También queda claro que el papel de la voluntad en la acción humana no puede ser comprendido sin ver la relación que la misma tiene con su raíz en el ser. BackgroundThe proper and ethical inclusion of PWLHIV and their young children in research is paramount to ensure valid evidence is generated to optimize treatment and care. Little empirical data exists to inform ethical considerations deemed most critical to these populations. Our study aimed to systematically review the empiric literature regarding ethical considerations for research participation of PWLHIV and their young children.MethodsWe conducted this systematic review in partnership with a medical librarian. A search strategy was designed and performed within the following (...) electronic databases: Ovid MEDLINE, Embase and CINAHL. We screened titles and abstracts using the following inclusion criteria: (1) a study population of PWLHIV or children under 5 years of age; and (2) collection of qualitative or quantitative data regarding ethics of research participation. Excluded were reviews, commentaries, policy statements, clinical care-related ethics concerns, abstracts, case studies, or studies unrelated to HIV research. Studies were appraised for quality, data were extracted, and studies were qualitatively analyzed using a principle-based ethical framework within the Belmont Report.ResultsOf the 7470 titles identified, 538 full-text articles were reviewed for eligibility and only three articles met full criteria for inclusion within this review. While we allowed for inclusion of studies involving young children born to mothers with HIV, only articles focused on PWLHIV were identified. Within the results of these studies, four themes emerged: (1) adequacy of informed consent; (2) consideration of paternal involvement; (3) balancing risks; and (4) access to research and treatment. A strength of this review is that it included perspectives of international research investigators, community leaders, and male partners. However, only two studies collected empiric data from PWLHIV regarding their experiences participating in researchConclusionResearchers and funding agencies should be aware of these considerations and appreciate the value of and critical need for formative research to ensure clinical trials involving PWLHIV promote ethical, well-informed research participation and, ultimately, improve care outcomes. More research is needed to create a comprehensive ethical framework for researchers when conducting studies with PWLHIV. (shrink)

Abstract:Vulnerability is an important criterion to assess the ethical justification of the inclusion of participants in research trials. Currently, vulnerability is often understood as an attribute inherent to a participant by nature of a diagnosed condition. Accordingly, a common ethical concern relates to the participant’s decisionmaking capacity and ability to provide free and informed consent. We propose an expanded view of vulnerability that moves beyond a focus on consent and the intrinsic attributes of participants. Traditionally, physicians and parents made decisions about children’s health care based on western practices. More recently, with legal and ethical development of informed consent and recognition for decision making, children are becoming active participants in their care. The extent to which this is happening is however blurred by lack of clarity about what children — of diverse levels of cognitive development — are capable of understanding. Moreover, when there are multiple surrogate decision makers, parental and professional conflict can arise concerning (...) children’s ‘best interest’. Giving children a voice and offering choice promotes their dignity and quality of life. Nevertheless, it also presents with many challenges. Case studies using pseudonyms and changed situational identities are used in this article to illuminate the complexity of ethical challenges facing nurses in end-of-life care with children and families. In the December 2006 edition of Harvard Business Review , Michael Porter and Mark Kramer argue that by approaching corporate social responsibility (CSR) based on corporate priorities, strengths and abilities, firms can develop socially and fiscally responsible solutions to current CSR issues, which will provide operational and competitive advantages. We agree that an effective approach to CSR includes a mapping of strategy, risk and opportunity. However, we also caution that the identification of these to the exclusion of societal input may (...) not be to the corporation's advantage. Instead, an investment in both strategic analysis and social capital can pay off from a social and an organizational standpoint. Compared with their larger counterparts, small- and medium-sized enterprises (SMEs) frequently have stronger relationships with their internal and external stakeholders that foster the development of social capital. As such, we believe that the sector offers a unique opportunity to identify additional models and frameworks in order to approach a strategic CSR model as espoused by Porter and Kramer. This paper explores a case study of one Canadian SME that uses a community development framework called Asset Based Community Development (ABCD) for its CSR programming. Because ABCD relies heavily on the development and maintenance of social capital and can be utilized to attain set objectives, we propose that it provides a supplementary framework