BackgroundIn the Canadian Alliance for Healthy Hearts and Minds (CAHHM) cohort, participants underwent magnetic resonance imaging (MRI) of the brain, heart, and abdomen, that generated incidental findings (IFs). The approach to managing these unexpected results remain a complex issue. Our objectives were to describe the CAHHM policy for the management of IFs, to understand the impact of disclosing IFs to healthy research participants, and to reflect on the ethical obligations of researchers in future MRI studies.MethodsBetween 2013 and 2019, 8252 participants (...) (mean age 58 ± 9 years, 54% women) were recruited with a follow-up questionnaire administered to 909 participants (40% response rate) at 1-year. The CAHHM policy followed a restricted approach, whereby routine feedback on IFs was not provided. Only IFs of severe structural abnormalities were reported.ResultsSevere structural abnormalities occurred in 8.3% (95% confidence interval 7.7–8.9%) of participants, with the highest proportions found in the brain (4.2%) and abdomen (3.1%). The majority of participants (97%) informed of an IF reported no change in quality of life, with 3% of participants reporting that the knowledge of an IF negatively impacted their quality of life. Furthermore, 50% reported increased stress in learning about an IF, and in 95%, the discovery of an IF did not adversely impact his/her life insurance policy. Most participants (90%) would enrol in the study again and perceived the MRI scan to be beneficial, regardless of whether they were informed of IFs. While the implications of a restricted approach to IF management was perceived to be mostly positive, a degree of diagnostic misconception was present amongst participants, indicating the importance of a more thorough consent process to support participant autonomy.ConclusionThe management of IFs from research MRI scans remain a challenging issue, as participants may experience stress and a reduced quality of life when IFs are disclosed. The restricted approach to IF management in CAHHM demonstrated a fair fulfillment of the overarching ethical principles of respect for autonomy, concern for wellbeing, and justice. The global pandemic needs to mark a turning point for the peoples of Aotearoa New Zealand. How can we make sure that our culturally diverse nation charts an equitable and sustainable path through and beyond this new world? In a less affluent future, how can we ensure that all New Zealanders have fair access to opportunities? One challenge is to preserve the sense of common purpose so critical to protecting each other in the face of Covid-19. How can we centre (...) what we have learnt about resilience within Māori and wider Pacific communities in our reforms? How can public understanding of Covid-19 science create a platform for the future social valuing of expertise? How can we ensure that the impact of Covid-19 in New Zealand results in a more sustainable, and inclusive workforce–for instance by expanding our perceptions of the value of our workers through promoting digital inclusion? To meet these challenges, we must reimagine our existing traditions of thought, breathing new life into perennial concepts and debates. Edgar: Or image of that horror?—King Lear, Act 5 scene 3... the raging and incredulous recounting (which enables man to bear with living)...—Faulkner, Absalom, Absalom! 161Psychoanalysis has not been brought to the bear on the study of Roman culture as thoroughly as it has engaged Hellenic studies, and to date most work has consisted of the psychoanalytic (...) literary study of symbolism or character, as witnessed recently by the Vergilian studies of Gillis (Eros), Putnam (“Possessiveness”), and Mitchell (“Violence”). 1 These studies have often yielded important insights into the Aeneid, and while they provide a foundation for the present examination, I believe that we can now further develop this line of thought by engaging such approaches as the psychoanalytic dynamics of reading, or the function of language in the unconscious, areas outlined in the important collection edited by Felman (Literature and Psychoanalysis) which presents Peter Brooks’ preliminary version of a Freudian practice of reading narrative. 2 In this paper I shall investigate Vergil’s narrative project in the light of [End Page 289] the model proposed more completely by Brooks in Reading for the Plot, whose focus on issues central to reading the Aeneid, such as the desire for the end and the function of repetition, could further our understanding of how the Aeneid works and why, in particular, its ending is so disturbing. 3 I intend this study as a contribution to the larger discussion of closure in classical texts that Don Fowler initiated recently in a more general survey of the prospects for such endeavors. 4 The Aeneid, I submit, problematizes the idea of an end through its deployment of the word finis, and then frustrates the reader’s desire for diegetic closure by merely stopping, not ending, the narrative, despite clear signals of its completion. 5 This inquiry thus engages the techniques of traditional literary formalism in the service of a text-centered psychoanalytic theory of narrative in the belief that the older forms of literary formalism and psychoanalysis are occasionally deficient in their rigidity, their reductiveness, their lack of concern with the relationship between text and reader, and simply because the older forms of psychoanalytic literary theory have been at times, in Brooks’ words (Psychoanalysis and Storytelling 20), “something of an embarrassment.”Far from the idea that a plot is a static self-sufficient entity, Brooks sees narrative emplotment as “a form of desire that carries us forward, onward through the text” (Reading for the Plot 37) in pursuit of meaning, a pursuit that gives pleasure. Stemming from the Freudian Eros of Beyond the Pleasure Principle, this desire seeks, as Brooks quotes Freud (37), “to combine organic substances into ever greater unities.” Our need for coherence, for knowledge, drives us continually toward discovering the “Masterplot” of the origins, unfoldings and ends of the plots of our lives and our stories. A plot’s beginning arouses certain desires and expectations that carry the reader to the end. “The sense of a beginning” [End Page 290] is “determined by the sense of an ending,” and present moments have “narrative meaning only because we read them in anticipation of the structuring power of those endings that will retrospectively give them the order and significance of plot” (94). Repetition, one of the most powerful narrative tools for the pleasurable ordering