Although being generally safe, the use of Deep Brain Stimulation has been associated with a significant number of patients experiencing postoperative psychological and neurological harm within experimental trials. A proportion of these postoperative severe adverse effects have lead to the decision to medically prescribe device deactivation or removal. However, there is little debate in the literature as to what is in the patient’s best interest when device removal has been prescribed; in particular, what should be the conceptual approach to ethically (...) guide the decision to remove or maintain implants. The purpose of this article is to examine the ethical issues raised when patients refuse brain device explantation despite medical prescription. In order to illustrate these issues, we report and discuss a clinical case involving a patient suffering from treatment resistant depression who experienced forms of postoperative self-estrangement, as well as suicidal attempts, but who resists giving consent to device explantation. One important concern regarding implantable Brain Computer Interfaces is the fear that the intervention will negatively change a patient’s sense of identity or agency. In particular, there is concern that the user will be psychologically worse-off following treatment despite postoperative functional improvements. Clinical observations from similar implantable brain technologies, such as deep brain stimulation, show a small but significant proportion of patients report feelings of strangeness or difficulty adjusting to a new concept of themselves characterized by a maladaptive je ne (...) sais quoi despite clear motor improvement. Despite the growing number of cases in the DBS literature, there is currently no accepted or standardized tool in neuroethics specifically designed to capture the phenomenological postoperative experience of patients implanted with DBS or BCI devices. Given potential risks of postoperative maladaptation, it is important for the field of neuroethics to develop a qualitative instrument that can serve as a shared method for capturing postoperative variations in patient experience of identity and agency. The goal of this article is to introduce an instrument we have developed for this purpose and call for further neuroethical efforts to assess the phenomenology of implantable brain device use. Reading the Communist Manifesto against the contemporary background of massive unemployment, the author argues that Marx's theory of work is no longer adequate to tackle the problem of `workers without work' and suggests that it has to be reformulated in such a way that its normative intuitions and its critical impulses can be maintained. In the first part, he presents a philosophical critique of Marxism that is inspired by Jürgen Habermas and Hannah Arendt. In the second part, he presents a (...) sociological critique of Marxism and argues that the theory of the alienation of work implies a justification of work and the work society. Finally, in the last part, the author presents an ideological critique of Marxism that is inspired by Marcel Mauss' sociology of the gift. Criticising the contractualist assumptions of workfare solutions, he proposes a package solution for a radical decommodification of the labour market by means of a disjunction of income and work. Han Fei (around 280 to 233 B.C.) advocates a strong and orderly state based on the absolute authority of the state and the law. Han Fei is usually not associated with human rights. His philosophy is difficult to reconcile with civil and political human rights, even if some of his political concepts support the realization of certain human rights. However, Han Fei’s ideas help us to gain a better understanding of the People’s Republic of China’s official human rights narrative. The (...) PRC emphasizes collective social and economic human rights, views the authority of the Communist Party as a prerequisite for the realization of human rights, and advocates rule by law, i.e. using the law as a tool of governance. Crisis management is a complex problem raised by the scientific community currently. Decision support systems are a suitable solution for such issues, they are indeed able to help emergency managers to prevent and to manage crisis in emergency situations. However, they should be enough flexible and adaptive in order to be efficient to solve complex problems that are plunged in dynamic and unpredictable environments. The approach we propose in this paper addresses this challenge. First, we expose a modelling of information (...) for an emergency environment. Then, we describe an architecture of a multiagent DSS that deals with these information in order to prevent and to manage crisis situations. We focus here on the first level of the system mechanism which intends to perceive and to reflect the evolution of the current situation. Finally, we provide an implementation and an experimentation of the approach using the RoboCupRescue Simulation System. (shrink)

In sum, major critics of the twentieth century continually insist that poetry's unique value lies in its ability to convey meanings for which there are no public criteria whatsoever. But there are no such meanings, and to praise a poem for conveying them is empty. Again, these critics assume that what we understand by emotion is to be identified simply with an inner experience or state of mind and that this state of mind is what is conveyed by, or gives (...) meaning and definition to, words which are used to refer to our emotional life. The emotions or qualities of emotions supposedly communicated by poetry alone, moreover, are said to elude public language altogether; they can be neither defined nor discussed but only embodied in images. But whatever the status these private experiences have for us, they as yet play no part in our language, even our poetic language, and reference to them certainly contributes nothing to our criticism of poetry. Criticism can play a central role in interpreting