Background Clinical empathy is an essential part of healthcare, and patient-centered care models require clinical empathy to be established. Despite this, little is known about its measurement in the neonatal scenario. Research Aim To measure clinical empathy in health professionals who work with medium and high-risk neonates and build a construct of this empathy. Research Design Single-center survey study. Participants and Research Context The Jefferson Scale of Empathy for Health Professionals questionnaire was applied to health professionals who work in an (...) intensive care unit and a medium-risk unit, in Brazil. Analysis was done using descriptive statistics and a factor analysis model, to build the construct of empathy. Overall empathy was calculated, and the domains’ punctuations were analyzed and compared to the maximum punctuation possible. The study followed the STROBE checklist. Ethical Considerations This study was approved by the Research Ethics Committee of the institution. All participants signed the informed consent form. Participants’ confidentiality and anonymity were protected. Findings Median empathy was 117 (IQR 113–124). The domain of Walking in the Patient’s Shoes had lower scores and represented 77.6% of the maximum punctuation possible. The factor analysis included three factors named Understanding, Experiences, and Treatment, and Emotional Relationships, explaining 64.3% of the overall variance. The domain Walking in the Patient’s Shoes was not included in the model. Conclusions In this scenario, clinical empathy should improve. There is a need to improve the domain of Walking in the Patient’s Shoes, in this case, the neonate, and provide more empathic care to them. (shrink)

O referido artigo propõe apresentar uma reflexão diante da contribuição de uma didática crítica intercultural e decolonial para a construção de novas formas de se viver em sociedade. Baseando na teoria de Vera Candau, o texto busca evidenciar a importância de uma educação que valorize a diversidade cultural, com ênfase na promoção da equidade racial, além dos desafios encontrados tanto nas questões estruturais da educação, tanto nas forças que buscam uma internacionalização e padronização de práticas pedagógicas. Para a exposição dessas (...) ideias, foi feita a análise de partes do Projeto Político Pedagógico (PPP) da Escola Afro-Brasileira Maria Felipa, localizada na cidade de Salvador, Bahia, cuja a intencionalidade da proposta pedagógica tem como intuito, desde sua concepção, em ser uma escola afro-brasileira. A escola possui um currículo voltado para a redução das desigualdades raciais, assim como na valorização e promoção da cultura preta e afro-brasileira. O texto ressalta ainda, a importância de práticas pedagógicas interculturais e decoloniais que celebrem as múltiplas identidades e culturas como maneira a se sonhar com futuros possíveis, assim com o papel crucial da formação continuada de professores, para que estes se sintam aptos e seguros para incluírem a diversidade como parte fundamental de uma educação transformadora. In recent years, supported by new scientific evidence, the conceptualization of cognitive reserve has been progressively enriched and now encompasses not only cognitive stimulating activities or educational level, but also lifestyle activities, such as leisure physical activity and socialization. In this context, there is increasing interest in understanding the role of psychological factors in brain health and cognitive functioning. In a previous study, we have found that these factors mediated the relationship between CR and self-reported cognitive functioning. In this study, (...) we have confirmed an association between two important constructs included in the psychological wellbeing and salutogenic models, “purpose in life” and “sense of coherence,” CR, as assessed using a questionnaire, and cognitive functioning, as evaluated using a comprehensive neuropsychological battery. Results from 888 middle-aged healthy participants from the Barcelona Brain Health Initiative indicate that both sense of coherence and CR were positively associated with verbal memory, reasoning and attention, working memory, and global