Ethical climate (EC) has been broadly described as how well institutions respond to ethical issues. Developing a tool to study and evaluate EC that aims to achieve sustained improvements requires a contemporary framework with identified relevant drivers. An extensive literature review was performed, reviewing existing EC definitions, tools and areas where EC has been studied; ethical challenges and relevance of EC in contemporary paediatric intensive care (PIC); and relevant ethical theories. We surmised that existing EC definitions and tools designed to (...) measure it fail to capture nuances of the PIC environment, and sought to address existing gaps by developing an EC framework for PIC founded on ethical theory. In this article, we propose a Paediatric Intensive Care Ethical Climate (PICEC) conceptual framework and four measurable domains to be captured by an assessment tool. We define PICEC as the collective felt experience of interdisciplinary team members arising from those factors that enable or constrain their ability to navigate ethical aspects of their work. PICEC both results from and is influenced by how well ethical issues are understood, identified, explored, reflected on, responded to and addressed in the workplace. PICEC encompasses four, core inter-related domains representing drivers of EC including: (1) organisational culture and leadership; (2) interdisciplinary team relationships and dynamics; (3) integrated child and family-centred care; and (4) ethics literacy. Future directions involve developing a PICEC measurement tool, with implications for benchmarking as well as guidance for, and evaluation of, targeted interventions to foster a healthy EC. (shrink)

In this paper I discuss a cardboard dissected globe made in 1813 by Edward Mogg, a cartographer and map seller, to instruct children in the principles of astronomy. Since little is known about the maker or the specific object, I draw on evidence beyond the sphere itself to construct an account of how the object might have been used. In particular I address conversation as a key part of astronomical education and examine the way in which the cardboard plates of (...) the sphere, the presentation slipcase and the accompanying booklet would have prepared young learners for polite astronomical discourse around large instruments in later life.Keywords: Dissected globe; Edward Mogg; Educational toys; Astronomy; Conversation. Paediatric Intensive Care Units (PICU) are complex interdisciplinary environments where challenging, high stakes decisions are frequently encountered. We assert that appropriate decisions are more likely to be made if the decision-making process is comprehensive, reasoned, and grounded in thoughtful deliberation. Strategies to overcome barriers to high quality decision-making including, cognitive and implicit bias, group think, inadequate information gathering, and poor quality deliberation should be incorporated. Several general frameworks for decision-making exist, but specific guidance is scarce. In this paper, we provide (...) specific guidance on collaborative complex decision-making for PICUs. The proposed approach is on principles of procedural justice and pragmatic hermeneutics. The process encompasses set-up/planning, information gathering, question formulation, analysis (perspectives, values, and principles), action plan development, decision documentation, and a review and appeal mechanism. The process can be adapted to suit other clinical contexts. Research evaluating the process, exploring how best to develop education for clinicians, and how to build a culture that values high quality deliberation, is worthwhile. (shrink)

Measuring outcomes using quantitative analytic methods is the hallmark of scientific research in healthcare. For clinical ethics support services (CESS), tangible outcome metrics are lacking and literature examining CESS quality is limited to evaluation of single cases or the influence on individual healthcare professional’s perceptions or behaviour. This represents an enormous barrier to implementing and evaluating ethics initiatives to improve quality. In this context, Kok _et al_ propose a theoretical framework for how moral case deliberation (MCD) can drive quality at (...) an organisational-level, termed _morisprudence_ 1. By offering mechanistic insights into how MCD can lead to organisational learning and quality improvement (QI), they provide a roadmap for actionable items to measure quality. As conceptualised, _morisprudence_ follows accumulation of organisational practical wisdom from a repertoire of ‘paradigmatic’ MCDs, aiding identification of patterns of morally important features. This narrative symposium examines the relationship of bioethics practice to personal experiences of illness. A call for stories was developed by Tod Chambers, the symposium editor, and editorial staff and was sent to several commonly used bioethics listservs and posted on the Narrative Inquiry in Bioethics website. The call asked authors to relate a personal story of being ill or caring for a person who is ill, and to describe how this affected how they think about bioethical questions and the (...) practice of medicine. Eighteen individuals were invited to submit full stories based on review of their proposals. Twelve stories are published in this symposium, and six supplemental stories are published online only through Project MUSE. Authors explore themes of vulnerability, suffering, communication, voluntariness, cultural barriers, and flaws in local healthcare systems through stories about their own illnesses or about caring for children, partners, parents and grandparents. Commentary articles by Arthur Frank, Bradley Lewis, and Carol Taylor follow the collection of personal narratives. (shrink)

