Background To characterize patients dying in a community hospital with or without attempting cardiopulmonary resuscitation and to describe patient involvement in, documentation of, and compliance with decisions on resuscitation. Methods All patients who died in Kalmar County Hospital during January 1, 2016 until December 31, 2016 were included. All information from the patients’ electronic chart was analysed. Results Of 660 patients female), 30 were pronounced dead in the emergency department after out-of-hospital CPR. Of the remaining 630 patients a DNAR order (...) had been documented in 558 patients. Seventy had no DNAR order and 2 an explicit order to do CPR. In 43 of these 70 patients CPR was unsuccessfully attempted while the remaining 27 patients died without attempting CPR. In 2 of 558 patients CPR was attempted despite a DNAR order in place. In 412 patients the DNAR order had not been discussed with neither patient nor family/friends. Moreover, in 75 cases neither patient nor family/friends were even informed about the decision on code status. Conclusions In general, a large percentage of patients in our study had a DNAR order in place. However, 27 patients died without CPR attempt or DNAR order. DNAR orders had not been discussed with the patient/surrogate in almost three fourths of the patients. Further work has to be done to elucidate the barriers to discussions of CPR decisions with the patient. (shrink)

Cette note de recherche prend pour point de départ un certain nombre d’observations empiriques sur les pratiques d’écritures relatives à la « rédaction web ». Son ancrage est celui de l’analyse du discours, envisagée comme discipline interprétative et lecture critique des énoncés mais aussi comme démarche susceptible d’apporter sa contribution à une didactique de l’écrit. Nous y livrons quelques premiers éléments de réflexion pour la caractérisation discursive des textes issus de la rédaction web, prenant en compte pour cela à la (...) fois la composante discursive de ces productions et les discours construisant la rédaction web comme pratique d’écriture. A partir de deux exemples, nous montrons que les normes dominantes de la rédaction web, liées notamment aux contraintes du support et à des enjeux économiques, sont diversement prises en compte suivant la sphère d'activité où sont produits les textes. Après avoir présenté l’institution récente de la kafâla, qui fut légalisée en Algérie en 1984 et au Maroc en 1993 en faveur des enfants abandonnés et en substitution au modèle prohibé de l’adoption, l’article propose une analyse des données recueillies lors d’une enquête ethnographique menée de 2005 à 2009. Elle révèle que l’enfant illégitime encourt davantage le risque d’être abandonné à la naissance s’il est de sexe masculin. En revanche, s’il est de sexe féminin, il bénéficie de plus de chances (...) de faire l’objet d’un don provisoire, direct et négocié, et surtout d’être adopté ou recueilli en kafâla suite à son abandon en institution. Dall-E2 and Stable Diffusion promote their text-to-image models based on their level of (photo)realism. The use of photographic language is not superficial or accidental but indicative of a broader tendency in computer science and data practice. To nuance the general application of photorealism, I position the term alongside photographic realism and computational photorealism. To contextualize important nuances between these three terms, contemporary examples from astrophotography are analysed and reconstructed using text-to-image models. From the comparative analysis, computational photorealism emerges as a (...) modified term that recognizes the relationship between photography and text-to-image models without conflating their ontological and epistemological differences. Through a fieldwork study of the practice of methadone substitution at a French addiction clinic, the classic ‘theory of action’ is criticized and an alternative one proposed. A sketch of the debates among French drug specialists, drug users and legislators shows the pertinence for these practitioners of such a theoretical question; further, the ethnographic data suggest that they can be seen as actually trying out an alternative to this ‘theory of action’.

