Introducción: El consentimiento informado es un procedimiento que brinda oportunidad a los pacientes de disponer de información y participar en la toma de decisiones médicas. Sin embargo, existe el criterio de que el nivel de entendimiento de la información aportada por los facultativos al paciente, sus familiares o ambos, es bajo y además, muchos médicos lo consideran como una herramienta de trabajo secundaria o que no es necesario realizarla con inmediatez. Objetivo: Evaluar la percepción y experiencia del personal sanitario de (...) la unidad de terapia polivalente del Hospital Docente Clínico Quirúrgico "Joaquín Albarrán" acerca del consentimiento informado. Método: Estudio descriptivo realizado en la unidad de terapia polivalente del Hospital "Joaquín Albarrán" de enero a marzo del 2013. Se aplicó una encuesta a 67 trabajadores que accedieron voluntariamente a responderla. Resultados: El 60 % de los encuestados tenía menos de dos años de trabajo en la unidad y solo el 45 %, conocimientos básicos de ética médica. Únicamente el 50 % de los interrogados consideró que este procedimiento puede mejorar la relación médico-paciente. Conclusiones: Es necesario incrementar el conocimiento sobre la utilidad del consentimiento informado entre los trabajadores de la unidad para aprovechar al máximo sus beneficios. Introduction: Informed consent is a procedure that offers full opportunity for patients to have access to information and participate in medical decision-makings. However, it is believed that the information given by the doctors are not fully understood by the patients, their families or both, and besides that many doctors consider it as a second tool or something that doesn't have to be done right away. Objective: Evaluate the perception and experience of the health staff of the polyvalent unit of the surgical clinical teaching hospital Joaquin Albarrán about informed consent. Method: Descriptive study carried out in the polyvalent unit of Joaquin Albarrán Hospital from January to march 2013. A survey was given to 67 workers who voluntarily agreed to answer it. Results: The 60% of the participants had been working in the unit for less than 2 years and only 45% of them had basic knowledge of medical ethics. Only 50% of the participants considered that this procedure could help to improve doctor-patient relationship. Conclusions: It is necessary to increase the knowledge of the usefulness of the informed consent among the workers of the unit to have full profit from it. (shrink)

T cells develop in the thymus by expressing a diverse repertoire of either αβ‐ or γδ‐T cell receptors (TCR). While many studies have elucidated how TCR signaling and gene expression control T cell ontogeny, the role of nutrient metabolism is just emerging. Here, we discuss how metabolic reprogramming and nutrient availability impact the fate of developing thymic T cells. We focus on how the PI3K/mTOR signaling mediates various extracellular inputs and how this signaling pathway controls metabolic rewiring during highly proliferative (...) and anabolic developmental stages. We highlight the role of the hexosamine biosynthetic pathway that generates metabolites that are utilized for N‐ and O‐linked glycosylation of proteins and how it impacts TCR expression during T cell ontogeny. We consider the dichotomy in metabolic needs during αβ‐ versus γδ‐T cell lineage commitment as well as how metabolism is also coupled to molecular signaling that controls cell fate. (shrink)

Background: The acquisition of experience is a major concern for nurses in intensive care units. Although the emotional component of the clinical practice of these nurses has been widely studied, greater examination is required to determine how this component influences their learning and practical experience. Objective: To discover the relationships between emotion, memory and learning and the impacts on nursing clinical practice. Research design: This is a qualitative phenomenological study. The data were collected from open, in-depth interviews. A total of (...) 22 intensive care unit nurses participated in this research between January 2012 and December 2014. Ethical considerations: The School of Nursing Ethics Committee approved the study, which complied with ethical principles and required informed consent. Findings: We found a clear relationship between emotion, memory and the acquisition of experience. This relationship grouped three dimensions: (1) satisfaction, to relieve the patient’s pain or discomfort, give confidence and a sense of security to the patient, enable the presence of family members into the intensive care unit and provide family members with a realistic view of the patient’s situation; (2) error experience, which nurses feel when a patient dies, when they fail to accompany a patient in his or her decision to abandon the struggle to live or when they fail to lend support to the patient’s family; and (3) the feel bad–feel good paradox, which occurs when a mistake in the patient’s care or handling of his or her family is repaired. Conclusion: Emotion is a capacity that impacts on nurses’ experience and influences improvements in clinical practice. Recalling stories of satisfaction helps to reinforce good practice, while recalling stories of errors helps to identify difficulties in the profession and recognise new forms of action. The articulation of emotional competencies may support the development of nursing ethics in the intensive care unit to protect and defend their patients and improve their relationships with families in order to maximise the potential for patient care. Recently, the concept of "gender identity" has enjoyed a great deal of attention in gender metaphysics. This seems to be motivated by the goal of creating trans-inclusive theory, by explaining trans people's genders. In this paper, we aim to unmotivate this project. Notions of "gender identity" serve important pragmatic purposes for trans people, such as satisfying the curiosity of non-trans people, and, relatedly, securing our access to important goods like legal rights and medical care. However, we argue that this does (...) not mean that "gender identity" is a metaphysically substantial thing that deserves extensive theoretical attention. There are reasons to be skeptical of such a concept. We trace the history of "gender identity," primarily to identify its roots in trans-antagonistic medical theory and its connections to the problematic "wrong-body" model--a legacy that has pathologized and flattened trans experience. Moreover, we argue that trans people primarily use "gender identity" to explain ourselves to non-trans people, rather than to discuss ourselves among ourselves. Thus, we urge theorists to resist the urge to substantiate gender identity. Instead, we encourage the development of new and better concepts, ones that attend to the lived realities of trans community and the flourishing gender terms and practices that are constructed there. If these lived realities are taken seriously, there is no need to “explain” trans people’s genders; we can simply see them as they are. (shrink)

