Experiential evidence shows that pain is associated with common meanings. These include a meaning of threat or danger, which is experienced as immediately distressing or unpleasant; cognitive meanings, which are focused on the long-term consequences of having chronic pain; and existential meanings such as hopelessness, which are more about the person with chronic pain than the pain itself. This interdisciplinary book - the second in the three-volume Meanings of Pain series edited by Dr Simon van Rysewyk - aims to better (...) understand pain by describing experiences of pain and the meanings these experiences hold for the people living through them. The lived experiences of pain described here involve various types of chronic pain, including spinal pain, labour pain, rheumatic pain, diabetic peripheral neuropathic pain, fibromyalgia, complex regional pain syndrome, endometriosis-associated pain, and cancer-related pain. Two chapters provide narrative descriptions of pain, recounted and interpreted by people with pain. Language is important to understanding the meaning of pain since it is the primary tool human beings use to manipulate meaning. As discussed in the book, linguistic meaning may hold clues to understanding some pain-related experiences, including the stigmatisation of people with pain, the dynamics of patient-clinician communication, and other issues, such as relationships between pain, public policy and the law, and attempts to develop a taxonomy of pain that is meaningful for patients. Clinical implications are described in each chapter. This book is intended for people with pain, their family members or caregivers, clinicians, researchers, advocates, and policy makers. (shrink)

Ideal epistemologists investigate the nature of pure epistemic rationality, abstracting away from human cognitive limitations. Non-ideal epistemologists investigate epistemic norms that are satisfiable by most humans, most of the time. Ideal epistemology faces a number of challenges, aimed at both its substantive commitments and its philosophical worth. This paper explains the relation between ideal and non-ideal epistemology, with the aim of justifying ideal epistemology. Its approach is meta-epistemological, focusing on the meaning and purpose of epistemic evaluations. I provide an account (...) on which the fundamental difference between ideal and non-ideal epistemic evaluations is that only the non-ideal epistemic ‘ought’ implies any substantive ‘can’. I argue that only ideal epistemic evaluations are ‘normatively robust’: they are neither conventional nor seriously context-sensitive. Non-ideal epistemic evaluations are normatively non-robust, exhibiting both conventionality and serious context-sensitivity from an interesting variety of distinct sources. For this reason, non-ideal epistemic evaluations won’t characterize the fundamental nature of epistemic rationality. Non-ideal epistemic rationality depends, not merely on what’s epistemically valuable, but also on modally contingent epistemic conventions and contextually contingent constraints on epistemic options. If we want a normatively robust theory of epistemic rationality, ideal epistemology is the only game in town. (shrink)

Many are reluctant to accept primitive modality into their fundamental picture of the world. The worry often traces to this thought: we shouldn't adopt any more primitive - that is, unexplained - notions than we need in order to explain all the features of the world, and primitive modal notions are not needed. I examine one prominent rival to modal primitivism, combinatorialism, and show that in order to account for all the modal features of the world the combinatorialist must adopt (...) two additional primitive notions. My own modal primitivist view takes as primitive the notion of incompatibility between properties or relations. I show how the non-modal notions that the combinatorialist must adopt as primitive may be analyzed using my notion. The upshot is that with respect to the number of primitive notions, my modal primitivist theory comes out ahead. (shrink)

Prediction‐based processes appear to play an important role in language. Few studies, however, have sought to test the relationship within individuals between prediction learning and natural language processing. This paper builds upon existing statistical learning work using a novel paradigm for studying the on‐line learning of predictive dependencies. Within this paradigm, a new “prediction task” is introduced that provides a sensitive index of individual differences for developing probabilistic sequential expectations. Across three interrelated experiments, the prediction task and results thereof are (...) used to bridge knowledge of the empirical relation between statistical learning and language within the context of nonadjacency processing. We first chart the trajectory for learning nonadjacencies, documenting individual differences in prediction learning. Subsequent simple recurrent network simulations then closely capture human performance patterns in the new paradigm. Finally, individual differences in prediction performances are shown to strongly correlate with participants’ sentence processing of complex, long‐distance dependencies in natural language. (shrink)