The idea that mythos and logos are incompatible, and that truth is a product of scientific and dialectical thinking, was certainly disproven by later Platonic philosophers. Deploying the works of Hesiod and Homer, Homeric Hymns and other such literature, they considered myth a valuable and significant augment to philosophical discourse. Plato’s denigration of myth gave his followers an incentive to read myth as allegory. The Stoics and first-century philosophers such as Philo, treated allegory as a legitimate interpretive strategy. The Middle (...) Platonists incorporated myth, for example, deifying the Monad and Dyad, as did 2nd century Platonists. Plutarch’s Isis and Osiris, for example, equates Isis and Osiris with form and matter: the god sows in matter logoi from himself. Porphyry’s allegorizing of Plato’s Cave of the Nymphs is another example. Plotinus is a strong influence on how the late Neoplatonists regarded myth. This paper argues that these philosophers’ use of allegory prepared the way for the Neoplatonists treatment of myth as inspired symbolism. Proclus and Syrianus, as reported by Hermias, did something more extreme by using mythology to construct inspired symbolic argument. Mythos becomes another type of logos, a vehicle for representing the invisible world of being, another kind of truth that can even serve a function in anagogic ascent. (shrink)

The humanist theory of the nude is one of the places where what can be called a ‘poor metaphysics’ developed during the Renaissance. To construct the concept of the nude as a representation of man in his own right, art theorists used common scholastic categories such as substance and accident, form and matter, potentiality and actuality, quantity and quality, whole and part, soul and body. Resolutely poor in its object – the human body, the work of art – and in (...) its form – technical treatise, fictional dialogue, or simple working notes – this reflection is nonetheless rich and original because of what constitutes its very weakness: the contamination of the Aristotelian metaphysical tradition with Neoplatonism, Vitruvianism, elements of natural philosophy, musical theory, and even Kabbalah. It testifies less to the permanence of scholastic metaphysics during the Renaissance than to the ingenious adaptation of its tools to new, humbler, and more rebellious objects of thought. (shrink)

Chora – le «cratère à mélanger» maternel, vannant et secouant de Platon – remplit «l’écart explicatif» entre les paradigmes formels «intelligibles et toujours existants» (48E5) et un monde encosmique «généré et visible». Proclus traite la gamme polysémique des termes utilisés par Platon pour chôra : hypodochê (réceptacle), kratêr (cratère à mélanger), etc., comme désignant des forces actives dans un univers où la sympathie cosmique règne, à partir des plus élevées, jusqu’aux plus basses manifestations de l’ «Un» transcendant. L’univers proclusien est (...) un «hénothéisme», dans lequel une «unité» dominante surgit sur la totalité de l’Être et du Devenir. Chôra permet à la fois au monde immatériel et au monde matériel de recevoir, de manière appropriée à chacun, les mêmes dons transcendants que leurs homologues intellectifs‑intelligibles superieurs. Aucun niveau de la réalité n’est dépourvu de cette donation. La réceptivité (hypodochê), indéterminée, mais toujours active, est présente à tous les niveaux. L’âme reçoit l’Intellect, la temporalité reçoit le temps monadique, la matière reçoit la structure géométrique polyédrique, etc. Chôra n’est pas l’equivalent de la matière, mais permet à la matière d’être imprimée par des paradigmes formels. Dans son rôle de «cratère à mélanger», elle protège la matière de sa potentielle itérabilité et dissolution infinie. Dans l’univers proclusien, le Bien atteint la dernière et la plus extrême de ses limites, il est espace et lieu, recevant à la fois l’expansivité et une capacité à donner place à la forme pour qu’elle puisse s’imposer à la matière. Après tout, les dons lui sont accordés depuis la plus haute hypostase, celle de la Limite et de l’Illimité, premier niveau après l’Un lui‑même. Vitale pour la création, la Chôra est alors la «mère» de tout. Il n’est donc pas étonnant que, comme nous le dit Proclus, la chôra soit, en fait, la déesse Rhéa. En hommage à la littérature révélatrice antique, comme les Oracles chaldaïques, Homère et Hésiode, Proclus fournit une élucidation non seulement philosophique, mais aussi théologique, de la chôra. In this contribution we will approach language and images in different modalities: speech and face, anticipated and imagined movements, illusions on the sound by the image. It will be the opportunity for us to revisit the Gestalt concepts which were considered obsolete since structuralism in Humanities. As instantiated by Gilbert Durand in The Anthropological Structures of the Imaginary (1999, French 1st ed. 1960), we shall recall that Gestalt is not—even implicitly—an exclusively static approach to cognition. On the contrary