through which the arguments of Porter and Kramer can be expanded. In applying the ABCD framework for CSR, we can begin to establish a programme that supports strategy, integrates employees and stakeholders towards a common vision, and creates unique and sustainable alternatives towards the resolution of social and corporate goals. In the years following the COVID‐19 pandemic, issues such as high job demands, burnout, and turnover continue to influence the nursing workforce, with heavier impacts to marginalized groups. Understanding the work and life contexts of nurses of color can help guide strategies for workplace equity and meaningful support. This qualitative study explored the experiences of nurses of color in the United States during the pandemic, focusing on feelings about the profession and job decisions. The overarching theme was “answering the call,” (...) with subthemes of “COVID shone a light,” “being consumed by COVID,” and “is it worth it?” Participants shared how their racial identities shaped their perceptions and job decisions in positive and negative ways, noting how racism impacted many facets of their work and added to the stressors felt in the workplace and the community. Findings provide insight into the underrepresented perspectives of nurses of color and suggest strategies to eliminate racism in nursing. (shrink)

A debate has been raging in the philosophy of mind for at least the past two decades. It concerns whether the mental can make a causal difference to the world. Suppose that I am reading the newspaper and it is getting dark. I switch on the light, and continue with my reading. One explanation of why my switching on of the light occurred is that a desiring with a particular content (that I continue reading), a noticing with a particular content (...) (that it is getting dark), and a believing with a particular content (that by switching on the light I could continue reading) occurred in me, and these events caused my switching on of the light. This explanation works by citing the intentional contents of mental phenomena as causes of that action. It is because the intentional causes have the contents that they do, and because those contents play a causal role in bringing about my action, that my action is causally explained. (shrink)

What is introspective know ledge of one’s own intentional states like? This paper aims to make plausible the view that certain cases of self-knowledge, namely the cogito-type ones, are enough like perception to count as cases of quasi-observation. To this end it considers the highly influential arguments developed by Sydney Shoemaker in his recent Royce Lectures. These present the most formidable challenge to the view that certain cases of self-knowledge are quasi-observational and so deserve detailed examination. Shoemaker’s arguments are directed (...) against two models of ordinary perception, the “object perception model” and the “broad perceptual model”. I argue that the core theses that Shoemaker associated with them are either dubious in their own right or applicable to certain cases of self-knowledge. Overall the aim is to show that there is such a variety of patterns in each case that simple analogies or disanalogies are unhelpful. (shrink)

Recently there has been a flurry of interest and activity, both scholarly and political, about the role and importance of fathers in child rearing. One manifestation of this interest is a movement that began in the United Kingdom, but is increasingly influential in the United States and Canada, asserting fathers’ rights in custody disputes following divorce. Advocates assert that fathers should have equal standing with mothers in such cases, and that current practice fails to grant them this standing. U ntil (...) the nineteenth century, most Western legal systems granted fathers property rights in their children and failed to grant mothers any rights at all. Thus, fathers were almost always able to claim custody successfully after divorce. In the latter part of the nineteenth century, the “Tender Years Doctrine” displaced this practice. This view holds that children, especially young children, have special need of maternal nurture, and that mothers are naturally more suited to the task of raising young children than fathers are. Several decades of scholarship on international nongovernmental organizations (NGOs) have established their important role in leading cosmopolitan political projects framed around moral ideals of global justice. But contemporary legitimacy crises in international liberalism call for a reexamination of NGOs’ global justice activism, considering how they should navigate the real-world moral contestations and shifting power dynamics that can impede their pursuit of justice. Recent work by deliberative-democratic theorists has argued that NGOs can help resolve disputes about global justice norms by facilitating (...) legitimate communicative exchanges among the diverse political voices of subjected global communities on the correct interpretation and implementation of global justice norms. In response, this essay argues for an expanded account of the political roles of NGOs in global justice activism, which reflects greater sensitivity to the multifaceted political dynamics through which power in real-world global politics is constituted and contested. It is shown that in some NGOs’ real-world operational contexts, structural power imbalances and social division