In Moody Minds Distempered philosopher Jennifer Radden assembles several decades of her research on melancholy and depression. The chapters are ordered into three categories: those about intellectual and medical history of melancholy and depression; those that emphasize aspects of the moral, psychological and medical features of these concepts; and finally, those that explore the sad and apprehensive mood states long associated with melancholy and depressive subjectivity. A newly written introduction maps the conceptual landscape, and draws out the analytic and (...) thematic interconnections between the chapters. Radden emphasizes and develops several new themes: the implications, theoretical phenomenological and moral, of recognizing melancholy and depressive states as mood states; questions of method, as they affect how we understand and characterize claims about melancholy and depression; and the persistence and force of cultural tropes linking such states to brilliance, creativity, and sagacity. Insights from literature and the history of medicine, psychology, and psychiatry are woven together with those from the more recent disciplines of feminist theory and cultural studies. This is interdisciplinary writing at its best-part analytic philosophy, and part history of ideas. (shrink)

Stories are an important indicator of our vision of the future. In the case of artificial intelligence (AI), dominant stories are polarized between notions of threat and myopic solutionism. The central storytellers—big tech, popular media, and authors of science fiction—represent particular demographics and motivations. Many stories, and storytellers, are missing. This paper details the accounts of missing AI narratives by leading scholars from a range of disciplines interested in AI Futures. Participants focused on the gaps between dominant narratives and the (...) untold stories of the capabilities, issues, and everyday realities of the technology. One participant proposed a “story crisis” in which these narratives compete to shape the public discourse on AI. Our findings indicate that dominant narratives distract and mislead public understandings and conceptions of AI. This suggests a need to pay closer attention to missing AI narratives. It is not simply about telling new stories, it is about listening to existing stories and asking what is wanted from AI. We call for realistic, nuanced, and inclusive stories, working with and for diverse voices, which consider (1) story-teller; (2) genre, and (3) communicative purpose. Such stories can then inspire the next generation of thinkers, technologists, and storytellers. (shrink)

The paper aims to develop a theoretical reflection on existing knowledge in public policy and its implementation in the nineties. This is a documental research design of bibliographical nature. The results reveal that the body of knowledge in public policy has been under the domain of rational choice theory and the assumptions of the neoclassical school. I conclude that, in the context of an alternative model to the neoliberal one, one need a different theoretical tool for alternative public policy that (...) consider social rationality moving away from a deterministic and technical policy. Este trabajo tiene como propósito desarrollar una reflexión teórica al conocimiento existente en materia de política pública y de su implementación en la década de los noventa. Esta es una investigación documental con un tipo de diseño de carácter bibliográfico. Los resultados revelan que el cuerpo de conocimientos en materia de política pública, ha estado bajo el dominio de la Teoría de la Elección Racional y de los supuestos de la Escuela Neoclásica. Se concluye que, en el marco de un modelo alternativo al neoliberal, se necesita un instrumental teórico diferente para una política pública alternativa. Esto pasa por considerar una racionalidad social que se aleje de una concepción determinista y técnica de la política. (shrink)

In this paper, I explore the implications of adopting one model of self rather than another in respect to one particular feature of our mental life. The need to explain synchronic unity in normal subjectivity, and also to explain the apparent and puzzling absence of synchronic unity in certain symptoms of severe mental disorder, I show, becomes more pressing with one particular model. But in the process of developing that explanation we learn something about subjectivity and perhaps also something about (...) brain functioning. (shrink)

Book synopsis: The book illustrates the concept of action in three different contexts - the justification of actions, people's life history, and pragmatism. The special feature of this book is that a comprehensive view of this kind marks a departure from the atomistic approach of action theory, which in itself raises a number of questions. If actions are not justified by mental states, how can persons then act for reasons? How can persons' actions over time be described, and what is (...) the connection with the question of personal identity? If there is to be a unified understanding of the person, does the practical have to take precedence over the theoretical, and what does this mean for epistemology, for example? The ten contributors to this volume engage in an instructive manner with these and similar questions in the three sections of the book. (shrink)

In this introduction to the edited volume, we briefly describe some of the current challenges faced by public mental health initiatives, at both the national and global level. We also include several general remarks on interdisciplinary methodology in public mental health ethics, followed by short descriptions of the chapters included in the volume.