we will (...) emphasize that forms can emerge from movements and stabilize in memory, thence morphology. And for speech, we will consider phonology as the stabilized outcome from audible and visible gestures of the mouth—caught in the time-course of a flow of some four in six syllables by second—via the perception of motor coordinations: which is necessary to master the most common expression of language between humans. We will adopt here also for the perceptual flows of spoken language, Multimodal Scene Analysis, the ongoing legacy of Gestalt Theory, including one of our main research topics: the gestural expression of the face and the hand for Deaf people, who practise as an augment French LPC (Langue française Parlée Complétée), adapted in 1967 from Cued Speech by its originator Dr Richard Cornett. Si les animaux d’élevage comme les animaux d’expérimentation intéressent depuis une trentaine d’années de plus en plus de personnes – de chercheurs, de militants –, les animaux dit de compagnie, en revanche, semblent toujours pâtir du préjugé selon lequel, parce qu’ils relèveraient d’une relation privée, ils seraient sans intérêt scientifique ou philosophique. Donna Haraway propose une façon tout à fait singulière de court-circuiter cet arrêt de la pensée afin de pouvoir re-problématiser cette relation à nouveau frais. Ce qui change immédiatement, (...) c’est le caractère non plus privé mais politique de cette co-relation : l’histoire biologique et politique qui nous unit à ces animaux dit de compagnie apparus massivement dans les années 1970 est toute aussi complexe que celle que nous partageons par ailleurs avec nos autres espèces compagnes. Le présent article établit deux résultats distincts mais complémentaires : il montre d’abord que Jan Patočka découvre, dans les années soixante-dix, le primat épistémologique de la donation, à partir duquel la question du monde dans son rapport au problème du donné se pose et se résout – le monde comme donné en totalité, le donné comme avance du monde ; il montre ensuite que cette découverte permet au philosophe tchèque de poser à nouveaux frais la question du sujet et de (...) faire émerger une nouvelle figure du sujet, dont les principales caractéristiques anticipent sur celles de l’adonné – le sujet comme adonné-au-monde. Cet article aboutit ainsi à montrer que les concepts fondamentaux de la phénoménologie de la donation (donné, adonné, contre-expérience, témoin, appel-réponse, etc.), quand ils sont appliqués à la phénoménologie de Patočka, ne fonctionnent pas comme des catégories anachroniques, mais au contraire comme des clés herméneutiques qui révèlent à la fois l’originalité et l’actualité de cette phénoménologie. Dans un régime comme celui de la République populaire de Chine, où prévaut la suprématie d’un Parti-État unique, la politique et le politique constituent, a fortiori plus qu’ailleurs, la chasse gardée de l’élite dirigeante du pays, à savoir les cadres du Parti communiste. Cet article tente de saisir qui sont les élites politiques aux commandes de la Chine d’aujourd’hui à travers l’analyse du modus operandi de l’École des cadres du Parti de Shanghai. L’étude de cette institution, qui participe à la (...) régulation de la circulation des élites et constitue l’un des lieux privilégiés de la transmission et de l’exercice du pouvoir, permet, par-delà son propre fonctionnement, d’éclairer la nature et le devenir même du régime. S’il apparaît que les forces modernisatrices, en assimilant davantage l’École du Parti à un institut d’administration publique, œuvrent à la professionnalisation du métier de cadre, c’est également et avant tout afin de mieux perpétuer l’hégémonie du Parti-État unique.In a regime where the one Party-State rule still prevails, as is the case in the People’s Republic of China, the polity and politics constitute – here more than anywhere else – the private ground of the country’s ruling elite, i.e. the cadres of the Communist Party. This article attempts to grasp who the political elites in control of contemporary China are, by analyzing the modus operandi of the Shanghai Party School for Cadres. The study of an institution that takes part in the circulation of the elites and hence constitutes one of the privileged playing fields for the transmission and the exercise of power, allows, beyond the understanding of its own working, to shed light on the very nature and evolution of the regime. If the modernizing forces appear to work towards the professionalization of the cadres’ craft by equating the Party School to an institute of public administration, they ultimately do so in order to better perpetuate the supremacy ofthe one-Party system. Strategies based on performance measurement and feedback are commonly used to support quality improvement among nurses. These strategies require practice change, which, for nurses, rely to a large extent on their capacity to coordinate with each other effectively. However, the levers for coordinated action are difficult to mobilize. This