Contemporary psychiatry is becoming more biologically oriented in the attempt to elicit a biological rationale of mental diseases. Although mental disorders comprise mostly functional abnormalities, there is a substantial overlap between neurology and psychiatry in addressing cognitive disturbances. In schizophrenia, the presence of cognitive impairment prior to the onset of psychosis and early after its manifestation suggests that some neurocognitive abnormalities precede the onset of psychosis and may represent a trait marker. These cognitive alterations may arise from functional disconnectivity, as (...) no significant brain damage has been found. In this review we aim to revise A.R. Luria’s systematic approach used in the neuropsychological evaluation of cognitive functions, which was primarily applied in patients with neurological disorders and in the cognitive evaluation in schizophrenia and other related disorders. As proposed by Luria, cognitive processes, associated with higher cortical functions, may represent functional systems that are not localized in narrow, circumscribed areas of the brain, but occur among groups of concertedly working brain structures, each of which makes its own particular contribution to the organization of the functional system. Current developments in neuroscience provide evidence of functional connectivity in the brain. Therefore, Luria’s approach may serve as a frame of reference for the analysis and interpretation of cognitive functions in general and their abnormalities in schizophrenia in particular. Having said that, modern technology, as well as experimental evidence, may help us to understand the brain better and lead us towards creating a new classification of cognitive functions. In schizophrenia research, multidisciplinary approaches must be utilized to address specific cognitive alterations. The relationships among the components of cognitive functions derived from the functional connectivity of the brain may provide an insight into cognitive machinery. (shrink)

In this article, we use the theory of Information Ethics to argue that deceased people have a prima facie moral right to privacy in the context of health data research, and that this should be reflected in regulation and guidelines. After death, people are no longer biological subjects but continue to exist as informational entities which can still be harmed/damaged. We find that while the instrumental value of recognising post-mortem privacy lies in the preservation of the social contract for health (...) research, its intrinsic value is grounded in respect for the dignity of the post-mortem informational entity. However, existing guidance on post-mortem data protection is available only in the context of genetic studies. In comparing the characteristics of genetic data and other health-related data, we identify two features of DNA often given as arguments for this genetic exceptionalism: relationality and embodiment. We use these concepts to show that at the appropriate Level of Abstraction, there is no morally relevant distinction between posthumous genetic and other health data. Thus, genetic data should not automatically receive special moral status after death. Instead we make a plea for ‘contextual exceptionalism’. Our analysis concludes by reflecting on a real-world case and providing suggestions for contextual factors that researchers and oversight bodies should take into account when designing and evaluating research projects with health data from deceased subjects. Organ availability for transplantation has become an increasingly complex and difficult question in health economics and ethical practice. Advances in technology have seen prolonged life expectancy, and the global push for organs creates an ever-expanding gap between supply and demand, and a significant cost in bridging that gap. This article will examine the ethical implications for the nursing profession in regard to the procurement of organs from an impoverished seller’s market, also known as ‘Transplant Tourism’. This ethical dilemma concerns itself (...) with resource allocation, informed consent and the concepts of egalitarianism and libertarianism. Transplant Tourism is an unacceptable trespass against human dignity and rights from both a nursing and collective viewpoint. Currently, the Australian Nursing and Midwifery Council, the Royal college of Nursing Australia, The Royal College of Nursing (UK) and the American Nurses Association do not have position statements on transplant tourism, and this diminishes us as a force for change. It diminishes our role as advocates for the most marginalised in our world to have access to care and to choice and excludes us from a very contemporary real debate about the mismatch of organ demand and supply in our own communities. As a profession, we must have a voice in health policy and human rights, and according to our Code of Ethics in Australia and around the world, act to promote and protect the fundamental human right to healthcare and dignity. Objective: To analyse the time variation of topics in bioethical publications as a proxy of the relative importance.Methods: We searched the Medline database for bioethics publications using the words “ethics or bioethics”, and for 360 specific topics publications, associating Medical Subject Heading topic descriptors to those words. We calculated the ratio of bioethics publications to the total publications of Medline, and the ratio of each topic publications to the total bioethics publications, for five-year intervals, from 1970 to 2004. We calculated (...) the time variation of ratios, dividing the difference between the highest and lowest ratio of each topic by its highest ratio. Four topics were described, selected to illustrate different patterns of variation: “Induced Abortion”, “Conflict of Interest”, “Acquired Immunodeficiency Syndrome”, “Medical Education.”Results: The ratio of bioethics publications to total Medline publications increased from 0.003 to 0.012. The variation of the topic’s ratios was higher than 0.7 for 68% of the topics. The Induced Abortion ratios decreased from 0.12 to 0.02. Conflict of Interest ratios increased from zero to 0.07. The Acquired Immunodeficiency Syndrome ratios were nearly zero in the first three intervals, had a peak of 0.06 during 1985–9, followed by a decrease to 0.01. Medical Education ratios varied few, from 0.04 to 0.03.Conclusions: There was an increase of bioethical publications in the Medline database. The topics in bioethics literature have an important time variation. Some factors were suggested to explain this variation: current legal cases, resolution of the issue, saturation of a discussion and epidemiologic importance. (shrink)

Is it necessary to have an ideal of perfection in mind to identify and evaluate true biotechnological human “enhancements”, or can one do without? To answer this question we suggest employing the distinction between ideal and non-ideal theory, found in the debate in political philosophy about theories of justice: the distinctive views about whether one needs an idea of a perfectly just society or not when it comes to assessing the current situation and recommending steps to increase justice. In this (...) paper we argue that evaluating human enhancements from a non-ideal perspective has some serious shortcomings, which can be avoided when endorsing an ideal approach. Our argument starts from a definition of human enhancement as improvement, which can be understood in two ways. The first approach is backward-looking and assesses improvements with regard to a status quo ante. The second, a forward-looking approach, evaluates improvements with regard to their proximity to a goal or according to an ideal. After outlining the limitations of an exclusively backward-looking view, we answer possible objections against a forward-looking view. Ultimately, we argue that the human enhancement debate would lack some important moral insights if a forward-looking view of improvement is not taken into consideration. (shrink)

On theories in Indian Philosophical Systems.