This article explores the relational and motivational leadership behaviors that may promote stewardship in organizations. I conceptualize stewardship as an outcome of leadership behaviors that promote a sense of personal responsibility in followers for the long-term wellbeing of the organization and society. Building upon the themes presented in the stewardship literature, such as identification and intrinsic motivation, and drawing from other research streams to include factors such as interpersonal and institutional trust and moral courage, I posit that leaders foster stewardship (...) in their followers through various relational, motivational, and contextually supportive leadership behaviors. This paper has two aims: to explore the affective dimensions of moral shock and the way it relates to normative marginalization of those furthest from dominant society and also, more specifically, to articulate the trans experience of constantly being under moral attack because the dominant ‘world’ normatively defines trans individuals out of existence. Toward these ends, I build on Katie Stockdale's recent work on moral shock, arguing that moral shock needs to be contextualized to ‘worlds’ of sense to understand how (...) marginalized people affectively experience shocking events. My focus is the trans experience of moral shock due to the way trans people are positioned outside of dominant society, which creates the conditions to experience cyclical, chronic shock. In the last 20 years the Turing test has been left further behind by new developments in artificial intelligence. At the same time, however, these developments have revived some key elements of the Turing test: imitation and adversarialness. On the one hand, many generative models, such as generative adversarial networks, build imitators under an adversarial setting that strongly resembles the Turing test. The term “Turing learning” has been used for this kind of setting. On the other hand, AI benchmarks are (...) suffering an adversarial situation too, with a ‘challenge-solve-and-replace’ evaluation dynamics whenever human performance is ‘imitated’. The particular AI community rushes to replace the old benchmark by a more challenging benchmark, one for which human performance would still be beyond AI. These two phenomena related to the Turing test are sufficiently distinctive, important and general for a detailed analysis. This is the main goal of this paper. After recognising the abyss that appears beyond superhuman performance, we build on Turing learning to identify two different evaluation schemas: Turing testing and adversarial testing. We revisit some of the key questions surrounding the Turing test, such as ‘understanding’, commonsense reasoning and extracting meaning from the world, and explore how the new testing paradigms should work to unmask the limitations of current and future AI. Finally, we discuss how behavioural similarity metrics could be used to create taxonomies for artificial and natural intelligence. Both testing schemas should complete a transition in which humans should give way to machines—not only as references to be imitated but also as judges—when pursuing and measuring machine intelligence. In this paper I provide a preliminary account of weakness of political will (political akrasia). My aim is to use theories from the weakness of will literature as a guide to develop a model of the same phenomenon as it occurs in collective agents. Though the account will parallel the traditional view of weakness of will in individuals, weakness of political will is a distinctly political concept that will apply to group agents such as governments, institutional actors, and other political (...) collectives. I further argue that blaming inaction by collective political agents on a “lack of political will” should be understood as blaming a kind of akratic break within the deliberative process, leading to an inability of the collective agent to capitalize on existing group attitudes in order to translate them into policy or legislation. My primary example throughout will be U.S. political inaction with respect to the climate crisis. (shrink)

