Medical nihilism is the view that we should have little confidence in the effectiveness of medical interventions. Jacob Stegenga argues persuasively that this is how we should see modern medicine, and suggests that medical research must be modified, clinical practice should be less aggressive, and regulatory standards should be enhanced.

This is the lightly revised text of my commentary/response to David Carr’s keynote address, “Phenomenology of Crisis,” at the 2024 meeting of the Husserl Circle.

Matteo Ricci (1552-1610) was an Italian Jesuit and one of the first Christian missionaries to China in the modern era. He was a genuine polymath—a translator, cartographer, mathematician, astronomer, and musician. Above all, Ricci was a missionary for the gospel. As we briefly examine his 1603 seminal work, The True Meaning of the Lord of Heaven, our hope is that we, as evangelical educators, will perceive some of the deeper principles necessary for our own missionary work among unreached people.

Substituted judgment has increasingly become the accepted standard for rendering decisions for incapacitated adults in the USA. A broad exception exists with regard to patients with diminished capacity secondary to depressive disorders, as such patients’ previous wishes are generally not honoured when seeking to turn down life-preserving care or pursue aid-in-dying. The result is that physicians often force involuntary treatment on patients with poor medical prognoses and/or low quality of life as a result of their depressive symptoms when similarly situated (...) incapacitated patients without such depressive symptoms would have their previous wishes honoured via substituted judgment. This commentary argues for reconsidering this approach and for using a substituted judgment standard for a subset of EMP/LQL patients seeking death. (shrink)

According to the Generality Constraint, mental states with conceptual content must be capable of recombining in certain systematic ways. Drawing on empirical evidence from cognitive science, I argue that so-called analogue magnitude states violate this recombinability condition and thus have nonconceptual content. I further argue that this result has two significant consequences: it demonstrates that nonconceptual content seeps beyond perception and infiltrates cognition; and it shows that whether mental states have nonconceptual content is largely an empirical matter determined by the (...) structure of the neural representations underlying them. Important study focuses on the revival and assimilation of ancient Greek mathematics in the 13th–16th centuries, via Arabic science, and the 16th-century development of symbolic algebra. This brought about the crucial change in the concept of number that made possible modern science — in which the symbolic "form" of a mathematical statement is completely inseparable from its "content" of physical meaning. As conceptual and theoretical discussions on environmental valuation approaches have advanced there is growing interest in the impact that valuation has on decision making. The perceived legitimacy of the outputs of valuation studies is seen as one factor influencing their impact on policy decisions. One element of this is ensuring that participants of valuation processes see the results as legitimate and would be willing to accept decisions based on these findings. Here, we test the perceived legitimacy to participants of two (...) approaches to deliberative monetary valuation, deliberated preferences and Deliberative Democratic Monetary Valuation, in the context of marine planning in the Clyde estuary in Scotland. We compare and contrast deliberated preference and deliberative democratic monetary valuation and track their emergence as responses to critiques of conventional stated preference approaches. We then present the results of our case study where we found that deliberative democratic monetary valuation produced valuations that were perceived as more legitimate that deliberated preference as the basis for decision making by those involved in the valuation process. John Morrison has argued that confidences are assigned in perceptual experience. For example, when you perceive a figure in the distance, your experience might assign a 55-percent confidence to the figure’s being Isaac. Morrison’s argument leans on the phenomenon of ‘completely trusting your experience’. I argue that Morrison presupposes a problematic ‘importation model’ of this familiar phenomenon, and propose a very different way of thinking about it. While the article’s official topic is whether confidences are assigned in perceptual experience, it (...) is also about two more general issues: how we can determine which properties are assigned in perceptual experience; and how we transition from perception to belief. This paper surveys and evaluates the answers that philosophers and animal researchers have given to two questions. Do animals have thoughts? If so, are their thoughts conceptual? Along the way, special attention is paid to distinguish debates of substance from mere battles over terminology, and to isolate fruitful areas for future research.