or volatility can undercut the operation of the ideal deliberative processes prescribed by democratic theory—calling for further attention to work focused on mitigating power imbalances, building solidarity, and organizing power in parallel or as a precursor to deliberative-democratic processes. The prevalence and complexity of local sustainable development challenges require coordinated action from multiple actors in the business, public, and civil society sectors. Large multi-stakeholder partnerships that build capacity by developing and leveraging the diverse perspectives and resources of partner organizations are becoming an increasingly popular approach to addressing such challenges. Multi-stakeholder partnerships are designed to address and prioritize a social problem, so it can be challenging to define the value proposition to each specific partner. Using a resource-based view, this (...) study examines partner outcomes from the perspective of the strategic interest of the partner as distinct from the strategic goal of the partnership. Based on 47 interviews with representatives of partner organizations in four Canadian case studies of community sustainability plan implementation, this article details 10 resources partners can gain from engaging in a multi-stakeholder partnership. Attention to these multi‐level factors is necessary to accurately and comprehensively understand and intervene to improve human health. The population health framework is a valuable conceptual framework to guide nurse researchers in identifying and targeting the broad range of determinants of (...) health. However, attention to the intermediate processes linking multi‐level factors and use of appropriate multi‐level theory and research methodology is critical to utilizing the framework effectively. Nurse researchers are well equipped to undertake such investigations but need to consider a number of political, societal, professional and organizational barriers to do so. By fully embracing the population health framework, nurse researchers have the opportunity to explore the multi‐level influences on health and to develop, implement and evaluate interventions that target immediate needs, more distal factors and the intermediate processes that connect them. This paper argues that efforts to understand historically remote patterns of thought are driven away from their original meaning if the investigation focuses on reconstruction of concepts. It is simply not appropriate to be looking for an archaic concept of soul, name or dream, for example, when considering the earliest documents which attest to their writers’ beliefs about certain types of phenomena. Instead, we propose to employ the notion of cognitive complex in order to investigate some important philosophical themes in (...) Ancient Egyptian, Ancient Iranian, and Ancient Near Eastern documents. Our principal theoretical claim is that archaic thought does not work with concepts but with complexes whose salient features are an over-abundance of properties, an over-production of connections, and weakness in abstraction. The basic level of complex formation may be the most inclusive level at which it is possible to form a mental image. Specific studies are focused on ancient texts which exhibit archaic patterns of thought. In Egyptian texts, “manifestation” seems to convey something which all categories of beings are capable of becoming, being and having, assuming and leaving; the “name” was considered to be an essential component of the individual’s survival; symbolic representations, such as images and words, are causally connected to the ‘objects’ the image or word signifies. In ANE records the human etemmu was plainly the corpse or skeleton of the dead person; on the other hand it was also the shadowy, volatile image of what he was during life. In ANE records the baffling idea of the divine me referred to an entire cultural area, an acquisition of civilized life; but at the same time it is also the result of an invention, a divine decision. The complexes involved in these archaic ideas about soul, name and dream are ideas fused with their ‘objects’; they have unstable traits and prototypical instances; and are thought at the most abstract level which have concrete images. The notion of “relational” autonomy—as described by feminist scholars such as Susan Sherwin and Anne Donchin—has been the subject of a significant body of literature over the last few years and has recently generated some interest within the field of bioethics. Although the focus of this interest has been the autonomy of ordinary moral agents, the analysis of relational autonomy can usefully be extended to apply to the autonomy of professionals, not only as individual moral agents, but in their roles (...) as professionals as well. In this paper, I argue that professional autonomy, rightly understood, is relational in nature. This understanding of professional autonomy stands to improve our understanding of professional ethics, as well as providing a particular, concrete example of what we mean when we call autonomy “relational” and “socially embedded. In this paper I outline and defend an introspectionist account of authoritative self-knowledge for a certain class of cases, ones in which a subject is both thinking and thinking about a current, conscious thought. My account is distinctive in a number of ways, one of which is that it is compatible with the truth of externalism.