In the paper, the role of a citizen, a public official, and an observer in the reproduction of bribery as a social norm are each analyzed. To make the analysis of this cellular-social form of corruption, three variables are incorporated: the agent’s perception of how widespread the corruption is, the agent’s available resources with which to act, and the role of moral values. Later, some scenarios of normalization and denormalization of corruption are explored, making different assumptions regarding the analysis and (...) assignment of agents to different roles in the interactions. What was found is that power relations among corrupt and non-corrupt agents are key for understanding the social reproduction of bribery, where inequality might facilitate the social reproduction of corruption. The role of the observer of the interaction of bribe-givers, bribe-takers, and bribe-resisting agents was also found to potentially tip the balance in the direction of normalization or denormalization of bribery. Hence, giving the observer tools to intervene in these interactions becomes essential to combat corruption. (shrink)

When asked which of our concepts are normative concepts, metaethicists would be quick to list such concepts as GOOD, OUGHT, and REASON. When asked why such concepts belong on the list, metaethicists would be much slower to respond. Matti Eklund is a notable exception. In his recent book, Choosing Normative Concepts, Eklund argues by elimination for “the Normative Role view” that normative concepts are normative in virtue of having a “normative role” or being “used normatively”. One view that Eklund aims (...) to eliminate is “the Metaphysical view” that normative concepts are normative in virtue of referring to normative properties. In addition to arguing that Eklund’s objection looks doubtful by its own lights, we argue that there are several plausible versions of the Metaphysical view that Eklund doesn’t eliminate, defending various claims about normative concepts and their relationships to deliberation, competence, reference, and possession along the way. The New Intuitionism brings together some of the world’s most important contemporary writers from such diverse fields as metaethics, epistemology and moral psychology to explore the latest implications of, and challenges to, Audi’s work. Background: Nurses are the largest professional group in healthcare and those who make more decisions. In 2014, the Committee on Bioethics of the Council of Europe launched the “Guide on the decision-making process regarding medical treatment in end-of-life situations” (hereinafter, Guide), aiming at improving decision-making processes and empowering professionals in making end-of-life decisions. The Guide does not mention nurses explicitly. Objectives: To analyze the ethical principles most valued by nurses working in palliative care when making end-of-life decisions and investigate if (...) they are consistent with the framework and recommendations of the Guide; to identify what disputed/controversial issues are more frequent in these nurses’ current end-of-life care practices. Design: Qualitative secondary analysis. Participants/context: Three qualitative datasets including 32 interviews from previous studies with nurses working in palliative care in Portugal. Ethical consideration: Ethical approval was obtained from the Ethics Research Lab of the Instituto de Bioética (Ethics Research Lab of the Institute of Bioethics) (Ref.04.2015). Ethical procedures are thoroughly described. Findings: All participant nurses referred to autonomy as an ethical principle paramount in end-of-life decision-making. They were commonly involved in end-of-life decision-making. Palliative sedation and communication were the most mentioned disputed/controversial issues. Discussion: Autonomy was highly valued in end-of-life care and decision-making. Nurses expressed major concerns in assessing patients’ preferences, wishes, and promoting advance care planning. Nurses working in palliative care in Portugal were highly involved in end-of-life decision-making. These processes embraced a collective, inclusive approach. Palliative sedation was the most mentioned disputed issue, which is aligned with previous findings. Communication also emerged as a sensitive ethical issue; it is surprising, however, that only three nurses referred to it. Conclusion: While the Guide does not explicitly mention nurses in its content, this study shows that nurses working in palliative care in Portugal are involved in these processes. Further research is needed on nurses’ involvement and practices in end-of-life decision-making. (shrink)

Background: The Directive 2001/20/EC was an important first step towards consistency in the requirements and processes for clinical trials across Europe. However, by applying the same rules to all types of drug trials and transposing the Directive’s principles into pre-existing national legislations, the Directive somewhat failed to meet its facilitation and harmonisation targets. In the field of ethics, the Directive 2001/20/EC conditioned the way of understanding and transposing the “single opinion” process in each country. This led to a situation in (...) which two models of research ethics committees organisation systems exist, being the model in which the “single opinion” is considered to be the decision made by a single ethics committee more effective and simpler in terms of administrative and logistic workload. Method: A survey was conducted in 10 European countries. Members of the European Clinical Research Infrastructures Network working party number 1, with expertise in the field of ethics, responded. Results: There is a major heterogeneity in the composition of ethics committees among the surveyed countries based on the number of members, proportion of experts versus lay members and expertise of the scientific members. A harmonised education of the ethics committees’ membership based in common curricula is recommended by the majority of countries. Conclusions: Despite the efforts for harmonisation of the European Clinical Trial Directive, from an ethical point of view, there remains a plurality of ethics committees' systems in Europe. It is important to comprehend the individual national systems to understand the problems they are facing. (shrink)