As the coronavirus pandemic continues apace in the United States, the dizzying amount of data being generated, analyzed and consumed about the virus has led to calls to proclaim this the first ‘data-driven pandemic’. But at the same time, it seems that this plethora of data has not meant a better grasp on the reality of the pandemic and its effects. Even as we have the potential to digitally track and trace nearly every single individual who has contracted the virus, (...) we have no idea exactly how many people have had the virus, been hospitalized, or died because of it, largely due to a confluence of factors, particularly active obfuscation and mismanagement by public authorities and misinformation spread through social media and right-wing media channels. But beyond these dynamics, there also lies the less nefarious ways that the everyday, subjective practices of data collection, analysis and visualization have the potential to themselves produce these very same dynamics where data is at once valorized and ignored, preeminent and completely useless. That is, the pandemic has revealed only the general inadequacy of our data infrastructures and assemblages to solving pressing social issues, but also the more general shift towards a ‘post-truth’ disposition in contemporary social life. But, as this paper argues, it would be a mistake to see the centrality of data as being somehow the opposite from the larger post-truth apparatus, as the two are instead fundamentally intertwined and co-produced. This thesis argues that the strongest account of moral rights entails that animals and other marginal cases hold rights. The thesis contends that mutual advantage social contract theories offer the strongest account of rights from a security perspective, and that such theories entail rights for animals and marginal cases. Both of these claims are widely contested. Chapter 1 examines the fundamental elements of a social contract theory as developed by Hobbes and Hume. Chapter 2 revises the fundamental elements of contract (...) theory to make them more persuasive, and derives from them an account of rights for animals and marginal cases. Chapter 3 examines the most plausible competing accounts of rights for animals and marginal cases: utilitarianism, neo-Kantianism, and capability theory. This paper explores the variety of ways that emerging sources of data are being used to re-conceptualize the city, and how these understandings of what the urban is shapes the design of interventions into it. Drawing on work on the performativity of economics, this paper uses two vignettes of the ‘new urban science’ and municipal vacant property mapping in order to argue that the mobilization of Big Data in the urban context doesn’t necessarily produce a single, greater understanding of the (...) city as it actually is, but rather a highly variegated series of essentialized understandings of the city that render it knowable, governable and intervene-able. Through the construction of new, data-driven urban geographical imaginaries, these projects have opened up the space for urban interventions that work to depoliticize urban injustices and valorize new kinds of technical expertise as the means of going about solving these problems, opening up new possibilities for a remaking of urban space in the image of these sociotechnical paradigms. Ultimately, this paper argues that despite the importance of Big Data, as both a discourse and practice, to emerging forms of urban research and management, there is no singular or universal understanding of the urban that is promoted or developed through the application of these new sources of data, which in turn opens up meaningful possibilities for developing alternative uses of Big Data for understanding and intervening in the city in more emancipatory ways. The aim of this study was to explore the ethical foundations for a caring conversation. The analysis is based on the ethics of Paul Ricoeur and deals with questions such as what kind of person the nurse ought to be and how she or he engages in caring conversations with suffering others. According to Ricoeur, ethics (the aim of an accomplished life) has primacy over morality (the articulation of aims in norms). At the ethical level, self-esteem and autonomy were shown (...) to be essential for a person (nurse) to act with respect and responsibility. The ethical relationship of a caring conversation was found to be asymmetrical, because of the passivity inflicted by suffering. This asymmetry was found to be potentially unethical if not balanced with reciprocity. In the ethical context, the caring conversation is one in which the nurse makes room through the ethos of caritas for a suffering person to regain his or her self-esteem, and thus makes a good life possible. From the publication of _Growing Up Absurd_ in 1960 until his death in 1972, Paul Goodman had the ear of the young radicals of the New Left, pouring forth books and articles on education, technology, decentralization, and of course, the war in Vietnam. Yet Goodman saw himself primarily as an artist rather than a political thinker or sociologist, and many of his books, even during the 1960s, were works of poetry, drama, and fiction. He had also practiced as a psychotherapist (...) and joined with Frederick Perls and Ralph Hefferkine in producing a new synthesis in psychological thought, Gestalt therapy, which has since become an international movement. In an age of specialization, few writers have taken on so braod a range of concerns. _Crazy Hope and Finite Experience_ is the final summing up of the thought and life of a self-described "old-fashioned man of letters." This book brings together for the first time five personal essays, all written near the end of his life, in which Goodman discusses his sense of the world and how he was "in" it, his politics, his spiritual and religious attitude, his sexuality, and his calling as a literary artist. For those already familiar with one or another aspect of his work, Goodman's self-assessment will provide new insight into the credo that underlies his whole career. For those learning about him for the first time, it offers a vivid sense of the man and his perspective. What is cruelty? What makes some people cruel? Under what conditions can cruelty grow? Taylor draws together aspects of psychology, sociology, philosophy, and her own field of neuroscience, illustrated with examples from history and the arts, in this thoughtful exploration of the nature and origins of cruelty, and how we might seek to reduce it.

