We give a sharper version of a theorem of Rosický, Trnková and Adámek [13], and a new proof of a theorem of Rosický [12], both about colimits in categories of structures. Unlike the original proofs, which use category-theoretic methods, we use set-theoretic arguments involving elementary embeddings given by large cardinals such as $\alpha$-strongly compact and $C^{(n)}$-extendible cardinals.

Reconstructive allografts using Vascularized Composite Allotransplantation are providing individuals living with upper limb loss and facial disfigurement with new opportunities for a sensate, esthetically acceptable, and functional alternative to current treatment strategies. Important research attention is being paid to how best to assess and screen candidates for VCA, measure optimal patient outcomes, and support patient adherence to lifelong behaviors and medical regimens. Far less attention, however, has been dedicated to the team science required for these complex VCA teams to form, (...) prepare, and provide the highest quality clinical and psychosocial care to those receiving VCA. VCA teams are unique in that they require specialized team members whose scope of practice may not otherwise overlap. The team also needs to constantly negotiate balancing patient safety with multiple risks throughout the transplant process. This study aimed to elucidate the team science needed for this highly innovative and complex area of medicine. Using in-depth qualitative interviews with 14 VCA team members and observations at team meetings, we found that careful consideration of team composition, team structure, and organizational commitment influences team performance and patient outcomes, but that to be efficient and truly effective, teams need to commit to developing processes that foster collaboration. These processes are action-oriented, strategic and interpersonal. Dedication and commitment to team science allows teams to manage conflict under stress and exercise ways to leverage strengths to provide optimal performance or patient psychosocial and clinical outcomes. This study can provide insight into quality improvement efforts for VCA teams and guidance for other transplant programs that wish to consider expansion into VCA. (shrink)

On the Contemplative Life is known for its depiction of a philosophical group of Jewish men and women known as the 'Therapeutae'. Yet the reasons for their depiction have been little understood. In the first commentary on the treatise in English for over 100 years, the social, cultural and political background of the times in which Philo lived are shown to be crucial in understanding Philo's purposes. As Alexandrian Jews were vilified and attacked, Philo went to Rome to present the (...) case for his community, faced with intense opposition. Side-stepping direct confrontation, Philo here cleverly presents the Therapeutae as the pinnacle of excellence, most especially in their communal meal, while ridiculing his accusers in a stinging parody of a festive banquet. The goals of vascular composite allotransplantation for hand are to maximize functional status and psychosocial wellbeing and to improve quality of life. Candidates are carefully vetted by transplant programs through an extensive evaluation process to exclude those patients with contraindications and to select those that are most likely to attain functional or quality of life benefit from transplant. Patient choice for any treatment, however, requires that candidates be able to understand the risks, benefits, and alternatives before choosing to proceed. This (...) study aimed to understand patients’ knowledge and perceptions about treatment options for hand loss, including hand transplant. This study will be used to inform a standardized education approach and develop conversation aids for use by clinicians and patients throughout the treatment decision process. Ten individuals who had experienced hand amputation or had congenital limb loss were interviewed to better understand previous and current decisions about treatment, experiences in adjusting to their treatment, and perceptions about hand VCA. From this qualitative interview data, four findings emerged: knowledge and education around VCA as a treatment option; adaptation of individuals with limb loss; fear of risk associated with transplantation; issues of aging and overuse injuries to existing limbs. Results suggests that there is opportunity for expanding education about all treatment options for patients with new loss, long-term loss, and congenital limb loss. Establishing a baseline of knowledge about all options–prosthetics, rehabilitative strategies, and VCA—can help patients evaluate their values and goals of treatment. Issues associated with aging, including overuse and injury, and adaptability over the life course should be included in considerations about treatment choices. Data indicate the need for routinely assessing patient preferences about treatment choice so patients can plan for their future as they adapt and age and as technology for treatments change. To assure that thorough information is provided for current and future decision-making, education about treatment choices and selection procedures for VCA should be standardized. (shrink)