The dichotomy between the qualitative and the quantitative has been a classic throughout the history of science. As will be seen, this dichotomy permeates all ontological levels of reality. In this work, phenomenological examples potentially related to semiosis are presented at the different levels established by Mario Bunge and Josep Ferrater Mora, contrasting the qualitative categorizations with the quantifiable physical reality. Likewise, the need to continue in the quantification of the biosemiotic and linguistic studies will be presented, while, in contrast, (...) the need to establish a qualitative framework in the little-addressed study of technosemiotics will be raised, of potential interest given the notable advances that are expected in communication systems for inert artifacts in the next years. In short, in the thesis defended here the qualitative precedes the quantitative in the defining path of science. (shrink)

This paper argues that, although Margaret Cavendish’s main philosophical contributions are not in philosophy of religion, she makes a case for a defense of God, in spite of the worst sorts of harms being present in the world. Her arguments about those harms actually presage those of contemporary feminist ethicists, which positions Cavendish’s scholarship in a unique position: it makes a positive theodical contribution, by relying on evils that contemporary atheists think are the best evidence against the existence of God. (...) To demonstrate that Cavendish’s work should be considered as early modern feminist theodicy, this paper will briefly introduce the contemporary feminist worry about theodicy as a project, show that Cavendish shares the contemporary feminist view about situated evil, and argue that her theodicy aims for agreement about how to eradicate great moral evils while preserving free will—and so, carves out a space for future female philosophers of religion who aim to be agents of healing in the face of such evil. I will relate some social and epistemic aspects involved in the conceptual development of the genetic model in the early Molecular Biology. I will use the link between Biology and Physics as a framework, which supports such assumptions, providing new methodologies, but mainly as a framework that provides a model of objectivity developed within Modern Tradition. Finally, I will interpret this relationship from the pragmatist perspective to embrace Rorty’s thesis on «the ontological priority of the social» which implies understanding biology (...) as a part of a general cultural politics. Philosophers of race and other writers in the Black and Latinx intellectual traditions have remarked on what it is like to live under “the racial gaze,” to be shaped and limited by the way whites perceive us. However, little work has been spent developing how the racial gaze functions in whites’, and other racially privileged people’s, moral psychology. I argue in this paper that there is a morally objectionable way of perceiving people of color. This claim builds on an insight (...) from Iris Murdoch that our perception can be morally evaluable and extends it to issues of race. I articulate how racial stereotypes and misvaluing distort one’s perception of people of color and that these distortions are organizing around a dominant conception for race that plays an important role in the oppression of people of color. I believe understanding racist perception lays a foundation for understanding the moral dimensions of interpersonal (as opposed to structural) racism. (shrink)

The objective of this work is to show that the cognitive and ontological connections between Benjamin’s and Zambrano’s works, more than similarities and differences, are based on the exercise of their expressive action. Along with the image that appears in the writing, both thinkers cite old poetic forms, which allows them to update philosophical, political and theological figurations to configure an experience of thinking in work.

This paper proposes that Hannah Arendt’s book Eichmann in Jerusalem: A Report on the Banality of Evil furnishes both philosophical and empirical elements to understand not only the Nazi crimes but also cases of wrongdoing by and within current organizations. It is suggested that Arendt provides three relevant standpoints to how wrongdoing is banalized within organizations: a critique of bureaucratic administration, an account of the role of interactive socialization, and a reflection on the cognitive and meaning-attribution processes. Podría decirse que uno de los debates que ha generado más discusiones, reflexiones y producciones al respecto en todas las corrientes de la filosofía ha sido sobre la existencia, naturaleza e influencia de Dios en el mundo. En el presente artículo se expondrán cinco paradojas planteadas por diversos filósofos que abrirían la posibilidad de negar las perfecciones de Dios desde su naturaleza: su omnipotencia y su bondad. Una vez planteado cada problema, se expondrán las posibles soluciones a cada uno, mostrando (...) que estos parten de ambigüedades, contradicciones o incomprensiones de la teoría cristiana. Key features of smart grids include the integration of renewable energy sources, decentralized generation and advanced distribution networks. At the heart of smart grids is a sophisticated metering system, consisting of intelligent electronic (...) devices that measure energy consumption and enable two-way communication between the utility company and the consumer. The information exchanged via these devices is critical, as it includes sensitive data such as energy consumption patterns and billing information. With the rise of the Internet of Things (IoT) and Industry 4.0, the security of this information is of utmost importance. This paper delves into the security challenges faced by advanced metering infrastructure (AMI) and highlights the crucial role played by cryptography in ensuring the secure exchange of sensitive information. Cryptographic protocols such as encryption, authentication and digital signatures are essential components of a secure smart grid infrastructure. These protocols work together to ensure the confidentiality, integrity and authenticity of the information exchanged between the utility company and the consumer. (shrink)