This essay some first steps toward the naturalization of what I call rational intentionality or alternatively type II intentionality. By rational or type II intentionality, I mean that full combination of rational powers and content-bearing states that is paradigmatically enjoyed by mature intact human beings. The problem I set myself is to determine the extent to which the only currently extant approach to the naturalization of the intentional that has the singular virtue of not being a non-starter can be aggregated (...) up into an account of rational intentionality. I have in mind a broadly defined family of accounts whose main members are the indicator/information-theoretic approach of Dretske (1988), the asymmetric dependence theory of Jerry Fodor (1987, 1990, 1994) and the teleo-semantics of Ruth Millikan (1984, 1993). Somewhat inaccurately, I will call this family of approaches the information-theoretic family. To be sure, there is only a rough family resemblance among the members of the information-theoretic family. Indeed, several intense quarrels divide the members of that family one from another, but the precise outcome of those internecine struggles is not directly relevant to the aims of this essay. ² Taken collectively, the information-theoretic family yields a compelling picture of the place of at least a crude form of intentionality -- what I call frog-like or type I intentionality -- in the natural order. Though frog-like or type I intentionality is, I think, a genuine species of intentionality, it may subsist in the absence of rational powers. The paper offers an account of justified resentment and its importance in preserving human dignity. I situate the argument in the context of Martha Nussbaum's recent work against anger and resentment. Drawing on Enlightenment thinkers, I show the importance of resentment in deterring injury, in creating greater solidarity and humanity, and in preserving human dignity. The paper also offers a preliminary analysis of the norms that help to ensure appropriately expressed resentment.

Maternal grandmothers play a key role in allomaternal care, directly caring for and provisioning their grandchildren as well as helping their daughters with household chores and productive labor. Previous studies have investigated these contributions across a broad time period, from infancy through toddlerhood. Here, we extend and refine the grandmothering literature to investigate the perinatal period as a critical window for grandmaternal contributions. We propose that mother-daughter co-residence during this period affords targeted grandmaternal effort during a period of heightened vulnerability (...) and appreciable impact. We conducted two focus groups and 37 semi-structured interviews with Himba women. Interviews focused on experiences from their first and, if applicable, their most recent birth and included information on social support, domains of teaching and learning, and infant feeding practices. Our qualitative findings reveal three domains in which grandmothers contribute: learning to mother, breastfeeding support, and postnatal health and well-being. We show that informational, emotional, and instrumental support provided to new mothers and their neonates during the perinatal period can aid in the establishment of the mother-infant bond, buffer maternal energy balance, and improve nutritional outcomes for infants. These findings demonstrate that the role of grandmother can be crucial, even when alloparenting is common and breastfeeding is frequent and highly visible. Situated within the broader anthropological and clinical literature, these findings substantiate the claim that humans have evolved in an adaptive sociocultural perinatal complex in which grandmothers provide significant contributions to the health and well-being of their reproductive-age daughters and grandchildren. Many examples of calibration in climate science raise no alarms regarding model reliability. We examine one example and show that, in employing Classical Hypothesis-testing, it involves calibrating a base model against data that is also used to confirm the model. This is counter to the "intuitive position". We argue, however, that aspects of the intuitive position are upheld by some methods, in particular, the general Cross-validation method. How Cross-validation relates to other prominent Classical methods such as the Akaike Information Criterion (...) and Bayesian Information Criterion is also discussed. (shrink)