The 'Therapeutae' were a Jewish group of ascetic philosophers who lived outside Alexandria in the middle of the first century CE. They are described in Philo's treatise De Vita Contemplativa and have often been considered in comparison with early Christians, the Essenes, and the Dead Sea Scrolls. But who were they really? This study focuses particularly on issues of history, rhetoric, women, and gender in a wide exploration of the group, and comes to new conclusions about the 'Therapeutae' and their (...) relationship with the Jewish allegorical school of exegesis in Alexandria. This article will address the overall responsibilities and current projects relating to human subject protections.OPRR implements the Department of Health and Human Services' (HHS) regulations for the protection of human subjects (45 CFR (...) 46 (1991)). These regulations fulfill the responsibilities set forth in the Public Health Service (PHS) Act with regard to institutional review boards (IRB) and other protections for human subjects involved in biomedical and behavioral research conducted or supported by HHS. The PHS Act also mandates a program to clarify ethical issues in biomedical research involving human subjects and to address violations of the rights of human subjects of research (P.L. 99-158 (1985)).OPRR staff negotiate assurances of compliance with institutions conducting Federally-supported research involving human subjects. The assurances describe the policies and procedures each institution will follow to fulfill the requirements of the regulations, including specific IRB composition and functions.In addition, OPRR cosponsors with the Food and Drug Administration and awardee institutions a series of regional workshops that address human subject protections issues. It remains unclear whether such genetic variants are associated with disease aggressiveness. The NCI-SPORE Genetics Working Group retrospectively collected clinicopathologic information and genotype data for 36 SNPs which at the time had been validated to be associated with PC risk from 25,674 cases with PC. Cases were grouped according to race, Gleason score and aggressiveness. Statistical analyses were used to compare the frequency (...) of the SNPs between different disease cohorts. After adjusting for multiple testing, only PC-risk SNP rs2735839 was significantly and inversely associated with aggressive and high-grade disease in European men. Similar associations with aggressive and high-grade disease were documented in African-American subjects. The G allele of rs2735839 was associated with disease aggressiveness even at low PSA levels in both European and African-American men. Our results provide further support that a PC-risk SNP rs2735839 near the KLK3 gene on chromosome 19q13 may be associated with aggressive and high-grade PC. Future prospectively designed, case-case GWAS are needed to identify additional SNPs associated with PC aggressiveness. (shrink)

Calhoun’s critical sociology relies not only on engagements with Habermas, Bourdieu and Taylor, but also on the middle range empirical traditions of American sociology. Through a review of his recent work on cosmopolitanism and globalization, community and solidarity, and public spaces and sociology, I propose that his search to explain different ways in which solidarity is developed offers a robust sociological foundation for the development of the most appropriate intellectual formation, and institutional sequel, to the emancipatory project that undergirds (...) critical theory’s hope and commitment. At the same time, we all need to rethink how both critical theory and more empirical sociologies can help us recognize the issues and agencies most vital for the address of contemporary cultural, social and biophysical needs. This paper advances a novel conception of the child’s best interest in regard to pediatric surgeries that do not promote the preventive or therapeutic health needs of children, or elective pediatric surgeries (EPS). First, children’s capacity for decision-making is examined, and the best decision-making model for EPS is identified as the Best Interest Standard. What follows is a discussion of the interests of children in the context of EPS, the correlation of fundamental interests to rights, and guidelines for weighing children’s (...) competing interests. Next, the rights and duties of parents as proxy decision makers are considered. Finally, a reinterpretation of the Best Interest Standard is proposed, identifying as paramount a child’s ability to make elective medical decisions for herself when she reaches maturity. (shrink)

: Catholic teaching has no moral difficulties with research on stem cells derived from adult stem cells or fetal cord blood. The ethical problem comes with embryonic stem cells since their genesis involves the destruction of a human embryo. However, there seems to be significant promise of health benefits from such research. Although Catholic teaching does not permit any destruction of human embryos, the question remains whether researchers in a Catholic institution, or any researchers opposed to destruction of human embryos, (...) could participate in research on cultured embryonic stem cells, or whether a Catholic institution could use any therapy that ultimately results from such research. This position paper examines how such research could be conducted legitimately in a Catholic institution by using an ethical analysis involving a narrative context, the nature of the moral act, and the principle of material cooperation, along with references to significant ethical assessments. It also offers tentative guidelines that could be used by a Catholic institution in implementing such research. (shrink)

Multiple scholars and institutions have asked what distinguishes public health research from public health practice. Most often, they ask in order to have a clear definition of what one does in various public health settings to assess oversight and/or regulation of human subjects research. More importantly, however, whether something is considered public health research or public health practice has real ethical implications in terms of the general moral considerations at stake and the obligations of public health researchers/practitioners to the populations (...) they serve or study.Numerous examples in recent history of research ethics, including the Kennedy Krieger Lead Abatement Study and EPA’s Children’s Environmental Exposure Research Study, suggest that an exploration of the ethics of public health, or more generally population-based research, may be warranted. Although we acknowledge that there are important ethical issues to consider in the implementation of public health practice, we leave that discussion for other authors. (shrink)