Over the past century, the “right to health” has been recognized by medical organizations and governments across the globe. This essay calls for recognition of a parallel “right to be sick” intended to reframe physician attitudes toward patient autonomy and the right to refuse care. Rather than seeking to discourage allopathic medicine or to question laws requiring involuntary treatment when indicated by the collective welfare, the goal is to have the medical and public health communities reconceptualize their attitudes toward individuals (...) who reject unsought guidance and care. Are evolution and creation irreconcilably opposed? Is 'intelligent design' theory an unhappy compromise? Is there another way of approaching the present-day divide between religious and so-called secular views of the origins of life? Jacob Klapwijk offers a philosophical analysis of the relation of evolutionary biology to religion, and addresses the question of whether the evolution of life is exclusively a matter of chance or is better understood as including the notion of purpose. Writing from a Christian point of view, (...) he criticizes creationism and intelligent design theory as well as opposing reductive naturalism. He offers an alternative to both and an attempt to bridge the gap between them, via the idea of 'emergent evolution'. In this theory the process of evolution has an emergent or innovative character resulting in a living world of ingenious, multifaceted complexity. (shrink)

Objective interpretations of probability are usually discussed in two varieties: frequency and propensity accounts. But there is a third, neglected possibility, namely, probabilities as deriving from ranges in suitably structured initial state spaces. Roughly, the probability of an event is the proportion of initial states that lead to this event in the space of all possible initial states, provided that this proportion is approximately the same in any not too small interval of the initial state space. This idea can also (...) be expressed by saying that in the types of situations that give rise to probabilistic phenomena we may expect to find an initial state space such that any "reasonable" density function over this space leads to the same probabilities for the possible outcomes. This "method of arbitrary functions" was introduced by Poincaré, studied and extended by Hopf and more recently by Eduardo Engel, Jan von Plato and Michael Strevens. The natural-range, or method-of-arbitrary-functions approach to probabilities is usually treated as an explanation for the occurrence of probabilistic patterns, whereas I examine its prospects for an objective interpretation of probability, in the sense of providing truth conditions for probability statements that do not depend on our state of mind or information. The main objection to such a proposal is that it is circular, i.e. presupposes the concept of probability, because a measure on the initial state has to be introduced, and density functions over the space are considered. In his transcendental approach, Cassirer argues that an objective world is not given and then simply copied by our cognitive faculties; rather, it is gained through the development of symbolic thought and perception. According to Cassirer, language plays a crucial role in this process of objectification. In this paper, the close relationship between language and symbolism in Cassirer’s philosophy will be delineated. This will also shed light on possible distinctions between human speech and animal communication. Furthermore, the relation of language (...) to the different functions of consciousness and to the symbolic forms of myth and science will be addressed. Finally, based on the former considerations of language and objectification, Cassirer’s conceptions of reality and truth will be explicated and critically evaluated. It will be argued that his approach offers several interesting and promising insights but is nonetheless in danger of falling short in explaining the difference between artefacts and natural kinds. Major depressive disorder is one of the most common serious illnesses worldwide; the disease is also among those with the lowest rates of treatment. Barriers to access to care, both practical and psychological, contribute significantly to these low treatment rates. Among such barriers are regulations in many nations that require a physician’s prescription for most pharmacological treatments including selective serotonin reuptake inhibitors (SSRIs). These rules are designed to protect patients. However, such regulations involve a tradeoff between the welfare of “visible” (...) victims, who might suffer negative consequences from a lack of regulation, and the well-being of invisible “victims,” who likely experience negative consequences that result from increased barriers to care. This article explores these tradeoffs and argues in favor of shifting SSRIs from prescription-only to over-the-counter status. (shrink)

The qualities of Abraham's God, as tradition has them, are personal presence and guidance, a God who makes and keeps promises, who “covenants” with him and communicates with him directly; and who acts justly, not in a forensic sense but in a practical way.