discussion paper offers a theoretical reflection on the challenges related to coordinating nurses' actions in the context of practice changes initiated by performance measurement and feedback strategies. We explore how Jürgen Habermas' (...) theory of Communicative Action may shed light on the issues underlying nurses' collective actions and self‐determination in practice change and the implications for the design of strategies based on performance measurement and feedback. Based on this theory, we propose differences between communicative and functional coordination according to the nature of the actions and the purposes involved. The domains of action underlying these coordination processes, which Habermas referred to as the lifeworld and the system, are then used to draw a parallel with aspects of nursing practice. Further exploration of these concepts allows us to consider the tensions between the demands of the system and the self‐determination of nurses within their practice. (shrink)

Precarious times have material consequences. Yet, feminist new materialist approaches demonstrate that the concepts of the “ma-terial” and of “matter” are radically different than what is com-monly held in the Western tradition. This article argues that femi-nist new materialism provides practical, essential, and ethical tools for political action in dynamic and entangled worlds. In such worlds (e.g., the Anthropocene), it is critically needed to establish an ethics of responsiveness, a culture of ethical living and dying with others. Yet, this ethic (...) must respond to and acknowledge our relational, entangled, dynamic, and agentic ontology. In response to this, this article proposes the “pluri-person,” a political figure that mobilizes contributions of feminist new materialism to produce an ethical, ontology-making, everyday practice/response to “Precari-ous Times.”. Modern literature, viewed superficially, presents two phenomena that attest its vitality rather than its decline, at least in our Vestern countries: the anormal development of the novel and the advent of criticism as a genre.The novel today appears to be omniscient and does not always succeed in disguising its didactic intent. Formerly, the various disciplines—once their methods were acquired and their end discerned—detached themselves from literature, leaving to it exclusively the domain of fiction. Devoured by the voracity of the novel, (...) not only the young and as yet uncertain sciences such as psychoanalysis, ethnology, and criminology, but older ones as well, like philosophy, history or law, come back to it and use it readily in maintaining their theses and in emphasizing their needs. From Italian physician Hieronymus Mercurialis’s contention that the stomach was ‘the king of the belly’, to its promotion by the end of the nineteenth century to the ‘monarch of humanity’ in patent medicine, to Byron Robinson’s discovery of the enteric nervous system in 1907 (a mesh of neural connectivity that led him to dub the gut ‘the second brain’), there has historically been a longstanding awareness of the expansive reach of the gut in the functions of the body. In the (...) nineteenth century, the authority of the gut and its allyship with the brain became a focus for writers thinking about the intersections of illness and ‘modern life’. In medical texts, domestic health manuals, patent medicine, and fiction, the electric telegraph in particular became a way of envisaging what we would now call the ‘gut-brain axis’. The telegraphic metaphor enabled a view of digestion as not simply a mechanical or chemical process, but one that was understood in terms of time, space, and communication. However, such a framework also suggested problems of connection that were common to both systems, emphasising not only the healthy body’s quasi-telegraphic networks but also its vulnerability to delay, disruption, and pathological incoherence. This article will explore the use of telegraphic technologies as proxies for theorising gastric connection and more broadly the concept of ‘gastric time’ as a key conceit for understanding digestion as a process that was and is subject to the idiosyncrasies of bodily rhythms. In the present context of aging populations, the question of how to support older workers who want to stay in employment longer is of particular importance, especially from a social justice perspective with regards to income. The challenges faced by organizations and governments are unprecedented. Interesting conclusions can be drawn from our research with regard to these challenges. First of all, the perception of retirement appears more or less unchanged over the years and remains very positive. Consequently, one of the (...) barriers to the employment of older workers may be the image of retirement itself, since it is still perceived as a gift or a right. Secondly, our results confirm that forcing people to stay longer in the labour market is a largely unpopular measure. Consequently, if public retirement