En el presente texto me propongo mostrar cómo puede entenderse el concepto de ‘metafísica’, no como el estudio de algo “sobrenatural” o “más allá de lo físico”, sino como un estudio de segundo orden que tendría por objeto de análisis los hipotéticos principios fundamentales y modelos de dependencia de la realidad. En este sentido, más que sostener juicios taxativos acerca de lo que las cosas son o qué tipo de entidades existen, las reflexiones de la metafísica podrían entenderse como análisis (...) acerca de las condiciones necesarias y suficientes para que una teoría de primer orden explique un aspecto de la realidad —o toda ella en su conjunto— de la mejor manera posible. Today’s medical ethics involve two different viewpoints based on how we understand the role of conscience in medicine and the purpose of healthcare. The first view, called the health-directed model, sees medicine as a way to improve health and promote healing, while also respecting the values of both patients and doctors. In this model, doctors need some discretionary space to decide how to achieve the best health outcomes in their practice. On the other hand, the service-provider model sees the main (...) goal of medicine as providing a service, especially healthcare, with a strong focus on protecting patient autonomy. In this view, doctors are required to provide care even when it goes against their personal beliefs. The goal of this article is to explore the foundations and arguments of these two medical models. Understanding the key ideas behind these models is important for deciding whether to support or oppose conscientious objection in medical ethics. Additionally, the article aims to figure out which model makes a stronger case and to offer advice on how to engage with the opposing view from a virtue ethics perspective. (shrink)

Quoting Flaubert through time, Mieke Bal and Michelle Williams Gamaker’s Madame B brings Madame Bovary’s reflections on love and emotions to the present day, in a productive anachronism. Their work produces an intertemporal space where the past is relevant for the present, and the present enables us to understand the past. Intimacy and routine are central in their exploration of Flaubert’s contemporaneity. Those issues are precisely one of the keys in Karl Ove Knausgård’s project of literary autobiography, where he expands (...) narration foreclosing the ellipsis and giving visibility to small things and emotions; a project with some resonances with Munch’s crude-obscene uses of intimacy. This essay explores how both proposals, Bal and Williams Gamaker in film, and Knausgård in literature, can serve us to connect present and past sensibilities and, more than that, demonstrate resistances to the hegemonic discourses of temporality. El Romanticismo hizo aportes fundamentales en la teoría de la pintura: por un lado centró la atención en su visualidad como arle de la apariencia y su idealización: por el otro, la coaccionó con el significado, poniéndola al servicio de la religión y el poder, estimuló el goce y la crítica del arte, pero los condenó también en favor de una actitud reverencial ante él. La pintura de los Nazarenos representa la intención romántica de una nueva pintura cristiana, alemana y (...) patriótica que habría de reorientar el espíritu de la época. Hegel se opuso radicalmente a este proyecto: su tesis del fin del arte no tiene sólo una explicación sistemática, sino que responde igualmente a un debate histórico concreto. El artículo tiene como objetivo describir las competencias informacionales a desarrollar durante la formación profesional. Se presenta los referentes teóricos a partir del empleo de un enfoque de sistema que supone el análisis y la síntesis, la inducción y la deducción como métodos de investigación, con el propósito de dar conocer los hitos en las universidades y organizaciones internacionales relacionadas. La modelación fue empleada para la construcción de un nuevo proyecto de desarrollo de competencias informacionales desde la perspectiva de la (...) formación profesional universitaria ajustada a las necesidades de la educación superior en Cuba. El nuevo proyecto se configura en tres unidades básicas (acceso, procesamiento y comunicación), con sus respectivos elementos de competencia, saberes esenciales e indicadores para la evaluación del desempeño. El artículo fundamenta la tesis de que el desarrollo de competencias informacionales debe ser sistemáticamente estructurado, planificado y ejecutado como parte integral de la formación curricular en interacción con los contenidos de las disciplinas docentes, bajo la dirección del profesor y la participación protagónica de los profesionales en formación. -/- ABSTRACT -/- The article aims at describing the competence to be developed during professional training. It presents the theoretical referents by means of an approach system that includes synthesis and analysis, induction and deduction as research methods in order to reveal the milestones of information skills development in universities and related international organizations. Modeling was used for the construction of a new skills development project from the perspective of university professional training according to the needs of Higher Education in Cuba. The new project is composed of three basic units (access, processing and communication), with their respective competence elements, essential knowledge and indicators for the evaluation of the performance. The article supports the theory that information development skills must be systematically structured, planned and implemented as an integral part of the curricular training in interaction with the contents of subjects, under the teacher´s guidance and the active participation of profesionals in training. -/- .