A new public centered long-range corporate planning orientation is proposed. Underlying this orientation is the rationale that the main purpose of business activity is the satisfaction of socially desirable needs, i.e. needs that are compatible with long run public welfare. Such need satisfaction will tend to bring about improvements in the quality of private and public life. Corporate planning should thus facilitate the performance of business activities designed to achieve the satisfaction of socially desirable needs.The determination of what are socially (...) desirable needs will be based on public consensus, which corporations can periodically assess, and incorporate into their long range plans. A long-range planning procedure, incorporating social welfare considerations is developed. It is suggested that a planning procedure such as this is essential for the existence of firms in the long run. (shrink)

In the age of change in the institutional and conceptual setting in which the ancient tradition of Jewish learning would go forward, what we see in the two most important figures of the transitional generation is only the end of the old, not the beginning of the new. Saul Lieberman continued the received tradition that learning means exegesis of texts, but did not fully master the logic of that received tradition and so distorted it. Salo W. Baron undertook a new (...) intellectual tradition, one that pursued historical study in place of the exegesis of texts, but did. not really grasp the requirements of the kind of new history that he proposed to write. Announcing the advent of social history into the academy of Jewish learning, what he wrote as the economic part of that social history was merely the paraphrase of received texts, with glosses that look suspiciously like a kind of free-associative exegesis. While the one scholar demonstrated the decadence of an old tradition of learning, the other succeeded in showing only the difficulty of actually mastering the discourse of an entirely new academic world. Physicians expressing opinions on medical matters that run contrary to the consensus of experts pose a challenge to licensing bodies and regulatory authorities. While the right to express contrarian views feeds a robust marketplace of ideas that is essential for scientific progress, physicians advocating ineffective or dangerous cures, or actively opposing public health measures, pose a grave threat to societal welfare. Increasingly, a distinction has been made between professional speech that occurs during the physician-patient encounter and public speech that transpires (...) beyond the clinical setting, with physicians being afforded wide latitude to voice empirically false claims outside the context of patient care. This paper argues that such a bifurcated model does not sufficiently address the challenges of an age when mass communications and social media allow dissenting physicians to offer misleading medical advice to the general public on a mass scale. Instead, a three-tiered model that distinguishes between citizen speech, physician speech and clinical speech would best serve authorities when regulating physician expression. (shrink)

He must then overcome two problems. First, the extrinsicness of semantic properties seems to deprive them of causal power . Secondly, on minimalist physicalist assumptions, no entity can instantiate a semantic property unless it simultaneously instantiates some physical properties. A representation's semantic property risks therefore to be deprived of causal efficacy by its physical properties . As the World Anti-Doping Code is up for revision, the paper proposes a framework for reading the Code based on a relatively literal approach and an almost exclusive focus on the ?spirit of sport? as a key element of the Code. The author argues that this single element can contribute to revealing the underlying rationale of the Code, as it serves to justify bans of doping substances and methods, in some cases without recurring to evidence sustaining the claims made. For (...) a substance/method to be banned, the Code requires that two out of three criteria be met: performance enhancement, health risk and the spirit of sport. The paper demonstrates how these provisions link to other parts of the Code, (under the rules of the Code 2009) in the absence of performance-enhancing properties combined with a health risk. The paper draws implications from these findings regarding terminological (im)precision, legal certainty/predictability, transparency/accountability, limits to arbitrary exercise of power and internal/external validity. Non-cognitive gadgets are fancy tools shaped to meet specific, local needs. Cecilia Heyes defines cognitive gadgets as dedicated psychological mechanisms created through social interactions and culturally, not genetically, inherited by humans. She has boldly proposed that many human cognitive mechanisms are gadgets. If true, these claims would have far-reaching implications for our scientific understanding of human social cognition. Here we assess Heyes’s cognitive gadget approach as it applies to mindreading. We do not think that the evidence supports Heyes’s thought-provoking thesis (...) that human children are taught to read minds the way they are taught to read words. We highlight a potential circularity lurking behind this analogy, and we explain why we are unpersuaded by Heyes’s anti-mentalistic proposal for handling data inconsistent with the gadget view, which others take to be evidence for mindreading in human infancy. We conclude that while human minds may well be filled with gadgets, mindreading is unlikely to be one of them. (shrink)