cognition. Moreover, the relation between CR and cognitive functioning in the different domains is partially mediated by SoC. When we controlled for brain integrity, introducing into the model neurofilament light chain measures, the mediator role of SoC was confirmed for reasoning and attention and global cognition. However, purpose in life was not associated with cognitive functioning. These results reveal the central role of the SoC construct, which mediates the association between classic CR estimates and cognitive functions, potentially representing a modifiable target for interventions that aim to promote brain health. The purpose of this study is to understand institutional violence (IV) in the relationships between health professionals, hospitalized children, and family members. This is a qualitative study developed at the pediatric inpatient unit of a university hospital in the city of Salvador, Bahia, Brazil. The research participants consisted of 39 health professionals who specialized in pediatrics and 10 family members of hospitalized children. Semi‐structured interviews were the method used for data collection. Using discourse analysis as a basis and taking a (...) Foucauldian perspective, the researchers observed that the expressions of IV could be traced to abusive power relations within the system. We found four discursive forms within the data set: communication problems as IV, violence through inattention and neglect, violence as an action and consequent materialization on the body, and psychological violence as a submission mechanism. Based on these findings, we argue that professionals, managers, the scientific community, and users might be able to better guarantee the safety of children by recognizing IV and effectively intervening in it. Los caminos espinosos en la construcción del conocimiento son un cometido al que solo el animal humano se ha enfrentado. El uso de un lenguaje proposicional le ha permitido iniciar construcciones racionales, que dan origen a las teorías más complejas en pos de comprender los enigmas del universo, y con ello iniciar la separación tajante entre episteme y doxa. Los discursos más enmarañados, rígidos y rigurosos convocan a científicos y epistemólogos. Enunciados con términos que, en ocasiones, se tornan oscuros y (...) poco comprensibles a quienes inician sus procesos formativos en el campo de la ciencia y la filosofía. Tal vez por ello se presenta un contraste actual en la enseñanza de las ciencias: por una parte, quienes apuestan por enseñar la rigidez de los enunciados científicos, y la conciben bajo un lenguaje unívoco, neutral y de carácter lógico-deductivo; y por otro lado, quienes la humanizan desde dinámicas móviles y cimentaciones semánticas artificiosas, las cuales, se alejan —como lo expone Héctor Palma en el prólogo del libro Bestiario epistemológico. Metáforas zoomórficas y de otras entidades en la enseñanza de las ciencias y la epistemología– de los enunciados depurados y se valen de nuevos discursos desde el ámbito literario, en los que abundan las metáforas. The US healthcare industry emits an estimated 479 million tonnes of carbon dioxide each year; nearly 8% of the country’s total emissions. When assessed by sector, hospital care, clinical services, medical structures, and pharmaceuticals are the top emitters. For 15 years, research has been dedicated to the medical structures and equipment that contribute to carbon emissions. More recently, hospital care and clinical services have been examined. However, the carbon of pharmaceuticals is understudied. This article will focus on the carbon emissions (...) of pharmaceuticals since they are consistently calculated to be among the top contributors to healthcare carbon and assess the factors that contribute to pharmaceutical carbon emissions. Specifically, overprescription, pharmaceutical waste, antibiotic resistance, routine prescriptions, non-adherence, drug dependency, lifestyle prescriptions, and drugs given due to a lack of preventive healthcare will be identified. Prescribing practices have environmental ramifications. Carbon reduction, when focused on pharmaceuticals, can lead to cleaner, more sustainable healthcare. (shrink)