plans offer limited income, governments and organizations should adopt a more voluntary approach to encourage older workers to stay in employment longer for a better income. Our results highlight the importance of the sector and type of job in the measures or incentives that could encourage older workers to stay in employment longer. Consequently, governments and organizations will probably have to adopt a contingent approach; i.e., all incentives do not necessarily fit all jobs or all sectors and social justice would require this be taken into account. We identified three sets of measures that could encourage older workers to stay in employment longer, and thus have access to better economic security: the reduction of working time, the flexibility of working time, and the individualization of retirement options and working time. The progressive reduction of their working time appears most interesting to our respondents. These measures appear to favour social justice in terms of income and right to employment at the end of active careers. (shrink)

"Literature and the Right to Marriage," which bears the same title as the special issue that it introduces, takes up the challenge of the claim that marriage is a universal right. Framing the questions that traverse the five essays in the collection with a close reading of the sections on marriage from Hegel's Elements of the Philosophy of Right, the authors argue that if marriage can be considered a right, this right both guarantees access to and is founded upon each (...) partner's vow to abide by his or her vows to the other. Hegel's text is important because it underscores the linguistic dimension of the marriage ceremony; and it thus helps to show that, if the right to marriage were universal, it must be understood as the right to an irreplaceable kind of ritual speech. Moreover, this text is remarkably modern in its emphasis upon love as the condition of rightful marriage. Hegel considered the exchange of marriage vows to be a political event at which love becomes ethical , and ethics becomes political ; and thus also the point at which the model for "ethical love" becomes the verbally solemnized love between a man and a woman rather than the priestly love of the neighbor. Marriage, for Hegel, is always a turning point, both in the lives of the couple and in the history of the state. But this means that the importance of any marriage cannot be measured in terms of a norm; it can only be measured in terms of the amorous situation that precedes the event of marriage. Hegel's analyses are indispensable for grasping the stakes of the essays collected in this special issue and for addressing contemporary questions about which couples have the right to marry and what marriage becomes when those who would marry already have shared long and complex lives together. This paper examines Irish campaigns for condom access in the early 1990s. Against the backdrop of the AIDS crisis, activists campaigned against a law which would not allow condoms to be sold from ordinary commercial spaces or vending machines, and restricted sale to young people. Advancing a conception of ‘transformative illegality’, we show that illegal action was fundamental to the eventual legalisation of commercial condom sale. However, rather than foregrounding illegal condom sale as a mode of spectacular direct action, we (...) show that tactics of illegal sale in the 1990s built on 20 years of everyday illegal sale within the Irish family planning movement. Everyday illegal sale was a long-term world-making practice, which gradually transformed condoms’ legal meanings, eventually enabling new forms of provocative and irreverent protest. Condoms ‘became legal’ when the state recognised modes of condom sale, gradually built up over many years and publicised in direct action and in the courts. (shrink)

The thymus is a unique primary lymphoid organ that supports the production of self‐tolerant T‐cells essential for adaptive immunity. Intrathymic microenvironments are microanatomically compartmentalised, forming defined cortical, and medullary regions each differentially supporting critical aspects of thymus‐dependent T‐cell maturation. Importantly, the specific functional properties of thymic cortical and medullary compartments are defined by highly specialised thymic epithelial cells (TEC). For example, in the medulla heterogenous medullary TEC (mTEC) contribute to the enforcement of central tolerance by supporting deletion of autoreactive T‐cell (...) clones, thereby counterbalancing the potential for random T‐cell receptor generation to contribute to autoimmune disease. Recent advances have further shed light on the pathways and mechanisms that control heterogeneous mTEC development and how differential mTEC functionality contributes to control self‐tolerant T‐cell development. Here we discuss recent findings in relation to mTEC development and highlight examples of how mTEC diversity contribute to thymus medulla function. (shrink)
