a deeper understanding of basic mechanisms in development and disease. Here, we review key findings of single-cell Hi-C and discuss possible biological reasons and consequences of the inferred dynamic chromatin spatial organization. Single-cell Hi-C expands the molecular biology toolbox for studies of chromatin structure in individual cells. This technique allows one to analyze cell-to-cell variability in TAD and loop structure, and to capture chromosome conformation in rare cell types such as oocytes, totipotent cells, and tumor progenitors. Algorithmic software for mathematical modeling of structural stability is considered, which is reduced to solving a partial generalized eigenvalues problem of sparse matrices, with automatic parallelization of calculations on modern parallel computers with graphics processors. Peculiarities of realization of parallel algorithms for different structures of sparse matrices are presented. The times of solving the problem of stability of composite materialsusing a three-dimensional model of "finite size fibers" on computers of different architectures are given. In mathematical modeling of physical and (...) technical processes in many cases there is a need to solve problems of algebraic problem of eigenvalues with sparse matrices of large volumes. In particular, such problems arise in the analysis of the strength of structures in civil and industrial construction, aircraft construction, electric welding, etc. The solving to these problems is to determine the eigenvalues and eigenvectors of sparse matrices of different structure. The efficiency of solving these problems largely depends on the effectiveness of mathematical modeling of the problem as a whole. Continuous growth of task parameters, calculation of more complete models of objects and processes on computers require an increase in computer productivity. High-performance computing requirements are far ahead of traditional parallel computing, even with multicore processors. High-performance computing requirements are far ahead of traditional parallel computing, even with multicore processors. Today, this problem is solved by using powerful supercomputers of hybrid architecture, such as computers with multicore processors and graphics processors, which combine MIMD and SIMD architectures. But the potential of high-performance computers can be used to the fullest only with algorithmic software that takes into account both the properties of the task and the features of the hybrid architecture. Complicating the architecture of modern high-performance supercomputers of hybrid architecture, which are actively used for mathematical modeling means a significant complication of efficient use of these resources in creating parallel algorithms and programs. here are problems with the creation of algorithmic software with automatic execution of stages of work, which are associated with the efficient use of computing resources, ways to store and process sparse matrices, analysis of the reliability of computer results. This makes it possible to significantly increase the efficiency of mathematical modeling of practical problems on modern high-performance computers, as well as free users from the problems of parallelization of complex problems. he developed algorithmic software automatically implements all stages of parallel computing and processing of sparse matrices on a hybrid computer. It was used at the Institute of Mechanics named after S.P. Tymoshenko NAS of Ukraine in modeling the strength problems of composite material. A significant improvement in the time characteristics of mathematical modeling was obtained. Problems of mathematical modeling of the properties of composite materials has an important role in designing the processes of deformation and destruction of products in various subject areas. Algorithmic software for mathematical modeling of structural stability is considered, which is reduced to solving a partial generalized problem of eigen values of sparse matrices of different structure of large orders, with automatic parallelization of calculations on modern parallel computers with graphics processors. The main methodological principles and features of implementation of parallel algorithms for different structures of sparse matrices are presented, which ensure effective implementation of multilevel parallelism of a hybrid system and reduce data exchange time during the computational process. As an example of these approaches, a hybrid algorithm of the iteration method in subspace for tape and block-diagonal matrices with a frame for computers of hybrid architecture is given. Peculiarities of data decomposition for matrices of profile structure at realization of parallel algorithms are considered. The proposed approach provides automatic determination of the required topology of the hybrid computer and the optimal amount of resources for the organization of an efficient computational process. The results of testing the developed algorithmic software for problems from the collection of the University of Florida, as well as the times of solving the problem of stability of composite materials using a three-dimensional model of "finite size fibers" on computers of different architectures. A few years ago no one would have suspected that the well‐known disorder of connective tissue, Marfan syndrome, could be caused by mutations in a recently discovered extracellular component, fibrillin. Likewise, nobody would have predicted that fibrillin represents a small family of proteins that are associated with several pheno‐typically overlapping disorders. The fibrillins are integral constituents of the non‐collagenous microfibrils, with an average diameter of 10 nm. These aggregates are distributed in the extracellular matrix of virtually every tissue. Microfibrillar bundles (...) provide the external coating to elastin in elastic fibers, and serve an anchoring function in non‐elastic tissues. At higher resolution, individual microfibrils have a “beads‐on‐a‐string” appearance resulting from the head‐to‐tail polymerization of multiple fibrillin aggregates. Structurally, fibrillin contains a series of repeated sequences homologous to the epidermal growth factor calcium‐binding motif. Characterization of fibrillin mutations in Marfan syndrome patients, together with the elucidation of the structure of the fibrillin proteins, have provided new insights, and raised new questions, about the function of the 10 nm microfibrils. For example, it is possible that the fibrillins, in addition to serving a structural function, might also be involved in regulating cellular activities and morphogenetic programs. It is fitting that the long search for the Marfan syndrome gene has brought a novel group of proteins to the forefront of extracellular matrix biology. (shrink)