and shaping our lives, but it will only be worth writing if its vocabulary has content. This book is a guide to working with social science concepts. Concepts are the prisms through which we see the social world. They are foundational to the social science enterprise, and the quality of investigations hinges in part on how well researchers make use of them. Most social science concepts are drawn from ordinary language used in everyday ways; however, many social scientists "reconfigure" ordinary words to meet their research needs. They tinker with the meanings of words to fit their (...) theoretical aims and make them precise, useful tools of measurement and comparison. This book examines social science concepts through an interpretivist lens with the aim of providing concrete conceptual guidance for future research. Specifically, this book seeks to: 1) identify characteristic dangers that attend the making and use of reconfigured concepts; 2) lay out ways in which a select number of interpretivist approaches have been used to mitigate these dangers; 3) introduce practical tools that rework these interpretivist approaches into explicit and useable research methods; 4) show concretely how these elucidation tools can be gainfully used by social scientists working both within and outside of the interpretivist tradition. The phenomenological effects of deep brain stimulation (DBS) on the self of the patient remains poorly understood and under described in the literature, despite growing evidence that a significant number of patients experience postoperative neuropsychiatric changes. To address this lack of phenomenological evidence, we conducted in-depth, semistructured interviews with 17 patients with Parkinson's disease who had undergone DBS. Exploring the subjective character specific to patients' experience of being implanted gives empirical and conceptual understanding of the potential phenomenon of DBS-induced self-estrangement. (...) Our study concluded that (1) the more patients preoperatively felt alienated by their illness, the more they experienced postoperative self-estrangement, and (2) the notion of self-estrangement seems to exist in association with certain common qualitative characters, namely, loss of control, which reflects a deteriorative estrangement, and distorted perception of capacities, which reveals a restorative estrangement. These findings indicate that subjective self-reports help us to understand some aspects of the potential phenomenon of DBS-induced self-estrangement. (shrink)

We argue that a fashionable interpretation of the theory of natural selection as a claim exclusively about populations is mistaken. The interpretation rests on adopting an analysis of fitness as a probabilistic propensity which cannot be substantiated, draws parallels with thermodynamics which are without foundations, and fails to do justice to the fundamental distinction between drift and selection. This distinction requires a notion of fitness as a pairwise comparison between individuals taken two at a time, and so vitiates the interpretation (...) of the theory as one about populations exclusively. This article discusses the concept of joint attention and the different skills underlying its development. Research in developmental psychology clearly states that the development of skills to understand, manipulate and coordinate attentional behavior plays a pivotal role for imitation, social cognition and the development of language. However, beside the fact that joint attention has recently received an increasing interest in the robotics community, existing models concentrate only on partial and isolated elements of these phenomena. In the line of Tomasello’s research, (...) we argue that joint attention is much more than simultaneous looking because it implies a shared intentional relation to the world. This requires skills for attention detection, attention manipulation, social coordination and, most importantly, intentional understanding. After defining joint attention and its challenges, the current state-of-the-art of robotic and computational models relevant for this issue is discussed in relation to a developmental timeline drawn from results in child studies. From this survey, we identify open issues and challenges that still need to be addressed to understand the development of the various aspects of joint attention and conclude with the potential contribution of robotic models. Deep brain stimulation (DBS) has been regarded as an efficient and safe treatment for Parkinson’s disease (PD) since being approved by the Food and Drug Administration (FDA) in 1997. It is estimated that more than 150 000 patients have been implanted, with a forecasted rapid increase in uptake with population ageing. Recent longitudinal follow-up studies have reported a significant increase in postoperative survival rates of patients with PD implanted with DBS as compared with those not implanted with DBS. Although DBS (...) tends to increase life expectancy for most patients with PD, this medical benefit does not come without attendant negative consequences. For example, emerging forms of iatrogenic harms are sometimes induced—harms which were not initially expected when clinicians proposed neurosurgery and patients or their guardians consented to the treatment. We report and discuss the clinical case of a patient who was implanted with DBS more than 20 years ago (at the time of writing) and is now experiencing unexpected stages of PD. This case illustrates how extending the life span without improving quality of life may introduce a burden of harms for patients and families. As well, this case shows why we should prepare for the expanding numbers of PD-implanted patients experiencing a gain of longevity but with severe stages of disease leading to diminution in quality of life. This newly observed effect of DBS treatment requires us to explore ethical questions related to iatrogenic harms, informed consent, end of life and caregivers’ burden. (shrink)
