Family Medicine Groups (FMGs) are the most recently developed primary care organizations in Quebec (Canada). Nurses within FMGs play a central role for patients with chronic diseases (CD). However, this complex role and the nursing activities related to this role vary across FMGs. Inadequate knowledge of nursing activities limits the implementation of exemplary nursing practices. This study aimed to describe FMG nursing activities with patients with CD and to describe the facilitators and barriers to these activities. A multiple‐case study was (...) performed with ten nurses practicing among patients with CD in FMGs. Five data sources were used to provide an in‐depth description of nursing activities and the facilitators and barriers to the development of these activities. After qualitative data analysis, findings show that nursing activities are clustered into five domains: Global assessment of the patient, Care management, Health promotion, Nurse–physician collaboration, and Planning services for patients with CD. Activities vary depending on contextual factors identified in each case. This multiple‐case study provides a clear description of nursing activities with patients with CD. There is a need for improved nursing activities and expertise in domains of activities that are less present in FMGs, such as case management and interprofessional collaboration. (shrink)

The purpose of the article is to offer a detailed exegetical analysis of the argument in chapter four of "de ente et essentia" in which aquinas argues for a distinction between "esse" and essence and to develop an interpretation of it on the basis of the analysis. I argue that the reconstructed argument shows that aquinas argues for a real distinction and that he establishes it earlier in the argument than some commentators have thought. There is increasing global pressure to ensure that pregnant women are responsibly and safely included in clinical research in order to improve the evidence base that underpins healthcare delivery during pregnancy. One supposed barrier to inclusion is the assumption that pregnant women will be reluctant to participate in research. There is however very little empirical research investigating the views of pregnant women. Their perspective on the benefits, burdens and risks of research is a crucial component to ensuring effective recruitment. The (...) Research In Pregnancy Ethics study set out to ascertain the views of pregnant women about research participation using an inductive thematic analysis. We conducted semi-structured interviews with 20 women who had participated in a double-blind randomised placebo controlled trial in Wellington while pregnant. Our results show that at least some pregnant women recognise the value and importance of research during pregnancy. The women we interviewed were deeply invested in the research process and outcomes. Key motivations for participating were altruism, playing a valuable civic role and the importance of research. The main perceived burdens related to inconvenience and time commitment. For some women, possible randomization to the placebo arm was regarded as a burden or disadvantage. (shrink)