It is shown, assuming the linear case of Schinzel's Hypothesis, that the first-order theory of the structure $\langle \omega; +, P\rangle$ , where P is the set of primes, is undecidable and, in fact, that multiplication of natural numbers is first-order definable in this structure. In the other direction, it is shown, from the same hypothesis, that the monadic second-order theory of $\langle\omega; S, P\rangle$ is decidable, where S is the successor function. The latter result is proved using a general (...) result of A. L. Semenov on decidability of monadic theories, and a proof of Semenov's result is presented. (shrink)

In lieu of an abstract, here is a brief excerpt of the content:BOOK REVIEWS 389 From the perspective of this reviewer, the presently most obvious fault in this lecture lies in Putnam's criticisms of "the coherence theorists, Ludwig, et al." (p. 97). In this criticism, it is apparently assumed that the adequacy of their proposed solution to the problem considered must be judged solely on the basis of what is presently known. Since Putnam himself acknowledges that no satisfactory interpretation of (...) quantum mechanics exists, this seems unwarranted. The superficiality of the criticism of this position makes this particularly notable. He states, for example, "to say that a wave gives an incomplete description of the state of a system is to say that systems whose state can be described by that wave function may really be in several quite different physical conditions." He then adopts the doubtful strategy of appealing to the meaning of completeness and incompleteness (they are not a matter of degree) in justification of the position that it is improper to say that a wave gives an incomplete descriptionl Despite the fact that, taken as a whole, his brief criticism of the coherence theory is somewhat ungracious. Putnam has set forth the principle issues sur~ rounding the problem with discerning lucidity. Paul K. Feyerabend, in "Problems of Empiricism," places a revisionist's version of empiricism in a carefully developed historical perspective. Empiricism as it has been known is both incomplete and weakened by undesirable assumptions. The incompleteness is to be removed by regarding empiricism as a cosmological hypothesis concerning the relation between man and the universe. The version of empiricism thus admitted has the character of a scientific theory. The undesirable assumptions are views no longer tenable of the nature and content of what is given in sense. Feyerbend understands sensations and perceptions as indicators on a par with indications of physical instruments, and likewise in need of interpretation. A philosophical method is proposed which requires theoretical pluralism instead of the theoretical monism that is (inappropriately) the ideal of much of contemporary empiricism (p. 149). The function of theoretical alternatives is to provide the means of criticizing accepted theory, to which he accords careful historical treatment. "Low level theories" are to guide us to crucial experiments, while "high level theories" are to represent the basic element of the universe. Here again the reader will find a theme which emerges erpeatedly in these lectures. This theme might be stated as follows. Both macrocosmic and microcosmic systems must ultimately be involved in scientific explanation. The crucial problem that emerges is how these two are to be related or in some degree integrated. By the way in which he develops the role of alternative theories, Feyerabend has affirmed one of the principle theoretical components of what Hegel was minded to call metaphysics. An additional observation, for which what I have said should furnish some justification: Many points made throughout this exposition (115 pp. in length) might properly be regarded as together constituting a rapprochement of empiricism and rationalism. It may very well occur to the thoughtful reader that Feyerabend, in his reconstruction of empiricism, has come very near to conceding all that a critical rationalist could ask. Other lectures included in the book are, "The Thermodynamics of Purpose," by David Hawkins, "The Physics of the Large," by Philip Morrison, and "The Ethical Dimension of Scientific Research," by Nicholas Rescher. In The Idea of a Social Science Winch, argues that, sociology is more properly conceived as a branch of philosophy than of empirical science. Winch falls victim here to the Humean assimilation of the empirical to the generalizable. He notes that much of our talk about social practice is in terms of conventions, so that explanations of social action can be given without recourse to statistical or experimental findings. But such talk depends nonetheless on the accuracy and detail with which (...) the situations in which actions occur are? recorded, and this is surely an empirical enterprise. It is the misleading conception of sociology as a discipline, characterized by common procedures, that leads Winch to espouse the assimilation of sociology to conceptual inquiry. We need to see instead that sociology embraces a group of questions and subjects so loosely connected that it would be mistaken to speak of, and idle to project, a procedure common to all of them. We show that that is a considerable under-estimate, the true number of symbols being that in the title, not counting 1 179 618 517 981 links between symbols that are needed to disambiguate the whole expression.

This chapter explores the philosophical development of Alexander’s empiricism, interest in experimental psychology, and its connection to realism and its subsequent rise in the early twentieth century. His early interests are most notably his rejection of British idealism and an empiricist tendency derived in part from classical empiricism and empirical developments in psychology. It is argued that Alexander arrives at realism through his work on psychology, thus revealing that the rise of realism has some psychological origins. His philosophical method is (...) also analysed in detail and it is shown that many of his arguments should be understood as following this method and that many objections to his arguments for realism and against his theory of perception can be resisted when this method is taken into consideration.