How should citizens respond to dirty-hands acts? This issue has been neglected in the theoretical literature, which has focused on the dilemma facing the politician and not on the appropriate responses of citizens. Nevertheless, dirty-hands scenarios pose a serious dilemma for the democratic citizens as well: we cannot simply condone the dirtyhanded act but should instead express our moral condemnation and disapproval. One way of doing this is through blame and punishment. However, this proposal is unsatisfactory, as dirty-hands agents commit (...) wrongdoing through no fault of their own. I argue that we ought to make conceptual space for an idea of no-fault responsibility – and a corresponding notion of no-fault forgiveness – according to which we can hold agents to obligations without blaming them. What type of speech act is a norm of action, when the norm is agreed upon as the conclusion of an argumentative dialogue? My hypothesis is that, whenever a norm of action is the conclusion of an argument, it should be analyzed as the statement of a norm and thus as a verdictive speech act. If the context is appropriate, and the interlocutors are sincerely (or institutionally) committed to their argumentative exchange and its conclusion, then this verdictive motivates and institutes (...) a new one with the force of an exercitive. The interlocutors’ recognition and acceptance that the new illocution has been performed lends the norm its exercitive force. (shrink)

Previous literature has demonstrated the important role that trust plays in developing and maintaining well-functioning societies. However, if we are to learn how to increase levels of trust in society, we must first understand why people choose to trust others. One potential answer to this is that people view trust as normative: there is a social norm for trusting that imposes punishment for noncompliance. To test this, we report data from a survey with salient rewards to elicit people’s attitudes regarding (...) the punishment of distrusting behavior in a trust game. Our results show that people do not behave as though trust is a norm. Our participants expected that most people would not punish untrusting investors, regardless of whether the potential trustee was a stranger or a friend. In contrast, our participants behaved as though being trustworthy is a norm. Most participants believed that most people would punish someone who failed to reciprocate a stranger’s or a friend’s trust. We conclude that, while we were able to reproduce previous results establishing that there is a norm of reciprocity, we found no evidence for a corresponding norm of trust, even among friends. In a dissonance case, a person sincerely and with conviction asserts that P, while his/her overall automatic behavior suggests that he/she believes that not-P. According to Schwitzgebel, this is a case of in-between believing. This article raises several concerns about Schwitzgebel's account and proposes an alternative view. I argue that the in-between approach yields incorrect results in belief self-ascriptions and does not capture the psychological conflict underlying the individual's dissonance. I advance the view that in relevant cases the dissonant individual (...) has two mutually contradictory beliefs. (shrink)

Fibring is recognized as one of the main mechanisms in combining logics, with great signicance in the theory and applications of mathematical logic. However, an open challenge to bring is posed by the collapsing problem: even when no symbols are shared, certain combinations of logics simply collapse to one of them, indicating that bring imposes unwanted interconnections between the given logics. Modulated bring allows a ner control of the combination, solving the collapsing problem both at the semantic and deductive levels. (...) The aim of this paper is to explore the viability of accounting for presumptions as a subtype of verdictives, within the framework of the Austinian approach to speech acts. The available set of felicity conditions is examined and worked out, in order to try and account in particular for a main feature of presumptions, namely, their function in shifting the burden of proof. In this essay I analyze some conceptual difficulties associated with the demand that global institutions be made more democratically accountable. In the absence of a world state, it may seem inconsistent to insist that global institutions be accountable to all those subject to their decisions while also insisting that the members of these institutions, as representatives of states, simultaneously remain accountable to the citizens of their own countries for the special responsibilities they have towards them. This difficulty seems insurmountable in (...) light of the widespread acceptance of a state-centric conception of human rights, according to which states and only states bear primary responsibility for the protection of their citizens' rights. Against this conception, I argue that in light of the current structures of global governance the monistic ascription of human rights obligations to states is no longer plausible. Under current conditions, states are bound to fail in their ability to protect the human rights of their citizens whenever potential violations either stem from transnational regulations or are perpetrated by non-state actors. In order to show the plausibility of an alternative, pluralist conception of human rights obligations I turn to the current debate among scholars of international law regarding the human rights obligations of non-state actors. I document the various ways in which these obligations could be legally entrenched in global financial institutions such as the WTO, the IMF and the World Bank. These examples indicate feasible methods for strengthening the democratic accountability of these institutions while also respecting the accountability that participating member states owe to their own citizens. I conclude that, once the distinctions between the obligations to respect, protect and fulfill human rights are taken into account, no conceptual difficulty remains in holding states and non-state actors accountable for their respective human rights obligations. This paper endorses a view of argumentation and arguments that relates both to a special type of speech action, namely, the performance of speech acts of arguing. Its aim is to advance an analysis of those acts that takes into account two kinds of norms related to their correct performance, namely, felicity conditions and objective requirements related to the “correspondence with the facts.” It assumes that the requirement that certain objective conditions be satisfied is among the set of felicity conditions (...) of speech acts of arguing. Taking this into account helps clarify the position and role of warrants in the performance and assessment of these acts. (shrink)

We present five studies investigating the effects of approach and avoidance behaviours when individuals do not enact them but, instead, learn that others have performed them. In Experiment 1, when participants read that a fictitious character (model) had approached a previously unknown product, they ascribed to this model a liking for the object. In contrast, they ascribed to the model a disliking for the avoided product. In Experiment 2, this result emerged, with a smaller effect size, even when it was (...) clear that the behaviours followed specific instructions from a third party. The model had been a mere executor instead of behaving autonomously. Finally, in Experiments 3, 4, and 5, we showed, with direct and indirect measures of attitudes, that reading that the model had approached vs avoided products was sufficient to create preferences in the participant for the approached one, regardless of whether it was explained that the model was a mere executor. This research highlights the largely unexplored effects of vicarious approach/avoidance behaviours. Theoretical and practical implications and possible developments of this line of research are discussed. (shrink)