They are widespread both in the United States and internationally; a National Institute of Health database reports over 106,000 active industry and government-sponsored trials (National Institutes of Health n.d.). While political and ethical philosophers today are familiar with critiques of confinement in both critical prison studies and critical animal studies, The Ethics of Captivity is unusual in that it brings these critiques of incarceration together, bridging human and nonhuman animal liberation movements. While Lisa Guenther’s recent book, Solitary Confinement: Social Death and Its Afterlives, also critiques the mass incarceration of both human and nonhuman animals, it is far more common to see human and animal liberation movements opposed on this (...) issue, as when the incarceration of humans is deplored for treating those individuals like animals. In 2013, one year after the enactment of the landmark Mental Health Act, a small team of psychiatrists, psychologists, and statisticians from the University of Ghana Medical School began a controversial field-based randomized-control trial entitled "Joining Forces" at the Mount Horeb prayer camp. Located about 45 minutes outside of Accra, Mt. Horeb is one of the largest and best known prayer camps in Ghana. Charles Taylor has been one of the most original and influential figures in contemporary philosophy: his 'philosophical anthropology' spans an unusually wide range of theoretical interests and draws creatively on both Anglo-American and Continental traditions in philosophy. In this paper I consider what may seem an "accidental" family relationship and stress its worrying attributes for contemporary family life. While we have made strides in being willing and able to "queer" kinship relationships, another frontier remains in thinking about the family. Not all forms of family relations are kinship or quasi-kinship relationships. I refer to a kind of family relationship that remains very largely invisible: that between the household's kin and their domestic servants. Throughout most of the twentieth (...) century writers about the family came to think of such relationships as unimportant because they were an historical curiosity, their resurgence re-raises unresolved and serious questions about the ethics and politics of the contemporary family. The important questions that I want to raise are these: if the family should serve as a school for justice, then what does the presence of servants teach? Historically and in contemporary terms, what does the nature of servitude have to do with autonomy and dependency? Does Frege have a metatheory for his logic? There is an obvious and uncontroversial sense in which he does. Frege introduces and discusses his new logic in natural language; he argues, in response to criticisms of Begriffsschrift, that his logic is superior to Boole's by discussing formal features of both systems. In so far as the enterprise of using natural language to introduce, discuss, and argue about features of a formal system is metatheoretic, there can be no doubt: Frege has (...) a metatheory. There is also an obvious and uncontroversial sense in which Frege does not have a metatheory for his logic. The model theoretic semantics with which we are familiar today are a post-Fregean development. The question I address in this paper is, does Frege have a metatheory in the following sense: do his justifications of his basic laws and rules of inference employ, or even require, ineliminable use of a truth predicate and metalinguistic variables? My answer is ‘no’ on both counts. I argue that Frege neither uses, nor has any need to use, a truth predicate or metalinguistic variables in his justifications of his basic laws and rules of inference. Quine's famous explanation of the need for semantic ascent simply does not apply to Frege's logic. The purpose of the discussions that are typically understood as constituting Frege's metatheory is, rather, elucidatory. And once we see what the aim of these particular elucidations is, we can explain Frege's otherwise puzzling eschewal of the truth predicate in his discussions of the justification of the laws and rules of inference. Each generation invents new practices and new writings of philosophy. Ours should have been able to introduce certain mutations that would at least be equivalent with those of cubism, abstract art, and twelve-tone serialism: it has only partially done so. But after all the deconstructions, after Wittgenstein, Heidegger, and Derrida, this demand takes on a different dimension: What do we do with philosophy itself? How do we globally change our relation to this thought, which keeps indicating that it is increasingly (...) conservative and repetitive? These two questions together have prompted what we call “non-philosophy.” Non-philosophy is not the negation of philosophy. It is the suspension of philosophy’s claim to think the real, and it is the invention of new usages of thought and language that disrupt the rational narrative of the real, which is precisely what every philosophy is. Non-philosophy should rather be understood à la the “non-Euclidean,” namely, as a generalization of the philosophical beyond its traditional limitation by the unitary or “Heraclitean” postulate. From then on, an infinite number of philosophical decisions that are no longer mutually exclusive will correspond with any real phenomenon. _Philosophy and Non-Philosophy_ is widely considered the first fully explicit elaboration of non-philosophy and one of its most important introductory texts. Herman Philipse’s Heidegger’s Philosophy of Being is an attempt to interpret, analyze, and ultimately discredit the whole of Heidegger’s thought. But Philipse’s reading of the texts is uncharitable, and the ideas he presents and criticizes often bear little resemblance to Heidegger’s views. Philipse relies on a crude distinction between “theoretical” and “applicative” interpretations in arguing that Heidegger’s conception of interpretation as a kind of projection is, like the liar’s paradox, formally self-defeating. But even granting the distinction, the charge of reflective (...) incoherence is fallacious and question-begging. Finally, Philipse advances the astonishing “interpretive hypothesis” that the seemingly morbid existential themes in Being and Time were part of a deliberate “Pascalian strategy” to win converts to Heidegger’s own idiosyncratic “postmonotheist worship of Being.” In short, notwithstanding its nearly comprehensive coverage of Heidegger’s works, the book does not represent a sufficiently serious effort to understand the complexities and obscurities of Heidegger’s thinking. Although what language users in different cultures say about their own language has long been recognized as of potential interest, its theoretical importance to the study of language has typically been thought to be no more than peripheral. Theorizing Language is the first book to place the reflexive character of language at the very centre both of its empirical study and of its theoretical explanation. Language can only be explained as a cultural product of the reflexive application of its own (...) creative powers to construct, regulate, and give conceptual form to objects of understanding. Language is itself, first and foremost, an object of cultural understanding. Theoretical analyses of language which have neglected its reflexive character, or simply taken its effects for granted, merely impose their own artificial structures on their analytical object. The first part of this book discusses the consequences of neglecting this reflexive character for the technical concepts and methods which are used in analysing different types of communicational phenomena. In the second part, normativity - a crucial aspect of language's reflexive nature - is examined. The book's third and final part focuses on particular issues in the history of linguistic thought which bear witness to the rhetoric of language theorizing as a reflexive form of inquiry. (shrink)