Medical assistance in dying (MAID) legislation in Canada followed much deliberation after the Supreme Court of Canada’s ruling inCarterv.Canada. Included in this deliberation was the Special Joint Committee on Physician Assisted Dying’s recommendation to extend MAID legislation beyond the inclusion of adults to mature minors. Children's agency is a construct advanced within childhood studies literature which entails eliciting children’s voices in order to recognise children as active participants in constructing their own childhoods. Using this framework, we consider the possible extension (...) of MAID legislation to most minors. We highlight important questions regarding how insights from children’s voices could be mobilised in the life or death context of MAID. We conclude that children’s voices have the potential to help determine their eligibility for MAID; however, incorporating children's voices in the context of MAID requires careful consideration due to the complexity of voice. Averageness is purportedly the result of stabilizing selection maintaining the population mean, whereas facial paedomorphosis is a product of directional selection driving the population mean towards an increasingly juvenile appearance. If selection is predominantly stabilizing, intermediate phenotypes reflect high genetic quality and mathematically average faces should be found attractive. If, on the other hand, directional selection is strong enough, extreme phenotypes reflect high genetic quality and juvenilized faces will be found attractive. To compare the effects of stabilizing and directional selection (...) on facial paedomorphosis (juvenilization), graphic morphing and editing techniques were used to alter the appearance of composite faces to make them appear more or less juvenile. Both facial models and judges of attractiveness were from the CSU-Long Beach campus. Although effect sizes for both preferences were large, the effect for averageness was nearly twice that found for juvenilization, an indication that stabilizing selection influences preferences for facial paedomorphosis more so than directional selection in contemporary humans. The Ceci et al. article is consistent with tenure being a necessary condition for controversial research. In the absence of tenure, as in the United Kingdom, professors have been fired and suspended for politically controversial issues. There are a variety of reasons why tenure does not ensure that professors will engage in controversial research, including career interests and the desire to be liked. This volume is an important supplement to the two volumes in the series of Cambridge Histories covering the philosophy of the Middle Ages. Dronke's book, which adopts the format of the latter volume, is intended to fill the gap between them. It contains sixteen contributions by fifteen scholars. The contributions are arranged in four parts. The four essays in part 1, "Background," provide useful summaries of the intellectual inheritance that provides the cultural environment for what has been called the twelfth-century (...) renaissance. These essays give us, for the first time I think, a clear and reasonably broad account of the historical and philosophical relations between twelfth-century thinkers and ancient thought. Part 2, "New Perspectives," contains four chapters, one on twelfth-century scientific speculations, one on the grammatical, logical, and semantic issues that grew out of interest in grammar, and two on logic during the period. These chapters are the most exciting in the book: they succeed in showing us not only some of what is new and distinctive in twelfth-century thought but also in taking us to the frontiers of some of the philosophically most interesting current research. Fredborg's "Speculative Grammar" and Jacobi's "Logic : The Later Twelfth Century" uncover some of the strange and intriguing roots of characteristically medieval developments in logic: issues such as the properties of terms, the theory of supposition, and fallacies; and methods such as the use of sophismata and instantiae. This paper is about the differences between probabilities and beliefs and why reasoning should not always conform to probability laws. Probability is defined in terms of urn models from which probability laws can be derived. This means that probabilities are expressed in rational numbers, they suppose the existence of veridical representations and, when viewed as parts of a probability model, they are determined by a restricted set of variables. Moreover, probabilities are subjective, in that they apply to classes of events (...) that have been deemed (by someone) to be equivalent, rather than to unique events. Beliefs on the other hand are multifaceted, interconnected with all other beliefs, and inexpressible in their entirety. It will be argued that there are not sufficient rational numbers to characterise beliefs by probabilities and that the idea of a veridical set of beliefs is questionable. The concept of a complete probability model based on Fisher's notion of identifiable subsets is outlined. It is argued that to be complete a model must be known to be true. This can never be the case because whatever a person supposes to be true must be potentially modifiable in the light of new information. Thus to infer that an individual's probability estimate is biased it is necessary not only to show that the estimate differs from that given by a probability model, but also to assume that this model is complete, and completeness is not empirically verifiable. It follows that probability models and Bayes theorem are not necessarily appropriate standards for people's probability judgements. The quality of a probability model depends on how reasonable it is to treat some existing uncertainty as if it were equivalent to that in a particular urn model and this cannot be determined empirically. Bias can be demonstrated in estimates of proportions of finite populations such as in the false consensus effect. However the modification of beliefs by ad hoc methods like Tversky and Kahneman's heuristics can be justified, even though this results in biased judgements. This is because of pragmatic factors such as the cost of obtaining and taking account of additional information which are not included even in a complete probability model. Finally, an analogy is drawn between probability models and geometric figures. Both idealisations are useful but qualitatively inadequate characterisations of nature. A difference between the two is that the size of any error can be limited in the case of the geometric figure in a way that is not possible in a probability model. (shrink)

This book offers a new approach to the analysis of the multiple meanings of English modals, conjunctions, conditionals, and perception verbs. Although such ambiguities cannot easily be accounted for by feature-analyses of word meaning, Eve Sweetser's argument shows that they can be analyzed both readily and systematically. Meaning relationships in general cannot be understood independently of human cognitive structure, including the metaphorical and cultural aspects of that structure. Sweetser shows that both lexical polysemy and pragmatic ambiguity are shaped by our (...) metaphorical folk understanding of epistemic processes and of speech interaction. Similar regularities can be shown to structure the contrast among root, epistemic and speech act uses of modal verbs, multiple uses of conjunctions and conditionals, and certain processes of historical change observed in Indo-European languages. Since polysemy is typically the intermediate step in semantic change, the same regularities observable in polysemy can be extended to an analysis of semantic change. (shrink)

What is introspective know ledge of one’s own intentional states like? This paper aims to make plausible the view that certain cases of self-knowledge, namely the cogito-type ones, are enough like perception to count as cases of quasi-observation. To this end it considers the highly influential arguments developed by Sydney Shoemaker in his recent Royce Lectures. These present the most formidable challenge to the view that certain cases of self-knowledge are quasi-observational and so deserve detailed examination. Shoemaker’s arguments are directed (...) against two models of ordinary perception, the “object perception model” and the “broad perceptual model”. I argue that the core theses that Shoemaker associated with them are either dubious in their own right or applicable to certain cases of self-knowledge. Overall the aim is to show that there is such a variety of patterns in each case that simple analogies or disanalogies are unhelpful. (shrink)