Over the last half century, United States debates about abortion focused at first on the question whether the fetus is a person with rights and later on whether involuntary conception—for instance, as a consequence of sexual assault—might mitigate a woman’s responsibilities toward the fetus she carries. This article argues that, whatever one’s position on these two questions, a third, morally salient dimension of most US women’s experiences of unwanted pregnancy deserves more attention: both abortion and birth burden women with their (...) inevitable moral failure to fulfill their responsibilities to persons who frequently have de facto last‐resort and unavoidable claims on them. Using Lisa Tessman’s work on moral failure and Pope Francis’s interventions on abortion and mercy, I argue that this moral anguish is not a simple emotional remainder. Structural evil, not necessity, is the primary driver of forced pregnancy choices that injure women and their children both materially and morally. Consequently, whether they abort or carry to term, women with unwanted pregnancies need mercy or forgiveness. But they also need compassionate solidarity: prophetic, active efforts to transform the social structures that make material harm and moral failure, and consequent moral anguish and moral injury, inevitable for many pregnant women. Within the Gricean framework in pragmatics, communication is understood as an inferential activity. Other approaches to the study of linguistic communication have contended that language is argumentative in some essential sense. My aim is to study the question of whether and how the practices of inferring and arguing can be taken to contribute to meaning in linguistic communication. I shall suggest a two-fold hypothesis. First, what makes of communication an inferential activity is given with its calculability, i.e. with the possibility (...) to rationally recover the assigned meaning by means of an explicit inference. Secondly, the normative positions that we recognize and assign each other with our speech acts comprise obligations and rights of a dialectical character; but this fact does not entail nor presuppose an argumentative nature in language or speech. Both inferring and arguing are needed, however, in the activity of justifying and assessing our speech acts. (shrink)

Experimental results in Ultimatum, Trust and Social Dilemma games have been interpreted as showing that individuals are, by and large, not driven by selfish motives. But we do not need experiments to know that. In our view, what the experiments show is that the typical economic auxiliary hypothesis of non-tuism should not be generalized to other contexts. Indeed, we know that when the experimental situation is framed as a market interaction, participants will be more inclined to keep more money, share (...) less, and disregard other participants’ welfare [Hoffman et al., 1994]. When the same game is framed as a fair division one, participants overall show a much greater concern for the other parties’ interests. The data thus indicate that the context of an interaction is of paramount importance in eliciting different motives. The challenge then is to model utility functions that are general enough to subsume a variety of motives and specific enough to allow for meaningful, interesting predictions to be made. For the sake of simplicity (and brevity), in what follows we will concentrate upon the results of experiments that show what appears to be individuals’ disposition to behave in a fair manner in a variety of circumstances [Camerer, 2003]., though what we are saying can be easily applied to other research areas. Such experimental results have been variously interpreted, each interpretation being accompanied by a specific utility function. We shall consider three such functions and the underlying interpretations that support them, and assess each one on the basis of what they claim to be able to explain and predict. (shrink)