of the plot, can also be a disruptive, painful force; Brooks recalls Freud’s account of the child’s game of throwing and returning a toy, an activity designed to master the unpleasant, regular disappearance of the mother. Similarly, neurotic patients, in narrating dreams, repeat distressing trauma, attempting to move from passivity to mastery. The pleasure principle thus conflicts with, and often yields to, the repetition compulsion. Narrative repetition both advances the plot and returns it to its origins, or it is a return of some repressed material. Repetition functions as a “binding of textual energies that allows them to be mastered by putting them into serviceable form” (94). DODDSARGUED that the first two hypotheses of Plato's Parmenides are the primary source of Plotinus' doctrines of the One and of Nous. I Dodds' main evidence was a list of parallels between the Parmenides and the Enneads? He argued further that the Neoplatonic interpretation of the Parmenides as positive metaphysics was neo-Pythagorean in origin. Several Plotinus scholars have (...) accepted and extended Dodds' view of the importance of the Parmenides for Plotinus. This group includes ]~mile Br6hier, ~ A. H. This article explores notions about Nietzsche’s career as a philologist and his fascination with the Greeks. It considers his interest in Homer and the Greek philosophers—in particular, Heraclitus and Pyrrho. For Nietzsche, ancient Greeks such as Heraclitus and Homer were interesting not because of their doctrines, but because of the example they themselves provided of certain psychological types. Like the ancient skeptics following Pyrrho, Nietzsche was generally more interested in the psychological consequences of philosophical doctrines than in their content, and (...) like those skeptics he often rejected any ambitions to limn the true nature of reality. Is there a rational and ethical basis for efforts to rescue individuals in dire straits? When does rescue have ethical support, and when does it reflect an irrational impulse? This paper defines a Rule of Rescue and shows its intuitive appeal. It then proceeds to argue that this rule lacks support from standard principles of justice and from ethical principles more broadly, and should be rejected in many situations. I distinguish between agent-relative and agent-neutral reasons, and argue that the Rule (...) of Rescue qualifies only in a narrow range of cases where agent-relative considerations apply. I conclude that it would be wise to set aside the Rule of Rescue in many cases, especially those involving public policies, where it has only weak normative justification. This paper argues the case for coming to see ‘medical politics’ as a topic or subject within medical education. First, its absence is noted from the wide array of paramedical subjects (medical ethics, history of medicine, the medical humanities, etc) currently given attention in both the medical education literature and in specific curricula. Second the author suggests that ‘the political’ is implicitly recognisable in the historical roots of medical ethics education, specifically in certain of the London Medical Group's activities, and (...) also that the medical profession, or indeed any profession, cannot be understood as an apolitical form of social organisation either in its institutional or scientific (epistemic) forms. Some brief suggestions for introductory and advanced topics in medical politics are discussed and the degree to which medical politics ought to be taken seriously and delivered as part of medical education is considered. Ultimately the author concludes that medical politics might be considered a useful subject within medical education, but it is perhaps best understood as a perspective or approach that can contribute to the development of a more expansive perspective within the extant paramedical subjects. And that is just how rationality works: it wants to leave no loose ends to understanding, seeking to enable us to understand things through to the bitter end. In the twelve chapters that make up Satisfying Reason, Rescher develops and defends the following perspective: That rationality is a cardinal virtue in cognitive matters. That this (...) is not something simple and cut-and-dried: in the pursuit of truth through the development of knowledge we face obstacles -- sometimes even insuperable ones. All that we can do is the best we can, realizing that even our very best may still be imperfect. Nevertheless, the venture is far from hopeless. While absolutes are unattainable in the cognitive venture, some solutions are situationally optimal, being comparatively the best that can be managed under the circumstances. That reason itself enables us to come to terms with this state of affairs, urging us to accept the best we can do as good enough. Satisfying Reason is an explanation of the presuppositions and methods of rational enquiry, an original exercise in metaknowledge, developing a systematic body of knowledge about the scope and limits of knowledge itself. Consumer involvement in clinical research is an essential component of a comprehensive response during emergent health challenges. During the COVID-19 pandemic, the moderation of research policies and regulation to facilitate research may raise ethical issues. Meaningful, diverse consumer involvement can help to identify practical approaches to prioritize, design, and conduct rapidly developed clinical research amid current events. Consumer involvement might also elucidate the acceptability of flexible ethics review approaches that aim to protect participants whilst being sensitive to the challenging context (...) in which research is taking place. This article describes the main ethical challenges arising from pandemic research and how involving consumers and the community could enable resolution of such issues. (shrink)

This paper proposes an ethical framework for rationing publicly financed health care. We begin by classifying alternative rationing criteria according to their ethical basis. We then examine the ethical arguments for four rationing criteria. These alternatives include rationing high technology services, non-basic services, services to patients who receive the least medical benefit, and services that are not equally available to all. We submit that a just health care system will not limit basic health care to persons unable to pay for (...) it. Furthermore, justice in health care requires limiting publicly-financed non-basic health care, striving for equality in access to basic health care, and relying on medical benefit to ration non-basic health care.