This paper assesses the scope and limits of a widely influential model of goal-ascription by human infants: the shared-intentionality model. It derives much of its appeal from its ability to integrate behavioral evidence from developmental psychology with cognitive neuroscientific evidence about the role of mirror neuron activity in non-human primates. The central question raised by this model is whether sharing a goal with an agent is necessary and sufficient for ascribing it to that agent. I argue that advocates of the (...) shared-intentionality model underestimate both the distinction between the target and the goal of a goal-directed action and the gap between sharing and ascribing a goal. Although considerable attention has been devoted to the concepts of “visible” and “invisible” victims in general medical practice, especially in relation to resource allocation, far less consideration has been devoted to these concepts in behavioral health. Distinctive features of mental health care in the United States help explain this gap. This essay explores three specific ways in which the American mental health care system protects potentially “visible” individuals at the expense of “invisible victims” and otherwise fails to meet the needs (...) of great numbers of people with serious psychiatric conditions: prioritization of the wrong patients, incentivization of excessive caution among providers, and a narrow definition of psychiatry's purview. While each of these practices has been discussed elsewhere in the literature, they are rarely considered as part of an interrelated and systemic problem. Reconceptualizing these three issues as aspects of the larger conflict between the interests of “visible” and “invisible” victims may prove a path toward reform. (shrink)

Ideal theorists aim to describe the ideally just society. Problem solvers aim to identify concrete changes to actual societies that would make them more just. The relation between these two sorts of theorizing is highly contested. According to the benchmark view, ideal theory is prior to problem solving because a conception of the ideally just society serves as an indispensable benchmark for evaluating societies in terms of how far they deviate from it. In this paper, I clarify the benchmark view, (...) argue that existing criticisms of it are unsuccessful, and develop a novel redundancy objection to the benchmark view and the claim of priority it allegedly entails. I then consider the extent to which ideal theory might facilitate problem solving without being prior to it and argue that it can only play a modest role in this regard. The upshot is that ideal theory is neither required for nor especially relevant to problem solving—but it is not completely irrelevant either. It facilitates problem solving to some limited degree, but no more, say, than theorizing about dystopia. (shrink)

Antonie Vos argues that Thérines’s quodlibet exhibits the influence of Scotus’s, yet no solid evidence has yet been provided for the chronology of their delivery or publication. Most scholars agree that they likely delivered their quodlibets in the same year: 1306. This paper works from the ITC document, Synodality in the Life and Mission of the Church (2018), to outline four essential components of an emergent theology of synodality: the claim that synodality is constitutive of the Church, an ecclesiology of communion, the ecclesial title People of God, and the sensus fidei. The paper then critically assesses the interplay of these four elements in the ITC document, along with the Vademecum and the Preparatory Document which were disseminated with the beginning of (...) the synodal process itself (2021). Certain issues from this interplay, it is argued, require further reflection as this emergent theology develops: the theological weight of consensus, the degree to which belonging to the Church is (or is not) essentially discursive in character, how the sensus fidei seems to imply that the practice of the faith is an integral element of meaningful synodal participation by Catholics, and indeed, finally, the participation of those who do not bear the name of Christian. Intelligence, as measured by grades and/or standardized test scores, plays a principal role in the medical school admissions process in most nations. Yet while sufficient intelligence is necessary to practice medicine effectively, no evidence suggests that surplus intelligence beyond that threshold is correlated with providing higher quality medical care. This paper argues that using perceived measures of intelligence to distinguish between applicants, at levels that exceed the level of intelligence required to practice medicine, is both unfair to applicants and fails (...) to serve the interests of patients.