Few have entertained the idea that Georg Cantor, the creator of set theory, might have influenced Edmund Husserl, the founder of the phenomenological movement. Yet an exchange of ideas took place between them when Cantor was at the height of his creative powers and Husserl in the throes of an intellectual struggle during which his ideas were particularly malleable and changed considerably and definitively. Here their writings are examined to show how Husserl's and Cantor's ideas overlapped and crisscrossed in the (...) areas of philosophy and mathematics, arithmetization, abstraction, consciousness and pure logic, psychologism, metaphysical idealism, new numbers, and sets and manifolds. In his Democracy across borders, Bohman articulates an ambitious political proposal for a future international order. Perhaps its most salient feature is the promise of global democracy without a world government. Global democracy is usually associated with the ideal of a world community unified under a set of global democratic institutions. Fear of the totalitarian consequences that such a concentration of power would generate often leads even the staunchest cosmopolitans to limit their democratic aspirations to the national level and merely (...) hope for the progressive implementation of the rule of law at the global level. In his book, Bohman tries to break with the widespread assumption that an increase in democratization across national borders must be purchased at the price of a concentration of power that dangerously increases the potential for political domination. According to Bohman, this assumption is rooted in a narrow understanding of the democratic ideal - an understanding that needs to be replaced by a more complex one. By drawing attention to these facts and to the relationship between Cantor’s and Husserl's ideas, I have tried to contribute to putting Frege's attack on Husserl "in the proper light" by providing some insight into some of the issues underling criticisms which Frege himself suggested were not purely aimed at Husserl's book. I have tried to undermine the popular idea that Frege's review of the Philosophy of Arithmetic is a straightforward, objective assessment of Husserl’s book, and to give some specific (...) reasons for thinking that the uncritical reading of Frege's review has unfairly distorted philosophers' perception of a work they do not know very well. Discursive theories of justice have been questioned for putting forward high-level principles that should nevertheless play a role in practical discourses in which the justice of a claim is at stake. Here, I will critically examine and systematize the main tenets in Rawls’s and Habermas’s discursive theories, and will suggest that the principles of justice (Rawls) and universalization (Habermas) can and play the role of mandates of optimalization in real deliberations on justice.

Whenever the government makes medical resource allocation choices, there will be opportunity costs associated with those choices: some patients will have treatment and live longer, while a different group of patients will die prematurely. Because of this, we have to make sure that the benefits we get from investing in treatment A are large enough to justify the benefits forgone from not investing in the next best alternative, treatment B. There has been an increase in spending and reallocation of resources (...) during the COVID-19 pandemic that may have been warranted given the urgency of the situation. However, these actions do not bypass the opportunity cost principle although they can appear to in the short term, since spending increases cannot continue indefinitely and there are patient groups who lose out when resources are redirected to pandemic services. Therefore, policy-makers must consider who bears the cost of the displaced healthcare resources. Failure to do so runs a risk of reducing overall population health while disproportionally worsening health in socially disadvantaged groups. We give the example of ethnic minorities in England who already had the worst health and, due to structural injustices, were hardest hit by the pandemic and may stand to lose the most when services are reallocated to meet the resource demands of the crisis. How can we prevent this form of health inequity? Our proposal is forward-looking: we suggest that the government should invest our resources wisely while taking issues of equity into account–that is, introduce cost–equity analysis. (shrink)

We report the results of an experiment designed to determine the effects of psychological proximity—proxied by awareness of pain and friendship—on moral reasoning. Our study tests the hypotheses that a moral agent’s emphasis on justice decreases with proximity, while his/her emphasis on care increases. Our study further examines how personality, gender, and managerial status affect the importance of care and justice in moral reasoning. We find support for the main hypotheses. We also find that care should be split into two (...) components, one related to protection and the other to the preservation of relationships. Although gender does not affect moral reasoning directly, we find that it does so indirectly via personality, controlling for age, professional status, and professional background. We do not find a significant effect of managerial status on ethics of justice, but do find that holding a managerial position has a negative impact on ethics of care. Regarding personality, we detect significant positive effects of conscientiousness on ethics of justice and of neuroticism on ethics of care. (shrink)