Surprisingly, though, God's love is depicted here as impersonal and indiscriminate, as similar to or even simply as a force of nature, even if a life-giving force: God `makes his sun to rise on the evil and the good, and sends rain on the just and the unjust' (Matthew 5:45). As virtuous leadership appears to be under severe pressure in all areas of life, it is the aim of this article to determine whether this biblical axiom holds suitable and valuable guidelines to counter contemporary leadership traps and assist leaders not to succumb to ambitious leadership cravings. The article determines whether the biblical axiom, the first shall be last and the last shall be first, holds the capacity to invert the corrupt pressure-cooker drives for both religious and secular leadership ambitions. (...) It examines the biblical and ethical wisdom of this teaching as well as the authoritative relevance it holds for leadership formation and practice. It determines whether a biblically infused value system still contains the moral capacity that contributes to the observance of resonant and ethical leadership capable of reversing pervasive situations of corrupt and incompetent leadership. In addition, the article contemplates the Christian weight of this biblical axiom to place those who are last, first in the service of leadership. In the same vein, it examines some of the ‘leadership traps’ experienced by persons who occupy lay, clerical, professional and secular positions. It endeavours to offer reflective biblical, moral and spiritual solutions that arise from the spirituality offered by Matthew 20:16 to current leadership demises.Contribution: The theme of the Leadership conference where this article was presented was the biblical verse: ‘The first shall be last and the last shall be first’. We study the relationship between algebraic structures and their inverse semigroups of partial automorphisms. We consider a variety of classes of natural structures including equivalence structures, orderings, Boolean algebras, and relatively complemented distributive lattices. For certain subsemigroups of these inverse semigroups, isomorphism of the subsemigroups yields isomorphism of the underlying structures. We also prove that for some classes of computable structures, we can reconstruct a computable structure, up to computable isomorphism, from the isomorphism type of its inverse semigroup of computable (...) partial automorphisms. Under what circumstances should the federal government pay for outpatient prescription drugs? Should the government (and by extension, taxpayers) pay for all of the drugs prescribed by health care providers, regardless of price or use—adhering to a medical standard? Or should taxpayers only pay for prescriptions supported by scientific evidence of effectiveness—a scientific standard?

This paper argues that judges assessing the scientific validity and the legal admissibility of forensic science techniques ought to privilege testing over explanation. Their evaluation of reliability should be more concerned with whether the technique has been adequately validated by appropriate empirical testing than with whether the expert can offer an adequate description of the methods she uses, or satisfactorily explain her methodology or the theory from which her claims derive. This paper explores these issues within two specific contexts: latent (...) fingerprint examination and the use of breath tests for the detection of alcohol. Especially in the forensic science arena, I suggest courts have often been seduced by superficially plausible explanations and descriptions of a technique or method, and permitted these to serve as a substitute for empirical testing. Thinking through these two examples illustrates both why evaluating the extent of testing should be the most important method by which courts assess reliability, and why, when other forms of explanatory evidence are readily available, we may nonetheless elect to make use of them. This paper suggests that these descriptions and explanations may at times usefully supplement evidence of testing, but should not generally be substituted for it. Finally, this paper embraces a kind of evidentiary pragmatism, in which the quantum of evidence required to establish legal reliability is determined not in the abstract, but in relation to the evidence that is, or ought to be, available as a result of reasonable research and investigation. The Federal Trade Commission has an important role to play in the governmental oversight of mobile health apps, ensuring consumer protections from unfair and deceptive trade practices and curtailing anti-competitive methods. The FTC’s consumer protection structure and authority is outlined before reviewing the recent FTC enforcement activities taken on behalf of consumers and against developers of mhealth apps. The article concludes with identification of some challenges for the FTC and modest recommendations for strengthening the consumer protections it provides.

We argue that strong moral objections to widespread implementation of uterine transplantation persist despite recent live births following the procedure. These objections relate not only to the serious medical risk to which live donors are currently subject but also to the strength of pronatalistic and biologistic social forces. We explore medical risk in light of various factors and treat questions relating uterine transplantation to gestational surrogacy.