Microfilament interactions with the plasma membranes of animal cells appear to vary with cell type and localization. In the erythrocyte, actin oligomers are associated with the membrane via spectrin and ankyrin. The ends of stress fibers in cultured cells, such as fibroblasts, are attached to the plasma membrane at focal adhesion sites and may involve the protein vinculin as a linking protein. In intestinal brush border microvilli a 110,000 dalton protein links the microfilament bundles to sites on the microvillus. (...) A transmembrane complex containing actin stably associated with a cell surface glycoprotein can be isolated from ascites tumor cell microvilli and can be obtained still associated with microfilaments by gentle extraction and gradient centrifugation of the microvilli. Recent evidence points to widespread underconnectivity in autistic brains owing to deviant white matter, the fibers that make long connections between areas of the cortex. Subjects with autism show measurably fewer long-range connections between the parietal and prefrontal cortices. These findings may help shed light on the current debate in the consciousness literature about whether conscious states require both prefrontal and parietal/temporal components. If it can be shown that people with autism have conscious states despite such underconnectivity, this would (...) constitute an argument for the claim that conscious states can exist in posterior cortex without associated prefrontal activity. This in turn lends support to a class of theories according to which microconsciousness is possible—consciousness in small areas of cortex without active connections to the prefrontal cortex, as opposed to the higher-order thought (HOT) theory of consciousness, according to which conscious states can only occur when posterior cortical areas (in the parietal or temporal lobes) have active connections to the prefrontal cortex. In this chapter, after listing several candidate examples of consciousness without accompanying prefrontal connections, I will argue that autism provides yet another such example. I will also examine a recent version of the higher-order theory that acknowledges these cases of consciousness without prefrontal activity and, instead depicts consciousness as requiring higher-order thoughts located in posterior cortex. In the final section, I will examine the consequences of these views for our understanding of the metaphysical nature of consciousness itself—the classic mind-body problem. (shrink)

The geometro-stochastic quantization of a gauge theory for extended objects based on the (4, 1)-de Sitter group is used for the description of quantized matter in interaction with gravitation. William Louis Poteat , a North Carolina intellectual of the Progressive Era, gained a reputation as a leading liberal in southern higher education. This book, which was recognized by a prestigious National Endowment for the Humanities publication grant, is useful for historians of science because it illustrates how scientific ideas, particularly Darwinism, were diffused into a rural and isolated society.From his student years to the end of his life, Poteat was intimately connected with a Southern Baptist college, Wake Forest. He (...) matriculated at Wake Forest and became a tutor there in 1878. Although almost entirely self‐taught in the sciences, he became a popular and respected professor of biology. As his knowledge of science came from reading contemporary papers and monographs rather than from traditional study, he developed a perspective more modern than that of most of his peers. He rose through the ranks, ultimately becoming president of Wake Forest in 1905.Relatively early in his career, Poteat began the work that earned him a place in southern history by writing articles and giving lectures on the ways in which science and religion were related. By the end of his first decade as a faculty member, he had become convinced of the importance of Darwin's theory of evolution and had decided that it posed no threat to traditional Christianity. In supporting this view, Poteat followed the lead of mainstream European and Northern American churches, which had no problem reconciling evolution with the Book of Genesis and its description of creation occurring in seven days. After all, who knew what a day meant in the mind of God? Many Southern Protestants, however, rejected this position.Randal Hall sees Poteat as an advocate of a liberal social philosophy fully in accord with the social gospel of the Progressive age. As a leading proponent for change in his state, Poteat “personified North Carolina Progressivism” , and his defense of Darwin was of a piece with his philosophy. But it is unclear whether Poteat's admiration for Darwin stemmed from an understanding of science or an obsession with eugenics, an obsession that in time came to dominate his thinking.Poteat vigorously used his position as president of the Southern Baptist Education Association to promote theories of eugenics. Although Darwin did not actively promote racism, followers of his thought such as Poteat could easily educe racist ideas from it. They could argue, for example, that individuals should speed up the process of producing superior humans by helping nature along through the process of selective breeding. Concerned with the large number of Americans who were institutionalized, and the even greater number who were barely educated, Poteat maintained that the government should control the right to reproduce and sterilize the unfit, including the “self‐centered, the coarse‐fibered, and the discourteous” . He thought that civilization would crumble unless such procedures were put in place, because there would be an “overproduction of under‐men” . Indeed, following this same line of thought, he opposed war because it foolishly wasted “our best blood” . He called for federal legislation to govern marriage and divorce, requiring applicants to meet his moral requirements and to pass a rigorous medical examination. Even in conservative North Carolina, these ideas provoked astonishment. A leading Raleigh newspaper accused Poteat of wanting to be the “boss of a human stock farm” .Although Randal Hall's book is primarily a biography of William Louis Poteat—and secondarily a history of Wake Forest as it evolved from a tiny Baptist school into a major college—it is also a study of the split personalities of Southern Progressives. On the one hand, they rejected rule by educated leaders; on the other, they wanted government to control the most intimate aspects of life, such as reproduction. Poteat thought that all these things should be overseen by a “genteel Christian community led by a liberal elite” . In emergencies If no advance decision has been made or is known, CPR should be attempted unless: the patient has refused CPR; the patient is clearly in the terminal phase of illness; or the burdens of the treatment outweigh the benefits. Advance decision making Competent patients should be involved in discussions about attempting CPR unless they indicate that they do not want to be. Where patients lack competence to participate, people close to them can be helpful in reflecting their views. Legal issues Patients' rights under the Human Rights Act must be taken into account in decision making. Neither patients nor relatives can demand treatment which the health care team judges to be inappropriate, but all efforts will be made to accommodate wishes and preferences. In England, Wales and Northern Ireland relatives and people close to the patient are not entitled in law to take health care decisions for the patient. In Scotland, adults may appoint a health care proxy to give consent to medical treatment.