Two hundred years ago, J.M.W. Turner packed up two large leatherbound sketchbooks, pencils, and watercolors and set off for the north of England. When he returned from the tour that he regarded as one of the most important of his career, Turner had completed more than two hundred sketches - works that later became the basis of more than fifty major oil paintings and watercolors. For this illustrated book, David Hill has taken photographs of many of the actual sites Turner (...) sketched on his northern tour. En este artículo se argumenta contra la interpretación metaética del kantianismo moral como una forma de constructivismo o antirealismo moral. Dado que los kantianos no comparten el expresivismo característico del antirealismo moral estándar, el constructivismo kantiano parece llevar a una posición inherentemente inestable que sólo puede desarrollarse o bien en un realismo consistente con el cognitivismo moral kantiano o en un decidido antirealismo moral. Tomando la ética del discurso de Habermas como ejemplo, aquí se contrasta una interpretación realista con una (...) interpretación antirealista del principio moral de universalización con la intención de mostrar que sólo la primera es compatible con el cognitivismo moral característico de las éticas kantianas, mientras que la segunda lleva inevitablemente a un planteamiento decisionista y, con ello, a un claro relativismo moral. There has been increased interest in the potential of maternal immunisation to protect maternal, fetal, and infant health. Maternal tetanus vaccination is part of routine antenatal care and immunisation campaigns in many countries, and it has played an important part in the reduction of maternal and neonatal tetanus. Additional vaccines that have been recommended for routine maternal immunisation include those for influenza and pertussis, and other vaccines are being developed. Maternal immunisation is controversial since regulators, professionals, and the public are (...) often reluctant to accept pharmaceutical interventions during pregnancy. So far, little attention has been given to the ethics of vaccination during pregnancy. In this Personal View we argue that maternal immunisation should be offered in response to concrete, severe risks of disease for mother and child, and we explain how this requirement of serious risk can be used to guide ethical decision-making about maternal immunisation. The present paper aims at presenting Averroes’ doctrine of act and potency in the framework of his general conception of metaphysics as a science. By tracing the origins of his doctrine back to Alexander of Aphrodisias, it shows that Averroes conceives act and potency as concomitant attributes of being qua being and as terms πρὸς ἕν and ἀφ’ ἑνός. According to this reading, the study of these two notions, considered as such, constitutes an essential step in Averroes’ metaphysical project, whose (...) ultimate goal is to account for the essence of the first of all forms: God. Partiendo del conocimiento que los árabes tuvieron de los textos neoplatónicos atribuidos a Aristóteles, tales como la Pseudo-Teología y el Liber de Causis, la autora de este estudio investiga el posible conocimiento que Avicena tuvo de este último libro, conocido en el mundo árabe por el título de Kalam fi mahd al-hayr. Se apoya, para ello, en el análisis de cuatro pasajes de la Metafísica de la gran enciclopedia filosófica Al-Sifa ("La curación").

O artigo propõe a análise, em espectro comparativo, de duas cenas da comédia aristofânica Acarnenses, a saber, a entrada de Anfíteo e o discurso dos embaixadores. Por meio da contraposição desses dois momentos da peça, sustento a ideia de que o instrumento de decisão democrática, a assembleia, constitui-se como terreno profícuo para a censura de pautas que privilegiem a aplicação de políticas dirigidas para o bem comum, ao mesmo tempo em que consagra a liberdade da fala dos que apresentam agendas (...) personalistas às custas da miséria do demos. Assim, funcionando como cortina de fumaça para o enriquecimento de líderes políticos, o discurso que apoia o prosseguimento da Guerra do Peloponeso é o único para o qual há liberdade de expressão, enquanto seu contrário, o discurso pacifista, deve ser censurado, até mesmo com o uso da violência. La denominada desigualdad vertical es uno de los temas centrales del debate internacional de los últimos años ya que su desmedido crecimiento ha alcanzado límites intolerables en términos éticos y económicos que no pueden ser ya ignorados por nadie. Al respecto se discute la manera de abordar el problema. En concreto el movimiento de los derechos considera que puede ofrecer herramientas útiles para enfrentar esta injusta situación, mientras otros dudan de la relación entre desigualdad económica y derechos humanos y de (...) la capacidad de los derechos humanos para luchar frente a este tipo de desigualdad. En esta tesitura se pueden distinguir posiciones “en contra”, “a favor” e “intermedias”. Analizar atentamente este debate y los conceptos que en él se manejan es de fundamental relevancia para valorar el problema de raíz que está detrás de los llamados derechos económicos, sociales y culturales, y que tiene que ver entre otras cosas con un problema de concepto y fundamentación de los derechos. Al respecto se destacan los argumentos esenciales y se muestran críticamente las definiciones y consideraciones más cuestionables con el objetivo de afianzar a continuación una noción de derechos sociales más resistente. Una solución razonable pasa por combinar el proyecto de suficiencia con el proyecto de igualdad volviendo a las bases del auténtico Estado social de Derecho que dio origen a estos derechos y que los configura como derechos de dignidad basados en la libertad real, la igualdad sustancial y también la solidaridad, ésta última prácticamente ausente en el debate.