Johnson conceptualizes the social responsibilities of digital media platforms by describing two ethical approaches: one emphasizing the discursive freedom of platform-users, the other emphasizing protecting users from harmful posts. These competing concerns are on full display in the current debate over platforms’ obligations during the COVID-19 pandemic. While Johnson argues both approaches are grounded in democracy, we argue that democratic commitments transcend the freedom/harm dichotomy. Instead, a commitment to democracy points toward social media companies’ responsibilities to structure their platforms in (...) ways that facilitate perspectival diversity and collective deliberation. ABSTRACT In Making Things Up, Bennett defends an impressive array of theses surrounding the notion of building. My focus is on Bennett’s use of modal recombination principles in her arguments, including in particular the principle that contingent fundamental entities are freely recombinable. I have argued that such principles are motivated by mere intuition, and that we have reasons to reject them. I discuss how worries about modal recombination principles affect three of her key arguments, which concern whether building is necessitating, (...) whether relative fundamentality is primitive, and whether building is fundamental. I argue that while Bennett’s appeal to modal recombination is unjustified, she has other resources available to her in each of the three cases. (shrink)

In this dissertation, I examine our mental health concepts to see what work they are currently doing as well as what work they could be doing. In 1976, Christopher Boorse stated that the mental health literature is a “web of obscurities” (p. 51). To resolve some of this confusion, I argue that we need to consider the goals we should have for our mental health concepts and then give accounts of those concepts that meet our stated goals. I argue that (...) our goals for our mental health concepts should be twofold: first, we need concepts that will work to alleviate the stigmatization of mental disorder as this stigmatization gets in the way of providing and receiving medical help, and second, we need concepts that will be useful tools for medical practitioners to use in their diagnosis and treatment practices. -/- I begin by arguing that our mental disorder concept is not currently a useful tool for medical practitioners. Moreover, I argue that our mental disorder concept is confused in such a way that it is unlikely that it could ever be a useful tool for medical practitioners. If we cannot give an account of the mental disorder concept that is a useful tool for medical practitioners, then perhaps we can give an account of the mental disorder concept that can be used toward our goal of alleviating the stigmatization of mental disorder. Before we can do that, however, we need an account of the stigmatization concept so that we can understand what our mental disorder concept needs to be alleviating. -/- I argue that stigmatization is a process whereby some entity becomes marked as disgraceful or shameful. This stigmatization process includes (1) an attitude against the entity for reasons found in the social ideology, and (2) acts against the entity. I then argue that acts of stigmatization take two forms: direct stigmatization (where stigmatizors take acts against the stigmatized entity) and indirect stigmatization (where potential victims of stigmatization take acts to avoid becoming victims of stigmatization). With that in mind, we learn that if we are to alleviate the stigmatization of mental disorder, we need to work on alleviating both direct and indirect stigmatization. It is at this point that I argue for a new account of our mental disorder concept that can be utilized to alleviate the direct and indirect stigmatization of mental disorder. With our mental disorder concept redesigned to achieve that goal, I turn to the goal of providing medical help for persons that need that help. I argue that we need to reconceptualize what it means to be a ‘diagnosable condition’ so that we can develop a new framework for diagnosis and treatment that will be more efficacious than our current system. In conclusion, I suggest that we can untangle the current confusion that is the mental health literature by imbuing our mental health concepts with specific purposes and then utilizing them to achieve those purposes as I outline in this dissertation. Hume argues that the difference between belief and mere conception consists in a difference in the manner of conception. His argument assumes that the difference between belief and mere conception must be a function of either the content conceived or of the manner of conception; however, it is unclear what justifies this assumption. I argue that the assumption depends on Hume’s confidence that we can know immediately that we believe when we believe, and that we can only have such knowledge (...) of intrinsic features of our perceptions. I then claim that Hume’s argument against the view that the difference between belief and mere conception is a function of the content conceived faces a difficulty, because it relies on an apparently implausible view about mental representation. I propose an interpretation of the argument that avoids the difficulty and explains Hume’s puzzling claim that his account of belief answers “a new question unthought of by philosophers.”. (shrink)

Children and pregnant women are often excluded from clinical research. This has resulted in a paucity of evidence on how medicines work for fetuses, neonates, infants, and adolescents. It also raises bioethics, scientific, and public health concerns. For over half a century, doctors have prescribed medicines to children largely on the basis of how they work in adults, despite children’s varied physiologies and differences in how their bodies absorb and metabolize drugs. Regulations and legislation have led to an increase in (...) the number of pediatric studies and to better drug labeling. However, children at all stages of their lives often remain “therapeutic orphans” owing to insufficient evidence about how medicines work for them. (shrink)
